Nociceptive and Neuropathic Pain Qualities in Men and Women with Acute Coronary Syndromes: A Complex Pain Presentation

Background: Cardiac pain arising from acute coronary syndrome (ACS) is a multi-factorial phenomenon. Historically, episodes of cardiac pain have been captured using a one-dimensional numeric pain rating scale. Lacking in clinical practice are acute pain assessments that employ a comprehensive evaluation of an emergent ACS episode. Aim: To examine the sensory-discriminative, motivational-affective and cognitive-evaluative dimensions of ACS-related pain. Methods: A descriptive-correlational, repeated-measure design was used to collect data on 121 ACS patients of their cardiac pain intensity. The (numeric rating scale-NRS 0-10 scale) measured chest pain “Now” and “Worst pain in the previous 2 hours over 8 hours” and the McGill Pain Questionnaire Short-Form (MPQ-SF) measured pain at 4 hours. Results: Mean age was 67.6 ± 13, 50% were male, 60% had unstable angina and 40% had Non-ST-elevation myocardial infarction. Cardiac pain intensity scores remained in the mild range from 1.1 ± 2.2 to 2.4 ± 2.7. MPQ-SF: 66% described pain as distressing and 26% reported pain was horrible or excruciating. Participants described ACS pain quality as acute injury (nociceptive pain: heavy, cramping, stabbing), as nerve damage (neuropathic: gnawing, hot-burning, shooting) and as a mixture of acute and chronic pain qualities (aching, tender and throbbing). Conclusions: Patients reported both nociceptive and neuropathic cardiac pain. It is unclear if pain perceptions are due to: i) pathophysiology of clot formation, ii) occurrence of a first or repeated ACS episode, or iii) complex co-morbidities. Pain arising from ACS requires an understanding of the interplay of ischemic, metabolic and neuropathophysiological mechanisms that contribute to complex cardiac pain experiences.

The present research paper presents the synthesis, characterization, biological and computational studies of 4-(benzylideneamino) benzoic acid derivatives(3a~3g). Derivatives 3a~3c displayed best antidiabetic potential with a glucose-lowering effect compared to the reference drug Glibenclamide. Biochemical parameters including plasma glucose, serum triglycerides, cholesterol, alanine amino transferase and aspartate amino transferase levels showed significant alterations in concentrations relative to the control. Similarly, the derivatives 3a, 3d and 3e displayed potent in vitro antibacterial potential. Molecular docking simulations delineated that the ligands and complexes were stabilized at the active site by electrostatic and hydrophobic forces, consistent with the corresponding experimental results. In silico study of the binding pattern predicted that the synthesized ligands, 3d and 3a could serve as a potential surrogate for hit-to-lead generation and the design of novel antibacterial drugs.

单分子荧光共振能量转移技术是研究无规蛋白结构的重要工具之一,通过搜索与实验测得的平均能量转移效率相匹配的末端距分布来推测相应构象系综的结构性质。由于末端距分布依赖于所采用的高分子链模型,因此选择恰当的高分子链模型对于蛋白分子结构推测的准确性起着至关重要的作用。为了更清楚地描述荧光效率-分子构象之间转换的物理基础,我们采用粗粒化链模型通过计算模拟进行大量的构象抽样,并在此基础上构建基于给定分子回转半径和非球形程度的条件化构象子系综,通过寻找与实验测量值具有最佳拟合的构象子系综,可以推测所测量的蛋白分子在无序状态的"最可几"回转半径和非球形程度。为了验证新方法的可行性,我们考察了具有90个残基长度的的天然无规蛋白Sic1（细胞周期激活酶抑制剂）,相对于依赖于高斯链模型或Sanchez模型等传统方法,运用新方法获得的分子尺寸更加塌缩,推测的分子回转半径和流体力学半径结果分别与小角X射线散射的核磁共振结果一致。传统方法推测分子尺寸偏差较大的原因主要是由于所依赖的模型忽视或者不能恰当处理排斥体积相互作用,过高估计了具有较小分子末端距的分子链分布几率,造成在较高变性剂浓度条件下推测的分子尺寸偏大。因此,我们可以推论,对于Sic1这样的天然无规蛋白,其构象特征不再适合用均一的的伸展或塌缩构象系综来描述,以一定方式将构象子系综组合而成的非均一构象系综可能更加适用。

A Needs-Based Rheumatologist Education Program on Treating to Target in Psoriatic Arthritis and Spondyloarthropathy: Insights and Challenges

Objectives: To determine if comparative practice data and education for rheumatologists would change physician behavior for monitoring and treating psoriatic arthritis (PsA) and spondyloarthritis (SpA). Methods: Participating rheumatologists each performed a chart audit on 20 patients with PsA and SpA. Accredited education (determined by a survey and chart audits) and results of chart audits (comparing to other rheumatologists) were provided for each participant (intervention). Eight months later, a repeat chart audit by each participant was conducted on another 20 PsA and SpA patients. Changes in measurements collected, treatment given and patient characteristics pre and post intervention were analyzed. Results: Nine rheumatologists received the intervention. At baseline, most routinely monitored PsA and SpA for clinical and laboratory markers. In PsA, there was no change post-intervention in performing SJC (96%), TJC (≥91%), ESR (≥70%), CRP (≥73%), and CDAI (25%). In SpA, there were increased measurements of inflammatory markers (54% pre vs. 61% post for CRP), more NSAID use and decreased physical exam measures and HAQ but no significant changes. There were no major treatment differences pre and post intervention including NSAIDs, DMARDs and biologics. Conclusions: The rheumatologists frequently performed measurements of disease activity, did not change significantly with educational intervention so there may have been little room for improvement and many patients were already in a low disease state. Calculation of composite scores did not increase in PsA. The validity of physical exam and BASDAI as a measurement of disease activity were noted as concerns in applying a treat-to-target approach in SpA. Significance and Innovation: This study did not show a significant change in behavior for rheumatologists who had education based on care gaps and needs assessment in psoriatic arthritis and spondyloarthropathy. Activities of Some Nigerian Medicinal Plants against <i>Aedes aegypti</i>

Extracts and constituents of medicinal plants have proven to be biodegradable, had low mammalian toxicity and induction of resistance, and comparable activities to the standard drugs. Therefore, methanolic extracts of some plants that are termite resistant or used ethnomedically as antimalarial and febrifuge were evaluated for activities against 4th-instar larvae of Aedes aegypti. A 61 % of these plants with these properties demonstrated larvicidal activities and may confirm the usefulness of these properties in choosing plant larvicides. This is the first report of larvicidal activities of stem barks and leaves of Blighia sapida and Baphia nitida, stem barks of Markhamia tomentosa and Newboldia laevis, and whole plants of Euphorbia macrophylla. Extracts of B. sapida stem bark, Costus specious root and Xylopia aethiopica seed, with LC50 1.71, 1.47 and 1.49 mg/ml at 48 h, respectively, were the most active and had significant activities that were comparable to Endosulphan. Hence, they may be used as plant larvicides in the control of dengue and yellow fever.

FSH/LH Ratio as a Predictor of the IVF Outcome in Young Women

In Vitro Fertilization (IVF) is the treatment for many causes of infertility. Many studies were done to investigate different factors that can affect the success rate. This study was conducted to evaluate if cycle day 3 (CD3) follicle-stimulating hormone (FSH)/luteinizing hormone (LH) ratio can be a predictor for the IVF outcome in young sub-fertile females ≤ 35 years with normal baseline FSH. This is a retrospective case control study conducted at the Centre of Fertility and Andrology Care (CFAC) in Egypt where 235 sub-fertile women underwent IVF. Patients were grouped based on CD3 FSH/LH ratio. Group A consisted of ≤35-year-old women with FSH/LH ratio ≤35-year-old women with FSH/LH ratio ≥ 2. The primary outcomes include the fertilization rate, implantation rate and the clinical pregnancy rate. The secondary outcomes include duration and the total dose of gonadotrophin used. We found that, there was no significant difference in the total dose of gonadotropin used during the IVF cycle. Also, there was no significant difference in the number of retrieved and fertilized oocytes and the number of good embryos. Clinical pregnancy rate was the same in both groups. In conclusion, in patients younger than 35 years, CD3 FSH/LH ratio is not a predictor for IVF outcome.

Detection of SARS-CoV-2 RNA in wastewater is a promising tool for informing public health decisions during the COVID-19 ***,approaches for its analysis by use of reverse transcription quantitative polymerase chain reaction(RT-q PCR)are still far from standardized *** characterize inter-and intra-laboratory variability among results when using various methods deployed across Canada,aliquots from a real wastewater sample were spiked with surrogates of SARS-CoV-2(gamma-radiation inactivated SARS-CoV-2 and human coronavirus strain 229E[HCoV-229E])at low and high levels then provided"blind"to eight *** estimates reported by individual laboratories were consistently within a 1.0-log_(10) range for aliquots of the same spiked *** laboratories distinguished between low-and high-spikes for both *** expected,greater variability was observed in the results amongst laboratories than within individual laboratories,but SARS-CoV-2 RNA concentration estimates for each spiked condition remained mostly within 1.0-log_(10) *** no-spike wastewater aliquots provided yielded non-detects or trace levels(<20 gene copies/mL)of SARS-CoV-2 *** appear linked to methods that included or focused on the solids fraction of the wastewater matrix and might represent in-situ SARS-CoV-2 to the wastewater ***-229E RNA was not detected in the no-spike ***,all methods yielded comparable results at the conditions *** behavior of SARS-CoV-2 and spiked surrogates in wastewater should be considered to evaluate method effectiveness.A consistent method and laboratory to explore wastewater SARS-CoV-2 temporal trends for a given system,with appropriate quality control protocols and documented in adequate detail should succeed.
