Traumatic rupture of the right diaphragmatic dome with herniation of a segment of the hail into the thorax is a rare lesion. It is often the result of a thoraco-abdominal trauma. It can generally lead to early or late cardiopulmonary complications due to compression. The objective of this clinical case is to study the physiopathological mechanisms, the diagnostic and therapeutic modalities of this complication. The diagnosis is often difficult in front of a diaphragmatic rupture since there are no specific clinical signs. In our case, the clinical picture on arrival was that of a high occlusion in an immediate postoperative context. The X-ray which makes it possible to visualize the ascended organs but more difficultly the rupture itself could not be carried out. Treatment is essentially surgical. Although the thoracic approach is preferred by several surgeons because of the difficulties of exposing the diaphragm in the presence of the liver, we chose the abdominal approach instead. The postoperative course is made up of parietal suppuration.

Objective: We initiated this work with the aim of studying the contribution of imaging in the diagnosis of acute intestinal obstruction at CIMED. Patients and methods: This was a prospective, descriptive and analytical study involving 96 patients collected at the radiology and medical imaging department of CIMED, from January 2022 to January 2023. Result: The age of our patients varied between 11 and 86 years with an average age of 36 years. There was a male predominance of 64.6% compared to 35.4% for women, i.e. a sex ratio of 1.82. The notion of previous surgery was found in 61.5% of our patients. Pain was present in all patients. Radiography of the ASP was performed in 89.6% of patients. It showed hydro-aerial levels in 96.5% of patients. Abdominopelvic CT was performed in 12 patients and made it possible to make the diagnosis of occlusion in all patients. The results of the positive diagnosis were concordant with those intraoperatively in 92% of cases. 8% of our patients, compared to the treatment, spontaneously resumed their transit, 91% benefited from surgical treatment and 1% died before surgery. The outcome was favorable in 80 patients or 83.3%, poor with death in 16 patients or 16.7% of cases. Conclusion: Acute intestinal obstruction remains a serious pathology for which ASP radiography often remains the only radiological examination performed urgently. However, abdominopelvic CT seems widely indicated thanks to its contribution both for the positive diagnosis and for the serious and etiological diagnoses. However, this imaging technique is largely underused in our practice due to its high cost and lack of availability.

Introduction: Innovations in surgical and anaesthetic techniques to provide a good maternal and child safety have made the cesarean section a routine intervention in obstetrics. However, its complications, especially the short or long term per and postoperative maternal ones, are not exceptional and can affect mothers’ vital prognosis. The objectives of this study were to describe the maternal per and postoperative complications encountered during the cesarean section. Patients and method: This is a prospective, descriptive and analytical study carried out at the maternity ward of kankan regional hospital over a 6 month period from January 1, 2018 to June 30, 2018. Results: During this period, out of a total of 2229 deliveries, 319 caesarean sections were performed i.e. a rate of 12.51%. Complications concerned 111 patients (34.79%). The average age was 28 with extremes of 15 and 45. Non-medical transportation concerned 71.17% of our patients. The cesarean section was performed in emergency situations in 82.8% of cases. Indications were dominated by the fetopelvic disproportion. There was a statistically significant relationship between labour duration and the occurrence of complications. Intraoperative complications were dominated by bleeding (25.22% of all complications and 8.77% of total cesarean sections). Post-operative complications were dominated by parietal suppurations (94.49% of all complications and 34.79% of the total number of cesarean sections). In most cases, patients simultaneously developed several complications. Conclusion: The maternal complications of cesarean sections remain considerable. If the increase in the rate of caesarean sections has contributed to the improvement of the mother-fetal prognosis, the surgical procedure itself is not without complications, which encourages us to review its indications for a better management. Introduction: Cataract surgery has undergone many changes with the size of incision progressively decreasing over time with an incision of 12.0 mm for intracapsular cataract extraction to 2.2 - 2.8 mm in phacoemulsification. However, phacoemulsification due to high cost and equipment maintenance cannot be employed widely in developing countries. The phacoalternative or Manual small-incision cataract surgery (MSICS) offers similar advantages with the merits of wider applicability, less time consuming, a shorter learning curve, and lower cost. Haven’t not being without complications like any other surgery We have identified the factors influencing the outcome of phacoalternative cataract surgery in order to improve our quality of care for our patients suffering from blindness induced by the world’s first leading cause of legal blindness. We have identified the factors influencing the outcome of cataract surgery. Patients and Methods: This was a prospective observational study of the descriptive type lasting six (6) months from March 1 to August 30, 2020 including all patients operated on for cataracts and having lower visual acuity at 3/10. The operating form included demographic data, the patient’s personal ophthalmological history, postoperative visual acuity, per and postoperative complications and the type of pathology involved. The analysis was carried out using epi-info 7.***.0.1 software. Results: During this study period, we collected 61 cases of failure of cataract surgery out of a total of 1182 operated eyes, i.e. a frequency of 5.16%. Women represented more than half of the sample with 74%. Almost all of our patients, i.e. 96.72%, were over the age of 60. Loss of visual acuity was the main complaint in all our patients, i.e. 100% followed by photophobia with 24.4% of cases. In this retrospective study, conducted over 7 years (2009-2016) at the ENT and Head and Neck surgery Department of the Donka National hospital, we report 2 cases of cervicofacial cystic lymphangiomas. They were a 28-month-old girl and a 2-year-old boy. The symptomatology was noted after their birth. Dyspnea and dysphagia were found in the boy. They had a satisfactory general condition. ENT examination noted a cystic-like tumor syndrome. Imaging showed evidence of a cystic lymphangioma of the cervicofacial region. Pathological examination confirmed the diagnosis. All patients underwent exeresis cervicotomy. We found adhesion of the lymphangioma cyst to the internal jugular vein in the children. The boy presented a paralysis of the chin branch of the facial nerve after the surgery. We did not find any tumor recurrence. However, cystic cervicofacial lymphangiomas are a particular aspect of surgical pathology in children in Africa. In spite of the advent of sclerosing products, surgery remains for us the treatment of choice.

Hypertensive Retinopathy in Guinean Hypertensive Women: Epidemiological, Clinical and Therapeutic Aspects

Introduction: Hypertensive retinopathy is the set of retinal and papillary manifestations secondary to acute or chronic acute or chronic high blood pressure. Its frequency increases with age in women, its treatment requires the balance of blood pressure figures. Purpose: To study in women the epidemiological, clinical and therapeutic aspects of Hypertensive retinopathy at the application center of the Diploma of Specialized Studies in Ophthalmology (CADES/O). Patients and Methods: Prospective study conducted on 92 women who met the inclusion criteria over a period of six (6) months, from October 1, 2021 to March 31, 2022 in whom the diagnosis of Hypertensive Retinopathy was retained according to the Kirkendall classification. Results: We collected 92 patients with an average age of 57.88 years. The hypertensive Retinopathy was present at 61.75%. The age group of 60 - 69 years was the most represented at 40.22%. The socio-professional stratum most affected was housewives. The duration of the evolution of high blood pressure was from 2 - 5 years representing 57.61% of our study cases;the most common grade of high blood pressure was Grade II at 60.87%. Treatment of hypertension was irregularly followed in 73.91%. Stage II according to Kirkendall’s classification was 58.70% common;the recurrent sign was retinal hemorrhage at 30.43%. Blood pressure balance was the treatment recommended to our patients at 80.48%. Conclusion: Hypertensive retinopathy is common in women and its management remains the balance of blood pressure figures with periodic and multidisciplinary monitoring.

Introduction: Otorhinolaryngology (ENT) diseases in patients living with Human Immunodeficiency Virus/Acquired Immunodeficiency Syndrome (HIV/ AIDS) are relatively common and of concern to ENT specialists and other health professionals. Aim: It was to describe the epidemiological and diagnostic aspects of ENT and cervicofacial diseases in patients living with HIV/ AIDS. Material and Methods: This was a descriptive study with prospective data collection, conducted over a period of six months (September 16, 2019 to March 16, 2020), carried out at the ENT and Infectious Diseases Departments of the Donka National hospital. Results: Of 522 HIV-positive patients, 208 (39.8%) presented with ENT diseases. Women represented 62.5% with a sex ratio of 0.6. The mean age was 42.21 years with extremes of 16 and 64 years. Housewives were the most represented (29.8%). Married people were the most affected (76.4%). Cervical involvement was present in 7.7% of patients. Otologic involvement accounted for 43.7%. Oral cavity and pharyngolaryngeal involvement accounted for 50.9%. Nasosinus involvement accounted for 55.7%. HIV type I was the most common (99.5%). Conclusion: Otorhinolaryngology diseases were frequent in HIV positive patients. They may constitute the first reason for consultation. However, these ENT diseases remain in appendix of the opportunistic diseases of HIV/AIDS taking the front stage.

Ocular Manifestations during Rheumatoid Arthritis in Conakry

Rheumatoid arthritis (RA) is a systemic inflammatory disease that primarily affects the joints and may also manifest with extra-articular symptoms. Ocular involvement is common in patients with rheumatoid arthritis and may represent the main clinical manifestation of the disease. Aim: To study the epidemiological and therapeutic particularities of ocular manifestations during rheumatoid arthritis in Conakry. Material and Methods: This was a descriptive cross-sectional study of 6 months duration from 1 January to 30 June 2018 on patients with RA. All patients with chronic inflammatory rheumatism in whom the diagnosis of rheumatoid arthritis had been made according to the diagnostic criteria of the American College of Rheumatology (ACR) 2010 and these patients benefited from a complete ophthalmologic examination. Results: We enrolled 22 patients. The mean age was 43.45 ± 16.07 years (range 21 and 79 years). The mean age at onset of symptoms was 39.4 years with a mean time to diagnosis of RA of 3.9 years. Females dominated with 77.27% of cases with a sex ratio of 0.22 M/F. Photophobia (39.29%), xerophthalmia (31.82%), lacrimation and decreased vision 5 cases or 22.73 each, pruritus (9.09%), foreign body sensation and red eye with 2 cases each or 9.09% were the most frequent symptoms. The different ophthalmological diagnoses found were dry eye 19 cases or 86.36%, allergic conjunctivitis 4 cases or 18.18%, ametropia 3 cases or 13.64%, cataract 2 cases or 9.09%, superficial punctate keratitis 2 cases or 9.09% and chronic glaucoma 1 case or 4.55%. The management of dry eye was based on tear substitutes. Cataract cases (9.09%) required surgical intervention while patients with ametropia (13.04%) received optical correction. Cases of allergic conjunctivitis (18.18%) were reported. Conclusion: Good coordination between the ophthalmologist and the rheumatologist is necessary to optimize the management of patients with rheumatoid arthritis.

Oral Lesions Observed in Tobacco Consumers: Socio-Demographical and Clinical Aspects in the Prefectures of kankan and Siguiri Republic of Guinea

Introduction: Oral lesions observed in Chewing Tobacco (CT) consumers are histological alterations caused by direct contact with the tissues of the oral cavity. The objectives of this study were to determine the socio-demographic status of consumers and describe the clinical characteristics of the oral lesions observed. Material and Method: It was a descriptive cross-sectional study lasting 12 months (January 2017 to January 2018), in the prefectures of kankan and Siguiri as a framework. Individuals who use chewing tobacco and whose oral examination revealed oral lesions were included in this study. Results: The frequency of oral lesions induced by the consumption of chewing tobacco was 90%. Dental pain was the most cited reason for using CT by respondents, i.e., 25.41%. The most concerned age group was that of 50 - 64 years or 23.81 with a male predominance of 84.13% (n = 53). A number of 61 respondents came from rural areas. Farmers were the predominant socio-professional group, i.e., 63.49% (n = 40). In 60.32% (n = 38) of respondents, the duration of CT consumption was greater than 10 years. Black villous tongues followed by neoplastic ulcerations were the most common oral lesions with 32 (50.79%) and 14 (22.22%) cases, respectively. The site of the lesion was the tongue in 47 consumers (74.61%) and in the lower vestibule in 15 users (23.80%). Among the oral diseases, mylolysis was the most common, i.e., 95.24% (n = 60). In 82.54% of cases (n = 52), Simplified Oral Hygiene Index (SOHI) was low and in 12.70% (n = 8) of respondents, SOHI was good. Conclusion: Chewing tobacco predisposes to a high risk of damage to the oral cavity and the development of potentially malignant lesions. In addition, it exerts some detrimental effects on the teeth and adjacent structures.

Alternative splicing is a tightly regulated process that contributes to cancer *** is a long noncoding RNA with alternative splicing and is implicated in the pathogenesis of several ***,whether deregulated expression of CRNDE is common and which isoforms are mainly involved in cancers remain *** this study,we report that CRNDE is aberrantly expressed in the majority of solid and hematopoietic *** investigation of CRNDE expression in normal samples revealed that CRNDE was expressed in a tissue- and cell-specific *** comparison of CRNDE expression in 2938 patient samples from 15 solid and hematopoietic tumors showed that CRNDE was significantly overexpressed in 11 malignancies,including 3 reported and 8 unreported,and also implicated that the overexpressed isoforms differed in various cancer ***,anti-cancer drugs could efficiently repress CRNDE overexpression in cancer cell lines and primary samples,and even had different impacts on the expression of CRNDE ***,experimental profiles of 12 alternatively spliced isoforms demonstrated that the spliced variant CRNDE-g was the most highly expressed isoform in multiple cancer ***,our results emphasize the cancer-associated feature of CRNDE and its spliced isoforms,and may provide promising targets for cancer diagnosis and therapy.
