Iron and Heart Failure: Current Concepts and Emerging Pharmacological Paradigms

Background: Emerging evidence has recognized that anemia and iron deficiency are recurrent comorbidities in chronic heart failure (HF) and several trials have established that iron administration improves myocardial asset and clinical scenario in HF. Purpose: Recent acquisitions suggest that iron deficiency represents a concrete bias in the pathogenetic mechanism of chronic HF, so we have investigated the putative role of the hepcidin/ferroportin axis in the cardiovascular setting to advocate novel pharmacological and clinical approaches. Methods: Here, after an excursus on iron metabolism, we first reviewed the ongoing studies on novel iron targeted compounds. Then, we summarize large clinical interventional studies conducted on patient suffering from iron deficiency and HF which have tested the effects of drugging iron regard QoL, hospitalizations and cardiovascular death. Results: Novel compounds such as hepcidin agonist (PTG 300), synthetic human hepcidin (LJPC-401) and anti FPN (Vamifeport) are ongoing in iron overloaded patients, while the hepcidin blocker (PRS-080) is under investigation in anemic patients. Noteworthy, novel insights could arise from the results of a Phase IV interventional study regarding the modification of hepcidin pathway in a large cohort of HF patients (n = 1992) by sodium glucose cotransporter 2 inhibitors. To date, several studies highlight the beneficial effect of iron administration in cardiovascular setting and latest evidences consider hepcidin level as a novel biomarker of cardiac injury and atherosclerosis. Conclusions: We advocate that data from ongoing studies will suggest novel iron targeted therapies for diagnosis, prognosis and therapy transferable in selected heart failed patients.

Study on crustal thickness and the prediction of prolific depressions:the Bohai Basin as an example

The present study is focused on evaluation of deep marine pelagic sediments of Late Cretaceous Kawagarh Formation of Kala-Chitta Range in the context of microfacies analysis,paleoenvironmental interpretation,planktonic foraminiferal biostratigraphy,sequence stratigraphy and diversification of species.A total of thirty three rock samples were collected from the measured *** microfacies are interpreted,namely planktonic foraminifera wackestone,planktonic foraminifera mudstone and sandy mudstone indicating low energy depositional environment i.e.,outer *** biostratigraphic studies show plentiful planktonic foraminifera species of Globotruncana,Heterohelix and ***,no association of benthic or siliceous organisms was *** the basis of available species assemblage,a single local planktonic foraminifera biozone i.e.,Globotruncana-Heterohelix-Globotruncanita Assemblage Biozone is *** biozone information is combined with published literature and Lower Santonian to Middle Maastrichtian age has been assigned to the Kawagarh *** trend of species occurrences evinces that species number decreases over time with pulsated rise in the Kawagarh *** Kawagarh Formation carbonates show an overall Transgressive Systems Tract(TST).The Kawagarh Formation of Pakistan evinces analogous characteristics to that of the Late Cretaceous Gurpi Formation of Iran based on the geologic age,outcrop lithology,microfacies,planktonic foraminiferal assemblages,depositional setting and sequence stratigraphy.

Influence of earthquake direction on the seismic response of irregular plan RC frame buildings

The nonlinear response of structures is usually evaluated by considering two accelerograms acting simultaneously along the orthogonal directions. In this study, the infl uence of the earthquake direction on the seismic response of building structures is examined. Three multi-story RC buildings, representing a very common structural typology in Italy, are used as case studies for the evaluation. They are, respectively, a rectangular plan shape, an L plan shape and a rectangular plan shape with courtyard buildings. Nonlinear static and dynamic analyses are performed by considering different seismic levels, characterized by peak ground acceleration on stiff soil equal to 0.35 g, 0.25 g and 0.15 g. Nonlinear dynamic analyses are carried out by considering twelve different earthquake directions, and rotating the direction of both the orthogonal components by 30° for each analysis(from 0° to 330°). The survey is carried out on the L plan shape structure. The results show that the angle of the seismic input motion signifi cantly infl uences the response of RC structures; the critical seismic angle, i.e., the incidence angle that produces the maximum demand, provides an increase of up to 37% in terms of both roof displacements and plastic hinge rotations.

Global Navigation Satellites Systems(GNSS)is frequently used for positioning services in various applications,e.g.,pedestrian and vehicular ***,it is well-known that GNSS positioning performs unreliably in urban *** shadow matching is a method of improving accuracy in the cross-street *** position and classification of observed satellite visibility between line-of-sight(LOS)and non-line-of-sight(NLOS)are essential for its *** the conventional LOS/NLOS classification,the classifiers are based on a single feature,extracted from raw GNSS measurements,such as signal noise ratio,pseudorange,elevation angle,*** in urban canyons,these measurements are unstable and unreliable due to the signal reflection and refraction from the surrounding ***,the conventional least square approach for positioning is insufficient to provide accurate initialization for shadow matching in urban *** our study,shadow matching is improved using the initial position from robust estimator and the satellite visibility determined by support vector machine(SVM).The robust estimator has an improved positioning accuracy and the classification rate of SVM classification can reach 91.5%in urban *** important issue is related to satellites with ultra-high or low elevation angles and satellites near the building boundary that are very likely to be *** solving this problem,the SVM classification shows the potential of about 90%classification accuracy for various urban *** the help of these approaches,the shadow matching has a mean error of 10.27 m with 1.44 m in the cross-street direction;these performances are suitable for urban positioning.

The increasing importance of treatment of lower urinary tract symptoms(LUTS),while avoiding side effects and maintaining sexual function,has allowed for the development of minimally invasive surgical therapies(MiSTs).Recently,the European Association of Urology guidelines reported a paradigm shift from the management of benign prostatic hyperplasia(BPH)to the management of nonneurogenic male *** aim of the present review was to evaluate the efficacy and safety of the most commonly used MisTs:ablative techniques such as aquablation,prostatic artery embolization,water vapor energy,and transperineal prostate laser ablation,and nonablative techniques such as prostatic urethral lift and temporarily implanted nitinol device(iTIND).MiSTs are becoming a new promise,even if clinical trials with longer follow-up are still *** of them are still under investigation and,to date,only a few options have been given as a recommendation for *** cannot be considered as standard of care and are not suitable for all *** and disadvantages should be underlined,without forgetting our objective:treatment of LUTS and re-treatment avoidance.
