Microvascular changes after conbercept therapy in central retinal vein occlusion analyzed by optical coherence tomography angiography

AIM: To investigate microvascular changes in eyes with central retinal vein occlusion(CRVO) complicated by macular edema before and after intravitreal conbercept injection and evaluate correlations between these changes and best-corrected visual acuity(BCVA) and retinal thickness. METHODS: Twenty-eight eyes of 28 patients with macular edema caused by CRVO were included in this retrospective study. All patients received a single intravitreal conbercept injection to treat macular edema. BCVA and the results of optical coherence tomography angiography(OCTA) automatic measurements of the vessel density in the superficial(SCP) and deep retinal capillary plexus(DCP), the foveal avascular zone(FAZ) area, the FAZ perimeter(PERIM), the vessel density within a 300-μm wide ring surrounding the FAZ(FD-300), the acircularity index(AI), the choriocapillaris flow area, and retinal thickness were recorded before and at one month after treatment and compared with the results observed in age-and sexmatched healthy subjects. RESULTS: The vessel density in the SCP and DCP, the FD-300, and the flow area of the choriocapillaris were allsignificantly lower in CRVO eyes than in healthy eyes, while the AI and retinal thickness were significantly higher(all P<0.05). After treatment, retinal thickness was significantly decreased, and the mean BCVA had markedly improved from 20/167 to 20/65(P=0.0092). The flow area of the choriocapillaris was also significantly improved, which may result from the reduction of shadowing effect caused by the attenuation of macular edema. However, there were no significant changes in SCP and DCP vessel density after treatment. The flow area of the choriocapillaris at baseline was negatively correlated with retinal ***: OCTA enables the non-invasive, layerspecific and quantitative assessment of microvascular changes both before and after treatment, and can therefore be used as a valuable imaging tool for the evaluation of the follow-up in CRVO pat

Background：The efficacy of montelukast （MONT）,a cysteinyl leukotriene receptor antagonist,in nonasthmatic eosinophilic bronchitis （NAEB）,especially its influence on cough associated life quality is still *** evaluated the efficacy of MONT combined with budesonide （BUD） as compared to BUD monotherapy in improving life quality,suppressing airway eosinophilia and cough remission in NAEB.Methods：A prospective,open-labeled,multicenter,randomized controlled trial was *** with NAEB （aged 18-75 years） were randomized to inhaled BUD （200 μg,bid） or BUD plus oral MONT （10 μg,qn） for 4 *** cough questionnaire （LCQ） life quality scores,cough visual analog scale （CVAS） scores,eosinophil differential ratio （Eos）,and eosinophil cationic protein （ECP） in induced sputum were monitored and compared.Results：The control and MONT groups contained 33 and 32 patients,respectively,with similar baseline *** with-in group improvement in CVAS,LCQ scores,Eos,and ECP was observed in both groups during *** 2-week treatment,add-on treatment of MONT was significantly more effective than BUD monotherapy for CVAS decrease and LCQ scores improvement （both P 〈 0.05）.Similar results were seen at 4-week assessment （both P 〈 0.05）.4-week add-on therapy of MONT also resulted in a higher percentage of patients with normal sputum Eos （〈2.5%） and greater decrease of ECP （both P 〈 0.05）.Conclusions：MONT combined with BUD was demonstrated cooperative effects in improvement of life quality,suppression ofeosinophilic inflammation,and cough remission in patients with NAEB.

Objective: The infiltration of gut-homing T cells and their polarization to proinflammatory Th17 cells plays an important role in inflammation caused by necrotizing enterocolitis(NEC). Increased production of gut-homing T cells in peripheral blood has been linked to several gastrointestinal inflammatory diseases;however, its relationship with NEC is not understood. We investigated whether gut-homing T cells were increased in peripheral blood of patients with NEC, and examined the characteristics of these cells and their clinical significance. Design: Gut-homing T cells were evaluated by flow cytometry in peripheral blood collected from 77 patients with NEC and 80 control neonates. An in vitro T cell polarization assay was performed to investigate the role of proinflammatory cytokines in regulatory T(Treg) cell polarization. In vivo Treg cell polarization analysis was performed using NEC mice treated with anti-interleukin-6(IL-6) receptor antibody. Results: A higher proportion of gut-homing CD4 T cells was found in the peripheral blood of NEC patients compared to that in control neonates. The elevated gut-homing CD4 T cells were primarily IL-17-producing Treg cells, which showed the phenotypic characteristics of conventional Treg and Th17 cells, and were negatively correlated with clinical severity. IL-6 promoted the polarization of Treg cells to IL-17-producing Treg cells, and blocking IL-6 signaling inhibited this conversion. Conclusion: The proportion of gut-homing IL-17-producing Treg cells was significantly increased in the peripheral blood of patients with NEC and retained suppressive features, indicating that these cells worked as negative regulators in peripheral blood;however, they may be pathogenic in the gut.
