Implementation of FAIR Guidelines in Selected Non-Western Geographies

This study provides an analysis of the implementation of FAIR Guidelines in selected non-Western geographies. The analysis was based on a systematic literature review to determine if the findability, accessibility, interoperability, and reusability of data is seen as an issue, if the adoption of the FAIR Guidelines is seen as a solution, and if the climate is conducive to the implementation of the FAIR Guidelines. The results show that the FAIR Guidelines have been discussed in most of the countries studied, which have identified data sharing and the reusability of research data as an issue(e.g., Kazakhstan, Russia, countries in the Middle East), and partially introduced in others(e.g., Indonesia). In Indonesia, a FAIR equivalent system has been introduced, although certain functions need to be added for data to be entirely FAIR. In Japan, both FAIR equivalent systems and FAIR-based systems have been adopted and created, and the acceptance of FAIRbased systems is recommended by the Government of Japan. In a number of African countries, the FAIR Guidelines are in the process of being implemented and the implementation of FAIR is well supported. In conclusion, a window of opportunity for implementing the FAIR Guidelines is open in most of the countries studied, however, more awareness needs to be raised about the benefits of FAIR in Russia and Kazakhstan to place it firmly on the policy agenda.

Analysis of Genetic Diversity and Relationships in a Tunisian Fig （Ficus carica） Germplasm Collection by Random Amplified Microsatellite Polymorphisms

The random amplified mirosatellite polymorphism method was performed in a set of Tunisian fig landraces using eighteen primer combinations. A total of sixty three random amplified microsatellite polymorphism （RAMPO） markers were scored and used either to assess the genetic diversity in these cultivars or to detect cases of mislabeling. Opportunely, data proved that the designed procedure constitutes an attractive and fast method with low costs and prevents radio exposure. As a result, we have identified the primer combinations that are the most efficient to detect genetic polymorphism in this crop. Therefore, the derived unweighted pair-group method with arithmetic averages （UPGMA） dendrogram illustrates the genetic divergence among the landraces studied and exhibits a typically continuous variation. Moreover, no evident correlation between the sexes of trees was observed. In addition, using these markers, discrimination between landraces has been achieved. Thus, random amplified mirosatellite polymor- phism is proved to be powerful for characterizing the local fig germplasm.

The Extent of Voluntary Information in the Annual Report and the Share Price Volatility： Application of DEA and SFA on the French Market

In the current paper, the authors investigate empirically the relationship between the share price volatility and the amount of voluntary information disclosed in the corporate annual report. The authors use the data envelopment analysis (DEA) and the stochastic frontier analysis (SFA) models on a sample of 50 listed French firms belonging to the Societes des Bourses Francaises (SBF250) index from 2004 to 2008. The authors focus on the extent of voluntary disclosure in the annual reports that have been measure by using a composite disclosure index. Then, the authors study the relevance of disclosure policies through the ability of the voluntary disclosure level to effectively reduce the share price volatility. The findings of DEA and SFA provide, in average, highly efficient scores of the sample, reveal the several dispositions taken by the French Exchange Market authorities and the initiative of French firms to improve the market stability. In addition, the findings highlight, in average, stability of the firms" efficiency scores over the studied period. The authors explain these findings by the stability of corporate disclosure policy as suggested in previous literature (e. g., Botosan, 1997). However, there are some differences between the findings of SFA and DEA models.

Metastatic Colorectal Cancer: Evolution of the Response Rate and Survival with Therapeutic Progress

Introduction: The therapeutic strategy for metastatic colorectal cancer (mCRC) has evolved greatly in the last two decades. This study aimed to compare patients with mCRC treated at the university Hospital of sousse (Tunisia) through three periods, in terms of treatment strategy;objective response rate (ORR) to chemotherapy;median overall survival (OS) and prognostic factors conditioning OS. Methods: This was a retrospective study covering the period from January 1994 to December 2011. It involved 401 patients with mCRC divided into three groups: group 1 (G1) including patients treated in the period 1994-1998, group 2 (G2) including patients treated in the period 1999-2005 and group 3 (G3) including patients treated in the period 2006-2011. Results: The ORR after 3 cycles of first line chemotherapy was 33%, 50% and 57.8% for G1, G2 and G3 respectively. The median OS was 13.8 months for G1, 19 months for G2 and 23 months for G3. The prognostic factors associated with a better OS for G3 were absence of initial tumor complication, normal initial level of carcinoembryonic antigen, normal liver function tests, surgery of the primary tumor, surgery of liver metastases and the ORR after three cycles of first line chemotherapy. Conclusion: The development of the therapeutic strategy, whatever of medical or surgical nature, has led to improved response rates and survival of patients treated for mCRC.

Tuberculosis Arthritis of the Sternoclavicular Joint

Tuberculosis remains a public health problem in developing country particularly in Tunisia. Bone location of tuberculosis infection is increasing and is often misdiagnosed due to the weakness of clinical presentation in early stages. Sternoclavicular joint tuberculosis is rare and unusual location of this disease. However, antibiotherapy and surgical debridement is still the basis of treatment. We report a case of sternoclavicular joint tuberculosis with a follow up of four years. The patient was treated surgically and put under antibiotherapy during twelve months. The site was sterilized. We report this case to show that debridement and antibiotherapy still efficient in tuberculous bone affection no matters the location.

Place of Tranexamic Acid and Fibrinogen Association in the Management of Severe Postpartum Haemorrhage

Introduction: Severe postpartum hemorrhage (PPH) is the main cause of death in Tunisia. Its management is multidisciplinary and requires perfect knowledge of a regularly updated protocol and consistent with available resources. We propose an evaluation of the impact of different therapeutic attitudes, in particular the combination of tranexamic acid and fibrinogen concentrates in the management of this hemorrhage. Material and Methods: This was a retrospective, descriptive and analytical study, conducted in the departments of Gynecology-Obstetrics and Surgical Resuscitation anesthesia of the Farhat Hached university Hospital, sousse, Tunisia. The study was conducted over six years (2009-2014), and included all parturients who were treated for severe PPH that occurred on a term greater than 24 weeks of amenorrhea. Patients were divided into two groups according to the study period: 1) Group 1 (G1): from 1 January 2009 to 31 December 2011, 2) Group 2 (G2): from January 1, 2012 to December 31, 2014, a group that benefited particularly from the combination of tranexamic acid and fibrinogen concentrates for the management of their PPH. Results: 166 patients were included: 57 in G1 and 109 in G2. The overall incidence of severe PPH was 3.15/1000 deliveries. We noted a significant decrease in the fall of hemoglobin in per and post haemorrhagic manifestations in favor of G2 with a p value of 0.003 and p = 0.025, respectively. The use of blood transfusion decreased significantly, in frequency and in number of packed red cells per patient, between the two groups;transfusion ratio was 1/1.7/1.5 for G1, and 1/2/1.8 for G2. Fluid therapy, use of macromolecules and catecholamines were less important in G2. The use of tranexamic acid, fibrinogen and the association of both increased significantly between the two groups (p 10-4). Haemostasis hysterectomies were less performed (p 10-3) in G2. Patellofemoral Outcome after Medial Unicompartmental Knee Replacement: A Retrospective Study of 25 Cases

Introduction: Osteoarthritis and impingement, in patellofemoral compartment after partial arthroplasty, have been rarely reported. The purpose of our study was to evaluate outcome at long-term follow-up. Methods: We have reviewed retrospectively twenty-five unicompartmental arthroplasties. The medial parapatellar approach was used in all cases. Patients have been evaluated with regard to patellar impingement and osteoarthritic changes revealed by radiographs. The mean follow-up was 10.4 years. The clinical scoring system of the Knee Society and specific questions regarding patellofemoral symptoms were used for functional outcome. Patellofemoral complications such as osteoarthritis, impingement and the position of the femoral component were diagnosed on X-ray views of the knee. Results: At the final follow-up, fifteen knees revealed lateral patellofemoral osteoarthritis and four knees had patellofemoral impingement. Excepted of two cases, all knees with patella impingement did not revealed degenerative modifications. Anterior pain of the knee was noted more frequently in knees with patellar complications. Patellar impingement was considered the first cause of knee pain, more than knees with degenerative modifications. Only one revision surgery was indicated in front of patellofemoral conflict. The occurrence of patellar impingement depends on the surgical procedure. The anterior position of the femoral component seems to increase the frequency of such complication. Conclusion: Several degenerative modifications may occur in the patello-femoral compartment following partial knee arthroplasty. Patellar impingement and total knee arthritis are the most reported modifications. Functional outcome may be affected progressively when these changes become obvious. Patellar impingement is the major cause of knee pain and needs revision.

CD4+/CD56+ Hematodermic Neoplasm Presenting in the Skin: A Tunisian Case Report and Current Review of the Literature

The CD4+/CD56+ hematodermic neoplasm is a rare aggressive systemic neoplasm for which effective therapies have not yet been established, it is clinically characterized by cutaneous involvement with spread to bone marrow, blood and poor prognosis with current chemotherapy regimens. Our objective is to report diagnosis and treatment difficulties of CD4+/CD56+ hematodermic neoplasm. We describe here a Tunisian man who presented with subcutaneous ulcerated lesion localized in the right leg and multiples generalized nodules. Skin biopsy showed an atypical lymphoid cell infiltration with an angiocentric pattern and extensive necrosis by immuno-histochemical analysis, these cells were positive for CD4, CD56, granzyme B and negative for CD8, CD123, CD20 and CD30. T-cell rearrangement and Epstein-Barr-virus (EBV) in situ hybridation studies were negative. The patient underwent 5 cycles chemotherapy SMILE regimen monthly sandwiched with radiotherapy on the residual lesions of the right leg with great tolerance but he relapsed within 8months with skin, blood, bone marrow, lung, and cerebrospinal involvement. Based on these findings, the patient was diagnosed with CD4+/CD56+ hematodermic neoplasm (blastic NK-like T-cell lymphoma) treated with one course of hyper-CVAD regimen, he died within 20 days with a septic chok. Despite the use of L-Asparaginase and radiotherapy the prognosis is very poor;we suggest the exploration for highly active drugs, hematopoietic stem cell transplantation (HSCT) is crucial to improve survival.

Beneficial Effect of an Oral Antioxidant Supplementation (Fertimax2) on IVF-ICSI Outcomes: A Preliminary Clinical Study

Background: Considerable evidence points towards a significant role of oxidative stress (OS) in the pathogenesis of sperm dysfunction. OS as a result of an inappropriate balance between oxidants and antioxidants in the semen can cause DNA damage and lipid peroxidation leading to failure of conception, miscarriage or potentially even childhood cancer. The objective of this study was to investigate whether a male antioxidant therapy can improve semen parameters and the results of in vitro fertilization (IVF) and intracytoplasmic sperm injection (ICSI). Methods: A total of forty eight infertile couples were considered. Male participants were administrated Fertimax2 antioxidant treatment for at least two months prior to their partner’s IVF-ICSI cycle. Sperm parameters (volume, concentration, progressive motility) and the IVF-ICSI outcomes were compared before and after the antioxidant treatment. The primary outcome measures were oocyte fertilization, cleavage and good embryo quality rates;the secondary outcomes were biochemical pregnancies, clinical pregnancies and implantation rates. Results: The principal finding that emerged from this study was that antioxidant therapy resulted in significant improvements in fertilization (p = 00.2), cleavage (p = 0.004) and good-embryo quality (p = 0.002) rates accompanied by a marked increase in clinical pregnancy (18.7% versus 2.5%) and implantation (11.8% versus 1.02%) rates. No significant changes in routine sperm parameters were observed. Conclusion: The Fertimax2 antioxidant therapy appears to influence favorably chances of conception in couples undergoing assisted reproduction treatment (ART).
