Liver trauma: What current management?

Background: The liver is the most commonly damaged organ in abdominal trauma. The management of liver trauma has experienced many changes over the last two decades. Currently there is a trend toward a non-operative treatment warranted by the successful pediatric experience and better results recorded in many trauma centers worldwide. This study aimed to evaluate outcomes of operative and non-operative management of liver trauma in our institution over the last five ***: The patients with a diagnosis of blunt or penetrating liver injuries, admitted and managed in our hospital from January 2012 to December 2016 were retrospectively studied. The patients were divided into 2 groups, operated and non-operated groups, according to the initial management considered appropriate at the time of patient admission. Clinical features and outcomes were ***: The study involved 83 patients, with a mean age of 33 years and a marked male predominance(85.5%). The most common type of lesions was blunt trauma and the main cause was road traffic accidents. Sixty-eight liver injuries(81.9%) were of low severity(grades Ⅰ,Ⅱ,Ⅲ), while 15(18.1%) were of high severity(grade Ⅳ or greater). Fifty-six patients(67.5%) had multiple injuries. Surgical treatment was performed in 26(31.3%) patients. Non-operative management was undertaken in 57 cases(68.7%). The morbidity and mortality rates were clearly lower in non-operative patients compared to those in the operated ***: Careful non-operative management is an adequate therapeutic strategy for the patients suffering from liver trauma with stable hemodynamics. Patients with complex hepatic trauma and especially those with other organ injuries continue to have significantly higher mortality.

How Much Digital Activism Plays a Role in Political Activism： A Case Study of Morocco＇s February 20th Movement

The present paper seeks to investigate the extent to which digital activism plays a role in political activism in Morocco through the intersection between women and young activists movement and the social media. By doing so the author argues that while facebook and the Moroccan political arena are Male dominated public areas, young female activists have emerged as prominent leaders in both spaces. The author will more particularly explain why female and youth activists are important in the current state of Moroccan politics. Next, the author provides a discussion of the February 20th Movement, and the movement＇s utilization of Facebook. It will show that young female activists involved in the February 20th Movement have used Facebook in unique and creative ways to further the Movement.

Trade-off between accuracy, cost, and QoS using a beacon-on-demand strategy and Kalman filtering over a VANET

Vehicular Ad-hoc Networks （VANETs） have been suggested as an active and powerful field of research to mitigate environmental problems and challenges. The main challenge in a VANET is to ensure routing with a good Quality of Service （QoS）. The Greedy Perimeter Stateless Routing （GPSR） protocol is one of the most promising position- based routing mechanisms used to overcome this challenge. Its effectiveness depends entirely on the information on a node＇s mobility and the precision of this information. By broadcasting periodic beaconing within trans- mission boundary ranges, GPSR can manage neighbors＇ mobility information and maintain up-to-date lists of neighbours. Nevertheless, information on the position of a neighboring vehicle quickly becomes outdated, which negatively influences the efficiency of the routing. In order to monitor information mobility and to increase the QoS in this challenging area, position estimation needs to he considered. Thus, in this study, we examine the position estimation problem, and propose an improvement to the GPSR protocol, named KF-GPSR, where each vehicle estimates in real time the position of its neighbors using the Kalman filter algorithm. Indeed, by employing this strong estimation technique, it is possible to reduce consid- erably the frequency of exchanged beacon packets, while maintaining high position accuracy. For greater reliability, we also propose an extension to KF-GPSR, called BOD-KF-GPSR, that uses the ＂beacon-on-demand＂ process only if a node needs to rediscover its neighborhood. Simulation experiments using the network simulator NS-2 are presented to demonstrate the ability and usefulness of our two proposals. Here, we compare the pro- posed protocols against diverse common protocols： GPSR, AODV, DSR, and ZRP. The results show that BOD-KF- GPSR achieves a significant enhancement in terms of its packet delivery ratio, routing cost, normalized routing load, end-to-end delay, and throughput.

Conventional direct torque control (DTC) is one of the excellent control strategies available to control the torque of the induction machine (IM). However, the low switching frequency of the DTC causes high ripples in the flux and torque that leads to an acoustic noise which degrades the control performances, especially at low speeds. Many direct torque control techniques were appeared to remedy these problems by focusing specifically on the torque and flux. In this paper, a state of the art review of various modern techniques for improving the performance of DTC control is presented. The objective is to make a critical analysis of these methods in terms of ripples reduction, tracking speed, switching loss, algorithm complexity and parameter sensitivity. Further, it is envisaged that the information presented in this review paper will be a valuable gathering of information for academic and industrial researchers.

Depression is a prevalent psychiatric disorder that often leads to poor quality of life and impaired *** during the acute phase of a major depressive episode aims to help the patient reach a remission state and eventually return to their baseline level of ***,especially selective serotonin reuptake inhibitors antidepressants,remains the most frequent option for treating depression during the acute phase,while other promising pharmacological options are still competing for the attention of *** psychotherapy is the second most common option for helping patients overcome the acute phase,maintain remission,and prevent *** therapy is the most effective somatic therapy for depression in some specific situations;meanwhile,other methods have limits,and their specific indications are still being *** medications,psychotherapy,and somatic therapies remains the most effective way to manage resistant forms of depression.

Delay Dependent Robust Stability of Singular Systems with Additive Time-varying Delays

This paper considers the problem of delay-dependent robust stability for uncertain singular systems with additive time-varying delays. The purpose of the robust stability problem is to give conditions such that the uncertain singular system is regular, impulse free, and stable for all admissible uncertainties. The results are expressed in terms of linear matrix inequalities （LMIs）. Finally, two numerical examples are provided to illustrate the effectiveness of the proposed method.

In software engineering as in all other engineering fields, the product to be built is divided into sub-parts that are independently constructed and subsequently assembled. This procedure reduces the complexity and improves the reuse of the developed products. MDE （model-driven engineering） is a recent initiative that adopts this approach. In this context, model composition has become an important artifact in the MDE domain that allows constructing and composing an efficient assembly process. This paper will attempt to identify the strength and the drawbacks of the existing composition model paradigms, and determine the various parameters that govern their behavior.

A two variable refined plate theory for the bending analysis of functionally graded plates

bending analysis of functionally graded plates using the two variable refined plate theory is presented in this *** number of unknown functions involved is reduced to merely four,as against five in other shear deformation theories. The variationally consistent theory presented here has, in many respects,strong similarity to the classical plate theory. It does not require shear correction factors,and gives rise to such transverse shear stress variation that the transverse shear stresses vary parabolically across the thickness and satisfy shear stress free surface *** properties of the plate are assumed to be graded in the thickness direction with their distributions following a simple power-law in terms of the volume fractions of the *** equations are derived from the principle of virtual displacements, and a closed-form solution is found for a simply supported rectangular plate subjected to sinusoidal loading by using the Navier *** results obtained by the present theory are compared with available solutions,from which it can be concluded that the proposed theory is accurate and simple in analyzing the static bending behavior of functionally graded plates.

Acute pancreatitis revealing ulcerative colitis—A case report

The association of acute pancreatitis with ulcerative colitis [UC] has been described in the literature. It is usually induced by drugs, but sometimes it may be idiopathic. This association remains rare. We report the case of a patient who was treated in our department. Medical observation: Mr. KA, a 60-year-old man, was admitted in our unit for management of acute epigastric. The clinical examination at the admission was normal. After eliminating a cardiac or surgical cause, the diagnosis of acute pancreatitis was made on the basis of the presence of a serum lipase up to 5 times the normal level and pain intensity. An abdominal scanner tomography was performed for the assessment of the pancreatitis. It has shown a pancreatitis stage C associated with a thick rectosigmoidien that was discovered incidentally. Symptomatology was enriched 10 days after by the occurrence of rectal bleeding. A lower endoscopy was performed after the improvement of the pancreatitis and had shown an ulcerative colitis on pancolitis which was confirmed by biopsy. In order to search other causes of this pancreatitis, other tests were made [a biliary IRM, endoscopic ultrasonography, autoimmune tests] and the results were negative. We concluded an idiopathic pancreatitis. The ulcerative colitis was classified as moderate and the patient was put on oral corticosteroids with degression. The evolution was marked by a clinical and biological improvement of pancreatitis and colitis. Conclusion: The association of idiopathic pancreatitis with ulcerative colitis is rare. The case of our patient is the first case reported in our series with 400 cases of ulcerative colitis diagnosed in our service.
