Uganda has been implementing the Global health Security Agenda(GHSA)since 2015 to build its capacity according to World health Organization(WHO)Benchmarks on International health Regulations *** country remains prone to outbreaks,with more than 20 disease outbreaks reported in the past five years,including Ebola virus disease,Crimean-Congo haemorrhagic fever,Marburg haemorrhagic fever,measles,yellow fever,coronavirus disease 2019(COVID-19),and *** resistance(AMR)is an ongoing *** scored capacity level 3 on infection prevention and control(IPC)and antimicrobial stewardship(AMS)in the 2017 Joint External Evaluation(JEE)*** gaps were being addressed after a self-assessment in *** paper describes the technical assistance approaches provided to Uganda by the Medicines,Technologies,and Pharmaceutical services Program,funded by the United States(U.S.)Agency for International Development,and implemented by Management sciences for *** program,through a One health approach,supported systematic capacity strengthening based on the JEE's capacity advancement framework for global health security,specifically relating to *** program's interventions impacted 32 WHO benchmark actions(7 for AMR multisectoral coordination,16 for IPC,and 9 for AMS),contributing to Uganda's strengthened GHSA *** success built on the AMR platform,the program trained 745 health workers in IPC for the Ebola virus and provided support for simulation exercises by eight district IPC *** program also worked with the Ministry of health to coordinate IPC for the COVID-19 response in five health regions,covering 45 districts and reaching 5,452 health workers at 858 health facilities.

Fabrication and evaluation of carboxymethyl guar gum-chitosan interpenetrating polymer network (IPN) nanoparticles for controlled drug delivery

Guar gum(GG)has drawn more interest from researchers in the last several decades for the controlled delivery of *** aim of the research is to fabricate and evaluate an interpenetrating polymer network(IPN)based nanocarriers system employing chemically modified GG to achieve controlled release of *** present study,we have reported the synthesis of carboxymethyl GG-chitosan IPN nanoparticles for controlled release of metformin *** guar gum(CMGG)was synthesized and employed for developing IPN nanoparticles in combination with chitosan in varying polymeric ratios.A cross-linking agent,sodium tripolyphosphate(Na-TPP)was used for the synthesis of IPN colloidal suspension of nanoparticles which was lyophilized to obtain *** carboxymethylation of GG was confirmed through Fourier transform infra-red spec*** mean particle size was 440.6 nm indicating a polydispersity index of *** drug entrapment efficiency was found to vary between 67.461.64%to 88.232.02%.The differential scanning calorimetry study revealed the presence of amorphous metformin hydrochloride as a homogenous dispersion in nanoparticles(NPs)*** vitro drug release study indicated controlled release ability of developed IPN matrix.A2 formulation(chitosan:CMGG-1:1)exhibited 78.110.27%release after 8 *** evaluation performed in alloxan-treated diabetic rat model exhibited a prolonged hypoglycemic efficacy over 6 h,for A2 formulation treated group,compared to the group receiving only metformin HCl,which demonstrated a reduction in blood glucose levels for up to 4 *** study findings demonstrated the efficacy of the CMGG-chitosan based IPN nanoparticulate system for sustained delivery of therapeutic agents.

A credible traffic prediction method based on self-supervised causal discovery

Next-generation wireless network aims to support low-latency, high-speed data transmission services by incorporating artificial intelligence(AI) technologies. To fulfill this promise, AI-based network traffic prediction is essential for pre-allocating resources, such as bandwidth and computing power. This can help reduce network congestion and improve the quality of service(QoS) for users. Most studies achieve future traffic prediction by exploiting deep learning and reinforcement learning, to mine spatio-temporal correlated variables. Nevertheless, the prediction results obtained only by the spatio-temporal correlated variables cannot reflect real traffic changes. This phenomenon prevents the true prediction variables from being inferred, making the prediction algorithm perform poorly. Inspired by causal science, we propose a novel network traffic prediction method based on self-supervised spatio-temporal causal discovery(SSTCD). We first introduce the Granger causal discovery algorithm to build a causal graph among prediction variables and obtain spatio-temporal causality in the observed data, which reflects the real reasons affecting traffic ***, a graph neural network(GNN) is adopted to incorporate causality for traffic prediction. Furthermore,we propose a self-supervised method to implement causal discovery to to address the challenge of lacking ground-truth causal graphs in the observed data. Experimental results demonstrate the effectiveness of the SSTCD method.

circRNA3669 promotes goat endometrial epithelial cells proliferation via miR-26a/RCN2 to activate PI3K/AKT-mTOR and MAPK pathways

The development of receptive endometrium(RE) from pre-receptive endometrium(PE) for successful embryo implantation is a complex dynamic process in which the morphology and physiological states of the endometrial epithelium undergo a series of significant changes, including cell proliferation and apoptosis. However, the molecular mechanisms are not yet fully understood. In this study, a higher circRNA3669 level was observed in PE than in RE of goats. Functional assays revealed that this overexpression promoted the proliferation of goat endometrial epithelial cells(GEECs) by activating the expression of genes related to the PI3K/AKT-mTOR and MAPK pathways,thereby inhibiting apoptosis in vitro. Furthermore, circRNA3669 functioned as a competing endogenous RNA(ceRNA) to upregulate Reticulocalbin-2(RCN2) expression at the post-transcriptional level by interacting with and downregulating miR-26a in GEECs. In addition, RCN2, which is highly expressed in the PE of goats, was found to be regulated by β-estradiol(E2) and progesterone(P4). Our results demonstrated that RCN2 also affected the key proteins PI3K, AKT, mTOR, JNK, and P38 in the PI3K/AKT-mTOR and MAPK pathways, thereby facilitating GEECs proliferation and suppressing their apoptosis in vitro. Collectively, we constructed a new circRNA3669-miR-26aRCN2 regulatory network in GEECs, which further provides strong evidence that circRNA could potentially play a crucial regulatory role in the development of RE in goats.

General anesthesia and Obstetric Anesthesia is the gold standard for a cesarean section but there are some cases where general anesthesia is unavoidable. The use of general anesthesia for cesarean delivery has decreased in recent years due to the widespread use of neuraxial techniques. The choice of anesthesia techniques for cesarean delivery depends on several factors, including the patient’s psychology and the attending physician’s experience. It is chosen because of its safety profile and its benefits to the mother and fetus. It may be indicated due to emergency, maternal refusal of regional techniques, or regional contraindications. Major complications include failed intubation, gastric content aspiration, and increased bleeding risk. This study aims to evaluate the impact of a newly launched team on obstetric anesthesia practice.
