Epidemiology and Clinical Features of Mycobacterium avium Complex Pulmonary Disease in a Brazilian Amazon Cohort

The incidence of Mycobacterium avium complex pulmonary disease (MACPD) has been globally increasing. The present study aims to provide a comprehensive analysis of clinical characteristics and treatment response among patients in the Amazon region of Brazil and conduct a retrospective cohort study at a prominent referral hospital in the State of Par, Brazil, from 2012 to 2020. The M. avium group represented 58.9% of cases, followed by M. intracellulare (35.7%), M. colombiense (3.6%), and M. chimaera (1.8%). The majority of patients (73.2%) were female and with an average age of 63 years. Primary clinical manifestations included persistent cough and sputum production. The Charlson comorbidity index (CCI) averaged 2.47, with 36.3% of patients having a CCI score equal to higher than 3. Notably, over 70% of patients had received treatment for pulmonary tuberculosis before initiating treatment for MAC infection. The fibrocavitary radiologic form was the most prevalent (55.4%), frequently exhibiting a bilateral distribution (53.6%). Antimicrobial susceptibility revealed a significant prevalence of MAC resistance to drugs in the therapeutic regimen. Despite this, most of the patients experienced clinical improvement (50%). In conclusion, this study highlights a higher prevalence of MAC infections among middle-aged women, with a history of pulmonary tuberculosis treatment and presenting the fibrocavitary radiological form was predominant.

In an attempt to improve immune responses and protective efficacy,we constructed tworecombinant bacille Calmette-Guérin(rBCG)strains expressing an 85B antigen(Ag85B)and early secretedantigenic target-6 kDa antigen(ESAT6)of M vcobacterium tuberculosis(MTB)fusion *** rBCGstrains have the same protein insertion but in a different order(Ag85B-ESAT6 and ESAT6-Ag85B).Thecultured supernatant of rBCG strains and the sera from the mice immunized with the fusion protein Ag85B-ESAT6 or ESAT6-Ag85B formed a band with a fraction size of 37 kDa,equalivalent to the sum of Ag85B *** weeks after BALB/c mice were immunized with BCG or rBCG,spleen lymphocytes showedsignificant proliferation in response to culture filtrate protein of *** with the BCG group,micevaccinated with rBCG elicited a high level increase of immunoglobulin G antibodies to culture filtrate proteinin the *** γ-interferon levels in the lymphocyte culture medium supernatants increased remarkably inthe rBCG1 group,significantly higher than that of the BCG immunized group(Pgroups(P>0.05).

Expression and significance of cyclooxygenase-2 in human lung cancer

Objective: We aimed to examine the expression of cyclooxygenase-2 and the relationship with the pathological types, TNM stage, lymph node metastasis, the degree of differentiation, smoking and the survival. Methods: Immunohistochemical staining method was used to examine the expression of cyclooxygenase-2 of 121 cases of lung cancer and three control groups. The data were statistically analyzed. Results: Compared with the health group, cyclooxygenase-2 was over expressed in the inflammatory tissue(P = 0.036), lung adenocarcinoma(P = 0.005) and squamous carcinoma(P = 0.047). Compared with patients without lymph node metastasis, cyclooxygenase-2 was over expressed(P = 0.033) in patients with lymph node metastasis. Compared with high differentiation group, cyclooxygenase-2 was over expressed(P = 0.004) in low differentiation group. Compared with non-smokers, the expression of cyclooxygenase-2 increased in smokers(P = 0.000). The median survival time of patients that the expression of cyclooxygenase-2 were negative was 9 months(95% CI, 5.6–12.4 months). The median survival time of patients that the expression of cyclooxygenase-2 were positive was 5 months(95% CI, 3–7 months). They was statistical difference(P = 0.001). Conclusion: Overexpression of cyclooxygenase-2 is associated with pathological types, TNM stage, lymph node metastasis, degrees of differentiation, smoking and prognosis in lung cancer.

Purpose Mixed infection is considered to play an important role in facilitating the stable coexistence of different strains,in altering treatment outcomes and in undermining the effectiveness of tuberculosis(TB) control *** study was to the first time to analyze influencing factors of mixed infection of TB in *** Between July 2015 and November 2020,64 children with 137 multiple specimens were included in the department of infection,children’s hospital of Chongqing medical university,***-genome sequencing(WGS) was developed to identify drug resistance patterns,genotype,and to group *** epidemiological and clinical data for each patient was extracted to analyze influencing factors of children with mixed infection of ***:Nine patients(14.1%) with 18 strains were included in the mixed infection group,and 55 patients(85.9%) with 110 strains were included in the nonmixed infection group according to genetic distance identified by *** addition,8 patients with 18 strains were microevolution in nonmixed infection *** statistical significance in the distribution of specimen source was found between the mixed infection group,microevolution group and nonmixed infection group(p>0.05).The distribution of drug-resistance patterns in the mixed infection and nonmixed infection groups was not different(p>0.05).The distribution of lineage 2 and lineage 4 genotypes in the mixed infection and nonmixed infection groups was statistically different(p=0.01).Lineage 4 and lineage 4.2.2 were found to be risk factors for *** mixed infection in children(OR=3.74 vs.10.08).Conclusion The incidence rate of mixed infection in children was nearly the same as that in *** microevolution within the host might not be related to the environment of infected organs in ***,mixed infection of *** might not lead to a poor prognosis in children with primary TB.

Molecular Epidemiology of <i>Mycobacterium tuberculosis</i>among Pulmonary tuberculosis Patients in Ilorin, Nigeria

Background: tuberculosis (TB) is an infectious disease and it is a public health concern due to the endemic nature of the disease. Nigeria’s health systems still rely heavily on out-dated tuberculosis diagnostic tests;including sputum smear microscopy and chest radiography. This necessitated the urgent need to explore a more reliable, effective method that can rapidly enhance simultaneous detection of Mycobacterium tuberculosis (MTB) and rifampicin (RIF) resistance. Aims: We evaluated the prevalence of MTB and rifampicin resistance and the influence of socio-demographic factors. Methods: A longitudinal, descriptive study that employs the Xpert MTB/RIF automated molecular method for rapid detection of rifampicin resistance. Secondary data were collected from the medical record from March 2017 to December 2018. Primary data were collected by direct clinical observations and analysis with gene expert machine from January 2019-March 2019. Results: Study populations were 1893 subjects pooled as secondary data at the Gene Expert laboratory, with a mean age of 44.92 years and were categorized based on sex, 1125 (59.4%) males and 768 (40.6%) females respectively. The MTB detected were 117 (15.7%) for males while 76 (9.9%) were positive for females. 948 (84.3%) were negative for males and 692 (90.1%) negative for females. Also, the categories for age were grouped into three but the older age groups (29 - 54) and (55 - 80) were more vulnerable to MTB having a p = 0.506 which is statistically non-significant;only 14 (5.5%) have RIF while 239 (94.5%) do not have RIF resistance strains. Logistic regression analysis of MTB detected showed a significant association (p 0.05) for age. Pulmonary tuberculosis in HIV/AIDS Patients Attending Art Clinic in Bududa General Hospital, Bududa District, Uganda

Background: tuberculosis disease affects survival among HIV co-infected patients on antiretroviral therapy. Prevalence of pulmonary tuberculosis in western Uganda is 15.3% and 7.2% in Tororo, Eastern Uganda. A study in Kampala revealed tuberculosis prevalence among immuno-incompetent group at 10%. To assess the prevalence of Pulmonary tuberculosis (PTB) infection in HIV/AIDS patients attending ART clinic in Bududa general hospital, Bududa district, a hospital-based, cross-sectional study was carried out. Methods and materials: Two hundred thirty eight (238) HIV/AIDS positive patients attending ART clinic were recruited using consecutive simple non-probability sampling on consenting. Data collection guide was used to capture the socio-demographic information from study participants and two sputum samples (spot and early morning samples) were obtained from each participant, and processed using Hot Ziehl Neelsen for Tubercle Bacilli. Data collected was entered into MS Excel spreadsheets and Statistical Package of Social Sciences was used for descriptive data analysis and outputs in form of percentages, figures and tables presented. Results: Out of 238 study participants recruited, 14/238 were positive for Acid Fast Bacilli (AFB);giving a prevalence of pulmonary tuberculosis at 5.9% (14/238) and the most affected age group was between 26 - 35 years with the prevalence of 3.4%. More females (65.6%) participated in the study than males (34.4%). Conclusion: tuberculosis remains a health challenge in HIV/AIDS positive people in Bududa district and there is need for early screening of all HIV patients for TB as part of their routine and intensification to follow up TB positive patients. We recommend large-scale studies on the trends in TB/HIV co-infection and associated factors should be carried out in this area. In addition, we recommend intensification of public awareness campaign about TB infection in relation to its transmission, prevention and control.

Application of Cystatin C Combined with Homocysteine Detection in AIDS and tuberculosis Complicated with Hypertension

Objective: To investigate the application of cystatin C combined with homocysteine detection in AIDS and tuberculosis complicated with hypertension. Methods: 57 patients with AIDS complicated with hypertension and 52 patients with tuberculosis complicated with hypertension from Guangxi Infectious Diseases Hospital Nanning Fourth People’s Hospital/Guangxi AIDS Clinical Treatment Center (Nanning) from October 2022 to March 2023, and 196 patients with simple hypertension from Guangxi Cardiovascular Diseases Hospital Nanning Third People’s Hospital were selected as research objects. And then the difference in the detection results of cystatin C and homocysteine among the three groups was compared. Results: The detection results of serum cystatin C and homocysteine in AIDS patients with hypertension and tuberculosis patients with hypertension were higher than those in the simple hypertension group, and the difference was statistically significant (P group and the tuberculosis hypertension group (P > 0.05). Conclusion: The detection of cystatin C combined with homocysteine has high clinical application value in AIDS with hypertension and tuberculosis with hypertension. When AIDS is combined with hypertension or tuberculosis is combined with hypertension, cystatin C and homocysteine are at a high level, while the concentration levels of cystatin C and homocysteine are relatively low in simple hypertension. Therefore, cystatin C combined with homocysteine detection can provide better laboratory evidence for clinical diagnosis and differential diagnosis, and is worth promoting and applying.
