Introduction: The incidence of acute renal failure (ARF) varies between 20% and 40% of cases for COVID-19 patients admitted to the intensive care unit, with very high mortality, but heterogeneous according to the different epidemic waves, probably due to the genetic variant phenomenon of the virus. The aim of this study is to determine the morbidity and mortality of COVID-19 patients admitted with ARF to the intensive care unit of the Grand H?pital Est Francilien (GHEF) according to the waves and variants. Methods: Cross-sectional observational study of COVID-19 patients with ARF admitted to the intensive care unit of the GHEF site in Meaux covering the period from March 1st 2020 to December, 31st 2021. Per-hospitalisation and outcome data were collected and analysed with SPSS version 25.0 software using the Chi-square or Fischer’s exact test or Student’s t-test and logistic regression for p Results: A total of 86 patients were included. The mean age was higher (70 ± 8.5) in patients in the fourth wave than in the other waves (p = 0.015), with male predominance in all waves without significant difference. Co-morbidities: hypertension, diabetes, heart disease, dyslipidaemia and arrhythmia complete with fibrillation were present in all waves. The majority of patients were classified as KDIGO 1 for the different waves (1st: 61.9%, 2nd: 86.5%, 3rd: 80%, and 4th: 75%), with the same trend according to variant (alpha: 80%, beta: 75%, delta: 81.3%, omicron: 75%). Mortality by the wave was: 1st: 28.5%, 2nd: 37.5%, 3rd: 23% and 4th: 11%) and by variant: alpha: 24.2%, beta: 44.8%, delta: 20.7%, omicron: 10.3%). Overall mortality was 33.7%. Men’s Experience of Difficulties during First Year Following Myocardial Infarction—Not Only Fatigue

Recovery from a myocardial infarction (MI) can be a very difficult process and some patients are also stricken by fatigue. The aim of the study was to describe the difficulties experienced by men with fatigue during the first year following their MI. The study was conducted using qualitative content analysis of semi-structured interviews with nine men in working age (mean age: 55 years) with verified fatigue (≥12 of 20 on the Multidimensional Fatigue Scale with a mean of 17) one year after their MI. During the first year after the MI the men suffering from fatigue experienced various difficulties in terms of both themselves and the care received. The analysis generated three themes with difficulties described by the interviewed men. 1) Going through involuntary change: not recognizing their body and the inability to recognize themselves. 2) Lacking individualized care: To be denied satisfactory health care treatment and to be denied respectful treatment. 3) Not being seen in the light of the social context: The MI affects the whole family and caregivers do not pay attention to the family. In order to make both the patient and relatives feel secure and cared for, it is essential that caregivers be aware of the importance of providing individualized care over time with particular attention for the patient’s social context.

Attitude, Practice, and Factors Affecting Contraceptive Use among Women Attending Postnatal care in a Tertiary health Facility in Jos North LGA, Plateau State, Nigeria

The high population growth rate has been associated with the increased level of poverty, morbidity, mortality, and decreased life expectancy. Modern contraception is an important factor in controlling fertility through prevention of unintended and unwanted pregnancies. Contraception (birth control) prevents pregnancy by interfering with the normal process of ovulation, fertilization, and implantation. Contraceptive use is still very low in Nigeria and Sub-Saharan Africa (SSA), where the levels of fertility and unmet need for family planning are high. The attitude of women toward contraception will usually affect the practice and utilization of contraceptives. The objective of this study is to assess the attitude, prevalence, and factors affecting contraceptive use among women attending Postnatal care in a tertiary health facility in Jos North L. G. A Plateau State, Nigeria. There was a descriptive cross-sectional study done between September to October 2019, with a sample size of 250 women. Respondents were determined using a multi-stage sampling method. Data collection was done using an interviewer-administered, pre-tested, structured questionnaire. The data was collected, entered, and analyzed using IBM Statistical package for social sciences (SPSS) version 20.0. Most women have a positive attitude 162 (64.8%) toward contraception while 88 (35.2%) had negative attitude toward contraception. Three quarters (75.2%) of women are willing to use contraception. The prevalence of contraceptive use among respondents was 35.6% while the prevalence of previous use of contraceptives among the respondents was 71.2%. Fifty (27.9%) used condom (barrier method), 40 (22.9%) used withdrawal method. Majority of the other respondents were spread across OCPs, injectables and implants. Major factors affecting the use of contraceptives were majorly attributed to informed choice with 60.1%, Partner’s decision (44.1%) and mild or no adverse reaction (12.8%).
