Peritoneal interposition flap reduces symptomatic lymphocele following transperitoneal robot-assisted radical prostatectomy and pelvic lymph node dissection: An updated meta-analysis

Background Robot-assisted radical prostatectomy with intraoperative pelvic lymph node dissection is the criterion standard for surgical treatment of nonmetastatic intermediate- and high-risk prostate cancer. However, this method is associated with symptomatic lymphocele (SLC), which is an important morbidity factor. To overcome this complication, several modifications of the technique have been developed, including the peritoneal interposition flap (PIF). We performed an updated systematic review and meta-analysis to investigate the efficacy and safety of this technique for preventing SLC and lymphocele (LC) formation. Materials and methods Searches were performed using databases and references from included studies and previous systematic reviews. Only randomized controlled trials and nonrandomized cohorts were included. Primary outcomes were the incidence of SLC and LC formation, and safety outcomes were defined as operation time, estimated blood loss, length of hospital stay, and urinary incontinence. Quality assessment was performed using the Newcastle-Ottawa Scale and Cochrane Collaboration's tool. Pooled treatment effects were estimated using odds ratios with 95% confidence intervals (CIs) for binary endpoints. Heterogeneity was examined using Cochran's Q test and I2 statistics; p values 25% were considered significant for heterogeneity. We used Mantel-Haenszel fixed-effect models in the analyses with low heterogeneity. Otherwise, the derSimonian and Laird random-effects model was used. Results The initial search yielded 510 results. After the removal of duplicate records and application of the exclusion criterion, 9 studies were fully reviewed for eligibility. Three randomized controlled trials and 5 retrospective cohorts met all the inclusion criteria, comprising 2261 patients, of whom 1073 (47.4%) underwent PIF. Managing inflammatory bowel disease(IBD)is becoming increasingly complex and personalized,considering the advent of new advanced therapies with distinct mechanisms of *** mucosal healing(MH)is a pivotal therapeutic goal in IBD management and can prevent IBD progression and reduce flares,hospitalization,surgery,intestinal damage,and colorectal *** proactive disease and therapy assessment is essential to achieve better control of intestinal inflammation,even if subclinical,to alter the natural course of *** monitoring of fecal calprotectin(FC)levels and interval endoscopic evaluations are cornerstones for evaluating response/remission to advanced therapies targeting IBD,assessing MH,and detecting subclinical ***,we comment on the article by Ishida et al Moreover,this editorial aimed to review the role of FC and endoscopic scores in predicting MH in patients with ***,we intend to present some evidence on the role of these markers in future targets,such as histological and transmural *** prospective multicenter studies with a stricter MH criterion,standardized endoscopic and histopathological analyses,and virtual chromoscopy,potentially including artificial intelligence and other biomarkers,are desired.

In this editorial,we comment on the article by Marangoni et al,published in the recent issue of the World Journal of Gastroenterology 2023;29:5618-5629,about“Diet as an epigenetic factor in inflammatory bowel disease”.The authors emphasized the role of diet,especially the interaction with genetics,in promoting the inflam-matory process in inflammatory bowel disease(IBD)patients,focusing on DNA methylation,histone modifications,and the influence of *** this editorial,we explore the interaction between genetics,gut microbiota,and diet,in an only ***,we provided dietary recommendations for patients with *** Western diet,characterized by a low fiber content and deficiency the micronutrients,impacts short-chain fatty acids production and may be related to the pathogenesis of *** the other hand,the consumption of the Mediter-ranean diet and dietary fibers are associated with reduced risk of IBD flares,particularly in Crohn’s disease(CD)*** to the dietary guidance from the International Organization for the Study of Inflammatory Bowel Diseases(IOIBD),the regular consumption of fruits and vegetables while reducing the consumption of saturated,trans,dairy fat,additives,processed foods rich in maltodextrins,and artificial sweeteners containing sucralose or saccharine is recommended to CD *** patients with ulcerative colitis,the IOIBD recommends the increased intake of natural sources of omega-3 fatty acids and follows the same restrictive recommendations aimed at CD patients,with the possible inclusion of red *** conclusion,IBD is a complex and hetero-geneous disease,and future studies are needed to elucidate the influence of epigenetics on diet and microbiota in IBD patients.

Performance of evolutionary optimized machine learning for modeling total organic carbon in core samples of shale gas fields

Rock samples' TOC content is the best indicator of the organic matter in source rocks. The origin rock samples' analysis is used to calculate it manually by specialists. This method requires time and resources because it relies on samples from many well intervals in source rocks. Therefore, research has been done to aid this effort. Machine learning algorithms can estimate total organic carbon instead of well logs and stratigraphic studies. In light of these efforts, the current work present a study on automating the total organic carbon estimation using machine learning approaches improved by an evolutionary methodology to give the model flexibility and precision. Genetic algorithms, differential evolution, particle swarm optimization, grey wolf optimization, artificial bee colony, and evolution strategies were used to improve machine learning models to predict TOC. The six metaheuristics were integrated into four machine learning methods: extreme learning machine, elastic net linear model, linear support vector regression, and multivariate adaptive regression splines. Core samples from the YuDong-Nan shale gas field, located in the Sichuan basin, were used to evaluate the hybrid strategy. The findings show that combining machine learning models with an evolutionary algorithms in a hybrid fashion produce flexible models that accurately predict TOC. The results show that, independent of the metaheuristic used to guide the model selection, optimized extreme learning machines attained the best performance scores according to six metrics. Such hybrid models can be used in exploratory geological research,particularly for unconventional oil and gas resources.

Multiple phase detector of M-ary phase shift keying symbols in code division multiple access systems

A novel iterative technique, the phase descent search detection was proposed. This technique constrained the solution （PDS） algorithm, for M-ary phase shift keying （M-PSK） symbols to have a unit magnitude and it was based on coordinate descent iterations where coordinates were the unknown symbol phases. The PDS algorithm, together with a descent local search （also implemented as a version of the PDS algorithm）, was used multiple times with different initializations in a proposed multiple phase detector; the solution with the minimum cost was then chosen as the final solution. The simulation results show that for highly loaded multiuser scenarios, the proposed technique has a detection performance that is close to the single-user bound. The results also show that the multiple phase detector allows detection in highly overloaded scenarios and it exhibits near-far resistance. In particular, the detector has a performance that is significantly better, and complexity that is significantly lower, than that of the detector based on semi-definite relaxation.

Effects of laying on of hands with and without a spiritual context on pain and functionality of older women with knee osteoarthritis:study protocol for a randomized controlled trial

Background： Laying on of hands （LooH） is a culturally-accepted therapy in several traditions. In Brazil, ＂Spiritism＂（third-largest religious tradition in number of followers） uses LooH with the name of ＂Spiritist Passe＂（SP）. However, there are few studies assessing SP＇s role in medical outcomes. The present study aims to investigate the effects of LooH, with and without a spiritual context, on pain, joint stiffness, and functional capacity of older women （≥60 years old） with knee osteoarthritis （KOA）. Methods/design： In this triple-blind, prospective randomized controlled trial, older women with KOA are assigned to three groups： LooH with a spiritual context group, LooH group without a spiritual context, and a control group receiving no intervention. Patients are assessed by a blinded researcher at baseline, 8 weeks, and 16 weeks. Pain, joint stiffness, and functional capacity are assessed using Western Ontario and McMaster universities Osteoarthritis Index. Other measures such as anxiety, depression, gait speed, and quality of life will also be assessed. To detect differences between groups, a post-intervention com- parison between the three groups and a mean change （post-pre） comparison among the three groups will be done using analysis of variance. All statistical analyses will be performed using an intention-to-treat approach and a per-protocol analysis. Discussion： A pragmatic design using SP, LooH without spiritual context, and no LooH may provide further scientific evidence on SP＇s feasibility and efficacy for KOA patients.

Needle chlorosis(NC)in Pinus taeda *** in Brazil becomes more frequent after second and third harvest rotation *** a study to identify factors contributing to yellowing needle chorosis(YNC),trees were grown in soils originating from contrasting parent materials,and soils and needles(whole,green and chlorotic portions)from 1-and 2-year-old branches and the first and second needle flush release at four sites with YNC on *** were analyzed for various elements and *** soils had very low base levels(Ca^(2+),Mg^(2+)and K^(+))and P,suggesting a possible lack of multiple *** symptoms started at needle tips,then extended toward the needle base with *** flush needles had longer portions with YNC than second flush needles *** from the lower crown also had more symptoms along their length than those higher in the *** were similar to those reported for *** chlorotic portions,Mg and Ca concentrations were well below critical values;in particular,Mg levels were only one third of the critical value of 0.3 g kg^(-1).Collectively,results suggest that Mg deficiency is the primary reason for NC of *** in various parent soils in Brazil.

Species composition and diversity of woody communities along an elevational gradient in tropical Dwarf Cloud Forest

Elucidating woody community diversity and structure change along environmental gradients is still a central issue of tropical forest ecology. We have evaluated changes in alpha and beta diversity, community composition and structure of woody communities along an elevational gradient in Dwarf Cloud Forests, Southeast Brazil. We selected five areas along an elevational gradient(1,300-1,600 m asl), and randomly allocated 10 plots(10 m × 20 m) in each area in the Ibitipoca State Park(ISP), Serra da Mantiqueira, southeastern Brazil. All woody species(diameter at breast height ≥ 5.0 cm) were collected for taxonomic identification. Thus, we analysed the woody communities based on a phytosociological approach. Overall, 147 species and 37 families were recorded, distributed in 2,303 individuals. No differences in the species richness pattern were observed between areas using the rarefaction and extrapolation curves. Significant differences in species composition and structure between areas were observed. The high beta diversity observed, corroborated by the Jaccard coefficient, increases with decreasing altitude. Our results showed that woody communities in the studied Dwarf Cloud Forests have a higher diversity and structural variability along elevational gradient.

Background：Due to high neutral lipids accumulation in the cytoplasm,in vitro-produced embryos from Bos primigenius indicus and their crosses are more sensitive to chilling and cryopreservation than those from Bos primigenius *** objective of the present study was to evaluate the effects of trans-10,cis-12 conjugated linoleic acid（CLA） on the development and cryotolerance of crossbred Bos primigenius taurus x Bos primigenius indicus embryos produced in vitro,and cultured in the presence of fetal calf *** zygotes（n = 1,692）were randomly assigned to one of the following treatment groups：1） Control,zygotes cultured in Charles Rosenkrans 2 amino acid（CR2aa） medium（n = 815） or 2） CLA,zygotes cultured in CR2 aa medium supplemented with 100 μmol/L of trans-10,cis-12 CLA（n =877）.Embryo development（cleavage and blastocyst rates evaluated at days 3 and 8 of culture,respectively）,lipid content at morula stage（day 5） and blastocyst cryotolerance（re-expansion and hatching rates,evaluated 24 and 72 h post-thawing,respectively） were compared between ***,selected mRNA transcripts were measured by Real-Time PCR in blastocyst ***：The CLA treatment had no effect on cleavage and blastocyst rates,or on mRNA levels for genes related to cellular stress and *** the other hand,abundance of mRNA for the 1-acylglycerol-3-phosphate0-acyltransferase-encoding gene（AGPAT）,which is involved in triglycerides synthesis,and consequently neutral lipid content,were reduced by CLA treatment.A significant increase was observed in the re-expansion rate of embryos cultured with trans-10,cis-12 CLA when compared to control（56.3 vs.34.4%,respectively,P = 0.002）.However,this difference was not observed in the hatching rate（16.5 vs.14.0%,respectively,P=0.62）.Conclusions：The supplementation with trans-10,ds-12 CLA isomer in culture medium reduced the lipid content of in vitro produced bovine embryos by reducing the gene expression of 1-acylglycerol 3-phosphate 0
