Current Epidemiology of Intracranial Metastases in Two university Teaching Reference Hospitals of the Town of yaounde, Cameroon: Analysis of 35 Cases Recorded in the Neurosurgery Departments

Background: The incidence of intracranial metastases (ICMET) has been steadily rising, and its frequency with respect to primary brain tumours is relatively high. Objective: The objectives of this study were to elucidate the current epidemiology and describe the clinical, diagnostic and therapeutic features of ICMET in yaounde. Method and findings: A descriptive cross-sectional study was done in the neurosurgery departments of the General and Central Hospitals of yaounde during the period from January 2016 to December 2022. We included all medical booklets of patients admitted for a tumoral intracranial expansive process with our target population being patients with histological evidence of ICMET, and did a retrospective inclusion of data using a pre-established technical form aimed at collecting sociodemographic data, clinical data, paraclinical data, and the treatment procedures. Analysis was done using the SPSS statistical software. A total of 614 cases of intracranial tumors were included among whom 35 presented histological evidence of ICMET. This gives a frequency of 5.7%. The sex ratio was 0.94, the mean age was 55.68 +/- 14.4 years, extremes 28 and 86 years and the age range 50 - 59 was affected in 28.57% of cases. The clinical presentation included signs of raised intracranial pressure (headache, blurred vision, vomiting) in 26 cases (74.3%), motor deficit 48.6%, seizures 17.1%. The mode of onset was metachronous in 71.4% and synchronous in 28.6%. The imaging techniques were cerebral CT scan in 82.9%, cerebral MRI in 40%, TAP scan in 22.9%. The metastatic lesions were supratentorial in 94.3% and single in 62.9%. The primary cancers found were breast cancer (31.4%), lung cancer (25.7%), prostate cancer (17.1%), thyroid cancer (5.7%), colon cancer (2.9%), and melanoma (2.9%). The Bipindi iron ore district is located in the central section of the Nyong Complex at the northwestern margin of the Congo Craton in Southwest *** iron district contains numerous iron mineralization hosted by the Mewongo,Bibole,Kouambo,and Zambi banded iron formations(BIFs).These BIFs contain magnetite as the main iron ore mineral associated with pyrite,and gangue minerals are quartz with minor chlorite and *** origin of iron ore from these BIFs was investigated using a combination of in-situ magnetite and whole-rock *** studied BIF ore samples have a narrow range of TFe between 30.90 wt.%and 43.20 wt.%,indicating a low-grade *** geochemical signatures of magnetite such as low contents of base metals(e.g.,Cu,Co,V,and Zn)and low Co/Zn ratios600 and>75,respectively),we suggest that magnetite was derived from a mixture of seawater and~0.1%low-temperature hydrothermal fluids in an oxidizing ***,low-temperature hydrothermal and later metamorphic fluids were necessary for the transformation of the protolith Nyong Complex BIFs to iron ore.

Evaluation of Procedural Pain in Neonates in Cameroon

Introduction: Acute pain associated with caregiving is a major cause of pain among neonates. Left untreated, it can lead to long-term neurosensory and psychoaffective consequences. In Cameroon, this subject has been scarcely explored, thus constituting an impediment to the management of care-induced pain. Objective: Assess procedural pain in neonates in Yaoundé. Material and Methods: We conducted a cross sectional study with prospective data collection over a period of eight months (October 2022 to May 2023) in three hospitals. We included neonates who were being cared for and were not crying prior to the onset of healthcare, whose parents consented to the study. Assessments were done using the DAN scale, which is specific to care-induced pain. Data was entered and analyzed using SPSS 23.0 software. Results: A total of 161 newborns were included. The hospital prevalence of care-induced pain in neonates was 85%. Neonatal sepsis was the main cause for admission (96.6%). The most common procedures were venous blood sampling (94.4%) and insertion of peripheral venous lines (93.8%). The pain intensity for these procedures was severe (83.9%). The most painful procedure was lumbar tap, followed by venous access procedures. Conclusion: Neonates in hospitals are subjected to many painful procedures. The pain experienced during these procedures is severe. The most nociceptive procedure is a lumbar puncture.

Reinforcing of Citizen’s Trust in E-Government: The Cameroon’s Case

The embracing of ICTs and related technologies has enhanced different approaches for governments worldwide to deliver services to their citizens in a smart way. However, the usage of e-government services by common citizens is recognized as one of the major setbacks of e-government development in both developed and developing countries. Moreover, government agencies in these countries are facing great challenges in keeping the citizens motivated enough to continue to use e-government services. This research aims to investigate the factors that influence citizens’ trust towards continue use of e-government services in Cameroon. The proposed research model consisted of three main constructs including technological, governmental, risk factors as well as six demographic characteristics (age, gender, educational level, income, internet experience and cultural perception). A five-point Likert scale questionnaire was designed to collect data physically and electronically, 352 valid questionnaires were retrieved. Simple and Multiple regression analysis methods were applied to build an adequate model based on the verification of hypotheses proposed. Based on results obtained, four demographic characteristics (age, education, occupation and income) have influence on citizens’ trust in e-government meanwhile gender and cultural affiliation have no influence. Furthermore, technological factors and governmental factors positively influence trust level in e-government, whereas risk factors have a negative influence on trust level. Deducing from the results, a list of recommendations is proposed to the government of Cameroon in order to reinforce citizens’ trust in e-government services.

Background: Klebsiella spp. are bacteria of medical importance for their role in opportunistic infections which are often difficult to treat because of resistance to one or several antimicrobials. The aim of this study was to determine antimicrobial resistance due to Extended Spectrum Beta-lactamase (ESBL), Class C cephalosporinase (AmpC) and carbapenemase enzymes in Klebsiella spp. isolated from patients consulted at four hospitals. Methodology: The study was cross-sectional and descriptive. A total of 4190 non-repetitive patients specimens from 13 types of clinical specimens were analysed from February to November 2020. Two hundred and twenty-five (225) Klebsiella spp isolates were identified using API 20E and antimicrobial susceptibility testing done according to the Kirby Bauer disc diffusion method. ESBL and AmpC phenotypes were determined by the combination disc method and carbapenemases by double disc synergy method, referenced by EUCAST guidelines for the resistance testing. Results: The frequency of the species was Klebsiella pneumoniae (69%, 155/255), K. oxytoca (14%, 31/255), K. ozaenae (12%, 27/225) and K. rhinoscleromatis (5%, 11/225). Isolates were most resistant to sulphomethoxazole trimethoprim (84%, 189/225), cepaholosporins (80%, 180/225), and least resistant to carbapenems (10.7%, 24/225). Two K. oxytoca and one K. pneumoniae were resistant to all antibiotics tested. Klebsiella pneumoniae had the most multidrug resistant isolates (59.4%, 134/225). Most isolates (83.6%, 188/225) expressed at least one enzyme, while 63.6% (143/225) of the isolates expressed at least two enzymes. Some isolates were ESBL (71.6%, 161/225), carbapenemase (10.7%, 24/225) and AmpC (6.6%, 15/225) producers. Three carbapenemases (Klebsiella pneumoniae carbapenemase-KPC, Metallo-Beta Lactamase-MBL and OXA-48) were detected. Introduction: Radiological investigation which is the gold standard to perform following anastomotic urethroplasty in order to evaluate the quality of micturition is costly. In our context, due to financial limitations, reconstructive urologists prefer to use the uroflowmetry in order to assess the micturition post-surgery. Therefore, the objective of the study was to assess the quality of micturition using the uroflowmetry after anastomotic urethroplasty. Methodology: We conducted an 11-year retrospective review (1st January 2006-31st December 2017) and a cross-sectional descriptive observational study for a period of 8 months (November 2017-June 2018) at the Urology and Andrology Department of the yaounde Central Hospital (YCH) of patients who were diagnosed with urethral stenosis and underwent an anastomotic urethroplasty at the YCH. We excluded patients who had incomplete files, patients lost to follow-up and did not do pre-operative uroflowmetry. Quality of micturition was evaluated using a uroflowmetry. Data was analyzed using EPI info 7.0. Parametric variables were reported as means and standard deviations and percentages and counts were used to report categorical variables. Results: We had a sample of 60 patients. The mean age was 42 ± 5 years with extremes ranging from 20 to 76 years. Twenty-seven (27) patients, or 45%, had no post-operative complications, and those who did had a urinary tract infection (26.70%). In our series, we had 82% excellent results (patient satisfied with his urination with bell-shaped urinary stream and urinary flow greater than or equal to 15 ml/sec);15% good results (patient with moderate dysuria with average urinary stream and urinary flow between 10 and 14.9 ml/sec) and 3% poor results (severe dysuria with urinary flow less than 10 ml/sec, urinary retention or urinary incontinence). Based on these results we can say that the success rate in our series was 97%. 96.70% of patients were satisfied against 3.3%

Simulation of Steel Reinforcement on the Nonlinear Behaviour of Slender Glulam Beam Columns by Using the Newton-Raphson Method

The current theory in NF EN 1995-1-1/NA of Eurocode 5, which is based on maximum deflection, has been investigated on softwoods. Therefore, this theory is not adapted for slender glulam beam columns made of tropical hardwood species from the Congo Basin. This maximum deflection is caused by a set of loads applied to the structure. However, Eurocode 5 doesn’t provide how to predict this deflection in case of long-term load for such structures. This can be done by studying load-displacement (P-Δ) behaviour of these structures while taking into account second order effects. To reach this goal, a nonlinear analysis has been performed on a three-dimensional beam column embedded on both ends. Since conducting experimental investigations on large span structural products is time-consuming and expensive especially in developing countries, a numerical model has been implemented using the Newton-Raphson method to predict load-displacement (P-Δ) curve on a slender glulam beam column made of tropical hardwood species. On one hand, the beam has been analyzed without wood connection. On the other hand, the beam has been analyzed with a bolted wood connection and a slotted-in steel plate. The load cases considered include self-weight and a uniformly applied long-term load. Combinations of serviceability limit states (SLS) and ultimate limit states (ULS) have also been considered, among other factors. A finite-element software RFEM 5 has been used to implement the model. The results showed that the use of steel can reduce displacement by 20.96%. Additionally, compared to the maximum deflection provided by Eurocode 5 for softwoods, hardwoods can exhibit an increasing rate of 85.63%. By harnessing the plastic resistance of steel, the bending resistance of wood can be increased by 32.94%.

Introduction: Urogenital prolapse is the descent of one or more elements of the abdominal-pelvic contents into the vaginal cavity. The anterior, middle and posterior anatomical compartments may be involved giving rise to several types of genital prolapse. However, there are various therapeutic modalities ranging from observational procedures to surgery. In yaounde, urogenital prolapse appears to be a rare clinical entity, and its treatment has not been clearly stated. There is limited data about this subject in our context. Our aim was to study the clinical and therapeutic aspects of urogenital prolapse in yaounde. Methods: We conducted a descriptive cross-sectional study with retrospective data collection at the yaounde Gynaecological Obstetric and Pediatric Hospital (YGOPH) and the yaounde Central Hospital (YCH) over a period of 05 months (from February 2022 to June 2022). All women aged ≥30 years and all women with documented urogenital prolapse admitted to the maternity and urology departments of these two health facilities were included. Data were collected using a structured questionnaire. Data was analyzed using IBM SPSS (statistical package for social sciences) software version 23.0 and the data were reported as mean, standard deviation, frequencies and percentages. Results: A total of 50 patients were recruited;the mean age was 45.8 ± 17 years. The age range was from 21 to 72 years. Majority of the patients were aged more than 55 years (32%). Most of the patients attended primary education (48%). Forty percent of the patients were housewives. The majority of the patients were from West Cameroon (26%). The most common reason for consultation was pelvic heaviness (54%). The most prevalent clinical sign was cystocele (56%), followed by hysterocele (54%). Introduction: Twin birth is the expulsion of two fetuses and their appendages out of the woman’s reproductive tract. It’s a high-risk delivery because of the high frequency of maternal, fetal and neonatal complications. Few studies exist on the comparative prognoses of twins. Our objective was to compare early neonatal complications in first and second twins. Methods: We carried out a cross-sectional prospective study from January 2 to April 30, 2020 (4 months) at the yaounde Gynaeco-Obstetric and Pediatric Hospital, and the yaounde Central Hospital. Data collected were analyzed using IBM SPSS version 23. Descriptive statistics were done in frequencies and percentages, means and standard deviations. Paired-sample student's t-test were used to compare means. McNemar’s Chi-square and Ficher’s exact tests were used to compare proportions. Statistical significance was set at p-values less than 0.05. Results: Of a total of 2183 deliveries during the study period, 43 (1.96%) were twin vaginal deliveries. Among the 43 women, 41 consented to have their newborns participate in the study. Overall, APGAR scores were better for the first twins at the first (p = 0.004) and fifth (p = 0.031) minutes than for the second twins. Although both twins had complications and that there were 4 neonatal deads of the second twin, there was no relationship between studied complications and the rank of the twin (p = 0.310). Conclusion: As the APGAR score seemed to be better for first twins, clinicians should pay more attention during twin births, in order to improve the prognosis of the second twin.
