Background: Rosacea is a skin problem not uncommonly encountered world-wide. There is a need for an effective and well-tolerated treatment for this disease. Objective: To evaluate the efficacy and side-effects of zinc sulfate in rosacea in a randomized, controlled, double-blind trial. Patients and methods: Patients with rosacea who attended the outpatient clinic of Dermatology and Venereology in Baghdad Teaching hospital were recruited into this study between October 2002 and August 2004. A disease severity score was calculated for each patient. The patients were randomly allocated to receive either zinc sulfate 100 mg or identical placebo capsules three times per day. Zinc sulfate and placebo capsules were given in a double blind manner. Following 3 months of starting the treatment, the patients crossed over, i.e. patients on placebo crossed over to zinc sulfate and those on zinc sulfate crossed over to placebo. Results: Twenty-five patients with rosacea were included in this study: 16 (64% ) females and nine (36% ) males. Nineteen patients completed the study: 11 (58% ) females and eight (42% ) males. Patient age ranged from 21 to 64 years with a mean ± SD of 48.2 ± 9.3 years. Duration of the disease ranged from 1 to 14 years with a mean ± SD of 4.4 ± 3.2 years. In the group started on zinc sulfate, the score before therapy ranged from 5 to 11 with a mean ± SD of 8 ± 2.0. The mean started to decrease directly after the first month of therapy with zinc sulfate to a significantly lower level. After shifting to placebo treatment, the mean started to rise gradually in the fifth month but remained significantly lower than the levels before therapy. In the group started on placebo, the score before therapy ranged from 5 to 9 with a mean ± SD of 7 ± 1.3. The mean remained high in the first 3 months of therapy while the patients were on placebo. Access to Supportive Health Services for People with Physical Disabilities: A Case of Health Facilities in Singida Rural District, Tanzania

Background: It is estimated that above one billion people (15%) of the world’s populations are living with disabilities. The poor and unfriendly infrastructure at health facilities is the core challenge for people with physical disabilities in accessing healthcare. This study aimed to explore at what extent the existing infrastructure and design of health facilities in Singida rural district, Tanzania supports people with physical disabilities to access healthcare. Methods: A cross sectional health facility-based assessment of all thirty-two functioning health facilities in the district was done between June and December 2020 using observational checklist and key informants’ interviews consist of measurement procedures of the architectural condition of health facilities. Observation checklist was designed based on standard of health facilities in Tanzania, national guideline for safe care standards for dispensaries, health centers and district hospital of 2014 and national guideline for water, sanitation and hygiene in healthcare facilities of 2017. Data were analyzed by SPSS-26 using descriptive statistics to obtain frequencies tables, percentage and figure. Qualitative data were analyzed by using NVivo-12 software. Results: The study revealed that there was no signage, entrance area, parking and toilets designated for people with physical disabilities in all health facilities. There was no special seat, examination table/bed and special weight scale for people with physical disabilities in all facilities. Also, the doors were not wide enough to support wheelchair user to maneuver in all health centers and dispensaries. In addition, door handles were not user friendly for wheelchair users. In fact, only the hospital and health centers had wheelchairs while 27 (96.4%) of all dispensaries had no wheelchairs. Furthermore, ramp and pathways were not available in all dispensaries. Background: Skin diseases are the most common cause of referrals to general practitioners and dermatologists, and are considered a health problem in developing countries. The aim of this study was to report the extent and patterns of skin diseases in southern Iran. Methods: All new cases of skin diseases referred to Shahid Mohammadi hospital, Bandar Abbas, Southern Iran over a 3-year period were retrospectively considered. The dermatologic diagnoses, sex and age were recorded for each patient and the diagnoses were classified according to the International Classification of Diseases (ICD-10). Results: A total of 6841 patients were referred to the dermatology clinic of the hospital (48.27%male and 51.73%female). Infectious and parasitic diseases, including infections of the skin and subcutaneous tissues, were found to be the most common skin diseases (32.1%), followed by dermatitis and eczema (24.5%), disorders of the skin and subcutaneous tissue such as pigmentation disorders (9.7%), papulosquamous disorders (3.8%), urticaria and erythema (2.8%), burns, corrosion and unspecified injuries (3.3%), radiation-related disorders (2.3%), neoplasm(0.8%) and diseases of the oral cavity, salivary glands and jaw(0.3%). Conclusions: It appears that certain skin diseases such as dermatitis and eczema, superficial mycoses and scabies cause serious health problems in the region. Emphasis should be placed on this group of common skin diseases at all levels of personal health care, and suitable public health policies should be implemented in order to manage this problem rationally.

Objective: To elucidate the possible mechanisms of immunotherapy for unexplained recurrent aborters using their husband’s mononuclear cells. Design: Prospective clinical study. Setting: Institutional practice at the Outpatient clinic for Infertility, Niigata university Medical hospital. Patient(s): Fifty-two unexplained recurrent aborters were chosen as an experimental group. Intervention(s): Each patient was injected with her husband’s mononuclear cells as immunotherapy. Peripheral blood was obtained from the patients. Main Outcome Measure(s): The percentage of CD4-positive cells, TH1 cells, TH2 cells, and the TH1/TH2 ratio were analyzed in the patients before and after immunotherapy. The same analyses were performed in the successful and the unsuccessful group. Result(s): To date, 42 of the 52 patients have become newly pregnant. Of the 42, 34 patients have already delivered (successful group) and 3 are now pregnant, while the remaining 8 cases experienced repeated abortion (unsuccessful group). The percentage of TH2 cells significantly increased in the total patient population, while the TH1/TH2 ratio significantly decreased in the total patient population and in the successful group. Conclusion(s): These findings suggest that immunotherapy with the husband’s mononuclear cells for unexplained recurrent abortion induces a dominant state of TH2 cells in the patients.

Objectives. The goal of this study was to examine quality of life and mood in long- term survivors of cervical and endometrial cancer. Relationships of quality of life and mood with demographic and disease factors were also investigated. Methods. One-hundred fifty-two survivors of cervical or endometrial cancer diagnosed and treated 5- 20 years ago at the university of Iowa hospitals and clinics participated. Eighty-nine healthy controls were recruited from a hospital gynecology clinic. Quality of life, mood, and demographics were assessed by questionnaires. Disease characteristics of cancer survivors were abstracted from medical records. Results. There were no significant differences in quality of life or depressive symptoms between the three groups. Cervical cancer survivors reported significantly more anxiety than endometrial cancer survivors, and more dysphoria, anger, and confusion than either endometrial cancer survivors or healthy controls. Greater depression and mood disturbance were reported by unemployed and unmarried cancer survivors. Treatment modality, stage of disease, and length of time since diagnosis were not related to quality of life or mood. Conclusions. Quality of life in cervical and endometrial cancer survivors approximates that of healthy controls by 5 years post-diagnosis. However, cervical cancer survivors report more negative mood than survivors of endometrial cancer or healthy controls. Cancer survivors who are unemployed or living alone may be especially at risk for mood and mental health difficulties.

Background: Zinc deficiency is associated with impaired immune function and an increased risk of infection. Supplementation can decrease the incidence of diarrhoea and pneumonia in children in resource-poor countries. However, in children with HIV- 1 infection, the safety of zinc supplementation is uncertain. We aimed to assess the role of zinc in HIV- 1 replication before mass zinc supplementation is recommended in regions of high HIV- 1 prevalence. Methods: We did a randomised double-blind placebo-controlled equivalence trial of zinc supplementation at Grey’s hospital in Pietermaritzburg, South Africa. 96 children with HIV- 1 infection were randomly assigned to receive 10 mg of elemental zinc as sulphate or placebo daily for 6 months. Baseline measurements of plasma HIV- 1 viral load and the percentage of CD4+ T lymphocytes were established at two study visits before randomisation, and measurements were repeated 3, 6, and 9 months after the start of supplementation. The primary outcome measure was plasma HIV- 1 viral load. Analysis was per protocol. Findings: The mean log10 HIV- 1 viral load was 5.4 (SD 0.61) for the placebo group and 5.4 (SD 0.66) for the zinc-supplemented group 6 months after supplementation began (difference 0.0002, 95% CI - 0.27 to 0.27). 3 months after supplementation ended, the corresponding values were 5.5 (SD 0.77) and 5.4 (SD 0.61), a difference of 0.05 (- 0.24 to 0.35). The mean percentage of CD4+ T lymphocytes and median haemoglobin concentrations were also similar between the two groups after zinc supplementation. Two deaths occurred in the zinc supplementation group and seven in the placebo group (p=0.1). Children given zinc supplementation were less likely to get watery diarrhoea than those given placebo. Watery diarrhoea was diagnosed at 30 (7.4% ) of 407 clinic visits in the zinc-supplemented group versus 65 (14.5% ) of 447 visits in the placebo group (p=0.001). Interpretation. Purpose: To report the clinical and histopathological features of malignant eyelid tumor cases treated in our clinic. We also compared the differences in the frequency of malignant eyelid tumor in various regions of Japan and worldwide. Methods: Retrospectively, we studied the records of the 38 cases of malignant eyelid tumor treated in Yamagata university hospital over the last 17 years. The statistical comparison with various countries was based on reports of case studies in those countries. Results: Data from our clinic: Among the total of 38 cases, 15 cases (39.5% )were diagnosed as basal cell carcinoma, 11 cases (28.9% ) as sebaceous gland carcinoma, and 4 cases (10.5% ) as squamous cell carcinoma. In addition, three cases were malignant melanoma, two adenocarcinoma, one Merkel cell carcinoma, one malignant peripheral nerve sheath tumor, and one malignant lymphoma. The ages of patients ranged from 45 to 92 years (mean, 72.0 ± 12.4 years). Most of the cases were treated by complete resection of the tumors and eyelid reconstruction. Radiation or cryotherapy were added when required. The prognosis of the cases with basal cell carcinoma and squamous cell carcinoma was good, and that of the other tumors was relatively poor. During the same period, in Caucasians, basal cell carcinoma constituted about 80% - 90% of the malignant eyelid tumors, whereas in Japan and Asian countries, basal cell carcinoma, sebaceous gland carcinoma, and squamous cell carcinoma each constituted about 20% - 40% . Conclusions: A racial difference in the incidence of basal cell carcinoma, sebaceous gland carcinoma, and squamous cell carcinoma can be considered in making a diagnosis.

UVB靶向光疗治疗斑块型银屑病

Objective: To determine whether targeted UV-B phototherapy is efficacious and safe in the treatment of localized psoriasis and whether there is a dose-response relationship. Design: Randomized, evaluator-blind, controlled study. Setting: Dermatology clinic in a large university-based hospital in Bangkok,Thailand. Patients: Fourteen patients with stable, localized, plaque-type psoriasis. Interventions: Patientswere randomized to receive different fluences of targeted UV-B phototherapy 3 times weekly based on predetermined minimal erythema doses(MEDs). Treatment fluences were constant throughout the study period of 4weeks. Follow-upwas carried out until lesions returned to original state. Main Outcome Measures: Modified psoriasis area and severity index. Results: All fluences of UV-B produced some clinical improvement and were very well tolerated. Fluences ranging from 1 to 6 multiples of MEDs resulted in clearance of lesions in some patients with 6 MEDs producing clearance in 77% of patients. The number of treatments required to clear psoriatic lesions when 2 to 6MEDs were used was 5.0 to 6.1 treatments. The only adverse events observed were erythema, which was asymptomatic in most subjects, and hyperpigmentation. Conclusions: Incoherent, targeted UV-B phototherapy is a safe and efficacious treatment modality for localized psoriasis. Its value in other UV-B responsive conditions should be further investigated.
