Background: Birth preparedness and complication readiness (BPCR) promotes timely information and utilization of skilled health attendance/services to pregnant women to prevent complications due to 3 delays that lead to maternal deaths. Objective: To determine nurses’ role in birth preparedness and complication readiness among pregnant women in university of calabar teaching hospital calabar. Methods: This study adopted a survey research design. Simple random sampling technique was used to select a sample size of 80 participants working in antenatal clinic, antenatal ward, gynecological ward, labour ward, postnatal ward, and Family planning unit. Data were collected using self-administered structured questionnaires. Data were analyzed using frequency, percentage and Pearson Moment Product Correlation Coefficient statistical methods of data analysis. Results: Findings of the study revealed that 53 (66.3%) of the respondents acknowledged nurse/midwives responsibility for the health education of mothers on dangers of pregnancy while 42 (52.5%) strongly agreed that nurses/midwives carry out discussion on plan for delivery with mothers. The study hypothesized that there is no significant relationship between the role of nurse/midwives and the practice of BPCR among pregnant women. The correlational analysis result that there is no revealed (r-cal = 0.67, r-crit = 0.58, df = 79, P > 0.05) thus rejecting the hypothesis statement as r-calculated was greater than r-critical. This implies that the role of nurse/midwives have very strong positive relationship with the practice of BPCR among pregnant women. Conclusion: Positive pregnancy outcome depends on nurse/midwives independent and interdependent roles to prevent delays, emergencies, have access to skilled care.

Objective:To ascertain the antimicrobial susceptibility profile of Pseudomonas aeruginosa(P. aeruginosa) recovered from surgical site infections(SSIs).Methods:The study was retrospective in nature and was compiled for a period of five years(1st February,2004-31st January,2009).Data were generated from the culture of post-operative wound swab specimens by the microbiology laboratory of university of calabar teaching hospital,Relevant information from the patients’ records was compiled,such as age,gender,type of surgical procedure,microorganisms recovered and their antibiotic sensitivity *** data was analysed by using Epi Info 6 statistical ***:Of the 4 533 wound swab specimens processed,673 were culture positive and P. aeruginosa was recovered from 13.1%of the culture positive specimens with its rate of recovery decreasing with age progression(P0.05).Most of the P. aeruginosa isolates were from general surgery wards and least from orthopaedic ***, ceftriaxone and augmentin were the most active antibiotics while ampicillin,tetracycline and cotrimoxazole were the least active antibiotics,with no antibiotic having a 100%activity against the ***:In view of the high resistance displayed by *** recovered from SSIs,adequate antiseptic procedures should be entrenched to avoid colonization of surgical wounds by this microorganism as well as others with similar sensitivity ***, ceftriaxone and augmentin may be considered for prevention of *** infection.

This research systematically examined the degradation,antibacterial effects,and biocompatibility of micro-arc oxidation(MAO)coatings with nano CuO and ZnO on extruded Mg *** copper(Cu)and Zinc(Zn)possess antibacterial *** findings demonstrated that adding ZnO will appreciably reduce the degradation rate of MAO-coating alloy due to the self-sealing micro ***+MAO coating exhibited excellent antibacterial performance,with an antibacterial rate of over 90%within 6 h co-cultured with Staphylococcus ***,the antibacterial rate of ZnO+MAO coating reached 90%after 12 h *** test using MG63 cell indicated that the incorporation of CuO and ZnO did not notably affect the cell viability rate of the ***,after 14 days of culture,the CuO+MAO and ZnO+MAO coated samples exhibited higher alkaline phosphatase(ALP)activity than the MAO-coated and uncoated samples,suggesting favorable osteogenic properties.

BACKGROUND The efficacy and safety of anti-tumor necrosis factor-α(TNF-α)monoclonal antibody therapy[adalimumab(ADA)and infliximab(IFX)]with therapeutic drug monitoring(TDM),which has been proposed for inflammatory bowel disease(IBD)patients,are still *** To determine the efficacy and safety of anti-TNF-αmonoclonal antibody therapy with proactive TDM in patients with IBD and to determine which subtype of IBD patients is most suitable for proactive TDM *** As of July 2023,we searched for randomized controlled trials(RCTs)and observa-tional studies in PubMed,Embase,and the Cochrane Library to compare anti-TNF-αmonoclonal antibody therapy with proactive TDM with therapy with reactive TDM or empiric *** and network meta-analyses were used to determine the IBD patient subtype that achieved clinical remission and to determine the need for *** This systematic review and meta-analysis yielded 13 studies after exclusion,and the baseline indicators were *** found a significant increase in the number of patients who achieved clinical remission in the ADA[odds ratio(OR)=1.416,95%confidence interval(CI):1.196-1.676]and RCT(OR=1.393,95%CI:1.182-1.641)subgroups and a significant decrease in the number of patients who needed surgery in the proactive vs reactive(OR=0.237,95%CI:0.101-0.558)and IFX+ADA(OR=0.137,95%CI:0.032-0.588)subgroups,and the overall risk of adverse events was reduced(OR=0.579,95%CI:0.391-0.858)according to the pairwise ***,the network meta-analysis results suggested that patients with IBD treated with ADA(OR=1.39,95%CI:1.19-1.63)were more likely to undergo TDM,especially in comparison with patients with reactive TDM(OR=1.38,95%CI:1.07-1.77).CONCLUSION Proactive TDM is more suitable for IBD patients treated with ADA and has obvious advantages over reactive *** recommend proactive TDM in IBD patients who are treated with ADA.

OBJECTIVE: Exploring the effect of Optimized New Shengmai powder(优化新生脉散方, ONSMP) on myocardial fibrosis in heart failure(HF) based on rat sarcoma(RAS)/rapidly accelerated fibrosarcoma(RAF)/mitogen-activated protein kinase kinase(MEK)/extracellular regulated protein kinases(ERK) signaling pathway. METHODS: Randomized 70 Sprague-Dawley rats into sham(n = 10) and operation(n = 60) groups, then established the HF rat by ligating the left anterior descending branch of the coronary artery. We randomly divided the operation group rats into the model, ONSMP [including low(L), medium(M), and high(H) dose], and enalapril groups. After the 4-week drug intervention, echocardiography examines the cardiac function and calculates the ratios of the whole/left heart to the rat's body weight. Finally, we observed the degree of myocardial fibrosis by pathological sections, determined myocardium collagen(COL) Ⅰ and COL Ⅲ content by enzyme-linked immunosorbent assay, detected the m RNA levels of COL Ⅰ, COL Ⅲ, α-smooth muscle actin(α-SMA), and c-Fos proto-oncogene(c-Fos) by universal real-time, and detected the protein expression of p-RAS, p-RAF, p-MEK1/2, p-ERK1/2, p-ETS-like-1 transcription factor(p-ELK1), p-c-Fos, α-SMA, COL Ⅰ, and COL Ⅲ by Western blot. RESULTS: ONSMP can effectively improve HF rat's cardiac function, decrease cardiac organ coefficient, COL volume fraction, and COL Ⅰ/Ⅲ content, down-regulate the m RNA of COL Ⅰ/Ⅲ, α-SMA and c-Fos, and the protein of p-RAS, p-RAF, p-MEK1/2, p-ERK1/2, p-ELK1, c-Fos, COL Ⅰ/Ⅲ, and α-SMA. CONCLUSIONS: ONSMP can effectively reduce myocardial fibrosis in HF rats, and the mechanism may be related to the inhibition of the RAS/RAF/MEK/ERK signaling pathway.

Medical dialogue generation(MDG) is applied for building medical dialogue systems for intelligent consultation. Such systems can communicate with patients in real time, thereby improving the efficiency of clinical diagnosis. However, predicting correct entities and correctly generating distinct responses remain a great challenge. Inspired by actual doctors’ responses to patients, we consider MDG a two-stage task: entity prediction and dialogue generation. For entity prediction, we design an ent-mac post pre-training strategy by leveraging external medical entity knowledge to enhance the pre-trained model. For dialogue generation, we propose an entity-aware fusion MDG method in which predicted entities are integrated into the dialogue generation model through different encoding fusion mechanisms, using information from different sources. Because the diverse beam search algorithm can produce responses with entities that deviate from the predicted entities, an entity-revised diverse beam search is proposed to correct the entities entailed in the generated responses and make the generated responses more distinct. The experimental results on the China Conference on Knowledge Graph and Semantic Computing 2021(A/B tests) and the International Conference on Learning Representations 2021(online test) datasets show that the proposed method outperforms several state-of-the-art methods, which demonstrates its practicability and effectiveness.
