Assessment of the Level of Knowledge about Chronic Renal Failure in 271 Hypertensive Patients in Brazzaville

Background and Objectives: Chronic kidney disease (CKD) is now a global public health problem. In low- and middle-income countries such as the Congo, access to dialysis is low and inequitable. The prevention of CKD involves raising awareness among patients at risk, such as those suffering from arterial hypertension (AH), by improving their knowledge of CKD. The objectives of our work were to determine the level of knowledge about CKD among hypertensive patients and to identify the factors associated with a low level of knowledge. Methodology: We conducted a 3-month descriptive and analytical cross-sectional study from 1 August to 30 October 2023 in 3 large public hospitals in Brazzaville (capital of the Republic of Congo). We included: hypertensive patients aged 18 and over who had freely consented to participate in our study and were able to answer the questions on the survey form. Patients with known hypertension who had been followed for less than 3 years and those with known chronic renal failure were not included. Results: The mean age was 58.4 ± 14.4 years (29 - 88 years). There were 121 men and 150 women (sex ratio = 0.8). All the patients were educated;37.2% with a higher level of education and 13.6% with primary education. 24 patients (9%) had a good level of knowledge about CKD and 153 (56%) had poor knowledge. A good level of knowledge was associated with the duration of hypertension, intellectual level and the existence of associated heart disease. Conclusion: Our study reveals a significant lack of knowledge about chronic kidney disease among hypertensive patients in Brazzaville.

Modeling the Drying Kinetics of Pigeon Pea [Cajanus cajan (L.) Millspaugh]

We set out to model the oven-drying kinetics of a legume known as pigeon pea, harvested in the Bouenza department in the south-west of the Republic of Congo. The drying kinetics of pigeon peas was carried out in an oven under experimental conditions using temperatures of: 50°C, 60°C and 70°C. Seven mathematical models were used to describe pigeon pea drying. During drying, water loss was faster and shorter at 70°C [10.446 g/25 g wet weight (wwb) for 320 min (5.3 h)] compared to 50°C [10.996 g/25 g wet weight (wwb) for 520 min (8.6 h)] and 60°C [10.616 g/25 g wet weight (wwb) for 420 min (7.0 h)] where it was slower and longer. With regard to modeling, and based on the principle of choosing the right model focusing on the high value of R2 and low values of χ2 and RMSE, two models were selected, the Midili model for temperatures of 50°C and 60°C and the Henderson and Pabis model modified for temperature of 70°C showed better results. The R2, χ2 and RMSE values calculated for pigeon pea are 0.99985, 3.93404E-5 and 0.00627;0.9997, 9.245E-5 and 0.00962;0.99996, 1.56332E-5 and 0.00395 respectively at 50°C, 60°C and 70°C.

The Congolese population is organised into households, which are thus headed by a chief who ensures the social well-being, development and integration into working life of the individuals in his charge. This study examines the functional principles of new housing design as an instrument for transforming the (current) failing economy into a strong and resilient one. Accordingly, a literature review of the practice of designing and building housing in human settlements in the Congo revealed the state of the art on this subject. An analysis of the existing housing stock from a demographic, social and economic point of view made it possible to identify the most common household sizes and numbers, as well as the lifestyle processes that determine the need for developed space. To this end, the experimental method was used to propose configuration plans for various new types of dwelling. To this end, the study highlighted the link between people’s standard of living and the effectiveness of their involvement in the local economy. To achieve the aims of the National Development Plan (NDP), particular attention must be paid to solving the housing problem. The existing housing stock actively contributes to the problems associated with unemployment and insecurity. Functional principles for the design of new types of housing have been developed. Four model types are proposed in line with the demographic structure of the population, their socio-economic characteristics and their lifestyle.

Impact of Tension-Type Headaches in the Workplace in Brazzaville

Introduction: Tension-type headaches are the most widespread of the primary headache disorders. Due to their high prevalence, tension-type headaches represent a major public health problem with an enormous socio-economic burden. Determining their impact remains a challenge. Objective: To assess the impact of occupational tension-type headache in Brazzaville and identify associated factors. Population and Methods: This was an analytical case-control study conducted in public and private companies in the city of Brazzaville over a period of four (04) months. The case population consisted of cephalalgic employees;the control population was drawn from the same companies and was free of tension-type headaches. Study variables were divided into socio-professional, clinical and individual impact variables. Individual impact variables were represented by: the HIT-6 score, which incorporates a very broad conception of disability, covering several domains, namely: severity of pain during attacks and the restrictive and limiting nature of attacks. Results: Individual impact was severe in 18 (62.1%) men and 11 (37.9%) women. Mean age was 36.3 6.14 years for cases with severe impact. The mean duration of headache was 40.3 32.7 months for cases with severe impact. Tension headache evolved in attacks in 22 (75.9%) cases with severe impact, and continuously in seven (24.1%) cases. The average number of attacks per month was 2.52 1.04 for cases with severe impact. Cases with severe impact included 14 (48.3%) with chronic headache and 15 (51.7%) with episodic headache. Pain of severe intensity present in 48.3% of cases was associated with a severe impact of tension-type headache: OR = 151.66 [2.36 - 44245.95] and p-value = 0.037. At least one days absence from work per year was observed in 47.4% of our cases. The number of days off work per year due to tension-type headache had an interquartile range between 0 and 3 days and extremes from 0 to 14 days. Objective: Chronic end-stage renal failure is a major public health problem in developing countries and is poorly documented. The objective of this study was to describe the epidemiological, diagnostic, therapeutic, and evolutionary aspects of patients admitted for end-stage renal failure to the Department of Nephrology at the university Hospital of Brazzaville. Patients and Methods: This was a descriptive cross-sectional study collecting prospective data that took place over 10 months. We included 128 patients who were exhaustively identified. Sociodemographic, clinical, therapeutic and developmental data were collected using a standardized and pretested pre-established survey form. The EPI info software enabled the analysis and processing of the data. Results: We reported a male predominance with a sex ratio of 2.5. The median age was 50 years. The first cause found was diabetes (55%) followed by hypertension (46%). The majority of patients had a dialysis emergency and half could not be dialyzed for financial reasons. Conclusion: Our results highlighted that certain epidemiological parameters of end-stage renal failure are constantly changing, such as frequency, while others remain static, including the main etiologies, gender, and age. However, mortality continues to increase and deserves more attention.

Experimental Study of the Influence of Intrinsic Parameters on the Thermal Reactivity of Sawdust, Polyethylene Terephthalate and Composite

Several works have been based on the study of thermal variations in biomass to derive more valuable products such as fuels capable of replacing oil in the event of a crisis or activated carbon used as an adsorbent material, widely used in industry for the elimination of unwanted materials, both in liquid and gaseous environments. A study of thermal parameters such as: heating speed, retention time, drying temperature, carbonization temperature, particle size, was carried out with the aim of determining the characteristic factors of the carbonization of Polyethylene terephthalate (PET), sawdust (SC) and sawdust/polyethylene terephthalate (CPS) mixture. The results of the immediate analysis revealed a very low level of ash in PET (0.013%) compared to the level of ash in sawdust (2.9%), as well as a high level of fixed carbon (82.960%), which suggests the presence of mineral oxides and a significant carbon matrix unlike PET, which indicates a very significant organic matrix (essentially made up of organic matter) with the absence of mineral oxides. The study of thermal parameters showed the water loss from Sawdust (SC) and the Sawdust/Polyethylene terephthalate (CPS) mixture, an increase with temperature, unlike that of PET whose variation is essentially zero. Without heat treatment, sawdust alone contains approximately 7% water. The optimal drying temperature for this study is 110˚C for a stay of 24 hours. It appears that the largest mass losses for the PET samples are between 87.19% and 96.05%, followed by that of the mixture, between 47.33% and 64.37%. And the lowest are observed, those of sawdust (from 24.02% to 62.6%). However, here we can say that the influence of the mass is not great, given the slight difference between the losses by temperature. The results of the study of the influence of grain size showed that the differences are insignificant, even if we vary the diameter of the grains from simple to triple. Traumatic Diaphragmatic Hernia in Children: A Case Report

Introduction: Traumatic diaphragmatic hernia is a rare condition in children complicating closed or penetrating trauma to the abdomen and thorax. We report the case of an 11-year-old girl with a traumatic diaphragmatic hernia. Case Presentation: An 11-year-old girl was seen in the paediatric surgery department for a thoracolumbar spine deformity and intermittent chest pain. These symptoms occurred after a domestic accident involving a fall from a low wall onto the thoracolumbar spine 5 months previously. The diagnosis was suggested by the presence of a left hemithoracic hydroaera and confirmed by a thoraco-abdominal CT scan. Surgical exploration revealed a linear rupture of the entire left hemi-diaphragm with herniation of the stomach, small intestine, cecum, transverse colon and omentum. We performed a double-layer suture of the diaphragmatic rupture with a non-absorbable suture without edge rejuvenation after the reduction of the hernia. The outcome was favourable with normal postoperative radiographs at one year follow-up. Conclusion: Traumatic diaphragmatic hernia, although uncommon and difficult to diagnose, is a condition that is relatively easy to manage surgically, even if it is discovered late. In all cases of trauma to the thoracolumbar spine, regular follow-up and repeat X-rays are necessary if pain persists.

Nutritional and Antioxidant Evaluation and Effect of Eggplant Consumption on Anthropometric and Hematologic Parameters in Wistar Rats

The aim of the present study was to evaluate the nutritional quality of green and purple eggplant, their antioxidant activity and their nutritional efficacy on Wistar rats. For nutritional quality, the parameters measured are dry matter, protein, lipid, ash, carbohydrate, iron, phosphorus, calcium, magnesium and energy content. For antioxidant activity, the parameters measured are 50% DDPH free radical inhibition concentration and total polyphenol content. Nutritional efficacy was evaluated in rats fed the control diet and in rats fed the three treated diets containing eggplant meal obtained by replacing 5%, 10%, and 15% of the control diet. The parameters measured are the amount consumed, the weight of the animal and target organs, and hematologic parameters. The results of the nutritional analysis show the following values: 13.31% protein, 2.66% lipids, 0.84% calcium, 0.12% magnesium, 0.43% phosphorus for the Solanum aethiopicum L. species and 13.47% protein, 3.66% lipids, 0.36% calcium, 0.22% magnesium, 0.35% phosphorus for the Solanum melongena L. species. In terms of antioxidant activity, we obtained DDPH inhibition percentages of 40.28 mg/ml for Solanum aethiopicum L. and 12.42 mg/ml for Solanum melongena L., respectively. Finally, hematologic and anthropometric tests showed that for the different diets used, weight loss and an increase in hematologic parameters were observed in the rats tested. This study showed that eggplant has interesting nutritional characteristics and antioxidant activity, contributing to an increase in weight and anthropometric parameters.

Application of the N + 2 Transversal Network Method to the Study of a Coupled Resonator Filter

This paper presents a new approach to synthesize admittance function polynomials and coupling matrices for coupled resonator filters. The N + 2 transversal network method is applied to study a coupled resonator filter. This method allowed us to determine the polynomials of the reflection and transmission coefficients. A study is made for a 4 poles filter with 2 transmission zeros between the N + 2 transversal network method and the one found in the literature. A MATLAB code was designed for the numerical simulation of these coefficients for the 6, 8, and 10 pole filter with 4 transmission zeros.
