Laparoscopic Janeway gastrostomy as preferred enteral access in specific patient populations:A systematic review and case series

BACKGROUND Nutrition is one of the fundamental needs of both patient and non-patient *** trends promote enteral feeding as a superior route,with the most common enteral access being the percutaneous endoscopic gastrostomy(PEG)as the first-line procedure,with surgical access including Witzel gastrostomy,Stamm Gastrostomy,Janeway gastrostomy(JG)as secondary *** To describe cases and technique of laparoscopic Janeway gastrostomy(LJG)and perform a systematic review of the *** We successfully performed two LJG procedures,after which we conducted a literature review of all documented cases of LJG from 1991 to *** surveyed these cases to show the efficacy of LJG and provide comparisons to other existing procedures with primary outcomes of operative time,complications,duration of gastrostomy use,and application *** data were then extracted and assessed on the basis of the Reference Citation Analysis(https://***/).RESULTS We presented two cases of LJG,detailing the simplicity and benefits of this *** subsequently identified 26 articles and 56 cases of LJG and extrapolated the data relating to our outcome *** could show the potential of LJG as a viable and preferred option in certain patient populations requiring enteral access,drawing reference to its favorable outcome profile and low complication *** The LJG is a simple,reproducible procedure with a favorable complication *** its technical ease and benefits relating to the gastric tube formed,we propose this procedure as a viable,favorable enteral access in patients with the need for permanent or palliative gastrostomy,those with neurologic disease,agitation or at high risk of gastrostomy dislodgement,or where PEG may be infeasible.

Gastroschisis and omphalocele are the two most common congenital defects of the anterior abdominal *** etiologies remain unclear,but while gastroschisis is associ-ated with young maternal age,maternal smoking,maternal alcohol and illicit drug use during pregnancy omphalocele is frequently associated with genetic disorders and other congenital anomalies[1].The incidence of gastroschisis in the United States(US)has increased with an estimated inci-dence of 4.6 per 10,000 live births(LB)between 2010 and 2014[2].A recent study capturing information on 20%of infants admitted to neonatal intensive care unit(NICU)in the US showed a decrease in gastroschisis hospitalizations while that of omphalocele remained stable[3].However,it lacks generalizability[3].Our aim was to provide updated trends of gastroschisis and omphalocele hospitalizations in the US between 2010 and 2018 using population-based and nationally representative databases.

Castleman’s Disease of the Pleura: Case Presentation and Review of Literature

Purpose. Castleman’s Disease is a very rare medical disease and the presence of this condition in the pleura is even rarer and there are only few reported cases. The presence of symptoms is also unusual and has been mostly described in multicentric cases of castleman’s disease instead of solitary lesions (unicentric castleman). We report a case of unicentric pleural castlema’s disease with presence of constitutional symptoms. Methods. A 36-year-old male without any significant past medical history who presented to the emergency due to an incidental large pleural effusion seen in chest radiograph, but referring chronic constitutional symptoms consistent of significant weight loss and chronic cough. Results. Chest computed tomography revealed a mass and several attemps with minimally invase approach failed to identify the lesion. The mass was excised through VATS but due to its significant vascularity, it had to be converted to open thoracotomy. Conclusion. The treatment of unicentric lesions is mainly complete surgical excision and the prognosis is excellent with some chances of recurrence. There are less than one dozen of reported pleural castleman reported in the literature. This case evidence than unicentric lesion may exhibit constitutional symptoms and confirmed than lesions above 5 cm can be removed by VATS but the chances for conversion to an open thoracotomy are high.

Echocardiographic findings in pulmonary embolism: An important guide for the management of the patient

Echocardiography can be used as an easy, inexpensive, devoid of complications and, for the most part, universally available tool for the risk stratification of patient with acute pulmonary embolism. Because of its low sensibility, an echocardiogram is best indicated in a patient in whom hypotension and or shock are suspected to be due to pulmonary embolism. Pertinent findings in the echocardiogram can justify the use of fibrinolytic therapy. Most common echocar-diographic findings in acute pulmonary embolism are: dilatation of the right ventricle, right ventricular dysfunction in some cases with preservation of the motility of the apex, dilatation of the inferior vena cava with lack of collapse during inspiration flattening of the interventricular septum suggesting right ventricular pressure overload and pulmonary hypertension based on the jet of tricuspid regurgitation if available.

Cardiac tamponade as the initial manifestation of severe hypothyroidism: A case report

Background: Hypothyroidism is a commonly seen condition. The presence of pericardial effusion with cardiac tamponade as initial manifestation of this endocrinological condition is very unusual. Objectives: In hypothyroidism pericardial fluid accu-mulates slowly, allowing adaptation and stretching of the pericardial sac, sometimes accommodating a large volume. Case Report: A 39 year-old female presented with chest pain, dyspnea and lower extremity edema for 1 day. Bradycardia, muffled heart sounds and severe hypertension were noticed. Chest radiograph showed an enlarged cardiac silhouette. A bedside echocardiogram revealed a cardiac tamponade, later she developed sudden hypotension and bradycardia that resolved after pericardiocentesis of 1 liter of pericardial fluid. The further laboratory evaluation revealed a TSH value of 69.3 miU/L and low T3 and free T4. The patient later developed reaccumulation of pericardial fluid with the need for creation of pericardial window. Conclusion: When the classic Beck’s triad is not present and bradycardia accompanies a cardiac tamponade, hypothyroidism should be strongly suspected. The requirement for thyroid hormone supplement is critical and is well reported. There is a chance of recurrence even after starting levothyroxine supplementation;and the associated hypertension usually requires treatment with more than one drug.

Electrocardiogram manifestations in hyperkalemia

Hyperkalemia is defined as serum potassium level of more than 5 mmol/L. Prompt identification of hyper-kalemia and appropriate management are critical, since severe hyperkalemia can lead to lethal cardiac dysrhythmias. There is a wide range of electrocardiogram (EKG) changes associated with hyperkalemia. The sequence of EKG changes has been previously described with limited information to correlate the level of potassium to a particular change in the EKG. This study aims to describe a correlation between the level of potassium and EKG changes in the presence or absence of certain diagnoses, to determine which EKG finding in the context of level of hyperkalemia, should be considered life-threatening and prompt emergency intervention. If a relationship between serum levels of potassium and EKG changes is significant, clinicians may be able to better monitor and treat hyperkalemic patients. This paper reviews the literature on hyperkalemia, potassium homeostasis and EKG changes attributed to elevated potassium.
