A 41- year- old Japanese woman presented at a local outpatient clinic because of a slightly pruritic pustular eruption on the trunk and extremities. Her skin lesion had been treated using an antifungal cream, but the condition had continued to worsen. According to the patient, the same pustular eruption had occurred during pregnancy, but diminished after delivery. She has no family history of skin eruption. On physical examination, she presented with well- circumscribed erythematous macules with small pustules on the borders, involving the axilla, groin, upper and lower extremities and upper back (Fig. 1). Bacterial and fungal cultures of the lesions were negative. A skin biopsy obtained from a lesion in the left forearm demonstrated a subcorneal pustule containing polymorphonuclear leukocytes (Fig. 2). A perivascular dermal infiltrate was present, consisting predominantly of mononuclear cells. There was no spongiosis or acantholysis. On direct immunofluorescence, there were no immunoglobulinA (IgA) and complement 3 (C3) deposits in the intercellular space of the epidermis. The clinical and histopathological findings were consistent with a diagnosis of subcorneal pustular dermatosis (SPD). Laboratory investigations showed hyper immunoglobulin A (IgA) immunoglobulinemia (479 mg/dL, normal, 110- 410) and a high titer of antinuclear antigen (speckled type) at 1: 160 (normal < 1: 40) and anti- single- strand (anti- ss) DNA- IgG at 1: 25 (normal: negative). Other findings were normal. Serum and urine immunoelectrophoresis showed no paraprotein bands. Administration of topical diflupredonate ointment and oral prednisolone improved but did not completely resolve the eruption. The patient was then treated with dapsone 75 mg daily, leading to reasonable control. Influence of Medical Model on Therapeutic Environment

In its development history, man has always tried to be close to nature, and get physical and spiritual help from the nature. According to the development of medical model from the deity worship to modern medical science, a near-nature and ecological therapeutic and recuperative model has been more favored, thus in modern therapeutic environment, proper landscape designs can be combined with effective medical methods to achieve physical and mental immunoregulation and balance using natural and quality climatic factors. This paper introduced the development of natural science and medicinal model in different periods, and analyzed their impact on environment, concluded the benefi ts of therapeutic medicine, and demonstrated the signifi cance of therapeutic environment and landscape healing as the therapeutic factors for facilitating human rehabilitation from the perspective of therapeutic medicine. Taking "evidence-based design" as the basis of therapeutic environment design, this paper proposed landscape design methods for the therapeutic environment, in order to provide people opportunities for closing to, sharing and participating in the nature, help people to relax physically and psychologically using the positive impact of natural environment on human spirits, relieve their stress, and achieve physical and psychological peace. In addition, the development tendency of therapeutic environment in China was predicted according to the actual conditions of urban society.

Reliability of foam posturography in assessment of postural balance in the patients with vertigo

The postural balance is a important aspect of assessment in vestibular rehabilitation *** the computerized dynamic posturography,the foam posturo-graphy performed by using foam in posturography is a useful measurement tool for postural ***,the reliability of foam posturography is critical in *** our knowledge,there is no report about the test-retest reliability of foam posturography in the patients with ver-tigo in *** foam posturography was taken on the patients with vertigo in stable state and repeated in 3 to 5 *** the two assessments of test and retest,all subjects stood under 4 sensory conditions,such as firm surface with eyes open(T1),firm surface with eyes closed(T2),foam surface with eyes open(T3)and foam surface with eyes closed(T4)for 30 seconds *** average sway velocity(SV)of center of pressure(COP)under feet of upright standing was recorded as research *** results of patients with vertigo were compared with those of normal *** demonstrated that all normal subjects did not fall in the two *** were 10 of 34 patients with vertigo at least fell one time during the two assessments,and the SVs of these subjects did not taken the analysis of test-retest *** results showed that:(1)The interclass correlation coefficients(ICC)of all 49 subjects including normal subjects and patients without falling were 0.887–0.973 according to the T1–T4 between two assessment.(2)The SVs between the normal subjects and patients were significantly differ-ent in the T2(t 52.018,P 50.049)and T4(t 53.905,P,0.001)in the first assessment and in the T4(t 53.715,P 50.001)in the ***(3)the cases of falling between two assessments were not significant different(x250.073,P 50.787)in the patients with *** is concluded from this study that the foam posturography has high test-retest reliability to assess the postural bal-ance in the patients with vertigo in stable state,indicating the foam pos

Introduction: Centropontine myelinolysis (CPM) is a rare pathology, a delayed neurological complication corresponding to massive demyelination of the protrusion. Its exact pathogenesis is poorly understood. Rapid correction of sodium hyponatremia has been implicated as a potent causative factor. We report a case of CPM despite a priori conservative correction of hyponatremia with a favorable course in a 61-year-old alcoholic-smoker diabetic. Case Presentation: A 61-year-old man with chronic alcoholism presented to the emergency department (D0) with physical asthenia and anorexia. He was treated for severe hyponatremia at 104 mmol/L by careful rehydration with saline before being transferred to a nephrological hospital. Magnetic resonance imaging (MRI) performed at D14 for locked-in syndrome showed osmotic demyelination syndrome (Figure 1). The evolution was favorable after 3 months of rehabilitation marked by a progressive and clear improvement of clinical signs. Conclusion: This observation suggests an evaluation of the benefit/risk ratio of the short-term prognosis of profound hyponatremia with that of the metabolic stress induced by a still too rapid correction. Particular attention should be paid to diabetic patients in the context of chronic alcoholism or nutritional deficiencies.

physical rehabilitation is an effective therapy to normalize weaknesses encountered with neurological disorders such as traumatic brain injury(TBI).However,the efficacy of exercise is limited during the acute period of TBI because of metabolic dysfunction,and this may further compromise neuronal *** we discuss the possibility to normalize brain metabolism during the early post-injury convalescence period to support functional plasticity and prevent long-term functional *** BDNF possesses the unique ability to support molecular events involved with the transmission of information across nerve cells through activation of its TrkB receptor,the poor pharmacokinetic profile of BDNF has limited its therapeutic *** flavonoid derivative,7,8-dihydroxyflavone(7,8-DHF),signals through the same TrkB receptors and results in the activation of BDNF signaling *** discuss how the pharmacokinetic limitations of BDNF may be avoided by the use of 7,8-DHF,which makes it a promising pharmacological agent for supporting activity-based rehabilitation during the acute post-injury period after *** turn,docosahexaenoic acid(C22:6n-3;DHA)is abundant in the phospholipid composition of plasma membranes in the brain and its action is important for brain development and *** is a major modulator of synaptic membrane fluidity and function,which is fundamental for supporting cell signaling and synaptic *** influences DHA function by normalizing DHA content in the brain,such that the collaborative action of exercise and DHA can be instrumental to boost BDNF function with strong therapeutic potential for reducing the deleterious effects of TBI on synaptic plasticity and cognition.

Neurologic complications and neurodevelopmental outcome with extracorporeal life support

Extracorporeal life support is used to support patients of all ages with refractory cardiac and/or respiratory failure. Extracorporeal membrane oxygenation(ECMO)has been used to rescue patients whose predicted mortality would have otherwise been high. It is associated with acute central nervous system(CNS) complications and with long- term neurologic morbidity. Many patients treated with ECMO have acute neurologic complications, including seizures, hemorrhage, infarction, and brain death. Various pre-ECMO and ECMO factors have been found to be associated with neurologic injury, including acidosis, renal failure, cardiopulmonary resuscitation, and modality of ECMO used. The risk of neurologic complication appears to vary by age of the patient, with neonates appearing to have the highest risk of acute central nervous system complications. Acute CNS injuries are associated with increased risk of death in a patient who has received ECMO support. ECMO is increasingly used during cardiopulmonary resuscitation when return of spontaneous circulation is not achieved rapidly and outcomes may be good in select populations. Economic analyses have shown that neonatal and adult respiratory ECMO are cost effective. There have been several intriguing reports of active physical rehabilitation of patients duringECMO support that is well tolerated and may improve recovery. Although there is evidence that some patients supported with ECMO appear to have very good outcomes, there is limited understanding of the longterm impact of ECMO on quality of life and long-term cognitive and physical functioning for many groups, especially the cardiac and pediatric populations. This deserves further study.
