Objective: Extracorporeal shockwave lithotripsy (SWL) currently plays an important role in the treatment of urinary tract lithiasis. The purpose of this article was to describe new concepts and procedural strategies that would improve results using SWL as a treatment for urolithiasis, thereby achieving better clinical practice.Methods: A systematic review process was carried in PubMed/PMC from January 2003 to March 2023. A narrative synthesis of the most important aspects has been made.Results: The important recommendations for the adequate selection of the candidate patient for treatment with SWL are summarized, as well as the new strategies for a better application of the technique. Aspects about intraoperative position, stone localization and monitoring, analgesic control, machine and energy settings, and measures aiming at reduced risk of complications are described.Conclusion: To achieve the therapeutic goal of efficient stone disintegration without increasing the risk of complications, it is necessary to make an adequate selection of patients and to pay special attention to several important factors in the application of treatment. Technological development in later generation devices will help to improve current SWL results.

The scientific community faces the challenge of measuring progress toward biodiversity targets and indices have been traditionally used.However,recent inventories in secondary tropical mountain forests using traditional biodiversity indices have yielded results that are indistinct with primary ones.This shows the need to develop complementary indices that goes beyond species count but integrates the distribution and conservation status of the species.This study developed endemicity and conservation importance index for tropical forest that incorporated the distribution and conservation status of the species.These indices were applied to Mt.Natoo,a remnant primary mossy forest in Buguias,Benguet,Philippines,that resulted to endemicity index of 81.07 and conservation importance index of 42.90.Comparing these with secondary forest sites with comparable Shannon-Wiener,Simpson,Evenness and Margalef’s indices,our endemicity and conservation indices clearly differentiates primary forest(our study site)with higher values from secondary forests with much lower values.Thus,we are proposing these indices for a direct but scientifically-informed identification of specific sites for conservation and protection in tropical forests.Additionally,our study documented a total of 168 vascular plant species(79 endemic and 12 locally threatened species)in Mt.Nato-o.Majority are of tropical elements for both generic and species levels with some temperate elements that could be attributed to the site's high elevation and semi-temperate climate.These are important baseline information for conservation plans and monitoring of tropical mossy forests.

This paper presents the quantitative research results of a comprehensive study titled“Current Profle of Older Adults in Castilla-la Mancha(Spain).”The study aims to understand and describe the characteristics of individuals aged 65 and above within this autonomous region,with a focus on the variables of sex,age,and habitat.A cross-sectional approach was employed,utilizing quantitative methods to collect data.To gather information,a customized questionnaire was developed to address the study objectives,covering various aspects such as sociodemographic factors,living conditions,leisure activities,social participation,and cultural interests.The survey was conducted via telephone,ensuring representation by employing gender quotas,age-group stratifcation,and considerations for habitat size.A representative sample of 1,065 participants was included in the study.The fndings reveal a degree of homogeneity in the lifestyles of older adults,with discernible variations infuenced by factors such as sex,habitat,and age.These results shed light on the diverse habits and experiences of older adults,emphasizing the signifcance of habitat conditions,social participation,and the aging process.

Background:Sports medicine(injury and illnesses)requires distinct coding systems because the International Classification of Diseases is insuf-ficient for sports medicine coding.The Orchard Sports Injury and Illness Classification System(OSIICS)is one of two sports medicine coding systems recommended by the International Olympic Committee.Regular updates of coding systems are required.Methods:For Version 15,updates for mental health conditions in athletes,sports cardiology,concussion sub-types,infectious diseases,and skin and eye conditions were considered particularly important.Results:Recommended codes were added from a recent International Olympic Committee consensus statement on mental health conditions in athletes.Two landmark sports cardiology papers were used to update a more comprehensive list of sports cardiology codes.Rugby union protocols on head injury assessment were used to create additional concussion codes.Conclusion:It is planned that OSIICS Version 15 will be translated into multiple new languages in a timely fashion to facilitate international accessibility.The large number of recently published sport-specific and discipline-specific consensus statements on athlete surveillance warrant regular updating of OSIICS.

Metastatic breast cancer leads to poor prognoses and worse outcomes in patients due to its invasive behavior and poor response to therapy.It is still unclear what biophysical and biochemical factors drive this more aggressive phenotype in metastatic cancer;however recent studies have suggested that exposure to fluid shear stress in the vasculature could cause this.In this study a modular microfluidic platform capable of mimicking the magnitude of fluid shear stress(FSS)found in human vasculature was designed and fabricated.This device provides a platform to evaluate the effects of FSS on MCF-7 cell line,an estrogen receptor positive(ER+)breast cancer cell line,during circulation in the vessels.Elucidation of the effects of FSS on MCF-7 cells was carried out utilizing two approaches:single cell analysis and bulk analysis.For single cell analysis,cells were trapped in a microarray after exiting the serpentine channel and followed by immunostaining on the device(on-chip).Bulk analysis was performed after cells were collected in a microtube at the outlet of the microfluidic serpentine channel for western blotting(off-chip).It was found that cells exposed to an FSS magnitude of 10 dyn/cm^(2) with a residence time of 60 s enhanced expression of the proliferation marker Ki67 in the MCF-7 cell line at a single cell level.To understand possible mechanisms for enhanced Ki67 expression,on-chip and off-chip analyses were performed for pro-growth and survival pathways ERK,AKT,and JAK/STAT.Results demonstrated that after shearing the cells phosphorylation of p-AKT,p-mTOR,and p-STAT3 were observed.However,there was no change in p-ERK1/2.AKT is a mediator of ER rapid signaling,analysis of phosphorylated ERαwas carried out and no significant differences between sheared and non-sheared populations were observed.Taken together these results demonstrate that FSS can increase phosphorylation of proteins associated with a more aggressive phenotype in circulating cancer cells.These findings provide additional information that may help

Climate change and extreme weather pose significant challenges to the traditional viticulture regions.Emerging high-altitude grape-producing regions with diverse orientations have shown great potential in coping with this challenge.Stable,high-quality wine grape production may be achieved by synchronizing the meso-and microclimate.To clarify the role of high altitude and row orientation in meso-and microclimate and the response of berries to it,we evaluated seven years(2012-2018)of climate data,two years of basic grape(Cabernet Sauvignon,Vitis vinifera L.)quality,and one-year microclimate from veraison to harvest.By comparing two locations(Sidon 2047 m,Sinon 2208 m)in Yunnan Province,China,we found that the average temperature has been stable at approximately 15℃ for seven years,with no extreme weather or,noticeable global warming.The light intensity(LI)in the north-south(NS)was more balanced than the east-west(EW)direction,and the east-west to the south(EW-S)canopy side was almost higher than the other sides.High LI was associated with high photosynthetically active radiation(PAR),ultraviolet(UV),and infrared(IR)light and vice versa.The north-south to the east(NS-E)and east-west to the north(EWN)sides were characterized by lower LI and higher UV and IR light,and higher total anthocyanin content.Most anthocyanin synthesis-related genes,for example,VvF3'H and VvF3'5'H,were highly expressed in NS-E from veraison to maturity.Perhaps UV and IR light induced their expression.This study provides new insights on the role of differently orientated rows in controlling grape quality due to varied light quality.The findings are globally significant,particularly in the context of climate change,and offer fresh insights into berry physiological responses and decision-making for the management of existing vineyards.

This study delves into the combustion behavior of various lignite types within a circulating fluidized bed boiler(CFBB),with a primary focus on the impact of different bed material sphericity ratios(0.5,0.7,and 0.9).Utilizing bed material with a sphericity ratio of 0.9 sourced from theÇan power plant and verified through experimentation,the research reveals several key findings.Notably,furnace temperatures tended to rise with higher sphericity ratios,albeit with variations between lignite types,particularly highlighting the complexity of this relationship in the case of GLI-Tunçbilek lignite.Pressure levels in the combustion chamber remained consistent across different sphericity ratios,indicating minimal influence on pressure dynamics.Improved combustion efficiency,especially at the bottom of the boiler,was observed at lower sphericity levels(0.5 and 0.7)forÇan lignite,as reflected in CO_(2) mole fractions.While NO_(x) emissions generally decreased with lower sphericity,the sensitivity to sphericity varied by lignite type,with Ilgın lignite showcasing low NO_(x) but high SO_(2) emissions,underscoring the intricate interplay between lignite properties,sphericity,and emissions.Overall,this study advances our understanding of CFBB combustion dynamics,offering insights valuable for optimizing performance and emissions control,particularly in lignite-based power.

In many countries, political and environmental pressures are currently combining to generate a perfect storm of circumstances that is reducing food availability, increasing food costs and thereby reducing the availability of food to many. The UK is currently considering new national food and land management policies, and attention is also being given to legislation to address diet-related health issues. Many now argue for a revolution in UK farming practices to reduce their impact on the natural environment. The UK is not alone in facing these and other challenges. Both the contribution of agriculture to greenhouse gas(GHG) emissions and the effects of climate change on food production are issues receiving worldwide attention.Regenerative agricultural practices can result in greater C capture, reduced GHG emissions, enhanced soil quality and enhanced biodiversity. However, it is questioned if such farming systems will be productive enough to feed a growing population with the food required for social and health benefits. To fully exploit the impact of new plant science in farmer fields, it is imperative to effectively link science to farming practices and conduct a broader conversation around the food revolution with social scientists and with the general public.
