INTRoDUCTIoN As we face the ongoing global pandemic of CoVID-19,doctors,nurses,ambulance officers,paramedics and many other health workers answer the call to serve in time-pressured,unfamiliar,chaotic and often-traumatic environments.1 We know how to look after ourselves in an infectious physical environment,but it is equally important to look after ourselves psychologically at this time.

背景血流动力学和全身氧输送的变化无法反映内脏低灌注，从而导致无法认识到对失血性休克的处理不足。液体复苏后扩容对改善失血性休克时全身和局部的氧供是必不可少的。我们假设，与传统的代血浆相比，应用少量的扩容剂7．5氯化钠／6％羟乙基淀粉（hydroxyethyl *** saline，HHES）溶液，可能提供较少的全身氧输送和胃血流灌注。我们为失血的狗进行临床上严重出血时常用的固定量液体单次输注，分别观察HHES、乳酸林格液（1actatedRinger，LR）和6％羟乙基淀粉（hydroxyethyl starch，HES）溶液对血管内扩容、早期全身氧合和胃灌注的影响。方法30只狗，出血30ml／kg，保持平均动脉压在40～50mmHg，持续45分钟后，分3组进行复苏：LR组（n=10），输注3倍出血量的液体；HES组（平均分子量130KDa，替代级0．4）（n=10），输注出血量等量的液体；HHES组（n=10），以4ml／kg的速度输注。测定血管内容量增加情况（用Evans蓝和血红蛋白稀释）、血流动力学、全身氧合、静动脉Co2分压差（Pv-aco2）和胃黏膜-动脉血Co2分压差（Pco2间隙）变化，测定时间点为基础值、出血45分钟后以及液体复苏5分钟、45分钟、90分钟时。结果由于HHES的高扩容效力增加了血容量，但它对血管内容量的扩张作用是各溶液中最小的（P〈0．05）。3种溶液对血流动力学影响相同，但相对于LR组和HES组，HHES组相混合静脉血Po，更低，全身氧合、Pv-aco2和胃黏膜Pco2间隙更大（P〈0．05）。结论对狗进行按血压调节的失血性休克和固定容量的复苏，尽管HHES有高效扩容作用，但相比LR和HES，由于输注量受限，它的血管内容量扩充能力较小，全身氧合和胃血流灌注较差。

Background: Endoscopic sclerotherapy (ES) and band ligation are standard treatments for esophageal varices. Unfortunately, recurrence is common and seems to be related to esophageal collateral vessels, easily identified by EUS. Eradication of these vessels might lead to a more durable therapeutic effect. objective: To compare ES with EUS-guided sclerotherapy of collateral vessels (EUS-ES). design: Randomized controlled trial. Setting: Endoscopy Unit, Division of Gastroenterology. universidade Federal de S o paulo, S o paulo, Brazil. Patients and Interventions: Fifty cirrhotic patientswith esophageal varices were randomized into 2 groups: ES (n = 25) and EUS-ES (n = 25). EUS-ES was targeted at collateral veins. Patients were followed-up for at least 6 months after eradication. Main outcome Measurements: Efficacy in eradication, complications, and recurrence of varices. Results: Varices were eradicated in 48 patients who adhered to the study protocol. The mean (SD) number of sessions until eradication was 4.3 (1.5) for the ES group and 4.1 (1.2) for the EUS-ES group. In ES group, 4 patients had mild bleeding. In EUS-ES group, 4 patients had pain. The mean (SD) length of the follow-up period was 22.6 (6.9) months for the ES group and 24.9 (8.1) months for the EUS-ES group. Recurrence was seen in 4 patients after ES and in 2 after EUS-ES (P = .32). The presence of collateral vessels was associated with recurrence (P = .003). Conclusion: EUS-ES is as safe and effective as ES in variceal eradication. Recurrence tends to be less frequent and occurs later. Persistence of esophageal collateral vessels after sclerotherapy is a risk factor for recurrence.

Hydropower production is one of the greatest threats to fluvial ecosystems and freshwater *** that we have entered the Anthropocene,there is an opportunity to reflect on what might constitute a‘sustainable’Anthropocene in the context of hydropower and riverine fish *** elements of existing practices that promote favorable social-ecological outcomes(i.e.,‘bright spots’)is timely given that there are plans to expand hydropower capacity in previously undammed rivers,intensify dam development in some of the world's largest river systems,and re-license existing *** approach this from a pragmatic perspective:for the foreseeable future,hydropower will likely remain an important source of renewable *** offer support for moving toward a more‘sustainable’Anthropocene,we provide syntheses of best practices during the siting,design,construction,operation,and compensation phases of hydropower development to minimize impacts on inland *** each phase,we offer positive examples(or what might be considered‘bright spots’)pertaining to some of the approaches described within our syntheses,acknowledging that these projects may not be viewed as without ecological and(or)societal detriment by all *** findings underscore the importance of protecting critical habitat and free-flowing river reaches through careful site selection and basinscale planning,infrastructure designs that minimize reservoir effects and facilitate safe passage of fish,construction of hydropower plants using best practices that minimize long-term damage,operating guidelines that mimic natural flow conditions,and compensation that is lasting,effective,inclusive,and locally *** from these‘bright spots’may require engagement of diverse stakeholders,professionals,and governments at scales that extend well beyond a given site,river,or even ***,environmental planning that integrates hydropower development into broader discussions

Metabolic syndrome is an epidemic that affects more and more people, increasing the probability of suffering metabolic and cardiovascular diseases in the short, medium or long term depending on the severity. The purpose of this article is to review the metabolic syndrome, assessing consensus, controversy and prevalence. The methodology was the preparation of a literature review on various health care databases, which were from 43 articles published from 2010 to 2015, in the general population. SM rate ranges from 0% to 90% depending on genders, ages and regions. There is still lack of consensus on cutoffs of diagnostic criteria. Thus, it is concluded that the prevalence of metabolic syndrome is being increased, urging the need for early diagnosis and treatment to promote the health of the global population.

Mesenchymal stem cell enhances chondral defects healing in horses

The purpose of this study was to evaluate the effect of intralesional Mesenchymal Stem Cells (MSC) on the treatment of experimentally induced articular chondral defects in horses, emphasizing the benefits of this application in veterinary medicine. Chondral defects were induced in the medial femoral trochlea of both hind limbs of four horses. Thirty days post induction;the horses were divided into two groups. The G1 was submitted to treatment with MSC and the G2 was the control group. Clinical evaluations, synovial fluid analysis and synovial Prostaglandin E2 (PGE2) assessment were performed prior to defects and fortnightly up to 120 and 150 days. Macroscopic, histopathological and histochemical evaluations were performed at the end of the experiment. The treatment with MSC reduced the intraarticular inflammatory process. The G1 showed lower PGE2 concentrations in the synovial fluid and greater percentage of mononuclear cells and lower percentages of lymphocytes and neutrophils. The treatment improved the macro and microscopic aspects of repair tissue. No difference was observed in the scores of lameness between the G1 and G2. The use of MSC in the treatment of chondral defects minimized joint inflammation, as confirmed by synovial fluid analysis. The treatment resulted in an improved repair tissue, verified by macroscopic examination, histochemical and histopathological analysis.
