Background: Costal fracture surgical is still a debate, therefore we shall select between early and delay surgical management. Case Report: We are reporting two cases of post road traffic clash delay ribs fractures osteosynthesis involving a 63-year-old man with multistage fractures on the left and pulmonary pinning of one of the costal arches, complicated by a homolateral haemothorax and a 41-year-old man with a bilateral flail chest. Conclusion: The simple postoperative course and the immediate postoperative improvement in the patient’s clinical respiratory condition enabled us to discuss the time frame for management, in this case the indication for early or later surgery.

Background: Breast cancer is the dominant cancer in women in both developed and developing countries. The objective of this study was to assess the knowledge, attitudes, practices and factors associated with early breast cancer screening among women in the Municipality of Abomey-Calavi in Benin. Methods: This was a cross-sectional, descriptive, analytical study with prospective data collection from October 1 to 8, 2018, involving 1740 women in the Municipality of Abomey-Calavi, aged 18 years or older and selected by WHO four-stage random cluster sampling. Consenting women who were mentally competent, 18 years of age or older at the time of the survey, and residing continuously in the Municipality of Abomey-Calavi for the last six months prior to the survey were included. On the other hand, women who belonged to a breast cancer prevention service, women in whom secondary screening was noted, or non-consenting women were not included. The initial minimum size was estimated by the Schwartz formula with a cluster effect of k = 2. Information was collected by questionnaire survey, entered with Epidata 3.1. Fr and analyzed with R Studio 3.5.1. software. Results: The mean age of the women surveyed was 32.0 ± 11.5 years with a range of 18 and 71 years. Regarding knowledge, the clinical manifestation known by the majority of women was the presence of a nodule (68.50%). In the series, 1308 (75.17%) declared having heard about breast cancer once before, either on the radio, television or from friends and 726 (55.50%) had heard about breast cancer screening. Five hundred and twelve (70.52%) of the 726 who had heard of breast cancer said they knew that breast cancer could be screened earlier. Breast self-examination was the most cited screening method (67.58%). The disease is of natural origin according to 37.84% of them. 目的使用心脏MRI分析高盐饮食(HSd)在活小鼠左心室(LV)质量、收缩功能和冠状动脉储备中的作用。方法 30只C57BL/6母鼠喂养1个月,包括15只对照组和15只HSd组。3个月后,采用三维(3d)高时间分辨率锰-增强MRI黑血序列评估LV容积、射血分数和质量,采用动态MR血管成像在静息期及腺苷诱导的充血期评估冠状动脉血流速度储备(CFVR)。切除心脏对LV湿质量及组织学的微血管重构进行评估。结果 HSd组全部心脏组织学研究均发现微血管重构,而对照组未发现一例。就心脏质量、LV容积和射血分数而言,HSd组和对照组之间没有区别。与对照组相比,由于HSd组的体质量较低[(22.3±0.9)g∶(24.0±1.4)g,Pd组具有较低的CFVR[(1.73±0.11)∶(1.94±0.12),Pd高时间分辨率锰-增强MRI评估活小鼠高血压性心脏病的表型是可行的。HSd与冠状动脉储备的早期损害存在相关性,早于心脏显著肥大的发生。要点①采用MRI评估小鼠高血压性心脏病的活体表型是可行的。②小鼠HSd与冠状动脉储备的早期损害有关。③小鼠高盐饮食引起冠状动脉储备的改变早于心室肥厚的发生。

背景:二叶式主动脉瓣反流可由瓣膜自身缺损或主动脉根部结构复合体中的某一个或多个结构功能异常导致。因此,成功的修复需同时矫治各方面的问题。作者对在矫治二叶式主动脉瓣反流时同时矫治瓣膜缺陷和主动脉根部功能异常的经验进行了回顾。方法和结果:1996—2004年间,作者治疗了68例主动脉反流患者。30例患者为单一的主动脉反流,其余38例伴有升主动脉瘤。所有患者接受标化联合手术治疗,包括:正中裂切除或瓣叶折叠,连合下瓣环成形术,强固瓣膜游离缘,窦管结合部位折叠。

Cardiomyopathies in Tropical Countries:Causes and Nosological Perspective

Background: Cardiomyopathy is the main cause of heart failure in developing countries, mainly in Africa. In those areas the concept of “tropical cardiomyopathy” is still used to design all unexplained cardiomyopathy. The primary aim of this review is first to review the main etiologies of cardiomyopathies observed in tropical countries and second to gain a better understanding of the nosological place of the so-called “tropical cardiomyopathies” in the current framework of cardiomyopathies. Methods and Results: We reviewed relevant references over the last forty years (June, 1976 to May 2012). Given literature data, endomyocardial fibrosis (EMF) is mainly diagnosed in sub-Saharan countries, as well as Brazil and India. Peripartum cardiomyopathy (PPCM) is observed with a higher prevalence than in temperate countries. Sickle cell anemia does not induce specific cardiomyopathy in all echocardiographic studies. Malnutrition and chronic anemia can induce reversible cardiac dysfunction. Myocardial involvement in parasitic infections is restricted to Chagas disease and probably to human African trypanosomiasis. Helminthiasis is not involved in the pathogenesis of cardiomyopathy except for the deleterious effect of high eosinophilia induced by some endemic diseases (filariasis, schistosomiasis). Primary cardiomyopathies (dilated, hypertrophic, and restrictive cardiomyopathy) have no specificity. Arrhythmogenic right ventricular dysplasia and left ventricular noncompaction are also reported and do not differ from elsewhere. Conclusions: The concept of tropical cardiomyopathy is no longer relevant as most of the cardiomyopathies observed in tropical countries have no specificity, with few exceptions (PPCM, EMF, Chagas disease). In this context, the European Society of Cardiology classification offers a simpler clinical approach and allows the inclusion of the rare tropical specificities.

The triggering receptor expressed on myeloid cells-1(TREM-1)is a receptor expressed on innate immune *** promoting the amplification of inflammatory signals that are initially triggered by Toll-like receptors(TLRs),TREM-1 has been characterized as a major player in the pathophysiology of acute and chronic inflammatory diseases,such as septic shock,myocardial infarction,atherosclerosis,and inflammatory bowel ***,the molecular events leading to the activation of TREM-1 in innate immune cells remain ***,we show that TREM-1 is activated by multimerization and that the levels of intracellular Ca 2+release,reactive oxygen species,and cytokine production correlate with the degree of TREM-1 ***-1 activation on primary human monocytes by LPS required a two-step process consisting of upregulation followed by clustering of TREM-1 at the cell surface,in contrast to primary human neutrophils,where LPS induced a rapid cell membrane reorganization of TREM-1,which confirmed that TREM-1 is regulated differently in primary human neutrophils and *** addition,we show that the ectodomain of TREM-1 is able to homooligomerize in a concentration-dependent manner,which suggests that the clustering of TREM-1 on the membrane promotes its *** further show that the adapter protein dAP12 stabilizes TREM-1 surface expression and ***-1 multimerization at the cell surface is also mediated by its endogenous ligand,a conclusion supported by the ability of the TREM-1 inhibitor LR12 to limit TREM-1 *** results provide evidence for ligand-induced,receptor-mediated dimerization of ***,our findings uncover the mechanisms necessary for TREM-1 activation in monocytes and neutrophils.

Acute heart failure(AHF)is a major cause of unplanned hospitalisations in the elderly and is associated with high *** prevalence has grown in the last years due to population aging and longer life expectancy of chronic heart failure *** international societies have provided guidelines for the management of AHF in the general population,scientific evidence for geriatric patients is often lacking,as these are underrepresented in clinical *** have a different risk profile with more comorbidities,disability,and frailty,leading to increased morbidity,longer recovery time,higher readmission rates,and higher ***,therapeutic options are often limited,due to unfeasibility of invasive strategies,mechanical circulatory support and cardiac ***,the in-hospital management of AHF should be tailored to each patient's clinical situation,cardiopulmonary condition and geriatric *** care should be considered in some cases,in order to avoid unnecessary diagnostics and/or *** discharge,a strict follow-up through outpatient clinic or telemedicine is can improve quality of life and reduce rehospitalisation *** aim of this review is to offer an insight on current literature and provide a clinically oriented,patient-tailored approach regarding assessment,treatment and follow-up of elderly patients admitted for AHF.
