Effect of cOVID-19 pneumonia infection control protocols on nosocomial infection incidence in trauma patients

Purpose:Nosocomial infection is a major threat to the health care system and patient *** the pandemic,new protocols were established in hospitals and communities to protect against the transmission of cOvID-19,which may have changed the incidence of nosocomial *** study was conducted to compare the incidence of nosocomial infection before and after the covID-19 ***:This was a retrospective cohort study performed on trauma patients who were admitted,from May 22,2018 to November 22,2021,to the largest level-1 trauma center in Shiraz,Iran(Shahid Rajaei trauma Hospital).All the trauma patients over 15 years old admitted during the study time were included in this *** who were declared dead upon arrival were *** were evaluated in 2 periods:before the pandemic(May 22,2018-February 19,2020)and after the pandemic(February 19,2020-November 22,2021).Patients were assessed based on demographic information(age,gender,length of hospital stay,and patient outcome),the occurrence of hospital infection,and the type of *** analysis was done using SPSS version ***:Overall,60,561 patients were admitted,with a mean age of 40 *** infection was diagnosed in 4.00%(n=2423)of all admitted *** incidence rate of post-COVID-19 hospitalacquired infections decreased by 16.28%(p<0.001)when compared to before the pandemic;in contrast,surgical site infection(p<0.001)and urinary tract infection(p=0.043)were responsible for this change,while hospital-acquired pneumonia(p=0.568)and bloodstream infection(p=0.156)were not significantly *** mortality was 1.79%,while 28.52%of all patients with nosocomial infections *** the pandemic,there was a 25.78%increase(p<0.001)in the overall incidence rate of mortality,which was also observed among patients with nosocomial infections(17.84%).Conclusion:The incidence of nosocomial infection has decreased during the pandemic,possibly due to the

The intervertebral disc(IVD) is the largest avascular tissue. Hypoxia-inducible factors(HIFs) play essential roles in regulating cellular adaptation in the IVD under physiological conditions. Disc degeneration disease(DDD) is one of the leading causes of disability, and current therapies are ineffective. This study sought to explore the role of HIFs in DDD pathogenesis in mice. The findings of this study showed that among HIF family members, Hif1α was significantly upregulated in cartilaginous endplate(EP) and annulus fibrosus(AF) tissues from human DDD patients and two mouse models of DDD compared with controls. Conditional deletion of the E3 ubiquitin ligase Vhl in EP and AF tissues of adult mice resulted in upregulated Hif1α expression and age-dependent IVD degeneration. Aberrant Hif1α activation enhanced glycolytic metabolism and suppressed mitochondrial function. On the other hand, genetic ablation of the Hif1α gene delayed DDD pathogenesis in Vhl-deficient mice. Administration of 2-methoxyestradiol(2ME2), a selective Hif1α inhibitor, attenuated experimental IVD degeneration in mice. The findings of this study show that aberrant Hif1α activation in EP and AF tissues induces pathological changes in DDD, implying that inhibition of aberrant Hif1α activity is a potential therapeutic strategy for DDD.

Sepsis,defined as life-threatening organ failure caused by a dysregulated host response to severe infection,is a major cause of death among intensive care unit *** targeting on immunomodulatory is a new research field in sepsis treatment.B-and T-lymphocyte attenuator(BTLA)is an inhibitory costimulatory factor molecule of B and T *** have shown that elevated expression of BTLA in lymphocytes can reduce mortality in sepsis,but its regulatory compounds and the underlying mechanism remains to be ***,we show that treatment with CP-673451 significantly decreases mortality of septic ***-673451 is a PDGFR kinase inhibitor which can promote the expression of BTLA,inhibit the release of chemokines such as CXCL13,and reduce first the chemotaxis of B cells to the peripheral blood and vital ***-673451 also inhibits both the release of cytokines and chemokines such as IL-1β,IL-6,IL-10,TNF-α,CCL1,CCL2 and CCL7 and reduces both the chemotactic ability of T *** suggests that CP-673451 may prevent septic death by inhibiting lymphocyte chemotaxis and alleviating“cytokine storm”.In conclusion,our study provides a new therapeutic target and an effective compound for sepsis treatment.

Musculoskeletal alterations in hepatocellular carcinoma(HCC)are less common than liver-related ***,they can significantly impact the quality of life and overall prognosis of patients with *** main obstacle in the clinical assessment of HCC-induced musculoskeletal alterations is related to effective and timely diagnosis because these complications are often asym-ptomatic and unapparent during routine clinical *** narrative literature review aimed to provide a comprehensive overview of the contem-porary literature related to the changes in the musculoskeletal system in patients with HCC,focusing on its clinical implications and underlying etiopathogenetic *** bone metastases are the most common skeletal alterations associated with HCC,which could be associated with an increased risk of low-trauma bone ***,previous studies reported that osteopenia,sarcopenia,and myosteatosis are associated with poor clinical outcomes in patients with *** though low bone mineral density and sarcopenia are consistently reported as reliable predictors of pretransplantation and post-transplantation mortality in HCC patients,these complications are frequently overlooked in the clinical management of patients with *** together,contemporary literature suggests that a multidisciplinary approach is essential for early recognition and clinical management of HCC-associated musculoskeletal alterations to improve patient *** research into the mechanisms and treatment options for musculoskeletal complications is warranted to enhance our understanding and clinical management of this aspect of HCC.
