OBJECTIVE: united nations Relief and Works Agency for Palestine Refugees in the Near East (UNRWA) provides primary health care services including care for diabetes and hypertension, with limited resources under difficult circumstances in Gaza, West Bank, Jordan, Lebanon and Syria. A total of 114,911 people with diabetes were registered with UNRWA health centres in 2011. The aim of this cross-sectional observational study was to assess the quality of diabetes care in the UNRWA primary health care centres. METHOD: The study population consisted of 1600 people with diabetes attending the 32 largest UNRWA health centres and treated there for at least one year. Between April and Sept. 2012 data from medical records, including results of clinical examinations and laboratory tests performed during the last one year, current management including self-care education and evidence of diabetes complications were collected and recorded in a previously validated data collection form (DCF). Patients were interviewed and clinically examined on the day of the audit and blood collected for HbA1c testing which was done at a central lab using High-performance liquid chromatography (HPLC) method (HLC®-723G8 Tosoh Corporation, Japan). Data was transferred from paper records into a computer and analysed with Epi-info 2000. RESULTS: Type 1 diabetes was present in 4.3% and type 2 diabetes in 95.7%. Co-morbid hypertension was present in 68.5%;90.3% were either obese (64.0%) or overweight (26.3%). Clinical management of diabetes was largely in line with UNRWA’s technical instructions (TI) for diabetes. Records for 2 hour postprandial glucose (2 h PPG), serum cholesterol, serum creatinine, and urine protein analysis were available in 94.7%, 96.4%, 91.4% and 87.5%, cases, respectively. Records of annual fundoscopic eye examination were available in 47.3% cases but foot examinations were less well documented. Accurate,consistent,and high-resolution Land Use&Cover(LUC)information is fundamental for effectively monitoring landscape dynamics and better apprehending drivers,pressures,state,and impacts on land ***,the availability of such national products with high thematic accuracy is still limited and consequently researchers and policymakers are constrained to work with data that do not necessarily reflect on-the-ground realities impending to correctly capture details of landscape features as well as limiting the identification and quantification of drivers and rate of ***,we took advantage of the Switzerland’s official LUC statistical sampling survey and dense time-series of Sentinel-2 data,combining them with Machine and Deep Learning methods to produce an accurate and high spatial resolution land cover map over the Lake Geneva *** suggest that time-first approach is a valuable alternative to space-first approaches,accounting for the intra-annual variability of classes,hence improving classification *** demonstrate that Deep Learning methods outperform more traditional Machine Learning ones such as Random Forest,providing more accurate predictions with lower *** produced land cover map has a high accuracy,an improved spatial resolution,while at the same time preserving the statistical significance(*** proportion)of the official national *** work paves the way towards the objective to produce a yearly high resolution land cover map of Switzerland and potentially implement a continuous land change monitoring *** further work is required to properly address challenges such as the need for increased temporal resolution for LUC information or the quality of training samples.

Black soils, characterized by their thick, dark horizons enriched with organic matter, epitomize highly fertile soils. However, their fertility precipitates intense land use, engendering challenges such as soil erosion, nutrient depletion, pollution, compaction, salinization, and acidification. Notably, these soils are significant contributors to global greenhouse gas emissions, primarily due to substantial losses in soil organic carbon. Despite these challenges, black soils are pivotal for global food production. This paper delineates the implementation of digital soil mapping for the global cartography of black soils and human interference on these soils. Predominantly distributed in Eastern Europe, Central and Eastern Asia, and North and South America, black soils cover an approximate area of 725 Mha, with the Russian Federation,Kazakhstan, and China collectively have over half of this area. Agriculturally,these soils underpin significant proportions of global crop yields, producing 66% of sunflower, 30% of wheat, and 26% of potato outputs. The organic carbon content in the upper 30 cm of these soils is estimated at 56 *** management of black soils is imperative for ensuring food security and addressing climate change on a global scale.

Effective waste management is a major challenge for Small Island Developing States (SIDS) like Maldives due to limited land availability. Maldives exemplifies these issues as one of the most geographically dispersed countries, with a population unevenly distributed across numerous islands varying greatly in size and population density. This study provides an in-depth analysis of the unique waste management practices across different regions of Maldives in relation to its natural and socioeconomic context. Data shows Maldives has one of the highest population density and per capita waste generation among SIDS, despite its small land area and medium GDP per capita. Large disparities exist between the densely populated capital Male’ with only 5.8 km2 area generating 63% of waste and the ~194 scattered outer islands with ad hoc waste management practices. Given Male’s dense population and high calorific waste, incineration could generate up to ~30 GW/a energy and even increase Maldives’ renewable energy supply by 200%. In contrast, decentralized anaerobic digestion presents an optimal solution for outer islands to reduce waste volume while providing over 40%–100% energy supply for daily cooking in local families. This timely study delivers valuable insights into designing context-specific waste-to-energy systems and integrated waste policies tailored to Maldives’ distinct regions. The framework presented can also guide other SIDS facing similar challenges as Maldives in establishing sustainable, ecologically sound waste management strategies.

Endosulfan residues and farmers’replacement behaviors of endosulfan in the north-west inland cotton region

We assessed the situation of endosulfan residues in cotton fields after the endosulfan ban came into effect and the current knowledge,attitude,and practice(KAP)of cotton farmers on the phase-out of endosulfan and the application of alternative *** samples(n=91)of cotton fields were collected from the major cotton-producing areas in China,namely the north-west inland cotton region,and the endosulfan residues were analyzed.A KAP survey was carried out for cotton farmers,and 291 questionnaires were *** influences of gender,age,education background,cotton planting years,publicity and training,income sources,and other factors on cotton farmers’KAP were *** results showed that endosulfan sulfate was the main endosulfan residue in the soil,followed byβ-endosulfan andα-endosulfan,the average residual contents were 0.569,0.139,and 0.060μg/kg,*** results of the KAP study showed that cotton farmers scored low on knowledge about the phase-out of endosulfan and the application of alternative technologies but high on attitude and *** number of family members,years of cotton planting,age,and the cottonplanting area had different degrees of influence on KAP *** training could significantly improve the KAP scores of cotton farmers;training should be more targeted and designed reasonably for key groups,such as men and the population under 30,followed by training them to use pesticides *** large-scale cotton growers,training should focus on green prevention and control technologies.

Access to off-farm employment has been expected to be a critical approach to ending hunger and all forms of malnutrition,two important targets of achieving Zero *** study aims to investigate the role of off-farm employment in improving dietary diversity through substitution effect and complementary effect with agricultural production activities and income *** study adopts Poisson/Tobit/Probit/OLS regressions and the instrument variable method based on the primary survey data collected among 1,282 households at 12 sites in environmentally and economically vulnerable areas of China,Nepal,Cambodia,Thailand,and Myanmar in *** results show that off-farm employment is positively associated with household dietary diversity and the consumption of flesh meat,fish and other aquatic animals,fruits,and milk and dairy products,which are rich in protein and *** results of mechanism analysis show that off-farm employment contributes to household dietary diversity by improving crop diversity,especially for poor households,boosting the probability of livestock raising for households with the middle one-third disposal income,and increasing household *** positive association between off-farm employment and household dietary diversity is much higher for households with the bottom one-third disposal income,low illiteracy,and from upper-middle income *** findings imply that off-farm employment does play a vital role in achieving multiple benefits of poverty alleviation,malnutrition reduction,and agrobiodiversity conservation in environmentally and economically vulnerable ***,it may enlarge the gaps in dietary diversity between households with low human capital and from low and lower-middle income countries and those with high human capital and from middle-high countries.
