Lower limb intracast pressures generated by different types of immobilisation casts

AIM To determine if complete, split casts and backslabs [plaster of Paris(POP) and fiberglass] generate different intracast pressures and pain. METHODS Increased swelling within casts was modeled by a closed water system attached to an expandable bag placed directly under different types of casts applied to a healthy lower limb. Complete fiberglass and POP casts, split casts and backslabs were applied. Twenty-five milliliter aliquots of saline were injected into the system and the generated intracast pressures were measured using a sphygmomanometer. The subject was blinded to the pressure scores to avoid bias. All casts were applied to the same right limb on the same subject to avoid the effects of variations in anatomy or physiology on intracast pressures. Pain levels were evaluated using the Visual Analogue Score after each sequential saline injection. Each type of cast was reapplied four times and the measurements were repeated on four separate occasions. Sample sizes were determined by a pre-study 90% power calculation to detect a 20% difference in intracast pressures between cast groups. RESULTS A significant difference between the various types of casts was noted when the saline volume was greater than 100 mL(P = 0.009). The greatest intracast pressure was generated by complete fiberglass casts, which were significantly higher than complete POP casts or backslabs(P = 0.018 and P = 0.008 respectively) at intracast saline volumes of 100 mL and higher. Backslabs produced a significantly lower intracast pressure compared to complete POP only once the saline volume within casts exceeded 225 mL(P = 0.009). Intracast pressures were significantly lower in split casts(P = 0.003). Split POP and fiberglass casts produced the lowest intracast pressures, even compared to backslabs(P = 0.009). In this note we consider Lagrangian Bonnet problem for Lagrangian surfaces in complex space forms. We first give a Bonnet type theorem for conformal Lagrangian surfaces in complex space forms, then we show that any compact Lagrangian surface in the complex space form admits at most one other global isometric Lagrangian surface with the same mean curvature form, unless the Maslov form is conformal. These two Lagrangian surfaces are then called Lagrangian Bonnet pairs. We also studied Lagrangian Bonnet surfaces in complex space forms, and obtain some characterizations of such surfaces.

The Ponseti method has become the gold standard for the treatment of idiopathic clubfoot. Its safety and efficacy has been demonstrated extensively in the literature, leading to increased use around the world over the last two decades. This has been demonstrated by the increase in Ponseti related Pub Med publications from many countries. We found evidence of Ponseti activity in 113 of 193 united Nations members. The contribution of many organizations which provide resources to healthcare practitioners in low and middle income countries, as well as Ponseti champions and modern communication technology, have helped to spread the Ponseti method around the world. Despite this, there are many countries where the Ponseti method is not being used, as well as many large countries in which the extent of activity is unknown. With its low rate of complication, low cost, and high effectiveness,this method has unlimited potential to treat clubfoot in both developed and undeveloped countries. Our listing of countries who have not yet shown presence of Ponseti activity will help non-governmental organizations to target those countries which still need the most help.

To date, there are very few studies on the second Hopf bifurcation in a driven square cavity, although there are intensive investigations focused on the first Hopf bifurcation in literature, due to the difficulties of theoretical analyses and numerical simulations. In this paper, we study the characteristics of the second Hopf bifurcation in a driven square cavity by applying a consistent fourth-order compact finite difference scheme recently developed by us. We numerically identify the critical Reynolds number of the second Hopf bifurcation located in the interval of(11093.75, 11094.3604) by bisection. In addition, we find that there are two dominant frequencies in its spectral diagram when the flow is in the status of the second Hopf bifurcation, while only one dominant frequency is identified if the flow is in the first Hopf bifurcation via the Fourier analysis. More interestingly, the flow phase portrait of velocity components is found to make transition from a regular elliptical closed form for the first Hopf bifurcation to a non-elliptical closed form with self-intersection for the second Hopf bifurcation. Such characteristics disclose flow in a quasi-periodic state when the second Hopf bifurcation occurs.

背景:肩关节上盂唇前后部(SLAP),是维持肩关节稳定的重要结构。SLAP的损伤常常造成肩关节的疼痛和功能障碍,但在临床上通过常规查体却很难确诊。目的:验证我们发现的一个新的临床试验用于诊断SLAP损伤。方法:选取因肩关节疼痛初次进行关节镜手术治疗的患者。术前均进行4项常用的SLAP损伤的临床检查法Speed、Yergason、O'Brien、及我们的新试验(Reverse Biceps Load test)进行检查做初步诊断。之后所有患者进行肩关节镜检查,术中镜下对有SLAP损伤的患者进行记录并分型。结果:从2010年2月至2010年8月大约有137名肩关节疼痛的患者进行了肩关节镜手术治疗,其中52名患者存在有SLAP或二头肌长头腱的损伤。其中对于SLAP及二头肌腱的损伤,新试验敏感度76.92%,特异性为43.53%。当肩关节后伸,前部极度外旋时SLAP及二头肌长头腱处于高张力状态。此时轻微的外力就可诱发疼痛,因此新试验具有较高的敏感性,但此位置时前关节囊及部分肩袖亦处于高张力状态,所以特异性会受一定影响。结论:新试验对于SLAP及二头肌腱损伤有较高的敏感性,可以作为SLAP损伤的筛选试验运用于临床。

Alcohol-induced Wnt signaling inhibition during bone fracture healing is normalized by intermittent parathyroid hormone treatment

Nearly half of orthopaedic trauma patients are intoxicated at the time of injury, and excess alcohol consumption increases the risk for fracture nonunion. Previous studies show alcohol disrupts fracture associated Wnt signaling required for normal bone fracture repair. Intermittent parathyroid hormone(PTH) promotes bone growth through canonical Wnt signaling, however, no studies have investigated the effect of PTH on alcohol-inhibited bone fracture repair. Male C57 BL/6 mice received two-3 day alcohol binges separated by 4 days before receiving a mid-shaft tibia fracture. Postoperatively, mice received PTH daily until euthanasia. Wnt/β-catenin signaling was analyzed at 9 days post-fracture. As previously observed, acute alcohol exposure resulted in a >2-fold decrease in total and the active form of β-catenin and a 2-fold increase in inactive β-catenin within the fracture callus. Intermittent PTH abrogated the effect of alcohol on β-catenin within the fracture callus. Upstream of β-catenin, alcohol-treated animals had a 2-fold decrease in total LRP6, the Wnt co-receptor, which was restored with PTH treatment. Alcohol nor PTH had any significant effect on GSK-3β. These data show that intermittent PTH following a tibia fracture restores normal expression of Wnt signaling proteins within the fracture callus of alcohol-treated mice.
