Water Quality Assessment of a Tropical Mexican Lake Using Multivariate Statistical Techniques

Water quality of Mexican tropical lake Chapala was assessed through multivariate statistical techniques, cluster analysis (CA) and principal component analysis (PCA) at ten different monitoring sites for ten physicochemical variables and six metals. This study evaluated and interpreted complex water quality data sets and apportioned of pollution sources to get better information about water quality. From descriptive statistics results, the highest concentrations of metals occurred during the dry season, and this trend was explained by the fact that an unusual rainy event occurred during the month of February 2009 and brought metals into the lake by runoffs from nearby mountains. According to international criteria for water consumption by aquatic organisms [USEPA], only Zn concentration values were below these criteria whereas the values of Ni, Pb, Cd and Fe were above the corresponding values set in these criteria (Ni: 52 μg·L-1, Pb: 2.5 μg·L-1, Cd: 0.25 μg·L-1, and Fe: 1000 μg·L-1). The correlations were observed by PCA, which were used to classify the samples by CA, based on the PCA scores. Seven significant cluster groups of sampling locations—(sites 4 and 5), (sites 3 and 9), (site 7), (site 10), (sites 2 and 6), (site 8) and (site 1)— were detected on the basis of similarity of their water quality. The results revealed that the stress exerted on the lake caused by waste sources follows the order: domestic > agricultural > industrial.

Genes, emotions and gut microbiota:The next frontier for the gastroenterologist

Most medical specialties including the field of gastroenterology are mainly aimed at treating diseases rather than preventing them. Genomic medicine studies the health/disease process based on the interaction of the human genes with the environment. The gastrointestinal(GI) system is an ideal model to analyze the interaction between our genes, emotions and the gut microbiota. Based on the current knowledge, this mini-review aims to provide an integrated synopsis of this interaction to achieve a better understanding of the GI disorders related to bad eating habits and stress-related disease. Since human beings are the result of an evolutionary process, many biological processes such as instincts, emotions and behavior are interconnected to guarantee survival. Nourishment is a physiological need triggered by the instinct of survival to satisfy the body's energy demands. The brain-gut axis comprises a tightly connected neuralneuroendocrine circuitry between the hunger-satiety center, the dopaminergic reward system involved in the pleasure of eating and the gut microbiota that regulates which food we eat and emotions. However, genetic variations and the consumption of high-sugar and high-fat diets have overridden this energy/pleasure neurocircuitry to the point of addiction of several foodstuffs. Consequently, a gut dysbiosis generates inflammation and a negative emotional state may lead to chronic diseases. Balancing this altered processes to regain health may involve personalized-medicine and genome-based strategies. Thus, an integrated approach based on the understanding of the gene-emotions-gut microbiota interaction is the next frontier that awaits the gastroenterologist to prevent and treat GI disorders associated with obesity and negative emotions.

Effect of a bacterial inoculum and additive on dry matter in situ degradability of sugarcane silage

The objective of this study was to evaluate the effect of adding a bacterial inoculum and a handmade additive to sugarcane silage（SCS） on the in situ digestibility of dry matter（DM）. The treatments were： T1） sugarcane silage（SCS） and T2） sugarcane silage with 1% inoculum and 1% additive（SCS＋）. The bacterial inoculum consisted of 10.0% molasses, 1.0% yogurt, 5.0% chicken manure, 0.5% urea, and 83.0% water, and the additive was formulated with 1.0% urea, 0.1% ammonium sulfate, and 0.25% phosphorus. In situ dry matter digestibility（DMD） was determined using the nylon bag technique with four cows equipped with ruminal fistulas. Cows were fed with ensiled sugarcane supplemented with 1 kg of commercial concentrate. 5 g of ground sample for each sugarcane treatment were weighted in nylon bags and incubated for 4, 8, 12, 24, 48, and 72 h in a completely randomized design with six replicates. The DMD（%） was higher（P〈0.05） for SCS＋ for all incubation times when compared with SCS. There were no differences in ruminal p H between the treatments for all the incubation times. The data suggested that the sugarcane silage with bacterial inoculum and additive could be an alternative for providing forage for ruminants during the season of low growth and quality grass.

BACKGROUND Digestive tract resections are usually followed by an anastomosis.Anastomotic leakage,normally due to failed healing,is the most feared complication in digestive surgery because it is associated with high morbidity and mortality.despite technical and technological advances and focused research,its rates have remained almost unchanged the last decades.In the last two decades,stem cells(SCs)have been shown to enhance healing in animal and human studies;hence,SCs have emerged since 2008 as an alternative to improve anastomoses outcomes.AIM To summarise the published knowledge of SC utilisation as a preventative tool for hollow digestive viscera anastomotic or suture leaks.METHODS PubMed,Science Direct,Scopus and Cochrane searches were performed using the key words“anastomosis”,“colorectal/colonic anastomoses”,“anastomotic leak”,“stem cells”,“progenitor cells”,“cellular therapy”and“cell therapy”in order to identify relevant articles published in English and Spanish during the years of 2000 to 2021.Studies employing SCs,performing digestive anastomoses in hollow viscera or digestive perforation sutures and monitoring healing were finally included.Reference lists from the selected articles were reviewed to identify additional pertinent articles.METHODS Given the great variability in the study designs,anastomotic models,interventions(SCs,doses and vehicles)and outcome measures,performing a reliable meta-analysis was considered impossible,so we present the studies,their results and limitations.RESULTS Eighteen preclinical studies and three review papers were identified;no clinical studies have been published and there are no registered clinical trials.Experimental studies,mainly in rat and porcine models and occasionally in very adverse conditions such as ischaemia or colitis,have been demonstrated SCs as safe and have shown some encouraging morphological,functional and even clinical results.Mesenchymal SCs are mostly employed,and delivery routes are mainly local injections and cell sheets followed by biosutur

DNA-guided hepatitis B treatment,viral load is essential,but not sufficient

Hepatitis B virus(HBV) infection is a global public health problem that concerns 350 million people worldwide.Individuals with chronic hepatitis B(CHB) are at increased risk of developing liver cirrhosis,hepaticde-compensation a nd hepatocellular carcinoma.To maintain undetectable viral load reduces chronic infection complications.There is no treatment that eradicates HBV infection.Current drugs are expensive,are associated with adverse events,and are of limited efficacy.Current guidelines try to standardize the clinical practice.Nevertheless,controversy remains about management of asymptomatic patients with CHB who are hepatitis B e antigen(HBeAg)-positive with normal alanine aminotransferase,and what is the cut-off value of viral load to distinguish HBeAg-negative CHB patients and inactive carriers.We discuss in detail why DNA level alone is not sufficient to begin treatment of CHB.

EEG Characteristics of Adolescents Raised in Institutional Environments and Their Relation to Psychopathological Symptoms

Children reared in institutional environments present emotional and cognitive affectations, as well as changes in the anatomy and functioning of certain cerebral areas, such as the prefrontal cortex. However, knowledge of the long run effect of these affectations remains scarce. Therefore, this study was designed to correlate the cortical electroencephalographic (EEG) and psychopathological parameters. Resting EEG from 10 institutionalized (IA) and 10 never-institutionalized adolescents (NIA) was obtained and this was parametrically correlated with the intensity of symptoms of depression, post-traumatic stress disorder (PTSD) and anxiety. Results showed that the IA group showed higher indices of depression, anxiety and PTSD, accompanied by low relative power (RP) in the fast bands, high activity in the slow bands in frontal areas, and higher alpha2 RP in temporal areas. In addition, IA showed higher EEG-correlations among frontal areas but lower EEG-correlations among prefrontal, parietal and temporal areas in the gamma band. We also found correlations between the EEG parameters and psychopathological scores. These EEG findings can contribute to improve our understanding of how stress experienced early in life may impact the development of the cortex, and how this, in turn, may predispose individuals to develop psychiatric conditions.

Effect of pesticides on phosphorylation of tau protein,and its influence on Alzheimer’s disease

Alzheimer’s disease(AD)is a progressive and neurodegenerative illness which results in alterations in cognitive development.It is characterized by loss/dysfunction of cholinergic neurons,and formation of amyloid plaques,and formation of neurofibrillary tangles,among other changes,due to hyperphosphorylation of tau-protein.Exposure to pesticides in humans occurs frequently due to contact with contaminated food,water,or particles.Organochlorines,organophosphates,carbamates,pyrethroids and neonicotinoids are associated with the most diagnosed incidents of severe cognitive impairment.The aim of this study was to determine the effects of these pesticides on the phosphorylation of tau protein,and its cognitive implications in the development of AD.It was found that exposure to pesticides increased the phosphorylation of tau protein at sites Ser198,Ser199,Ser202,Thr205,Ser396 and Ser404.Contact with these chemicals altered the enzymatic activities of cyclin-dependent kinase 5 and glycogen synthase kinase 3 beta,and protein phosphatase-2A.Moreover,it altered the expression of the microtubule associated protein tau gene,and changed levels of intracellular calcium.These changes affected tau protein phosphorylation and neuroinflammation,and also increased oxidative stress.In addition,the exposed subjects had poor level of performance in tests that involved evaluation of novelty,as test on verbal,non-verbal,spatial memory,attention,and problem-solving skills.

Hepatitis B Virus Genotype H and Environmental Factors Associated to the Low Prevalence of Hepatocellular Carcinoma in Mexico

Purpose: Hepatocellular carcinoma (HCC) is a leading health problem worldwide. Any agent causing chronic liver damage and cirrhosis is a risk factor for HCC. Genetic and environmental factors may be responsible for regional variations in the occurrence of HCC worldwide. The aim of this review was to describe the risk factors that may be contributing to low prevalence of HCC in the Mexican population. Methods: An electronic systematic search was conducted in four databases to retrieve studies on hepatocellular carcinoma inMexico. Results: Eighteen publications gave a total of 1042 HCC cases with a percentage that ranged from 0.25% to 1.87%. Cirrhosis was registered in 7 studies while the main etiologies were: HCV (66%), HBV (11%) and alcoholism (6.6%). Conclusions: In the last 50 years, the studies performed inMexicohave shown a very low incidence and/or mortality rate of HCC. These findings contrast from those reported in high endemic regions, such asAsia, where viral hepatitis and HCC are prevalent. One significant difference is the predominance of HBV genotype H in Mexico and HBV/B and C in Asia. InMexico, high endemic areas of HBV infection have been detected, mainly among the native population;however, infection seems to resolve very quickly, due to a prominent immunological response among the population. Other factors are that patients with liver cirrhosis die prematurely before that HCC can be detected. Furthermore, an environmental factor that may exert a protective effect against HCC, in spite of the high consumption of potentially aflatoxin-contaminated food products, is the neutralization of these substances by alkaline treatment. This study shows that genetic and environmental factors associated to HCC among the Mexican population are different from others reported worldwide.

Protein tolerance to standard and high protein meals in patients with liver cirrhosis

AIM To investigate the plasma amino acid response and tolerance to normal or high protein meals in patients with cirrhosis.METHODS The plasma amino acid response to a 20 g mixed protein meal was compared in 8 biopsy-proven compensated cirrhotic patients and 6 healthy subjects.In addition the response to a high protein meal(1 g/kg body weight) was studied in 6 decompensated biopsy-proven cirrhotics in order to evaluate their protein tolerance and the likelihood of developing hepatic encephalopathy(HE) following a porto-caval shunt procedure.To test for covert HE,the "number connection test"(NCT) was done on all patients,and an electroencephalogram was recorded in patients considered to be at Child-Pugh C stage.RESULTS The changes in plasma amino acids after a 20 g protein meal were similar in healthy subjects and in cirrhotics except for a significantly greater increase(P decreased significantly after the meal in the cirrhotic group.After the high protein meal there was a marked increase in the levels of most amino acids,but only small changes occurred in the levels of taurine,citrulline,cysteine andhistidine.The BCAA/AAA ratio was significantly higher 180 and 240 min after the meal.Slightly elevated basal plasma ammonia levels showed no particular pattern.Overt HE was not observed in any patients.CONCLUSION Patients with stable liver disease tolerate natural mixed meals with a standard protein content.The response to a high protein meal in decompensated cirrhotics suggests accumulation of some amino acids but it did not precipitate HE.These results support current nutritional guidelines that recommend a protein intake of 1.2-1.5 g/kg body weight/day for patients with cirrhosis.

Clostridium difficile (C. difficile) is a spore-forming, toxin-producing, gram-positive anaerobic bacterium that is the principal etiologic agent of antibiotic-associated diarrhea. Infection with C. difficile (CDI) is characterized by diarrhea in clinical syndromes that vary from selflimited to mild or severe. Since its initial recognition as the causative agent of pseudomembranous colitis, C. difficile has spread around the world. CDI is one of the most common healthcare-associated infections and a significant cause of morbidity and mortality among older adult hospitalized patients. Due to extensive antibiotic usage, the number of CDIs has increased. Diagnosis of CDI is often difficult and has a substantial impact on the management of patients with the disease, mainly with regards to antibiotic management. The diagnosis of CDI is primarily based on the clinical signs and symptoms and is only confirmed by laboratory testing. despite the high burden of CDI and the increasing interest in the disease, episodes of CDI are often misdiagnosed. The reasons for misdiagnosis are the lack of clinical suspicion or the use of inappropriate tests. The proper diagnosis of CDI reduces transmission, prevents inadequate or unnecessary treatments, and assures best antibiotic treatment. We review the options for the laboratory diagnosis of CDI within the settings of the most accepted guidelines for CDI diagnosis, treatment, and prevention of CDI.
