Epidemiology of ocular surface squamous neoplasia in veterans: a retrospective casecontrol study

Background:A number of risk factors have been evaluated in ocular surface squamous neoplasia,but few studies have assessed risk factors specific to the armed forces veteran ***:We conducted a retrospective case-control study on 55 patients and 55 age-matched controls with biopsy-proven ocular surface squamous neoplasia from the Miami Veterans Administration hospital Eye clinic to investigate potential risk factors encountered by veterans,including service-specific ***-specific risk factors included ionizing radiation exposure,Agent Orange exposure,deployment to Southwest Asia,and exposure to the series of biochemical warfare tests known as Project Shipboard Hazard and *** was analyzed with SPSS(SPSS Inc.,Chicago,IL)using t-tests,chi-squared,and logistic regression analysis,with a p-value of<0.05 considered statistically ***:The strongest risk factor for ocular surface squamous neoplasia was lifetime sun exposure both directly assessed via historical quantification of exposure by dermatology practitioners(Odds Ratio(OR)5.4,95%Confidence Interval(CI)2.27–12.847,p<0.005),and using the surrogate markers of basal cell carcinoma(OR 3.157,95%CI 1.286–7.748,p=0.010)and pingueculae(OR 5.267,95%CI 2.104–13.186,p<0.005).Of the veteran-specific risk factors,Agent Orange exposure and Southwest Asia deployment were not associated with an increased risk of ocular surface squamous *** to ionizing radiation and involvement in Project Shipboard Hazard and Defense were not documented among any cases or ***:The results of our study are consistent with prior established risk factors,namely highlighting the important role of sun exposure in ocular surface squamous neoplasia among veterans.

Detection and Characterization of Methicillin Resistant Staphylococcus aureus from Toilet and Classroom Door Handles in Selected Secondary Schools in Nairobi County

Background: Staphylococcus aureus is found on all surfaces especially in public areas like hospitals and schools and on frequently touched areas like toilet and classroom door handles. Methicillin resistant Staphylococcus aureus (MRSA) is a strain of Staphylococcus aureus which is resistant to methicillin. There are two types of MRSA: Community acquired methicillin resistant Staphylococcus aureus (CA-MRSA) and hospital acquired methicillin resistant Staphylococcus aureus (HA-MRSA). MRSA in the community presents a significant reservoir that could enter into healthcare facilities and spread among patients and also a risk for immune compromised persons in the community. Methodology: The study aimed at determining the prevalence of MRSA isolated from toilet and classroom door handles as a potential source of infection to the students and the workers in selected schools in Nairobi, Kenya. The study also compared the prevalence of MRSA between boarding and non-boarding girls, boys and mixed (both girls and boys in the same school) secondary schools. Twelve secondary schools in Nairobi County were randomly selected and 306 samples from both the toilet and classroom door handles were collected using sterile swabs and transported to the laboratory. Isolation of Staphylococcus aureus was done by the use of selective media Mannitol salt agar, antibiotic susceptibility of isolates was done by disk diffusion method, and molecular detection of mecA and PVL genes were done by polymerase chain reaction (PCR). Results: The prevalence of S. aureus was 20% and 15% were MRSA positive by both Antimicrobial Susceptibility Test and PCR detection. 20% showed the presence of PVL genes, 8% showed the presence of both genes and 56% of isolates with mecA gene had PVL genes. Conclusion: The presence of MRSA in this study emphasizes the need to formulate hygiene measures to prevent possible spread of MRSA and other transmissible pathogens to students and workers in the schools.

Factors Affecting Japanese HPV-Vaccination: Findings from the Semi-Structured Interviews with Adolescent Girls and Caregivers

The objective of the present study was to qualitatively assess the obstructive and facilitative factors affecting adolescent girls and their caregivers when the adolescent had received or was considering receiving the Human Papilloma Virus (HPV) vaccination. Using these data, we propose recommendations for medical and nursing staff concerned with HPV vaccination. Participants were 20 adolescent girls (aged 10 - 19 years) and their caregivers, who had visited any of the 3 pediatric clinics in the Tokyo metropolitan area during a specified period since HPV vaccination began in Japan. The girls and their caregivers were separately interviewed by 2 child and/or family nursing care specialists with a semi-structured interview. The responses were qualitatively analyzed by 2 specialists, and the obstructive and facilitative factors affecting participants’ decision to receive HPV vaccination were extracted from the responses. Among the 20 sets of participants, 7 adolescents had completed HPV vaccination, 9 were going to receive vaccination, and 4 had not received any vaccination. The obstructive/facilitative factors related to considering or receiving HPV vaccination and actual vaccination were extracted and 4 main categories of factors were identified. Facilitators toward HPV-vaccination of daughters included clear future self-image and visions, fear Cervical Cancer (CC) and desire to escape from CC, having discussion with mothers about HPV-vaccination and CC, and to have a boyfriend. Barriers toward vaccination included the mothers’ reluctance to explain the sexual matters about HPV-vaccination to their daughters and difficulty with find the appropriate clinic or hospital to HPV-vaccination. Relevant factors about vaccination included positive family attitudes toward vaccination, having family system allowing consultation and having a public financial support for vaccination for daughters. Identifying pain-related concerns in routine follow-up clinics following oral and oropharyngeal cancer

Epidemiology of Depression and Suicide Ideation in Patients With Psoriasis:A Metaanalysis of Prospective Cohort Studies

Objective:Psoriasis is an immune-mediated inflammatory,chronic,recurrent skin disease associated with a high risk of developing psychiatric disorders,especially depression and suicidal ideation,leading to functional disability and poor quality of *** study was conducted to comprehensively review and assess the epidemiologic association between psoriasis and the risk ratios(RRs)of depression or suicidal ***:Five databases(PubMed,Wanfang Database,CNKI,The Cochrane Library,and EMBASE)were searched for prospective cohort studies on the prevalence of depression and/or suicidal ideation in patients with psoriasis updated to February 2,*** independent reviewers evaluated and extracted the data,which were then pooled into a summary RR with corresponding 95%confidence interval(CI)using random-effects ***:Sixteen cohort studies comprising 1,166,840 patients with psoriasis and 3,294,205 controls were eligible for the final *** pooled RR for depression was 1.43(95%CI=1.13-1.81)in patients with psoriasis and1.55(95%CI=1.40-1.71)in patients with psoriatic *** the subgroup analysis,Asian patients with psoriasis(RR=1.38,95%CI=1.17-1.63)had a lower pooled RR for depression than non-Asian patients(RR=1.45,95%CI=1.07-1.97),and patients with moderate-to-severe psoriasis(RR=1.69,95%CI=1.15-2.50)showed a higher RR for depression than patients with mild psoriasis(RR=1.60,95%CI=1.06-2.42).We also found no increase in the RR for suicidal ideation among people with psoriasis(RR=1.25,95%CI=0.95-1.65).Conclusion:Patients with psoriasis are at an increased risk of *** patients with psoriasis,those with psoriatic arthritis,those who are non-Asian,and those with moderate-to-severe psoriasis are at higher risk for ***,the available evidence does not support an association between psoriasis and suicidal ideation.

成人包皮环切可以预防大多数生殖道感染,显著降低男性获得性HPV、HSV和HIV的感染风险。有关男性包皮环切对军队服役人员健康益处的报道很少。前线的士兵不能保持阴囊与包皮足够的局部卫生条件,容易受到感染,影响执行任务和作战,辅助治疗也会直接给军队增加开支。在服役期间和在派遣之前进行包皮环切可以节省大量时间和成本,对改善官兵生殖健康状况起到长效的益处。这些研究发现正在受到非洲各国军队卫生部门的重视,将成为未来军队卫生和服务的发展方向。在HIV流行可能恶化的亚洲,在军队倡导男性包皮环切术预防生殖道感染和性传播疾病,对提高战斗力和降低医疗成本,增进全社会公众健康状况与国民经济将具有十分显著的战略意义。

Urinary bladder cancer(UBC)is a heterogeneous disease with highly variable clinical outcomes and responses to *** some advances in the molecular understanding of UBC,this knowledge still has not been translated to the clinic in terms of improvements in the prognosis and treatment of *** urinary bladder tumor models representative of the human disease in terms of histology and behavior are needed to study factors involved in tumor initiation,progression and ***,accurate model systems would facilitate identification of new therapeutic targets and predictive markers that could lead to optimization of existing therapies and development of new *** established cancer cell lines derived from human urinary bladder tumors representing different grades and stages have been used as experimental models for UBC *** cell lines reflect some of the genetic and morphologic alterations observed in human urothelial carcinoma and serve as simplified models to study the behavior of cancer cells in ***,their translational potential is limited due to the artificial conditions,in which the cells are maintained,grown and *** models offer a more complex and realistic model for the establishment,development,and progression of tumors as well as to evaluate new therapeutic *** the years,the authors'group has worked with several UBC cell lines,established and characterized chemically induced UBC models,and patient-derived xenografts *** this study,the authors will provide a summary of the UBC models developed by their group,analyze their translational potential and weaknesses,and define areas that remain to be explored.

Inquiry Based on Evidence and its Practices in Two Healthcare Facilities of Bujumbura

Background: Inquiry evidence-based practice (IBP) improves healthcare quality, reliability, and patient outcomes as well as reduces variations in care and costs. IBP and its practice in health care promote also many advantages, such as improvements of practices based on the attitudes and cognitive ideas. This study aims to assess the inquiry based on evidence (IBP) and its practices in two Health Care Facilities (HCFs) of Bujumbura to help the practitioners to understand its importance. Methods: A cross-sectional study design was used to analyze the importance of IBP and its practice in these two hospitals. The probability-sampling technique was used also to select 104 nurses from the Military hospital of Kamenge and 55 nurses from the Van Norman clinic. A questionnaire was used to collect data with two mains components, demographic data and knowledge and attitudes addressing the following parameters: evidence practice during inquiry, nursing theory, current analysis in nursing care oriented the evidence, prioritization of care, rational diagnostic, monitoring and assessment. Results: The findings from this study revealed a poor knowledge and attitude among participants towards Inquiry Based Practice. In all variables, participants were scoring less than 10%. However, majority of participants (76.5%) know the indicators of patients’ satisfaction with nursing interventions through survey-based practice and 74.1% argued to analyze their information collected. Conclusion: This study revealed a weak awareness on IBP and its importance during nursing practice among participants as for almost all variables, participants were scoring less than 10%, except for the indicators of patients’ satisfaction with nursing interventions through survey-based practice (76.5%).
