Do patients with gastroesophageal reflux disease and somatoform tendencies benefit from antireflux surgery?

BACKGROUND The clinical presentation of gastroesophageal reflux disease(GERD) shows a large symptom variation also in different intensities among patients. As several studies have shown, there is a large overlap in the symptomatic spectrum between proven GERD and other disorders such as dyspepsia, functional heartburn and/or somatoform *** To prospectively evaluate the GERD patients with and without somatoform disorders before and after laparoscopic antireflux *** In a tertiary referral center for foregut surgery over a period of 3 years patients with GERD, qualifying for the indication of laparoscopic antireflux surgery, were investigated prospectively regarding their symptomatic spectrum in order to identify GERD and associated somatoform disorders. Assessment of symptoms was performed by an instrument for the evaluation of somatoform disorders[Somatoform Symptom Index(SSI) > 17]. Quality of life was evaluated by Gastrointestinal Quality of Life Index(GIQLI).RESULTS In 123 patients an indication for laparoscopic antireflux surgery was established and in 43 patients further medical therapy was suggested. The portion of somatoform tendencies in the total patient population was 20.48%(34 patients).Patients with a positive SSI had a preoperative GIQLI of 77(32-111). Patients with a normal SSI had a GIQLI of 105(29-140)(P ders, the GIQLI was improved from preoperative GIQLI 75(47-111) to postoperative 95(44-122)(P ders have significantly worse levels of quality of life. The latter patients can also benefit from laparoscopic fundoplication, however they will not reach a normal level.

A long pulse electron cyclotron resonance heating（ECRH）system has been developed to meet the requirements of steady-state operation for the EAST superconducting tokamak,and the first EC wave was successfully injected into plasma during the 2015 spring *** system is mainly composed of four 140 GHz gyrotron systems,4 ITER-Like transmission lines,4 independent channel launchers and corresponding power supplies,a water cooling,control ＆inter-lock system *** gyrotron is expected to deliver a maximum power of 1 MW and be operated at 100-1000 s pulse *** No.1 and No.2 gyrotron systems have been *** the initial commissioning,a series of parameters of 1 MW 1 s,900 k W 10 s,800 k W 95 s and650 k W 753 s have been demonstrated successfully on the No.1 gyrotron system based on calorimetric dummy load *** plasma heating and MHD instability suppression effects were observed in EAST *** addition,high confinement（H-mode）discharges triggered by ECRH were obtained.

Assembly and metrology of NIF target subassemblies using robotic systems

With European Laser Facilities such as the Extreme Light Infrastructure(ELI) and the Helmholtz International Beamline for Extreme Fields(HIBEF) scheduled to come online within the next couple of years, general Atomics, as a major supplier of targets and target components for the High Energy density Physics community in the United States, is gearing up to meet their demand for large numbers of low cost targets. Using the production of a subassembly for the national Ignition Facility's fusion targets as an example, we demonstrate that through automation of assembly tasks, the design of targets and their experimental setup can be fairly complex while keeping the assembly time and cost as a minimum.A six-axis Mitsubishi robot is used in combination with vision feedback and a force–torque sensor to assemble target subassemblies of different scales and designs with minimal change of tooling, allowing for design flexibility and short assembly setup times. Implementing automated measurement routines on a Nikon NEXIV microscope further reduces the effort required for target metrology, while electronic data collection and transfer complete a streamlined target production operation that can be adapted to a large variety of target designs.

Multifunctional core/shell type,luminescent-plasmonic material composed of lanthanide doped microspheres(≈50μm)and gold nanoparticles(Au NPs;≈10-20 nm)deposited onto their surface,were successfully prepared(Nd^3+:YAS@Au).The material was synthesized to combine the luminescence properties of the Nd^3+-doped microspheres,*** resonance with plasmonic activity of the surface Au NPs,*** enhanced Raman scattering(SERS)effect,within a single,micro-sized *** luminescent-plasmonic microspheres were used as the active SERS substrate for detection of the organic probe,and for generation of Whispering Gallery Modes(WGM),which red-shift together with increasing laser power(temperature elevation).The products obtained were analysed with optical,scanning and transmission electron microscopy(SEM and TEM),as well as by Raman,absorption and photoluminescence spectroscopies.

Secondary Education in Argentina： A Winding Road

This paper is aimed at exploring the state of the secondary education in Argentina, with special emphasis on the different changes and approaches of inclusive education public policies introduced in the last years. Argentina has been experiencing profound changes in relation to the population groups with access to the different educational levels, presenting painful differences in the quality and effectiveness of education given in different population and geographic sectors. Secondary education in Argentina is currently the educational level on which all eyes are focused. On the one hand, due to the progressive obligatoriness, but to a greater extent a large number of secondary school age population are effectively excluded from the possibility of exercising the right to quality education. The paper tries to examine, through an analysis of the strengths, weaknesses, opportunities, and threats of the Argentinian educational system, what are the critical points of inflection where to incorporate elements of innovation to achieve better quality and inclusion standards in a country marked by deep levels of inequality in the access and quality of education in its different population groups. The research has been conducted through a descriptive methodology, using quantitative and qualitative analysis of primary and secondary sources. It is set on a bibliographical review based on reports and studies of recent publications, coming from official, national, and international public institutions. The paper aims to be a ftrst step in a diagnostic analysis of Argentinian education, but in a comparative manner, contrasting indicators and local indexes with international and regional trends in the subject. The main results point out that a deep transformation of the secondary school is needed, in such a way as to respond to the many signs of inequality, fragmentation and poor quality of the most vulnerable population groups.

Synthesis and Characterization of Hydrogels Based on Potato Starch/Poly(vinyl Alcohol)/N,N′-Methylenebisacrylamide

Several hydrogels were synthesized by free-radical polymerization in an aqueous medium based on potato starch(PS),poly(vinyl alcohol)(PVA),and N,N′-Methylenebisacrylamide(MBAm),being possible to study these hydrogels as a function of the proportion of components *** this way,the products generated from the synthesis were characterized by swelling and deswelling kinetics,the first swelling being verified with Schott and statistical models,allowing to contrast the proximity between the experimental and theoretical ***,water retention in soil(R%),spectroscopy(FTIR),morphological(SEM),and thermal(TGA and DSC)analysis allowing to know the intrinsic characteristics of the material,increasing in general terms the knowledge of this type of *** this context,it was possible to verify the characteristics and effectiveness of the synthesis and crosslinking of the main *** experimental results obtained show that the synthesized hydrogels present representative swellings consistent with kinetic and statistical models,optimal thermal stability depending on the amount of crosslinker,and excellent water retention in environments such as soil,presenting it as an excellent alternative to be used in agro-industrial applications in an eco-friendly way.

Momentum Transport in DIII-D Discharges with and Without Magnetohydrodynamics （MHD） Activity

Two phases of a DIII-D discharge with and without magnetohydrodynamics （MHD） activity are analysed using ONETWO code. The toroidal momentum flux is extracted from experimental data and compared with the predictions by neoclassical theory, Gyro-Landau fluid transport model （GLF23） and Multi-Mode model （MMM95）. It is found that without MHD activities GLF23 and MMM95 provide a reasonable description while with MHD activity no model alone can fully describe the experimental momentum flux. For the phase with MHD activity a simple model of resonant magnetic drag is tested and it cannot fully explain the plasma slowing down observed in experiment.

Covered nitinol stents for the treatment of esophageal strictures and leaks

AIM:To compare 2 different types of covered esophageal nitinol stents(Ultraflex and Choostent) in terms of efficacy,complications,and long-term ***:A retrospective review of a consecutive series of 65 patients who underwent endoscopic placement of an Ultraflex stent(n = 33) or a Choostent(n = 32) from June 2001 to October 2009 was ***:Stent placement was successful in all patients without hospital *** significant differences in patient discomfort and complications were observed between the Ultraflex stent and Choostent *** median follow-up time was 6 mo(interquartile range 3-16 mo).Endoscopic reintervention was required in 9 patients(14%) because of stent migration or food *** significant difference in the rate of reintervention between the 2 groups was observed(P = 0.8).The mean dysphagia score 1 mo after stent placement was 1.9 ± 0.3 for the Ultraflex stent and 2.1 ± 0.4 for the Choostent(P = 0.6).At 1-mo follow-up endoscopy,the cover membrane of the stent appeared to be damaged more frequently in the Choostent group(P = 0.34).Removal of the Choostent was possible up to 8 wk without ***:Ultraflex and Choostent proved to be equally reliable for palliation of dysphagia and *** of the Choostent was easy and safe under mild sedation.

Small water bodies are critical for maintaining freshwater biodiversity,but are among the least investigated aquatic *** examined physical and chemical variables at two reaches in Arroyo Tambillos,a small,high-elevation Andean stream,in NW Mendoza province,Argentina,across four seasons to examine how local factors and seasonality affected the structure of the macroinvertebrate *** Arroyo Tambillos community was numerically dominated by ephemeropterans(mainly Massartellopsis irarrazavali)while the dipteran Chironomidae were the most species *** macroinvertebrate abundance was highest in summer,driven mainly by taxa that were secondary in dominance(Austrelmis *** Andesiops peruvianus),while richness did not differ between ***,benthic composition was different in Fall 2013(March)compared to other seasons,largely because of the increased abundance of *** correspondence analysis discriminated Chironomidae species distributions in Fall 2013 by substrate type(i.e.,big and small boulder substrate).By contrast,discharge,velocity,and depth were the variables which most affected the macroinvertebrate abundance and *** to our expectations,most community changes observed occurred in fall instead of ***-kryal stream communities like the one described here have become increasingly important for conserving mountain stream biodiversity as anthropogentic impacts and climate change increasingly impact lower stream ***,monitoring high-altitude streams like the Arroyo Tambillos may be critical for preventing the future loss of unique and sensitive stream biota.

Influence of extrusion temperature on microstructure and mechanical properties of Mg-4Y-4Sm-0.5Zr alloy

The solution-treated Mg-4Y-4Sm-0.5Zr alloy was extruded at temperatures from 325℃ to 500℃.Dynamic recrystallization(DRX) completely occurs when the alloy is extruded at 350℃and *** grains of the extruded alloy are obviously refined by the occurrence of *** average grain size of the extruded alloy increases with increasing the extrusion temperature,leading to a slight decrease of the ultimate tensile strength(UTS) and the yield strength(YS) .On the contrary,the UTS and YS of the extruded and aged alloy increase with increasing the extrusion *** of UTS of 400 MPa,YS larger than 300 MPa and elongation(EL) of 7%are achieved after extrusion at 400℃ and ageing at 200℃ for 16 *** grain refinement and precipitation are efficient strengthening mechanisms for the Mg-4Y-4Sm-0.5Zr alloy.
