Application of ultrasonography-elastography score to suspect porto-sinusoidal vascular disease in patients with portal vein thrombosis

Background:Porto-sinusoidal vascular disease(PSVD)and portal vein thrombosis(PVT)are causes of portal hypertension characterized respectively by an intrahepatic and a pre-hepatic obstacle to the flow in the portal *** PVT may be a consequence of PSVD,in PVT patients at presentation,a pre-existing PSVD should be *** these patients the identification of an underlying PSVD would have relevant implication regarding follow-up and therapeutic management,but it could be *** this setting ultrasonography may be valuable in differential *** aim of the study was to use ultrasonography to identify parameters to discriminate between PSVD and“pure”PVT and then to suspect PVT secondary to a pre-existing ***:Fifty-three patients with histologically proven PSVD and forty-eight patients affected by chronic PVT were enrolled and submitted to abdominal ultrasonography with elastography by acoustic radiation force impulse(ARFI).Results:ARFI was higher and superior mesenteric vein(SMV)diameter was wider in PSVD patients than in PVT ***,a prognostic score was obtained as linear combinations of the two parameters with a good discrimination capacity between PSVD and PVT(the area under the curve=0.780;95%confidence interval:0.690-0.869).Conclusions:A score based on ARFI and SMV diameter may be useful to suspect an underlying PSVD in patients with PVT and to identify a subgroup of patients to be submitted to liver biopsy.

ABCB1 and ABCB19 auxin transporters have synergistic effects on early and late Arabidopsis anther development

Arabidopsis abcbl abcb19 double mutants defective in the auxin transporters ABCB1/PGP1 and ABCB19/PGP19 are altered in stamen elongation, anther dehiscence and pollen maturation. To assess the contribution of these transporters to stamen development we performed phenotypic, histological analyses, and in situ hybridizations on abcbl and abcbl9 single mutant flowers. We found that pollen maturation and anther dehiscence are precocious in the abcbl but not in the abcb19 mutant. Accordingly, endothecium ligniflcation is altered only in abcbl anthers. Both abcbl and abcb1 abcb19 stamens also show altered early development, with asynchronous anther Iocules and a multilayer tapetum. DAPI staining showed that the timing of meiosis is asynchronous in abcbl abcb19 anther Iocules, while only a small percentage of pollen grains are non- viable according to Alexander＇s staining. In agreement, TAM （TARDY ASYNCHRONOUS MEIOSIS）, as well as BAM2 （BARELY ANY MERISTEM）involved in tapetal cell development--areoverexpressed in abcbl abcb19 young flower buds. Corre- spondingly, ABCB1 and ABCB19 mRNA localization supports the observed phenotypes of abcbl and abcbl abcb19 mutant anthers. In conclusion, we provide evidence that auxin transport plays a significant role both in early and late stamen development： ABCB1 plays a major role during anther development, while ABCB19 has a synergistic role.

Strength and stiffness reduction factors for infilled frames with openings

Framed structures are usually infilled with masonry walls. They may cause a significant increase in both stiffness and strength, reducing the deformation demand and increasing the energy dissipation capacity of the system. On the other hand, irregular arrangements of the masonry panels may lead to the concentration of damage in some regions, with negative effects; for example soft story mechanisms and shear failures in short columns. Therefore, the presence ofinfill walls should not be neglected, especially in regions of moderate and high seismicity. To this aim, simple models are available for solid infills walls, such as the diagonal no-tension strut model, while infilled frames with openings have not been adequately investigated. In this study, the effect of openings on the strength and stiffness of infilled frames is investigated by means of about 150 experimental and numerical tests. The main parameters involved are identified and a simple model to take into account the openings in the infills is developed and compared with other models proposed by different researchers. The model, which is based on the use of strength and stiffness reduction factors, takes into account the opening dimensions and presence of reinforcing elements around the opening. An example of an application of the proposed reduction factors is also presented.

Thermal and catalytic pyrolysis of a synthetic mixture representative of packaging plastics residue

Biochronology,paleobiogeography and faunal turnover in western Mediterranean Cenozoic mammals

Cenozoic terrestrial mammals from Sardinia contribute substantial information for reconstructing the complex his-tory of the western *** occurrence of endemic perissodactyls in Eocene marine and marsh deposits suggests the existence of ecological or physical barriers between the Corso-Sardinian massif and the Iberian-Occitanic *** the end of the Oligocene,isolation of Sardinia was almost complete,although a migration from Europe occurred at the beginning of the Early Miocene,as indicated by the unbalanced endemic fauna from *** the Late Miocene,the Tusco-Sardinian palaeobioprovince came into existence as an isolated region inhab-ited by the quite diversified,but notably endemic,Oreopithecus *** was definitely isolated from Tuscany by the Messinian,but temporary connections with the European mainland possibly allowed the colonization of forerunners of some Sardinian Pliocene *** the Plio-Pleistocene,Sardinia maintained permanent ***,sea level drop,resulting in a relatively short distance between Sardinia and the European mainland,allowed different migratory *** the Late Pliocene to the Late Pleistocene–Holocene,two main mammalian faunal complexes(FC)can be recognized:the Nesogoral FC(Late Pliocene-Early Pleistocene)and the Microtus(Tyrrhenicola)FC(late Early Pleistocene–Early Holocene).At the transition from Nesogoral to Microtus(Tyrrhenicola)FC,approximately 47%of the genera and 76%of the species disappeared,while approximately 58%of the genera and 71%of the species appeared.A noticeable turnover followed the arrival of Neolithic man and his accompanying ***,Praemegaceros was still present at about 7000 years BP,while Microtus(Tyrrhenicola)and Prolagus are respectively recorded in the Bronze and Iron Ages.

Accuracy of the automated cell counters for management of spontaneous bacterial peritonitis

AIM： To evaluate the accuracy of automated blood cell counters for ascitic polymorphonuclear （PMN） determination for：（1） diagnosis, （2） efficacy of the ongoing antibiotic therapy, and （3） resolution of spontaneous bacterial peritonitis （SBP）. METHODS： One hundred and twelve ascitic fluid samples were collected from 52 consecutive cirrhotic patients, 16 of them with SBP. The agreement between the manual and the automated method for PMN count was assessed. The sensitivity/specificity and the positive/negative predictive value of the automated blood cell counter were also calculated by considering the manual method as the ＂gold standard＂ RESULTS： The mean ＋ SD of the difference between manual and automated measurements was 7.8 4- 58 cells/ram3, while the limits of agreement were ＋124 cells/mm3 [95% confidence interval （CI）：＋145 to ＋103] and -108 cells/mm3 （95% CI： -87 to -129）. The automated cell counter had a sensitivity of 100% and a specificity of 97.7% in diagnosing SBP, and a sensitivity of 91% and a specificity of 100% for the efficacy of the ongoing antibiotic therapy. The two methods showed a complete agreement for the resolution of infection. CONCLUSION： Automated cell counters not only have a good diagnostic accuracy, but are also very effectivein monitoring the antibiotic treatment in patients with SBP. Because of their quicker performance, they should replace the manual counting for PMN determination in the ascitic fluid of patients with SBP.

Neurotrophic factors and corneal nerve regeneration

The cornea has unique features that make it a useful model for regenerative medicine studies. It is an avascular, transparent, densely innervated tissue and any pathological changes can be easily detected by slit lamp examination. Corneal sensitivity is provided by the ophthalmic branch of the trigeminal nerve that elicits protective reflexes such as blinking and tearing and exerts trophic support by releasing neuromediators and growth factors. Corneal nerves are easily evaluated for both function and morphology using standard instruments such as corneal esthesiometer and in vivo confocal microscope. All local and systemic conditions that are associated with damage of the trigeminal nerve cause the development of neurotrophic keratitis, a rare degenerative disease. Neurotrophic keratitis is characterized by impairment of corneal sensitivity associated with development of persistent epithelial defects that may progress to corneal ulcer, melting and perforation. Current neurotrophic keratitis treatments aim at supporting corneal healing and preventing progression of corneal damage. Novel compounds able to stimulate corneal nerve recovery are in advanced development stage. Among them, nerve growth factor eye drops showed to be safe and effective in stimulating corneal healing and improving corneal sensitivity in patients with neurotrophic keratitis. Neurotrophic keratitis represents an useful model to evaluate in clinical practice novel neuro-regenerative drugs.

Body size structure of Pleistocene mammalian communities: what support is there for the “island rule”?

Islands are often regarded by scientists as living laboratories of evolution and an optimal context for the study of forces influencing evolution and *** main issues have been attentively scrutinized and debated:the loss of biodiversity and the peculiar changes undergone by island settlers,primarily changes in size of endemic *** time,several hypotheses have been formulated to explain the causal mechanism of body size *** of those islands where mainland taxa migrate more than once provide the most interesting data to answer the question of whether or not trends of insular taxa result from a predictable response to differences in competition and availability of niches between insular and mainland *** contribute to the debate,the body size structure of the Pleistocene mammalian faunas from two Mediterranean islands,Sicily and Crete,were analyzed and compared with the structure of coeval mainland *** results obtained suggest that:(i)size of endemic species does not directly depend on the area of islands;(ii)evolution and size of endemic species seems somewhat affected by the degree of isolation(constraining coloniza-tion from mainland)and physiography(sometimes permitting adaptive radiation);(iii)in unbalanced insular communities,the shift in size of non-carnivorous species largely depends on the nature of competing species;and(iv)body size of carnivorous species mainly depends on the size of the most available ***,it is rational to suppose that the body size of insular mammals mainly results from the peculiar biological dynamics that characterizes unbalanced insular *** interaction,particularly the intraguild competition,is the major driver behind the evolution of insular communities,leading towards an optimization of energy balance through a change in body size of endemic settlers.

Ascitic fluid analysis for diagnosis and monitoring of spontaneous bacterial peritonitis

Polymorphonuclear (PMN) cell count in the ascitic fluid is essential for the diagnosis and management of spontaneous bacterial peritonitis (SBP). To date, PMN cell count is routinely performed by traditional manual counting. However, this method is time-consuming, costly, and not always timely available. Therefore, considerable efforts have been made in recent years to develop an alternative test for a more rapid diagnosis and monitoring of SBP. The use of urinary reagent strips was proposed to achieve an "instant" bedside diagnosis of SBP. A series of reports evaluated the urine strip test for SBP diagnosis and reported promising results. However, a recent large multicenter study revealed a surprising lack of diagnostic effi cacy of the urine screening test for SBP diagnosis. Another method, more recently proposed as an alternative to the manual PMN count, is the measurement of lactoferrin in ascitic fluid, but the data available on the diagnostic value of this test are limited to a single study. However, both urinary reagent strips and ascitic lactoferrin tests are qualitative methods and need, therefore, to be further confirmed by standard cytology of the ascitic fluid. To date, the only quantitative method proposed as a valid alternative to manual PMN counting is automated blood cell counters, commonly used in all laboratories for blood cell counting. Data available in the literature on the diagnostic performance of this method are limited but very promising, and this tool seems to have the potential to replace the manual counting method.
