Effects of neoadjuvant chemotherapy vs chemoradiotherapy in the treatment of esophageal adenocarcinoma:A systematic review and meta-analysis

BACKGROUND Neoadjuvant therapy is an essential modality for reducing the clinical stage of esophageal cancer;however,the superiority of neoadjuvant chemotherapy(nCT)or neoadjuvant chemoradiotherapy(nCRT)is unclear.Therefore,a discussion of these two modalities is necessary.AIM To investigate the benefits and complications of neoadjuvant modalities.METHODS To address this concern,predefined criteria were established using the pICO protocol.Two independent authors performed comprehensive searches using predetermined keywords.Statistical analyses were performed to identify significant differences between groups.potential publication bias was visualized using funnel plots.The quality of the data was evaluated using the Risk of Bias Tool 2(RoB2)and the GRADE approach.RESULTS Ten articles,including 1928 patients,were included for the analysis.Significant difference was detected in pathological complete response(pCR)[pp=0.015;OR:0.4;95%CI:0.22-0.71)favoring the nCRT,and renal failure(p=0.039;OR:1.04;95%CI:0.66-1.64)favoring the nCT.No significant differences were observed in terms of survival,local or distal recurrence,or other clinical or surgical complications.The result of RoB2 was moderate,and that of the GRADE approach was low or very low in almost all cases.CONCLUSION Although nCRT may have a higher pCR rate,it does not translate to greater long-term survival.Moreover,nCRT is associated with higher 30-d mortality,although the specific cause for postoperative complications could not be identified.In the case of nCT,toxic side effects are suspected,which can reduce the quality of life.Given the quality of available studies,further randomized trials are required.

Neurotrophins and neural stem cells in posttraumatic brain injury repair

Traumatic brain injury(TBI)is the main cause of disability,mental health disorder,and even death,with its incidence and social costs rising steadily.Although different treatment strategies have been developed and tested to mitigate neurological decline,a definitive cure for these conditions remains elusive.Studies have revealed that vari-ous neurotrophins represented by the brain-derived neurotrophic factor are the key regulators of neuroinflammation,apoptosis,blood-brain barrier permeability,neurite regeneration,and memory function.These factors are instrumental in alleviating neu-roinflammation and promoting neuroregeneration.In addition,neural stem cells(NSC)contribute to nerve repair through inherent neuroprotective and immunomodulatory properties,the release of neurotrophins,the activation of endogenous NSCs,and in-tercellular signaling.Notably,innovative research proposals are emerging to combine BDNF and NSCs,enabling them to synergistically complement and promote each other in facilitating injury repair and improving neuron differentiation after TBI.In this review,we summarize the mechanism of neurotrophins in promoting neurogen-esis and restoring neural function after TBI,comprehensively explore the potential therapeutic effects of various neurotrophins in basic research on TBI,and investigate their interaction with NSCs.This endeavor aims to provide a valuable insight into the clinical treatment and transformation of neurotrophins in TBI,thereby promoting the progress of TBI therapeutics.

Data-driven discovery of partial differential equations(pDEs)has recently made tremendous progress,and many canonical pDEs have been discovered successfully for proof of concept.However,determining the most proper pDE without prior references remains challenging in terms of practical applications.In this work,a physics-informed information criterion(pIC)is proposed to measure the parsimony and precision of the discovered pDE synthetically.The proposed pIC achieves satisfactory robustness to highly noisy and sparse data on 7 canonical pDEs from different physical scenes,which confirms its ability to handle difficult situations.The pIC is also employed to discover unrevealed macroscale governing equations from microscopic simulation data in an actual physical scene.The results show that the discovered macroscale pDE is precise and parsimonious and satisfies underlying symmetries,which facilitates understanding and simulation of the physical process.The proposition of the pIC enables practical applications of pDE discovery in discovering unrevealed governing equations in broader physical scenes.

Development of RpA-Cas12a-fluorescence assay for rapid and reliable detection of human bocavirus 1

Human bocavirus(HBoV)1 is considered an important pathogen that mainly affects infants aged 6–24 months,but preventing viral transmission in resource-limited regions through rapid and affordable on-site diagnosis of individuals with early infection of HBoV1 remains somewhat challenging.Herein,we present a novel faster,lower cost,reliable method for the detection of HBoV1,which integrates a recombinase polymerase amplification(RpA)assay with the CRISpR/Cas12a system,designated the RpA-Cas12a-fluorescence assay.The RpA-Cas12a-fluorescence system can specifically detect target gene levels as low as 0.5 copies of HBoV1 plasmid DNA per microliter within 40 min at 37℃without the need for sophisticated instruments.The method also demonstrates excellent specificity without cross-reactivity to non-target pathogens.Furthermore,the method was appraised using 28 clinical samples,and displayed high accuracy with positive and negative predictive agreement of 90.9%and 100%,respectively.Therefore,our proposed rapid and sensitive HBoV1 detection method,the RpA-Cas12a-fluorescence assay,shows promising potential for early on-site diagnosis of HBoV1 infection in the fields of public health and health care.The established RpA-Cas12a-fluorescence assay is rapid and reliable method for human bocavirus 1 detection.The RpA-Cas12a-fluorescence assay can be completed within 40 min with robust specificity and sensitivity of 0.5 copies/μl.

Optimal operation of Internet Data Center with pV and energy storage type of UpS clusters

With the development of green data centers,a large number of Uninterruptible power Supply(UpS)resources in Internet Data Center(IDC)are becoming idle assets owing to their low utilization rate.The revitalization of these idle UpS resources is an urgent problem that must be addressed.Based on the energy storage type of the UpS(EUpS)and using renewable sources,a solution for IDCs is proposed in this study.Subsequently,an EUpS cluster classification method based on the concept of shared mechanism niche(CSMN)was proposed to effectively solve the EUpS control problem.Accordingly,the classified EUpS aggregation unit was used to determine the optimal operation of the IDC.An IDC cost minimization optimization model was established,and the Quantum particle Swarm Optimization(QpSO)algorithm was adopted.Finally,the economy and effectiveness of the three-tier optimization framework and model were verified through three case studies.

Tissue adhesive,ROS scavenging and injectable pRp-based‘plasticine’for promoting cartilage repair

platelet-rich plasma(pRp)that has various growth factors has been used clinically in cartilage repair.However,the short residence time and release time at the injury site limit its therapeutic effect.The present study fabricated a granular hydrogel that was assembled from gelatin microspheres and tannic acid through their abundant hydrogen bonding.Gelatin microspheres with the gelatin concentration of 10wt%and the diameter distribution of 1-10 lm were used to assemble by tannic acid to form the granular hydrogel,which exhibited elasticity under low shear strain,but flowability under higher shear strain.The viscosity decreased with the increase in shear rate.Meanwhile,the granular hydrogel exhibited self-healing feature during rheology test.Thus,granular hydrogel carrying pRp not only exhibited well-performed injectability but also performed like a‘plasticine’that possessed good plasticity.The granular hydrogel showed tissue adhesion ability and reactive oxygen species scavenging ability.Granular hydrogel carrying pRp transplanted to full-thickness articular cartilage defects could integrate well with native cartilage,resulting in newly formed cartilage articular fully filled in defects and well-integrated with the native cartilage and subchondral bone.The unique features of the present granular hydrogel,including injectability,plasticity,porous structure,tissue adhesion and reactive oxygen species scavenging provided an ideal pRp carrier toward cartilage tissue engineering.

High-sensitive state perception method for inverter-fed machine turn insulation based on FrFT-Mel

Amidst the swift advancement of new power systems and electric vehicles,inverter-fed machines have progressively materialized as a pivotal apparatus for efficient energy conversion.Stator winding turn insulation failure is the root cause of inverter-fed machine breakdown.The online monitoring of turn insulation health can detect potential safety risks promptly,but faces the challenge of weak characteristics of turn insulation degradation.This study proposes an innovative method to evaluate the turn insulation state of inverter-fed machines by utilizing the fractional Fourier transform with a Mel filter(FrFT-Mel).First,the sensitivity of the high-frequency(HF)switching oscillation current to variations in turn insulation was analyzed within the fractional domain.Subsequently,an improved Mel filter is introduced,and its structure and parameters are specifically designed based on the features intrinsic to the common-mode impedance resonance point of the electrical machine.Finally,an evaluation index was proposed for the turn insulation state of inverter-fed machines.Experimental results on a 3kW permanent magnet synchronous machine(pMSM)demonstrate that the proposed FrFT-Mel method significantly enhances the sensitivity of turn insulation state perception by approximately five times,compared to the traditional Fourier transform method.

Stochastic programming based coordinated expansion planning of generation,transmission,demand side resources,and energy storage considering the DC transmission system

With the increasing penetration of wind and solar energies,the accompanying uncertainty that propagates in the system places higher requirements on the expansion planning of power systems.A source-grid-load-storage coordinated expansion planning model based on stochastic programming was proposed to suppress the impact of wind and solar energy fluctuations.Multiple types of system components,including demand response service entities,converter stations,DC transmission systems,cascade hydropower stations,and other traditional components,have been extensively modeled.Moreover,energy storage systems are considered to improve the accommodation level of renewable energy and alleviate the influence of intermittence.Demand-response service entities from the load side are used to reduce and move the demand during peak load periods.The uncertainties in wind,solar energy,and loads were simulated using stochastic programming.Finally,the effectiveness of the proposed model is verified through numerical simulations.

Control method based on DRFNN sliding mode for multifunctional flexible multistate switch

To address the low accuracy and stability when applying classical control theory in distribution networks with distributed generation,a control method involving flexible multistate switches(FMSs)is proposed in this study.This approach is based on an improved double-loop recursive fuzzy neural network(DRFNN)sliding mode,which is intended to stably achieve multiterminal power interaction and adaptive arc suppression for single-phase ground faults.First,an improved DRFNN sliding mode control(SMC)method is proposed to overcome the chattering and transient overshoot inherent in the classical SMC and reduce the reliance on a precise mathematical model of the control system.To improve the robustness of the system,an adaptive parameter-adjustment strategy for the DRFNN is designed,where its dynamic mapping capabilities are leveraged to improve the transient compensation control.Additionally,a quasi-continuous second-order sliding mode controller with a calculus-driven sliding mode surface is developed to improve the current monitoring accuracy and enhance the system stability.The stability of the proposed method and the convergence of the network parameters are verified using the Lyapunov theorem.A simulation model of the three-port FMS with its control system is constructed in MATLAB/Simulink.The simulation result confirms the feasibility and effectiveness of the proposed control strategy based on a comparative analysis.

Robust optimal dispatch strategy of integrated energy system considering CHp-p2G-CCS

Integrated energy systems(IESs)can improve energy efficiency and reduce carbon emissions,essential for achieving peak carbon emissions and carbon neutrality.This study investigated the characteristics of the CHp model considering p2G and carbon capture systems,and a two-stage robust optimization model of the electricity-heat-gascold integrated energy system was developed.First,a CHp model considering the p2G and carbon capture system was established,and the electric-thermal coupling characteristics and p2G capacity constraints of the model were derived,which proved that the model could weaken the electric-thermal coupling characteristics,increase the electric power regulation range,and reduce carbon emissions.Subsequently,a two-stage robust optimal scheduling model of an IES was constructed,in which the objective function in the day-ahead scheduling stage was to minimize the start-up and shutdown costs.The objective function in the real-time scheduling stage was to minimize the equipment operating costs,carbon emission costs,wind curtailment,and solar curtailment costs,considering multiple uncertainties.Finally,after the objective function is linearized with a ψ-piecewise method,the model is solved based on the C&CG algorithm.Simulation results show that the proposed model can effectively absorb renewable energy and reduce the total cost of the system.
