Diagnosis and treatment of biliary mucinous cystic neoplasms: A single-center experience

Background: Biliary mucinous cystic neoplasms(BMCNs) are rare hepatobiliary cystic tumors, which can be divided into noninvasive and invasive types. This study aimed to investigate the diagnosis, treatment, and prognosis of BMCNs in a large single center. Methods: We analyzed 49 patients with BMCNs confirmed by postoperative pathology at the first Afflliated Hospital, Zhejiang University school of Medicine between January 2007 and December 2021. Results: Among the 49 patients, 37 were female(75.5%), and the average age was 57.04 years. Common symptoms included abdominal discomfort, jaundice and fever, while 22 patients(44.9%) had no symptoms. Serum carbohydrate antigen(CA) 19-9 and CA125 concentrations were elevated in 34.8% and 19.6% of patients, respectively. Forty-eight patients had tumors in the intrahepatic bile ducts and only one had a tumor in the extrahepatic bile duct. Forty-eight patients with noninvasive intrahepatic BMCNs were further analyzed in terms of pathological features: 34(70.8%) had low-grade intraepithelial neoplasms(LGINs), and 14(29.2%) had high-grade intraepithelial neoplasms(HGINs). The potential immunohistochemical markers of BMCNs were cytokeratin(CK) 19, CK7, estrogen receptor and progesterone receptor. Follow-up data for 37 patients with intrahepatic BMCNs were obtained. The median overall survival(OS) of BMCNs was not reached. The longest survival time was 137 *** 5-and 10-year OS rates were 100% and 85.4%, respectively. The 5-and 10-year recurrence-free survival(RFS) rates were 93.9% and 80.2%, respectively. Conclusions: BMCNs are rare cystic neoplasms that commonly occur in middle-aged females. BMCNs can only be diagnosed and classified by postoperative pathology, as there are no specific clinical presentations, serological indicators or imaging modalities for preoperative diagnosis. Complete surgical resection is necessary for BMCNs, and the postoperative prognosis is favorable.

Airborne transmission is among the most frequent types of nosocomial *** years have witnessed frequent outbreaks of airborne diseases,such as severe acute respiratory syndrome(SARS)in 2002,middle East respiratory syndrome(MERS)in 2012,and coronavirus disease 2019(COVID-19),with the latter being on the rampage since the end of 2019 and bringing the effect of aerosols on health back to the fore(Gralton et al.,2011;Wang et al.,2021).An increasing number of studies have shown that certain highly transmissible pathogens can maintain long-term stability and efficiently spread through aerosols(Leung,2021;Lv et al.,2021).As reported previously,influenza viruses that can spread efficiently through aerosols remain stable for a longer period compared to those that *** World Health Organization(WHO)has stated that aerosol-generating procedures(AGPs)play an important role in aerosol transmission in hospitals(Calderwood et al.,2021).AGPs,referring to medical procedures that produce aerosols,including dental procedures,endotracheal intubation,sputum aspiration,and laparoscopic surgeries,have been reported to be significantly associated with an increased risk of nosocomial infection among medical personnel(Hamilton,2021).

Exact Tail Asymptotics for a Queueing System with a Retrial Orbit and Batch Service

This paper discusses a queueing system with a retrial orbit and batch service, in which the quantity of customers’ rooms in the queue is finite and the space of retrial orbit is infinite. When the server starts serving, it serves all customers in the queue in a single batch, which is the so-called batch service. If a new customer or a retrial customer finds all the customers’ rooms are occupied, he will decide whether or not to join the retrial orbit. By using the censoring technique and the matrix analysis method, we first obtain the decay function of the stationary distribution for the quantity of customers in the retrial orbit and the quantity of customers in the queue. Then based on the form of decay rate function and the Karamata Tauberian theorem, we finally get the exact tail asymptotics of the stationary distribution.

To the Editor:Hepatocellular carcinoma(HCC)is one of the main causes of cancer related death worldwide,and new cases are expected to continue to increase in future[1].Surgical resection is still the best way to remove the tumor and improve patient’s ***,resection of caudate lobe often presents a technical challenge,even to accomplished hepatic *** lobe is located anterior to the inferior vena cava(IVC)and posterior to the bifurcation of the portal vein(PV),generally consists of three regions:the paracaval portion,the left Spiegel lobe and the process portion[2].The anatomy of the paracaval portion includes the liver parenchyma ventral to the hepatic IVC and the area between the Spiegel lobe and the right lobe adjacent to the middle hepatic vein(MHV)ventrally,which was classified by Couinaud as segment IX[3].Here,we presented a case of HCC with successful downstaging therapy,as well as variation of MHV,which provides a safe path for anterior transection of segment IX.

Brain Cancer Treatment with Gene Editing

The first method is used gene editing to knock out the PD-L1 receptor located on the T cell surface so that PD-1 on the cancer cell surface cannot combine with the PD-L1,in that case,T cell can identify the abnormal cell and kill *** the beginning,researchers use protein-guided editing technology,but it is not easy to control and not specific enough,so they choose to use CRISPR-Cas9 to edit the target *** with the traditional protein-guided nucleases,CRISPR-Cas9 system is more easy-handle,highly specific,and it is an more efficient tool for engineering eukaryotic genomes;because CRISPR-Cas9 system aims to edit the targeting genes by tiny RNAs guiding the Cas9 nuclease to the target site by base *** second treatment is mainly used“fighting cancer with cancer”.Because living tumor cells have the ability to home and target tumors,thus,if those living tumor cells can be engineered to secrete therapeutic agents,the tumor cells can be effectively ***’s team picked the agent interferon-β(IFN-β).However,this idea of treatment is limited by the premature cell death due to autocrine *** researchers solved this problem by first using CRISPR Cas9 to knock out the IFN-β–specific receptor(IFNAR1)in inherently IFN-β–sensitive syngeneic tumor cells,and subsequently engineered them to constitutively produce IFN-βfor tumor cell targeting and simultaneous *** therapeutic cells are further designed to coexpress granulocyte-macrophage colony-stimulating factor(GM-CSF)that facilitates the differentiation,proliferation,and recruitment of dendritic cells(DCs).The last approach can stop cancer cell repairing their DNA when it gets damaged.

Individuals may gather information about environmental conditions when deciding where to breed in order to maximize their lifetime *** can obtain social information by observing conspecifics and heterospecifics with similar ecological *** studies have shown that birds can rely on social information to select their nest *** location of active nests and the reproductive success of conspecifics and heterospecifics can provide accurate predictions about the quality of the breeding *** short-lived species can facultatively reproduce two and/or more times within a breeding ***,few studies have focused on how multiplebrooding individuals select nest sites for their second breeding *** this study,we use long-term data to test whether the Japanese Tit(Parus minor)can use social information from conspecifics and/or heterospecifics(the Eurasian Nuthatch Sitta europaea,the Daurian Redstart Phoenicurus auroreus and the Yellow-rumped Flycatcher Ficedula zanthopygia)to select a nest site for the second breeding *** results showed that the nest boxes occupied by tits on their second breeding attempt tended to be surrounded by more breeding conspecific nests,successful first nests of conspecifics,and fewer failed first nests of conspecifics than the nest boxes that remained unoccupied(the control group).However,the numbers of breeding heterospecific nests,successful heterospecific nests,and failed heterospecific nests did not differ between the nest boxes occupied by tits on their second breeding attempt and the unoccupied nest ***,the tits with local successful breeding experience tended to choose areas with more successful first nests of conspecifics than those without successful breeding ***,we suggest that conspecifics'but not heterospecifics'social information within the same breeding season is the major factor influencing the nest site selection of Japanese Tits during second breeding attempts.

Limit Test and pH Determination of 5-Hydroxymethylfurfural in Jiulongteng Honey

[Objectives]To explore the effect of storage time on pH and 5-hydroxymethylfurfural content in Jiulongteng honey.[Methods]The pH of Jiulongteng honey was determined by neutralization titration with sodium hydroxide standard *** content of 5-hydroxymethylfurfural in Jiulongteng honey was determined by *** conditions:ZORBAX SB-C 18 column(250 mm×4.6 mm,5μm)from Agilent Co.,Ltd.,acetonitrile-0.1%formic acid solution(5:95)as mobile phase,flow rate of 0.8 mL/min,5-hydroxymethylfurfural detection wavelength of 284 nm,guanosine detection wavelength of 254 nm.[Results]The pH of 12 batches of Jiulongteng honey was 3.70-3.84 in the new honey stage,3.92-4.05 in the old honey stage 1,and 4.25-4.53 in the old honey stage 2;5-hydroxymethylfurfural was not detected in the new honey stage,5-hydroxymethylfurfural was detected in FM-001 in the old honey stage 1,and 5-hydroxymethylfurfural was detected in most samples in the old honey stage 2.[Conclusions]The pH and 5-hydroxymethylfurfural content of 12 batches of Jiulongteng honey met the requirements within 3 years of *** was no 5-hydroxymethylfurfural in Jiulongteng honey,but with the extension of storage time,the detection amount of 5-hydroxymethylfurfural increased significantly even if Jiulongteng honey was stored at low ***,5-hydroxymethylfurfural can be used as an important indicator of honey freshness.
