BACKGROUND： There is an increasing frequency of pancreaticoduodenectomy（PD） and PD with superior mesenteric-portal vein（SMPV） resection in elderly cancer patients. The study aimed to investigate the safety and the survival benefits of PD and PD with SMPV resection in patients under or over 70 years of ***： We divided 296 patients who had undergone PD and PD with SMPV resection into two groups according to their ages： under or over 70 years old. The clinical data were compared between the two ***： Preoperative comorbidity rate was higher in elder patients than in younger patients（P=0.001）. The elder patients were more likely to have postoperative complications（P=0.003）.Specifically, complications above grade III were more likely to occur in the elderly patients（P=0.030）. Multivariable analysis showed that age（adjusted OR=2.557, P=0.015） and hypertension（adjusted OR=2.443, P=0.019） were independent predictors of postoperative complications. There was no significant difference in the mortality rates between the two groups（P=0.885）. In the PD with SMPV resection series, elderly patients were more likely to have postoperative complications（P=0.063）, but this difference was not statistically *** was no difference in the survival rate of patients with pancreatic ductal adenocarcinoma between the two *** type（PD vs PD with SMPV resection） did not affect the survival of ***： Age affects postoperative complication in patients undergoing either PD or PD with SMPV ***, extensive experience and advanced perioperative management lower the complication rate to an acceptablelimit. Upper gastrointestinal cancer burden in Hebei Province, China: A population-based study

AIM To investigate the incidence and mortality rates of upper gastrointestinal cancer(UGIC) in Hebei Province, China, and to identify high-risk populations to improve UGIC prevention and *** Data for UGIC patients were collected from 21 population-based cancer registries covering 15.25% of the population in Hebei Province. Mortality data were extracted from three national retrospective death surveys(1973-1975, 1990-1992 and 2004-2005). The data were stratified by 5-year age groups, gender and area(high-risk/non-high-risk areas) for analysis. The age-period-cohort and grey system model were *** The crude incidence rate of UGIC was 55.47/100000, and the adjusted rate(Segi's population) was 44.90/100000. Males in rural areas had the highest incidence rate(world age-standardized rate = 87.89/100000). The crude mortality rate of UGIC displayed a decreasing trend in Hebei Province from the 1970 s to 2013, and the adjusted rate decreased by 43.81% from the 1970s(58.07/100000) to 2013(32.63/100000). The mortality rate declined more significantly in the high-risk areas(57.26%) than in the non-high-risk areas(55.02%) from the 1970 s to 2013. The median age at diagnosis of UGIC was 65.06 years in 2013. There was a notable delay in the median age at death from the 1970s(66.15 years) to 2013(70.39 years), especially in the high-risk areas. In Cixian, the total trend of the cohort effect declined, and people aged 65-69 years were a population at relatively high risk for UGIC. We predicted that the crude mortality rates of UGIC in Cixian and Shexian would decrease to 98.80 and 133.99 per 100000 in 2018, *** UGIC was the major cause of cancer death in Hebei Province, and males in rural areas were a high-risk population. Objective： Population-based cancer registration data in 2012 from all available cancer registries in Henan province were collected by Henan Office for cancer Research and Control. The numbers of new cancer cases and cancer deaths in Henan province with compiled cancer incidence and mortality rates were estimated. Methods： In 2015, all registries＇ data in Henan province were qualified for the national cancer registry annual report in 2012. The pooled data were stratified by area （urban/rural）, gender, age group （0, 1-4, 5-9, 10-14, ..., 85＋） and cancer type. New cancer cases and deaths were estimated using age-specific rates and corresponding population of Henan province in 2012. The Chinese census data in 2000 and Segi＇s population were applied for age-standardized rates. All the rates were expressed per 100,000 person-years. Results： Qualified 19 cancer registries （4 urban and 15 rural registries） covered 16,082,688 populations of Henan province in 2012. The percentage of cases with morphologically verified （MV%） and death certificate- only cases （DCO%） were 69.84% and 2.30%, respectively, and the mortality to incidence rate ratio （M/ I） was 0.64. It was estimated that there were 248,510 new cancer cases and 158,630 cancer deaths in Henan province in 2012. The incidence rate was 266.17/100,000 （288.61/100,000 in males and 241.86/100,000 in females）, the age-standardized incidence rates by Chinese standard population （ASIRC） and by world standard population （ASIRW） were 208.95/100,000 and 206.41/100,000 with the cumulative incidence rate （0-74 years old） of 24.30%. The crude incidence rate in urban areas was higher than that in rural areas. However, after adjusted by age, the cancer incidence rate in rural was higher than that in urban areas. The crude mortality of all cancers in Henan province was 169.90/100,000 （201.23/100,000 in males and 135.95/100,000 in females）.
