Transurethral resection of the prostate is an independent risk factor for biochemical recurrence after radical prostatectomy for prostate cancer

Biochemical recurrence(BCR)is important for measuring the oncological outcomes of patients who undergo radical prostatectomy(RP).Whether transurethral resection of the prostate(TURP)has negative postoperative effects on oncological outcomes remains controversial.The primary aim of our retrospective study was to determine whether a history of TURP could affect the postoperative BCR rate.We retrospectively reviewed patients with prostate cancer(PCa)who had undergone RP between January 2009 and October 2017.Clinical data on age,prostate volume,serum prostate-specific antigen levels(PSA),biopsy Gleason score(GS),metastasis stage(TNM),D’Amico classification,and American Society of Anesthesiologists(ASA)classification were collected.Statistical analyses including Cox proportional hazard models and sensitivity analyses which included propensity score matching,were performed,and the inverse-probability-of-treatment-weighted estimator and standardized mortality ratio-weighted estimator were determined.We included 1083 patients,of which 118 had a history of TURP.Before matching,the non-TURP group differed from the TURP group with respect to GS(P=0.047),prostate volume(mean:45.19 vs 36.00 ml,P<0.001),and PSA level(mean:29.41 vs 15.11 ng ml?1,P=0.001).After adjusting for age,PSA level,T stage,N stage,M stage,and GS,the TURP group showed higher risk of BCR(hazard ratio[HR]:2.27,95%confidence interval[CI]:1.13–3.94,P=0.004).After matching(ratio 1:4),patients who underwent TURP were still more likely to develop BCR according to the adjusted propensity score(HR:2.00,95%CI:1.05–3.79,P=0.034).Among patients with PCa,those with a history of TURP were more likely to develop BCR after RP.

DOK3 promotes proliferation and inhibits apoptosis of prostate cancer via the NF-κB signaling pathway

Background: DOK3 (Downstream of kinase 3) is involved primarily with immune cell infiltration. Recent research reported the role of DOK3 in tumor progression, with opposite effects in lung cancer and gliomas;however, its role in prostate cancer (PCa) remains elusive. This study aimed to explore the role of DOK3 in PCa and to determine the mechanisms involved. Methods: To investigate the functions and mechanisms of DOK3 in PCa, we performed bioinformatic and biofunctional analyses. Samples from patients with PCa were collected from west china Hospital, and 46 were selected for the final correlation analysis. A lentivirus-based short hairpin ribonucleic acid (shRNA) carrier was established for silencing DOK3. A series of experiments involving the cell counting kit-8, bromodeoxyuridine, and flow cytometry assays were performed to identify cell proliferation and apoptosis. Changes in biomarkers from the nuclear factor kappa B (NF-κB) signaling pathway were detected to verify the relationship between DOK3 and the NF-κB pathway. A subcutaneous xenograft mouse model was performed to examine phenotypes after knocking down DOK3 in vivo . Rescue experiments with DOK3 knockdown and NF-κB pathway activation were designed to verify regulating effects. Results: DOK3 was up-regulated in PCa cell lines and tissues. In addition, a high level of DOK3 was predictive of higher pathological stages and worse prognoses. Similar results were observed with PCa patient samples. After silencing DOK3 in PCa cell lines 22RV1 and PC3, cell proliferation was significantly inhibited while apoptosis was promoted. Gene set enrichment analysis revealed that DOK3 function was enriched in the NF-κB pathway. Mechanism experiments determined that knockdown of DOK3 suppressed activation of the NF-κB pathway, increased the expressions of B-cell lymphoma-2 like 11 (BIM) and B-cell lymphoma-2 associated X (BAX), and decreased the expression of phosphorylated-P65 and X-linked inhibitor of apoptosis (XIAP). To the Editor:china is the most populated country,and it is aging more quickly than almost any other country in the world.According to Worldometer,[1]the population size of china,as of 2020,is around 1.4 billion,with the proportion of the 60-year olds and older being 17.30%in 2017 and expected to reach 35%in 2050.Older age tended to be closely correlated with multimorbidity.Ward and Schiller[2]reported that 65%of men and 72%of women aged>65 years in the US had at least two chronic conditions in 2010.Accordingly,older adults living with multiple chronic conditions tend to be readmitted after discharge,further leading to increased expenditure and more utilization of medical resources.Many studies have reported higher read mission rates in elderly patients.However,previous studies have focused on investigating the factors that influence read mission for a single disease,rather than for the chronic comorbid conditions in the initial hospitalization and the changes in these conditions from initial hospitalization to read mission among the elderly in china.Furthermore,most studies have focused on Caucasian patients and have been conducted in the US.Little is known about Chinese patients due to the lack of large-scale data.Identifying the common chronic comorbid conditions and the changes in these conditions from initial hospitalization to read mission among older people is important as it will help in the understanding of the chronic conditions leading to avoidable read mission and facilitate the development of efficient intervention strategies for the reduction of read missions.

Sex-specific association between coffee consumption and incident chronic kidney disease: a population-based analysis of 359,906 participants from the UK Biobank

Background:The risk for chronic kidney disease(CKD)is influenced by genetic predisposition,sex,and lifestyle.Previous research indicates that coffee is a potentially protective factor in CKD.The current study aims to investigate whether sex disparity exists in the coffee–CKD association,and whether genetic risk of CKD or genetic polymorphisms of caffeine metabolism affect this association.Methods:A total of 359,906 participants from the UK Biobank who were enrolled between 2006 and 2010 were included in this prospective cohort study,which aimed to estimate the hazard ratios for coffee intake and incident CKD using a Cox proportional hazard model.Allele scores of CKD and caffeine metabolism were additionally adjusted for in a subsample with qualified genetic data(n=255,343).Analyses stratified by genetic predisposition,comorbidities,and sex hormones were performed.Tests based on Bayesian model averaging were conducted to ascertain the robustness of the results.Results:Coffee was inversely associated with CKD in a dose-dependent manner.The effects of coffee did not differ across different strata of genetic risk for CKD,but were more evident among slower genetically predicted caffeine metabolizers.Significant sex disparity was observed(P value for interaction=0.013),in that coffee drinking was only associated with the risk reduction of CKD in females.Subgroup analysis revealed that testosterone and sex hormone-binding globulin(SHBG),but not estradiol,modified the coffee–CKD association.Conclusions:In addition to the overall inverse coffee–CKD association that was observed in the general population,we could also establish that a sex disparity existed,in that females were more likely to experience the benefit of the association.Testosterone and SHBG may partly account for the sex disparity.

To the Editor:Testicular tumor represents 1%of male neoplasms and 5%of urogenital tumors.Non-seminomatous germ cell tumor(NSGCT)accounts for nearly 40%of all testicular cancers.The survival of patients with NSGCT has been improved because of advanced multimodal therapies(orchiectomy,and if necessary,subsequent chemotherapy).[1]However,to the best of our knowledge,survival of NSGCT patients has not been studied in a large Chinese cohort yet.Therefore,this study assessed the clinical characteristics,treatments,and outcomes of patients with NSGCT.

Complete androgen blockade vs.medical castration alone as adjuvant androgen deprivation therapy for prostate cancer patients following radical prostatectomy:a retrospective cohort study

Background:Till date,the optimal treatment strategy for delivering adjuvant androgen deprivation therapy(ADT)in localized and locally advanced prostate cancer(PCa),as a lower stage in PCa progression compared with metastatic PCa,is still unclear.This study compares the efficacy of castration alone with complete androgen blockade(CAB)as adjuvant ADT in patients with localized and locally advanced PCa undergoing radical prostatectomy(RP).Methods:Patients diagnosed with PCa,without lymph node or distant metastasis,who received RP in west china Hospital between January 2009 and April 2019,were enrolled in this study.We performed survival,multivariable Cox proportional hazard regression,and subgroup analyses.Results:A total of 262 patients were enrolled,including 107 patients who received castration alone and 155 patients who received CAB.The survival analysis revealed that there was no significant difference between the two groups(hazard ratios[HR]=1.07,95%confidence intervals[95%CI]=0.60-1.90,P=0.8195).Moreover,the multivariable Cox model provided similarly negative results before and after adjustment for potential covariant.Similarly,there was no significant difference in the clinical recurrence between the two groups in both non-adjusted and adjusted models.Furthermore,our subgroup analysis showed that CAB achieved better biochemical recurrence(BCR)outcomes than medical castration alone as adjuvant ADT for locally advanced PCa(P for interaction=0.0247,HR=0.37,95%CI=0.14-1.00,P=0.0497).Conclusion:Combined androgen blockade achieved better BCR outcomes compared with medical castration alone as adjuvant ADT for locally advanced PCa without lymph node metastasis.

Association between antibiotic exposure and adverse outcomes of children and pregnant women: evidence from an umbrella review

Background Antibiotics are widely prescribed among children and pregnant women,but their safety profile is controversial.This study aimed to summarize and appraise current evidence for the potential impact of antibiotic exposure on pregnancy outcomes and children’s health.Methods PubMed,Embase,Web of Science and the Cochrane database of Systematic Reviews were searched from inception to June 2022.Meta-analyses of any study design comparing the impact of antibiotic exposure with nonexposure among children,pregnant women and prepregnant women on adverse health outcomes of children and pregnancy were retrieved.The quality of evidence was assessed by a Measurement Tool to Assess Systematic Reviews 2(AMSTAR2)and the Grading of Recommendations,Assessment,Development and Evaluation(GRADE).data were reanalyzed,and the credibility of the evidence was determined.Results Out of 2956 studies identified,19 articles with 39 associations were included.Totally 19 of the associations(48.72%)were statistically significant with a P value≤0.05,while only six were supported by highly suggestive evidence.Children with postnatal antibiotic exposure had a higher risk of developing asthma odds ratio(OR):1.95,95%confidence interval(CI):1.76–2.17,wheezing(OR:1.81,95%CI 1.65–1.97)and allergic rhinoconjunctivitis(OR:1.66,95%CI 1.51–1.83),with prediction intervals excluding the nulls.Quality assessed by both AMSTAR2 and GRADE of included meta-analyses were very low in general.Conclusions Antibiotic exposure in early life was associated with children’s long-term health,especially in cases of allergic diseases.Prenatal exposure might also influence children’s health in some aspects but requires more high-quality evidence.Potential adverse effects of antibiotics on pregnancy outcomes were not observed in our study.Studies with higher quality and better quantification of antibiotic exposure are needed in the future.

Oncological outcomes of patients with ductal adenocarcinoma of the prostate receiving radical prostatectomy or radiotherapy

Objective:To evaluate the oncological outcomes of ductal adenocarcinoma of the prostate(DAC)managed with radical prostatectomy(RP)or radiotherapy(RT)and optimize the proper treatment modality to DAC comprehensively.Methods:The cohorts included a total of 528 patients from the Surveillance,Epidemiology and End Results(SEER)database,354 receiving RP and 174 receiving RT.Cox proportional hazards regressions were performed to assess cancer specific mortality(CSM)and overall mortality(OM)between treatment groups.A competing risk analysis was further conducted.Subgroup analyses by age and level of prostate-specific antigen(PSA)were performed.Propensity score matching was implemented.Results:Patients managed with RP had lower risks of CSM and OM compared with RT(before matching:Hazard ratio[HR]=0.24,95%confidence interval[CI]0.13-0.47 and HR=0.26,95%CI 0.17-0.40,respectively;after matching:HR=0.18,95%CI 0.04-0.82 and HR=0.28,95%CI 0.11-0.70,accordingly).Subgroup analyses demonstrated that patients in the middle tertile of the age or with lower tertile PSA level managed with RP took lower risks of OM significantly(HR=0.18,95%CI 0.06-0.57,p<0.01 and HR=0.17,95%CI 0.06-0.54,p<0.01).Conclusion:Among patients with DAC,treatment with RP was associated with better survival outcomes in comparison with RT.Patients with DAC in the middle tertile of the age and with lower tertile PSA level benefited the most from RP.

2020 was an unprecedented year,with rapid and drastic changes in human mobility due to the COVID-19 pandemic.To understand the variation in commuting patterns among the Chinese population across stable and unstable periods,we used nationwide mobility data from 318 million mobile phone users in china to examine the extreme fluctuations of population movements in 2020,ranging from the Lunar New Year travel season(chunyun), to the exceptional calm of COVID-19 lockdown,and then to the recovery period.We observed that cross-city movements,which increased substantially in chunyun and then dropped sharply during the lockdown,are primarily dependent on travel distance and the socio-economic development of cities.Following the Lunar New Year holiday,national mobility remained low until mid-February,and COVID-19 interventions delayed more than 72.89 million people returning to large cities.Mobility network analysis revealed clusters of highly connected cities,conforming to the social-economic division of urban agglomerations in china.While the mass migration back to large cities was delayed,smaller cities connected more densely to form new clusters.During the recovery period after travel restrictions were lifted,the netflows of over 55% city pairs reversed in direction compared to before the lockdown.These findings offer the most comprehensive picture of Chinese mobility at fine resolution across various scenarios in china and are of critical importance for decision making regarding future public-health-emergency response,transportation planning and regional economic development,among others.
