Clinical outcomes with a new design in multifocal intraocular lens:a pilot study

Purpose:To evaluate the clinical and visual outcomes,quality of near vision and the influence of photic phenomena in patients bilaterally implanted with a new Precizon Presbyopic multifocal intraocular lens(IOL).Methods:In this prospective consecutive case series,20 eyes of 10 patients were included(mean age 63.80±12.55 years).Uncorrected and corrected visual acuity(far,intermediate and near),subjective refraction,binocular defocus curve,contrast sensitivity(CSV-1000)and quality of vision and satisfaction questionnaires were *** follow-up was 12 months after ***:At 12 months after surgery,uncorrected distance visual acuity(UDVA)improved with surgery(p=0.001)with a value of 0.08±0.08 *** near visual acuity(UNVA)was 0.22±0.12 logMAR and distance corrected near visual acuity(DCNVA)was 0.16±0.13 *** distance visual acuity(UIVA)was 0.22±0.10 *** sensitivity outcomes were similar to normal population in photopic conditions and slightly reduced in mesopic conditions of *** curve showed that this multifocal IOL was able to provide a visual acuity(VA)equal or better to 0.16 logMAR between defocus levels of+1.00 to−2.50 *** patient satisfaction was obtained in quality of vision and satisfaction questionnaires ***:The Precizon Presbyopic NVA IOL(OPHTEC BV)provides good visual *** multifocal IOL provides a high percentage of spectacle independence due to good VA at far,intermediate and near distances and satisfactory contrast *** patient satisfaction was observed in quality of vision and satisfaction questionnaires with a low percentage of patients manifesting photic phenomena.

Keratoconus is a corneal degeneration that usually appears during puberty and may seriously deteriorate the quality of life of the *** corneal disease is today the first indication of corneal transplantation in young *** the last decade of the XX century,keratoplasty procedures were the only alternative to treat this pathological *** the beginning of the XXI century,intracorneal ring segments implantation was proposed as a therapeutic choice for treating keratoconus *** then,several published articles have reported the benefits of this surgical procedure in treating this type of corneal ectatic *** purpose of the present investigative work is to summarize the characteristic of the intracorneal ring segments and also to review the different features published in the literature in relation to this surgical technique for the treatment of keratoconus patients.

After nearly three decades of innovation in excimer laser,today we are presented with a state of the art generation targeting minimally invasive refractive surgery with high speed laser,faster trackers,pupil monitoring systems and better customization *** systems are capable of delivering better treatments with less induced postoperative high order *** results reported by many authors had confirmed the superiority in efficiency and safety profiles of this generation compared to previous ***,current technology is facing major challenges in the correction of high hyperopic errors and in presbyopic treatments,with upgrades in ablation centration and thermal control needed,which will ensure better biomechanical results,as a step closer to perfection in refractive surgery.

Background:To evaluate the refractive outcomes and the optical performance as well as the quality of life in patients implanted with a new diffractive multifocal intraocular lens(IOL).Methods:Prospective,clinical study including 41 cases of patients who underwent cataract surgery and were divided in two groups:group 1,including 20 eyes implanted with the multifocal IOL SeeLens MF(Hanita Lenses,Israel);group 2,21 eyes implanted with the Acrysof SA60AT *** acuity,defocus curve,intraocular aberrations,contrast sensitivity function and quality of life were assessed during a follow up period of 6 ***:Significant improvement was observed in the uncorrected distance visual acuity(UDVA)and corrected distance visual acuity(CDVA)in both groups(p<0.02).The multifocal group showed better results in terms of uncorrected near visual acuity(UNVA)and distance-corrected near visual acuity(DCNVA)(p<0.01).Comparison of both groups showed better visual acuities for the multifocal IOL group in defocus levels from-3.0 D to-1.50 D(p≤0.01).At 6 months,there was a significant reduction of the internal higher order aberrations(p≤0.04).A significant increase in scotopic contrast sensitivity was detected for 6 cycles/°spatial frequency during follow up(p=0.04),but no significant changes were observed for the rest of spatial frequencies(p≥0.06).Visual Functioning Index(VF-14)questionnaire showed that patients reported high levels of satisfaction when performing daily ***:The SeeLens MF IOL is able to successfully restore distance,near and intermediate visions after cataract *** also provides functional intermediate vision with optimal intraocular optical quality.

Patients’dissatisfaction with multifocal intraocular lenses managed by exchange with other multifocal lenses of diferent optical profles

Background:The aim of the study was to evaluate the outcomes of dissatisfed patients reporting poor visual quality following implantation of multifocal intraocular lenses(MF-IOLs),managed by IOL exchange with another multifocal optical ***:This is a retrospective series of ***-IOL exchange was done in 15 dissatisfed patients(30 eyes)with the perception of poor visual quality for far distance afected by neuroadaptation *** underwent a bilateral exchange of a MF-IOL with another MF-IOL of a diferent optical *** outcomes and complications were *** including Quality of Vision(QoV),Visual Function Index-14(VF-14)and its Rasch-revised version(VF-8R)and a satisfaction questionnaire were also used for outcome ***:The mean elapsed time from implantation to explantation-reimplantation was 11.8 *** QoV scores improved signifcantly across all the three *** function improved with a change in VF-14 score from 60.41±24.81 to 90.16±10.91(P<0.001).The VF-8R score improved as *** uncorrected distance visual acuity improved from 0.24 to 0.12 logMAR after exchange(P<0.001)and corrected distance visual acuity improved from 0.15 to 0.04 logMAR(P<0.001).Safety and efcacy indexes reached 1.46 and 1.16,*** patients’satisfaction following MF-IOL exchange,80%of the patients reported they would have the MF-IOL reimplantation procedure ***:Patient dissatisfaction with neuroadaptation failure following MF-IOL implantation can be managed in 80%of our cases by MF-IOL exchange with a diferent MF-IOL optical profle.

Refractive errors are frequently found following cataract surgery and refractive lens *** biometric analysis,selection and calculation of the adequate intraocular lens(IOL)and modern techniques for cataract surgery all contribute to achieving the goal of cataract surgery as a refractive procedure with no refractive ***,in spite of all these advances,residual refractive error still occasionally occurs after cataract surgery and laser in situ keratomileusis(LASIK)can be considered the most accurate method for its ***-based procedures,such as IOL exchange or piggyback lens implantation are also possible alternatives especially in cases with extreme ametropia,corneal abnormalities,or in situations where excimer laser is *** our review,we have found that piggyback IOL is safer and more accurate than IOL *** aim is to provide a review of the recent literature regarding target refraction and residual refractive error in cataract surgery.

Artificial neural network to guide intracorneal ring segments implantation for keratoconus treatment:a pilot study

Background:To analyze the clinical results of an artificial neural network(ANN)that has been processed in order to improve the predictability of intracorneal ring segments(ICRS)implantation in ***:This retrospective,comparative,nonrandomized,pilot,clinical study included a cohort of 20 keratoconic eyes implanted with intracorneal ring segments KeraRing(Mediphacos,Belo Horizonte,Brazil)using the ANN(ANN group)and 20 keratoconic eyes implanted with KeraRing using the manufacturer’s nomograms(nomogram group).Uncorrected distance visual acuity(UDVA),corrected distance visual acuity(CDVA)(visual acuity is expressed in decimal value and in LogMAR value in brackets),manifest refraction,corneal topography,tomography,aberrometry,pachymetry and volume analysis(Sirius ***,Firenze,Italy)were performed during the preoperative visit;and the two groups,ANN group and nomogram group,did not differ significantly preoperatively in all of the parameters *** preoperative values were compared with the results obtained at the third-month ***-Whitney test and Wilcoxon test were used for the statistical ***:The spherical equivalent and the keratometric values decreased significantly in both *** CDVA improved from 0.60±0.23(0.22 LogMAR)pre-operatively to 0.73±0.21(0.14 LogMAR)post-operatively in the ANN group(p<0.005),and from 0.54±0.19(0.27 LogMAR)pre-operatively to 0.62±0.19(0.21 LogMAR)post-operatively in the nomogram group(p<0.01),with statistically significant difference between the two groups(p<0.05),being better in the ANN ***-like aberrations decreased significantly in the ANN group,while in the nomogram group they did not change significantly,but no statistically significant difference was found between the two ***:ANN to guide ICRS provides an increase in the visual acuity,reduction in the spherical equivalent and improvement in the optical quality of keratoconus *** gives better results wh

Cellular therapy of the corneal stroma,with either ocular or extraocular stem cells,has been gaining a lot of interest over the last *** publications from different research groups are showing its potential benefits in relation to its capacity to improve or alleviate corneal scars,improve corneal transparency in metabolic diseases by enhancing the catabolism of the accumulated molecules,generate new organized collagen within the host stroma,and its immunosuppressive and immunomodulatory *** extraocular stem cells do not require a healthy contralateral eye and they do not involve any ophthalmic procedures for their *** stem cells have been the most widely assayed and have the best potential to differentiate into functional adult keratocytes in vivo and in *** embryonic stem cells have been partially abandoned due to ethical implications,the discovery of the induced pluripotent stem cells(iPSC)has opened a new and very promising field for future research as they are pluripotent cells with the capacity to theoretically differentiate into any cell type,with the special advantage that they are obtained from adult differentiated *** delivery into the corneal stroma has been experimentally assayed in vivo in multiple ways:systemic versus local injections with or without a *** preliminary human clinical data is already available although still very limited,and further research is necessary in order to consolidate the clinical applications of this novel therapeutic line.

Refractive surgery today: is there innovation or stagnation?

Refractive surgery,both corneal and intraocular,has undergone an explosive evolution during the last 25 *** first excimer laser surgery performed now already over 25 years ago is a historical landmark which started the scientific age of corneal refractive *** that momentous occasion,refractive surgery was a buccaneer procedure that lacked consistent scientific knowledge about the long-term outcomes of the procedures that were practiced(mainly radial and astigmatic keratotomy).In other words,there was a complete lack of objective outcome analysis coupled with almost no perspective on what the future development of this subspecialty was going to hold[1].A classic example of this is Radial Keratotomy,which was extensively performed in Europe,the US and Latin America at the end of the 80’s and early 90’*** the PERK study was able to detect and announce the trend towards hyperopic flattening of these corneas years later when they approached the technique in a systematic way by studying a relatively small number of cases[2-4].
