Chronic hepatitis B is a major health problem in China. The universal vaccination program since 1992 has changed the epidemiology of hepatitis B virus infection in China from highly to moderately endemic. The most prevalent hepatitis B virus strains in China are genotypes B and C, whereas those in western provinces are genotypes D and C/D hybrid. Chronic hepatitis B poses a heavy burden to the society in China. Different treatment strategies have been explored to improve patient outcomes in a cost-effective manner. However, antiviral drugs with a low genetic barrier to resistance are still extensively used because of the generally low income and limited resources in China. Individualized antiviral therapy is closely associated with translational medicine, which utilizes information from studies on genomics, immune biomarkers, and fibrosis. The results of these studies are crucial in further improving treatment outcomes.

Efficacy and safety of sofosbuvir and daclatasvir in treatment of kidney transplantation recipients with hepatitis C virus infection

AIM To assess the efficacy and safety of sofosbuvir and daclatasvir regimens for kidney transplantation(KT) patients with hepatitis C virus(HCV) *** This study enrolled a prospective cohort of consecutive Chinese KT patients with HCV infection. They were given sofosbuvir combined with daclatasvir, with or without ribavirin. They were monitored regularly during and after the treatment. RESULTS Six patients were recruited in our prospective study cohort. All patients were male and naive to directacting antiviral treatment. The treatment duration was 12 wk. Most patients(4/6) were infected with HCV genotype 1b. HCV RNA was undetectable at week 4 after treatment and at the end of treatment in all patients. Sustained virological response rate at 12 wk was 100%(6/6). Two patients had to accept a half dose of sofosbuvir due to serum creatinine elevation during treatment. Kidney function in the remaining patients was stable. No serious adverse events(AEs) were observed. No patient discontinued antiviral therapy due to side effects. CONCLUSION Sofosbuvir and daclatasvir for treatment of KT recipients with HCV infection are highly efficient and safe. Patients tolerated the medications well, and no serious AEs were observed. Larger prospective cohort studies are needed to validate these results.

Expression of inducible nitric oxide synthase in human gastric cancer

INTRODUCTIONInducible nitric oxide synthase (iNOS) is an enzyme that catalyzes the formation of nitric oxide (NO) from L-arginine. iNOS expression and activity results in the production of high levels of NO[1]. The generation of physiological levels of NO is important for mucosal function and it also exerts a cytoprotective effect on the gastrointestinal mucosa. However, increased iNOS expression has been observed in patients with chronic inflammatory diseases of the gastrointestinal tract, such as ulcerative colitis[2,3], and gastritis[4] and it has been speculated that increased NO may induce DNA damage[5,6] and angiogenesis[7]. Nonetheless, the role of iNOS in human GI neoplasia is largely unkown. Previous studies have demonstrated increased iNOS expression in breast cancer[8,9], and increased iNOS activity and protein levels have been demonstrated in colorectal cancer[10] and adenocarcinoma of the esophagus[11]. However, to date, the role of iNOS in gastric carcinogenesis has not been elucidated.
