More than 80%of all cases of deafness are related to the death or degeneration of cochlear hair cells and the associated spiral ganglion neurons,and a lack of regeneration of these cells leads to permanent hearing ***,the regeneration of lost hair cells is an important goal for the treatment of ***1 is a basic helix-loop-helix(bHLH)transcription factor that is critical in both the development and regeneration of cochlear hair ***1 is transcriptionally regulated by several signaling pathways,including Notch and Wnt *** the post-translational level,it is regulated through the ubiquitin-proteasome *** vitro and in vivo studies have revealed that manipulation of these signaling pathways not only controls development,but also leads to the regeneration of cochlear hair cells after *** progress toward understanding the signaling networks involved in hair cell development and regeneration has led to the development of new strategies to replace lost hair *** review focuses on our current understanding of the signaling pathways that regulate Atoh1 in the cochlea.

Bilateral congenital dacryocystoceles with intranasal mucoceles in the neonate: Case report and review of literature

Congenital dacryocystoceles with intranasal mucoceles are an uncommon entity. The overwhelming majority of reported cases are unilateral in nature. We present a rare case of a neonate with bilateral congenital dacryocystoceles with intranasal mucoceles. Management of dacryocystoceles includes medical and surgical treatment. Literature review revealed 186 reported cases of congenital dacryocystoceles with 82 associated with intranasal mucoceles. Prevalence rates ranged between 11% to 100%. Our case highlights the importance of nasal endoscopy in the work up and identification given the high prevalence rate of intranasal mucocele component in patients with congenital dacryocystoceles.

To present the clinical characteristics and the misdiagnosis rate of acute coronary syndrome manifested primarily as throat discomfort,we conducted a multicentric and retrospective study in the cardiology and otorhinolaryngology *** of patients with primary complaint of throat discomfort,absence of chest pain at onset,and an ultimate diagnosis of acute coronary syndrome,as well as patients with pharyngitis(as controls)were collected from May 2015 to April *** patients’main manifestations were *** regression results showed that chest tightness,dyspnea,perspiring,and exertional throat symptoms were significantly associated with acute coronary syndrome,with odds ratios of 8.3(95%CI 2.2–31.5),10.9(95%CI 1.8–66.9),25.4(95%CI 3.6–179.9),and 81.2(95%CI 13.0–506.7).A total of 25(56.82%)out of 44 acute coronary syndrome patients,who were first admitted to the otorhinolaryngology department,were misdiagnosed,with a 12%(3/25)mortality *** discomfort can be the principal manifestation of acute coronary *** patients exhibit high misdiagnosis and mortality *** throat symptoms,chest tightness,perspiring,and dyspnea were important indicators of acute coronary syndrome in patients whose main complaint was throat *** awareness of this condition will result in prompt diagnosis and reduce morbidity and mortality.

Background: Age-related macular degeneration (AMD) and cardiovascular disease share common risk factors. Inflammatory biomarkers, including C-reactive protein (CRP),interleukin 6 (IL- 6), soluble tumor necrosis factor alpha receptor 2, soluble intercellular and vascular adhesion molecules (intercellular adhesion molecule 1 and vascular cell adhesion molecule 1), and lipid biomarkers, including lipoprotein(a) and apolipoprotein B, have all been associated with cardiovascular disease. We previously found an association betweenAMD and CRP in a cross-sectional analysis, but the prospective relationships between AMD, CRP, and the other cardiovascular disease markers are unknown. Objective: To test the hypothesis that baseline cardiovascular disease biomarkers are associated with subsequent increased risk for progression ***, Setting, and Participants: This prospective cohort study involved 251 participants aged 60 years and older who had some sign of nonexudative AMD and visual acuity of 20/200 or better in at least one eye at baseline. The AMD status was assessed by standardized grading of fundus photographs, and stored fasting blood specimens obtained at baseline were analyzed for levels of the various biomarkers. The average follow-up time was 4.6 years. Main Outcome Measures: Relationship between biomarkers and incidence rates of progression of AMD. Results: Comparing the highest quartile with the lowest quartile, CRP was associated with progression of AMD, with a multivariate adjusted relative risk (RR) of 2.10 (95% confidence interval (CI), 1.06- 4.18; P for trend, .046) controlling for body mass index, smoking, and other cardiovascular variables and a multivariate adjusted RR of 2.02 (95% CI, 1.00- 4.04; P for trend, .06) controlling additionally for antioxidant nutrients. Objective: To evaluate modifiable risk and protective factors for age-related macular degeneration (AMD) among elderly twins. Methods: The US Twin Study of Age-Related Macular Degeneration comprises elderly male twins from the National Academy of Sciences-National Research Council World War II Veteran Twin Registry. To determine genetic and environmental risk factors for AMD, twins were surveyed for a prior diagnosis of AMD and underwent an eye examination, fundus photography, and food frequency and risk factor questionnaires. This environmental component of the study includes 681 twins: 222 twins with AMD (intermediate or late stages) and 459 twins with no maculopathy or early signs. Risk for AMD according to cigarette smoking and dietary fat intake was estimated using logistic regression analyses. Results: Current smokers had a 1.9-fold increased risk (95%confidence interval, 0.99-3.68, P=.06) of AMD while past smokers had about a 1.7-fold increased risk (95%confidence interval, 1.2-2.6, P=.009). Increased intake of fish reduced risk of AMD, particularly for 2 or more servings per week (P trend =.04). Dietary omega-3 fatty intake was inversely associated with AMD (odds ratio, 0.55; 95%confidence interval, 0.32-0.95) comparing the highest vs lowest quartile. Reduction in risk of AMD with higher intake of omega-3 fatty acids was seen primarily among subjects with low levels (below median) of linoleic acid intake, an omega-6 fatty acid (P trend<.001). The attributable risk percentage was 32%for smoking and the preventive fraction was 22%for higher omega-3 intake. Conclusions: This study of twins provides further evidence that cigarette smoking increases risk while fish consumption and omega-3 fatty acid intake reduce risk of AMD.

The objective of this manuscripts to review current knowledge regarding exosomes as they relate to the physiology and pathology of the human nose as well as their role as biomarkers of chronic rhinosinusitis with nasal polyps (CRSwNP). Exosomes are 30-150 nm membrane-bound vesicles secreted by virtually all cell types. Exosomes contribute to the rapid inter-epithelial transfer of proteins and mediate innate immunosurveillance and defense mechanisms in the human nasal cavity. Exosomes also protect their cell specific cargo from degradation by nucleases and proteases and mirrorCRS related tissue protein perturbations more effectively than whole mucus. Thus, exosomal isolation and analysis may be used to non-invasively monitor disease severity, prognosis, and potentially even treatment response. Recent studies of exosomes in CRS suggest they can be used to study the immunopathology of chronic sinonasal inflammation. Furthermore, their relative accessibility suggests that exosomal proteomescan be used as non-invasive, serial, and quantitative biosignatures for rhinosinusitis that can be sampled in clinic in order to predict disease severity, prognosis, and treatment response. Exosomal research has also led to important revelations regarding their physiologic function as they seem to play an important role in innate immunosurveillance and defense. However, exosomal research is still nascent and cost-effectiveness as well as feasibility of implementation in the routine workup for CRS have to be further explored.
