Is Alzheimer’s Disease an Adaptability Disorder? What Role Does Happiness Have in Treatment, Management and prevention

A case presentation indicating the importance of “happiness” in childhood causing memory block until the patent presented with probable mixed vascular and neurodegenerative memory loss at 60 years of age is presented to highlight the role of emotional factors in causing the disease. The question of whether Alzheimer’s disease is an adaptability disorder is raised, given the patient blocked out her memory of her childhood experience. The importance of “happiness” as a treatment goal raises issues of advocacy and Guardianship as well as capacity, which is addressed by actual case reference and court action in defence of the patient’s rights to have their wishes respected and observed. Functional mental capacity assessment, using the Functional Mental state Measure (FMsM) gives a greater indication of neuronal reserve than standard cognitive testing, as it helps to unravel the dilemma associated with pure cognitive assessment in Alzheimer’s Disease as well as vascular dementia patients and patients who, despite retained and intact functional capacity and ability to express their wishes, i.e. “best interest”, are “wrongly” placed under Guardianship. Maladaptive responses, to control the change in external environment that are sensed or perceived, and which lead to disorder or to susceptibility to disease, exemplify a General systems Theory approach, in which appropriate and adequate responses to environmental change, in behavioural terms, by a person, whether independently, or as the recipient or giver, or both, leads to functional interaction and happiness.

Medical Ethics: Context Is the Key Word

Medical Ethics is no different to Ethics, but relates specifically to the doctor-patient relationship. This relationship is based on mutual trust—trust in the doctor’s expertise and knowledge and trust that the patient is consulting the doctor, for his/her/their own health, wellbeing and welfare, without agenda or bias. In other words, the basis of the doctor-patient relationship is one of mutual trust and confidence. Thus, what is imparted between them is held in the strictest confidence. The doctor must act within the law. No third party influence can be brought to bear on this relationship without the patient’s consent. In the case of minors the relationship is between the child’s guardians as well as the doctor who must act within the law. In the category of “elderly” or adult guardianship or advocacy, those appointed and in positions of trust must take the person’s wishes into account if the patient has capacity to make decisions or has indicated their wish prior to any such appointment. Capacity is defined as consistent belief or wish, awareness of what one does not wish for, satisfaction when wishes are respected and fulfilled and the opposite when they are not. Ethical practice is determined by taking context into account in decision-making and ensuring the sanctity of the patient’s rights and wishes. Abuse is just the opposite, where actions taken or not taken are against a person’s wishes. personal wish determines best interest. Best interest may also be defined in terms of social functioning and psychological wellbeing. Discussion of ethics in terms of General systems Theory is also addressed, and affects an action on the environment or others, i.e. ethics in the wider medical context may pertain if the person has a contagious disease and a period of quarantine or “isolation” or barrier” nursing is required, and where precedent is given to society’s best interest over the person’s wishes, as part of an educational process and as required for strategic dis

A cellphone-based colorimetric multi-channel sensor for water environmental monitoring

The development of colorimetric analysis technologies for the commercial cellphone platform has attracted great attention in environmental monitoring due to the low cost,high versatility,easy miniaturization,and widespread ownership of *** work demonstrates a cellphone-based colorimetric multi-channel sensor for quantifying multiple environmental contaminants simultaneously with high sensitivity and *** improve the sensitivity of the sensor,a delicate optical path system was created by using a diffraction grating to split six white beams transmitting through the multiple colored samples,which allows the cellphone CMOs camera to capture the diffracted light for image *** proposed sensor is a universal colorimetric detection platform for a variety of environmental contaminants with the colorimetry assay in the range of 400–700 *** introducing the diffraction grating for splitting light,the sensitivity was improved by over six folds compared with a system that directly photographed transmitted *** a successful proof-of-concept,the sensor was used to detect turbidity,orthophosphate,ammonia nitrogen and three heavy metals simultaneously with high sensitivity(turbidity:detection limit of 1.3 NTU,linear range of 5–400 NTU;ammonia nitrogen:0.014 mg/l,0.05–5 mg/l;orthophosphate:0.028 mg/l,0.1–10 mg/l;Cr(VI):0.0069 mg/l,0.01–0.5 mg/l;Fe:0.025 mg/l,0.1–2 mg/l;Zn:0.032 mg/l,0.05–2 mg/l)and reliability(relative standard deviations of six parallel measurements of 0.37%–1.60%and recoveries of 95.5%–106.0%in surface water).The miniature sensor demonstrated in-field sensing ability in environmental monitoring,which can be extended to point-of-care diagnosis and food safety control.

Cross-layer Adaptive Resource Allocation Algorithm with Diverse Qos Requirements for single-Cell OFDMA systems

The orthogonal frequency division multiple access( OFDMA) based communication system has been considered as the main trend of next-Generation communication system. But the existing resource allocation algorithm designed for such system is always with high complexity thus hard to be realized. To solve such problem with the constraints of spectrum efficiency and buffer state,a novel cross-layer resource allocation algorithm( RAA) is proposed in this paper. The goal of our RAA is to maximize the system throughput while satisfying several practical constraints,such as fairness among services,head of line( Ho l) delay and diverse quality of service( Qo s) requirements. Due to these constraints,finding the optimal solution becomes a Nphard problem. Therefore in this paper a novel method to solve such problem with acceptable complexity is proposed within following steps: firstly,based on the link state we formulate the ideal subchannel allocation strategy as a convex optimization problem,which can be efficiently solved by our proposed lagrange multiplier technique subchannel allocation( lMTsA) algorithm; secondly,according to the obtained channel allocation matrix,a power allocation algorithm based on the water-filling power allocation( WpA) idea is deployed to get the optimal power allocation matrix combining with adaptive modulation and coding( AMC); finally,through a greedy algorithm,the ultimate subchannel and power allocation matrix can be obtained based on iterative method. The simulation results illustrate that we can achieve the higher throughput and better Qo s performance than the widely-used maximum throughput( MT) algorithm and round robin( RR) algorithm.

Ultrasensitive detection of micro RNA-21 in human serum based on the confinement effect enhanced chemical etching of gold nanorods

Natural enzymes,such as horseradish peroxidase(HRp),are a class of important biocatalysts with the high specificity,but their catalytic efficiency is usually ***,the higher catalytic efficiency induced by the confinement effect is promising in optical sensing *** this work,a dark-field light scattering sensing platform was fabricated by the confinement effect of HRp from hybridization chain reaction(HCR)and then released to solution by the toehold-mediated strand displacement reaction(TsDR).Then,HRp catalyzed the 3,3,5,5-tetramethylbenzidine(TMB)to TMB^(2+)with the assistance of hydrogen peroxide,which etched the gold nanorods(Au NRs)with the weakened light *** single-particle assay was established based on the decreased light scattering intensity of AuNRs under dark-field *** proposed assay revealed excellent analytical performance within a linear range from 25 pmol/l to 600 pmol/l,and a low limit of detection of 3.12 pmol/***,it also manifested satisfactory recovery of mi RNA-21 in human serum *** high sensitivity,excellent specificity,and universal applicability make this sensing platform promising for disease diagnosis.

New algorithm for hub-and-spoke topological virtual networks embedding problem

The virtual network embedding/mapping problem is a core issue of the network virtualization. It＇s mainly concerned with how to map virtual network requests to the substrate network efficiently. previous researches mainly focus on developing heuristic algorithms for general topology virtual network request. In this paper, we propose a new algorithm for the hub-and-spoke （HAs） topology virtual request. The characters of the HAs topology are exploited to develop the new algorithm. The simulation results show that the new algorithm greatly enhances the performance of revenue/cost （R/C） ratio while the HAs topology request arrives.

Timing Advanced Estimation Algorithm of low Complexity Based on DFT spectrum Analysis for satellite system

In satellite mobile communication system, relative movement of the satellite and the terminal will cause a large Doppler offset. Timing advanced estimation with Zadoff-Chu sequence is sensitive to the frequency offset. When the frequency offset is larger than one times subcarrier spacing, the value of peak cannot be detected at the receiving end. To suppress the larger Doppler frequency shift, this paper proposes a novel timing advanced estimation scheme(TAE-MCD) for satellite communication system. In this algorithm, t r a n s m i t t e d s i g n a l i s d i v i d e d i n t o Z C sequence and its conjugate sequence. Using multiplication and DFT operation to find the estimated peak at the receiving end, and make subtraction with the obtained sequences at last. The scheme can not only inhibit the adverse effects of large Doppler frequency shift in timing estimation effectively, but also reduce the computational complexity at the receiving end and improve the work efficiency of the hardware. simulations results show that TAEMCD outperform the existing timing advanced estimation methods, on the condition of no additional time and frequency resource are needed.

Combination immunotherapy has shown promising potential for enhancing the objective response rate compared to immune checkpoint blockade(ICB)***,combination therapy with multi-drugs is limited by the different properties of the agents and inconsistent synergistic targeted ***,based on a universal triterpene template and the anticancer active agent ursolic acid(UA),a cytomembrane-coated biomimetic delivery nanoplatform(UR@M)prepared by the selfassembly of a pD-l1 targeted CRIspR/Cas9 system and UA was designed for hepatocellular carcinoma(HCC)***@M showed enhanced tumor accumulation in vivo with homologous tumor targeting,and CRIspR in the nanosystem exhibited potent gene-editing efficiency of 76.53% in vitro and 62.42% in vivo with no off-target *** activated the natural immune system through the TlR-2-MyD88-TRAF6 pathway,which synergistically enhanced the proliferation of natural killer cells and dendritic cells and realized excellent immune cytotoxic T cell infiltration by combining with the ICB of *** strategy of work along both lines based on innate immune and adaptive immunity displayed a significant effect in tumor ***,the UA-templated strategy“killed three birds with one stone”by establishing a self-assembly nanosystem,inducing tumor cell death,and promoting synergistic immunostimulation for HCC treatment.
