Alcoholism and cirrhosis,which are two of the most serious health problems worldwide,have a broad spectrum of clinical *** diseases are influenced by genetic susceptibility and cultural traits that differ globally but are specific for each *** contrast to other regions around the world,Mexicans present the highest drinking score and a high mortality rate for alcoholic liver disease with an intermediate category level of per capita alcohol *** has a unique history of alcohol consumption that is linked to profound anthropological and social *** Mexican population has an admixture genome inherited from different races,Caucasian,Amerindian and African,with a heterogeneous distribution within the ***,genes related to alcohol addiction,such as dopamine receptor D2 in the brain,or liver alcoholmetabolizing enzymes,such as alcohol dehydrogenase classⅠpolypeptide B,cytochrome P450 2E1 and aldehyde dehydrogenase class 2,may vary from one individual to ***,they may be inherited as risk or non-risk haplogroups that confer susceptibility or resistance either to alcohol addiction or abusive alcohol consumption and possibly liver ***,in this era of genomics,personalized medicine will benefit patients if it is directed according to individual or population-based *** association studies will be required to establish novel strategies for the prevention,care and treatment of liver disease in Mexico and worldwide.

Conditioned Taste Aversion Diminishes Sugar Intake

Animals learn to avoid particular food when some of its sensory properties are associated with gastrointestinal discomfort. Twenty rats were exposed to free access to food and a sugar solution for ten days in first phase. During second phase, experimental group received a dose of LiCl. Control group received an injection without LiCl. Both groups had free access to a sugar solution and food restriction for three days. In the final phase, both groups returned to the conditions of first phase. Results showed a significant decrease in sugar intake after aversive conditioning regarding the intake registered in the initial phase in experimental group. Control group did not show any differences in its sugar intake before and after the experimental manipulation. The procedure carried out is discussed as a means to decrease sugar intake.

CD36 genetic variation, fat intake and liver fibrosis in chronic hepatitis C virus infection

AIM To analyze the association of the CD36 polymorphism(rs1761667) with dietary intake and liver fibrosis(LF) in chronic hepatitis C(CHC) patients. METHODS In this study, 73 patients with CHC were recruited. The CD36 genotype(G > A) was determined by a TaqM an real-time PCR system. Dietary assessment was carried out using a three-day food record to register the daily intake of macronutrients. Serum lipids and liver enzymes were measured by a dry chemistry assay. LF evaluated by transient elastography(Fibroscan~)and APRI score was classified as mild LF(F1-F2) and advanced LF(F3-F4).RESULTS Overall, the CD36 genotypic frequencies were AA(30.1%), AG(54.8%), and GG(15.1%), whereas the allelic A and G frequencies were 57.5% and 42.5%, respectively. CHC patients who were carriers of the CD36 AA genotype had a higher intake of calories attributable to total fat and saturated fatty acids than those with the non-AA genotypes. Additionally, aspartate aminotransferase(AST) serum values were higher in AA genotype carriers compared to non-AA carriers(91.7 IU/L vs 69.8 IU/L, P = 0.02). Moreover, the AA genotype was associated with an increase of 30.23 IU/L of AST(β = 30.23, 95%CI: 9.0-51.46, P = 0.006). Likewise, the AA genotype was associated with advanced LF compared to the AG(OR = 3.60, 95%CI: 1.16-11.15, P = 0.02) or AG + GG genotypes(OR = 3.52, 95%CI: 1.18-10.45, P = 0.02).CONCLUSION This study suggests that the CD36(rs1761667) AA genotype is associated with higher fat intake and more instances of advanced LF in CHC patients.

Treatment Processes of Leachate from a Landfill by Advanced Oxidation Fenton and Ozone-UV

In this paper it is presented the results of advanced oxidation of leachates from a technified sanitary landfill located in the State of Querétaro, Mexico. One characteristic of already stabilized leachates from sanitary landfills like this case, is their difficult degradation, mainly because the organic matter contained is recalcitrant. For the samples collect, four sites were selected, where three points per site were sampled, measuring at each site the parameters: temperature, pH, conductivity, redox potential (ORP) and dissolved oxygen (DO) and leachate samples were collected. On the other hand, the Chemical Oxygen Demand (COD) of crude leachates, leachates acidified and leachates oxidized by Fenton reagent and Ozone-UV combined were analyzed. COD was used to monitor the degradation kinetics. With the results, the ArcGIS software was applied to study the distribution of temperature, dissolved oxygen and COD mainly in the leachate lagoon. For the application of Fenton reagent in the crude leachate oxidation, the pH was first adjusted and Fe2+/H2O2 ratio was optimized. The efficiency of Ozone-UV treatments was studied through COD degradation kinetics. The graphs of in (Ci/Co) vs time, showed that the kinetic processes are of order one, with very acceptable regression coefficients (R2) and extraordinarily similar speed constants (K). With Fenton oxidation, the highest percentage of COD degradation was achieved and with Ozone-UV oxidation, it was possible to practically degrade all the COD.

Relationship between Change of Diet and Poverty in Mexico: A Stochastic Analysis

Background: In this article, we seek to break the paradigm of traditional estimates (deterministically) to estimate the probability of transition from poverty and diet change in Mexico through a stochastic model while providing a comparative study in the time between the diet change and poverty. Methods: A model based on the theory of Markov applied to the different dimensions of poverty and diet type from aggregate data from government agencies was used. Also likely future state changes were estimated and Monte Carlo simulation was used to find a balance between the transition probabilities of the different states. Results: It was shown that there was a high probability of consuming more fat than protein and carbohydrates in Mexico. In the case of poverty, it was found that poverty of patrimony presented the highest probability of change. Estimates for 2030 show as well that the Mexican population will have equal probabilities of state transition to the type of diet and poverty, as long as you consider changing some current values of both consumption and poverty. Conclusions: It was shown that there was indeed a close relationship between poverty of patrimony and an unbalanced diet where the probability of fat intake was high. The stochastic approach had enabled us, in addition to linking poverty and changing diet, to prevent the Mexican population of future scenarios that could be dramatic and, to avoid this situation, alternatives of change of state consumption and poverty had been proposed.

Synthesis and Characterization of MgSb2O6 Trirutile-Type in Low Presence Concentrations of Ethylenediamine

The nanoparticles MgSb206 with trirutile-type structure were prepared by colloidal method using ethylanediamine （0.5 and 0.75 mL）, following a calcination at 800 ℃. After, those powders were analyzed by x-ray obtaining a tetragonal structure with cell parameters, a = 4.64 A and c = 9.25 A and special group P42/mnm. Using a scanning electron microscopy were analyzed the powders calcined at 800 ℃, showing the formation of micro-plates, micro-rods, nanoparticles and morphology as rice grains. Employing a transmission electron microscopy, were found nanostructures hexagonal shaped with sizes of- 33.6 nm （0.5 mL） and 28.6 nm （0.75 mL）.

Efficacy of the vaccination in inflammatory bowel disease

Inflammatory bowel disease (IBD) is associated with conditions that may predispose to infections, such as the lack of an appropriate innate immune response to infectious agents, malnutrition, surgery, and immunosuppressive and biological drugs. Some of these infections may be preventable by vaccination. Therefore, for this particular patient population, the benefits of implementing a well-established immunization protocol in daily clinical practice are potentially even greater than for the general population. In recent years international consensus guidelines have been published, but in spite of theses recommendations, studies have shown that a significant number of patients with IBD remain inadequately immunized. Another important issue regarding immunization in this population is that vaccine efficacy among patients receiving immunosuppressive therapies has been variable. In a healthy population, a humoral immune response to hepatitis B vaccination (HBV) is expected in > 90%, whereas a much lower rate is achieved in the IBD patients. Immunosuppressive, anti-tumor necrosis factor therapy and disease activity have been implicated in the impaired efficacy of the vaccination. The serological response to HBV should be confirmed and patients with an inadequate response should receive a second full series of vaccine. Modified dosing regimens, including doubling the standard antigen dose, might increase the effectiveness. Response to influenza, pneumococcal and tetanus immunization is still not clear, as there are studies that show a normal response to the vaccination while others demonstrate a lack of efficacy. We pose a series of questions on the efficacy of the different vaccinations recommended for IBD patients and attempt to answer them using scientific evidence.

Protein tolerance to standard and high protein meals in patients with liver cirrhosis

To investigate the plasma amino acid response and tolerance to normal or high protein meals in patients with cirrhosis. METHODSThe plasma amino acid response to a 20 g mixed protein meal was compared in 8 biopsy-proven compensated cirrhotic patients and 6 healthy subjects. In addition the response to a high protein meal (1 g/kg body weight) was studied in 6 decompensated biopsy-proven cirrhotics in order to evaluate their protein tolerance and the likelihood of developing hepatic encephalopathy (HE) following a porto-caval shunt procedure. To test for covert HE, the “number connection test” (NCT) was done on all patients, and an electroencephalogram was recorded in patients considered to be at Child-Pugh C stage. RESULTSThe changes in plasma amino acids after a 20 g protein meal were similar in healthy subjects and in cirrhotics except for a significantly greater increase (P < 0.05) in isoleucine, leucine and tyrosine concentrations in the cirrhotics. The baseline branched chain amino acids/aromatic amino acids (BCAA/AAA) ratio was higher in the healthy persons and remained stable-but it decreased significantly after the meal in the cirrhotic group. After the high protein meal there was a marked increase in the levels of most amino acids, but only small changes occurred in the levels of taurine, citrulline, cysteine and *** BCAA/AAA ratio was significantly higher 180 and 240 min after the meal. Slightly elevated basal plasma ammonia levels showed no particular pattern. Overt HE was not observed in any patients. CONCLUSIONPatients with stable liver disease tolerate natural mixed meals with a standard protein content. The response to a high protein meal in decompensated cirrhotics suggests accumulation of some amino acids but it did not precipitate HE. These results support current nutritional guidelines that recommend a protein intake of 1.2-1.5 g/kg body weight/day for patients with cirrhosis.

Ultra-high molecular weight polyethylene (UHMWPE) has been used in orthopedics as one of the materials for artificial joints in knee, hip and spine prostheses, most of the implanted joints are designed so that the metal of the prosthesis is articulate against a polymeric material, however the main problems is the average life time of the UHMWPE due to wear, and the particles generated by the friction of the metal on the articulation of the polymer are the most common inducer of osteolysis, generating a loosening of the implant leading to an imminent failure resulting in the total replacement of the prosthesis. In this investigation a numerical model of abrasive wear was made using the classic Archard wear equation applied to dynamic simulation of finite element analysis (FEA) of the micro-abrasion test using a subroutine written in Fortran language linked to the finite element software to predict the rate of wear. The results of the numerical model were compared with tests of abrasive wear in the laboratory, obtaining a margin of error below 5%,concluding that the numerical model is feasible for the prediction of the rate of wear and could be applied in knowing the life cycle of joint prostheses or for the tribological analysis in industrial machinery or cutting tools. The wear coefficient (K) was obtained from the grinding tests depending on the depth of stroke of the crater, which was analyzed by 3D profilometry to obtain the wear rate and the wear constant.

Hepatitis C virus-specific cytotoxic T cell response restoration after treatment-induced hepatitis C virus control

Hepatitis C virus(HCV)-specific cytotoxic T cell(CTL) response plays a major role in viral control during spontaneous infection resolution. These cells develop an exhausted and pro-apoptotic status during chronic onset, being unable to get rid of HCV. The role of this response in contributing to sustained viral response(SVR) after anti-HCV is controversial. Recent studies show that after successful interferon-based anti-HCV treatment, HCV traces are still detectable and this correlates with a peak of HCV-specific CTL response activation, probably responsible for maintaining SVR by subsequent complete HCV clearing. Moreover, SVR patients' serum is still able to induce HCV infection in na?ve chimpanzees, suggesting that the infection could be under the control of the immune system after a successful treatment, being transmissible in absence of this adaptive response. At least theoretically, treatmentinduced viral load decrease could allow an effective HCV-specific CTL response reestablishment. This effect has been recently described with anti-HCV interferonfree regimes, based on direct-acting antivirals. Nevertheless, this is to some extent controversial with interferon-based therapies, due to the detrimental immunoregulatory α-interferon effect on T cells. Moreover, HCV-specific CTL response features during anti-HCV treatment could be a predictive factor of SVR that could have clinical implications in patient management. In this review, the recent knowledge about the role of HCV-specific CTL response in the development of SVR after anti-HCV treatment is discussed.
