Background:Given the pervasive issues of obesity and diabetes both in puerto rico and the broader United States,there is a compelling need to investigate the intricate interplay among body mass index(BMI),pregesta-tional,and gestational maternal diabetes,and their potential impact on the occurrence of congenital heart defects(CHD)during neonatal development.Methods:Using the comprehensive System of Vigilance and Surveillance of Congenital Defects in puerto rico,we conducted a focused analysis on neonates diagnosed with CHD between 2016 and 2020.Our assessment encompassed a range of variables,including maternal age,gestational age,BMI,pregestational diabetes,gestational diabetes,hypertension,history of abortion,and presence of preeclampsia.Results:A cohort of 673 patients was included in our study.The average maternal age was 26 years,within a range of 22 to 32 years.The mean gestational age measured 39 weeks,with a median span of 38 to 39 weeks.Of the 673 patients,274(41%)mothers gave birth to neonates diagnosed with CHD.Within this group,22 cases were linked to pre-gestational diabetes,while 202 were not;20 instances were associated with gestational diabetes,compared to 200 without;and 148 cases exhibited an overweight or obese BMI,whereas 126 displayed a normal BMI.Conclusion:We identiﬁed a statistically signiﬁcant correlation between pre-gestational diabetes mellitus and the occurrence of CHD.However,our analysis did not show a statistically signiﬁcant association between maternal BMI and the likelihood of CHD.These results may aid in developing effective strategies to prevent and manage CHD in neonates.

Effect of bond length and radius on superconducting transition temperature for FeAs-based superconductors

By comparing the data of lattice parameters of more than 50 new FeAs-based high temperature superconductors in two syetems, the effect of bondlength on superconducting transition temperature (T C ) was found that, for both FeAs superconductor systems with similar ionic radii of cation A, the bond length L As-A between Arsenic atom and its nearest neighbor cation at the A site is in an inverse proportion to T C , i.e. the larger the bond length L As-A , the lower the T C . In addition, we also found a noticeable effect of ionic radius on T C , in which deviation from the tendency line of bond length vs. T C decreases with the increasing ionic radius. Both bond length-T C and ionic radii-T C relations indicate that the interaction of cation A and As ion have significant effect on superconductivity. The analysis on the electronic structure indicates that there exists the proximity of the secondary-outer p-orbit of the cation A and the 4s orbit of the As ion, both in energy space and in real space. Some high frequency individual vibrating modes would be established through exchange coupling on their inner orbits. It is worth of mention that the superconducting condensation is influenced by the interaction between the cation A and the As atoms.

This editorial commented on an article in the World Journal of Gastroenterology titled“Risks of Reactivation of Hepatitis B Virus in Oncological Patients Using Tyrosine Kinase-Inhibitors:Case Report and Literature Analysis”by Colapietro et al.In this editorial,we focused on providing a more comprehensive exploration of hepatitis B virus reactivation(HBVr)associated with the usage of tyrosine kinase inhibitors(TKIs).It includes insights into the mechanisms underlying HBV reactivation,the temporal relationship between TKIs and HBV reactivation,and preventive measures.The aim is to understand the need for nucleos(t)ide analogs(NAT)and serial blood tests for early recognition of reactivation and acute liver injury,along with management strategies.TKIs are considered to be an intermediate(1%-10%)of HBVr.Current guidelines stipulate that patients receiving therapy with high or moderate risks of reactivation or recent cancer diagnosis must have at least tested hepatitis B surface antigen,anti-hepatitis B core antigen(HBc),and anti-hepatitis B surface antibody.Anti-HBc screening in highly endemic areas means people with negative tests should be vaccinated against HBV.Nucleoside or nucleotide analogs(NAs)like entecavir(ETV),tenofovir disoproxil fumarate(TDF),and tenofovir alafenamide(TAF)form the basis of HBV reactivation prophylaxis and treatment during immunosuppression.Conversely,lamivudine,telbivudine,and adefovir are generally discouraged due to their reduced antiviral efficacy and higher risk of fostering drug-resistant viral strains.However,these less effective NAs may still be utilized in cases where ETV,TDF,and TAF are not feasible treatment options.

Academic Stress among Nursing Students in a Private university at puerto rico, and Its Association with Their Academic Performance

Background: Nursing programs have distinguished themselves for being rigorous. The college experience emerges with new challenges and demands that require adaptation in order to cope with academic stress. Situation that could lead to a non adaptive institutional environment. Objective: The purpose of this research was to determine and analyze possible repercussions of academic stress in nursing students in a puerto Rican private university and how it affects academic performance in students. Materials and Methods: The selected methodology is a cross-sectional study. The simple random sample was comprised of 168 nursing students who were enrolled in associate degree (29.2%) and bachelor’s degree programs in nursing (70.8%). Data was collected through standardized Academic Stress (CEA, by its acronym in Spanish). Results: The study shows that academic stressors are correlated with the grade point average. In addition, correlation between academic stress and the performance rate was found, especially in terms of the difficulty to approve enrolled credits. Conclusions: Nursing students suffer from stress regardless their academic degree. Academic stress affects academic performance of students despite their use of management strategies.

Prevalence of refractive errors in children of puerto rico

AIM:To determine the prevalence of refractive error in 5-to 17-year-old schoolchildren in puerto rico.METHODS:A quantitative descriptive study of 2867 children aged 5 to 17y from all seven educational regions of puerto rico was conducted from 2016–2019.Refractive error was determined via static and subjective refraction.Children with distance acuity≤20/40 or near visual acuity≤20/32 had a cycloplegic refraction.Data analysis included descriptive statistics,correlation coefficient,Kruskal-Wallis,Chi-square,and t test calculations.RESULTS:Twenty percent of the children had a spherical equivalent refractive error≤-0.50 D,3.2%had a spherical equivalent≥+2.00 D,and 10.4%had astigmatism≥1 D.There was a statistically(but non-clinically)significant myopic change in spherical equivalent refractive error with age(Ppuerto rico.Individual children must be monitored to examine the need for treatment of myopia progression.

Predicting depression intensity from microblogs and social media posts has numerous benefits and applications,including predicting early psychological disorders and stress in individuals or the general public.A major challenge in predicting depression using social media posts is that the existing studies do not focus on predicting the intensity of depression in social media texts but rather only perform the binary classification of depression and moreover noisy data makes it difficult to predict the true depression in the social media text.This study intends to begin by collecting relevant Tweets and generating a corpus of 210000 public tweets using Twitter public application programming interfaces(APIs).A strategy is devised to filter out only depression-related tweets by creating a list of relevant hashtags to reduce noise in the corpus.Furthermore,an algorithm is developed to annotate the data into three depression classes:‘Mild,’‘Moderate,’and‘Severe,’based on International Classification of Diseases-10(ICD-10)depression diagnostic criteria.Different baseline classifiers are applied to the annotated dataset to get a preliminary idea of classification performance on the corpus.Further FastText-based model is applied and fine-tuned with different preprocessing techniques and hyperparameter tuning to produce the tuned model,which significantly increases the depression classification performance to an 84%F1 score and 90%accuracy compared to baselines.Finally,a FastText-based weighted soft voting ensemble(WSVE)is proposed to boost the model’s performance by combining several other classifiers and assigning weights to individual models according to their individual performances.The proposed WSVE outperformed all baselines as well as FastText alone,with an F1 of 89%,5%higher than FastText alone,and an accuracy of 93%,3%higher than FastText alone.The proposed model better captures the contextual features of the relatively small sample class and aids in the detection of early depression intensity prediction from tweet

Can lithium enhance the extent of axon regeneration and neurological recovery following peripheral nerve trauma?

The clinical"gold standard"technique for attempting to restore function to nerves with a gap is to bridge the gap with sensory autografts.However,autografts induce good to excellent recovery only across short nerve gaps,in young patients,and when repairs are performed a short time post nerve trauma.Even under the best of conditions,<50%of patients recover good recovery.Although many alternative techniques have been tested,none is as effective as autografts.Therefore,alternative techniques are required that increase the percentage of patients who recover function and the extent of their recovery.This paper examines the actions of lithium,and how it appears to trigger all the cellular and molecular events required to promote axon regeneration,and how both in animal models and clinically,lithium administration enhances both the extent of axon regeneration and neurological recovery.The paper proposes more extensive clinical testing of lithium for its ability and reliability to increase the extent of axon regeneration and functional recovery.

This paper considers stability analysis of a Susceptible-Exposed-Infected-Recovered-Virus(SEIRV)model with nonlinear incidence rates and indicates the severity and weakness of control factors for disease transmission.The Lyapunov function using Volterra-Lyapunov matrices makes it possible to study the global stability of the endemic equilibrium point.An optimal control strategy is proposed to prevent the spread of coronavirus,in addition to governmental intervention.The objective is to minimize together with the quantity of infected and exposed individuals while minimizing the total costs of treatment.A numerical study of the model is also carried out to investigate the analytical results.

The Intergovernmental Panel on Climate Change(IPCC)recognises the pivotal role of renewable energies in the future energy system and the achievement of the zero-emission target.The implementation of renewables should provide major opportunities and enable a more secure and decentralised energy supply system.Renewable fuels provide long-term solutions for the transport sector,particularly for applications where fuels with high energy density are required.In addition,it helps reducing the carbon footprint of these sectors in the long-term.Information on biomass characteristics feedstock is essential for scaling-up gasification from the laboratory to industrial-scale.This review deals with the transformation biogenic residues into a valuable bioenergy carrier like biomethanol as the liquid sunshine based on the combination of modified mature technologies such as gasification with other innovative solutions such as membranes and microchannel reactors.Tar abatement is a critical process in product gas upgrading since tars compromise downstream processes and equipment,for this,membrane technology for upgrading syngas quality is discussed in this paper.Microchannel reactor technology with the design of state-of-the-art multifunctional catalysts provides a path to develop decentralised biomethanol synthesis from biogenic residues.Finally,the development of a process chain for the production of(i)methanol as an intermediate energy carrier,(ii)electricity and(iii)heat for decentralised applications based on biomass feedstock flexible gasification,gas upgrading and methanol synthesis is analysed.
