Patient Perceptions and Effects of Single Community Pharmacy Systems： Key Elements for Transitions of Care

The primary objective of the study was to evaluate baseline patient perception on single versus multiple community pharmacy systems. The secondary objective was to determine effects of a pharmacist provided educational intervention on perception of using a single community pharmacy system. This was a prospective survey based study implemented in a single grocery store chain pharmacy and one location of a large retail pharmacy. An anonymous pre-survey was administered to eligible patients. The primary investigator then delivered a brief educational intervention followed by an anonymous post survey. This aimed to determine if there was a change in perception of components assessed in the pre-survey. Data analysis was performed using descriptive statistics. Subjects reported using approximately 1.5 pharmacies. Major reasons included cost, location, and immunizations. 63% of subjects answered that their pharmacist will not have an accurate medication list when using multiple pharmacies, however, 92% of subjects responded that the pharmacist should be aware of all medications they take. Education on multiple pharmacy use may help protect against its risks. Cognizance of medications filled at other pharmacies and close communication between patients and pharmacists may help foster pharmacist-patient relationships while reducing the risk of multiple pharmacy use.

Current state of knowledge on the excretion of mRNA and spike produced by anti-COVID-19 mRNA vaccines;possibility of contamination of the entourage of those vaccinated by these products

The massive COVID-19 vaccination campaign is the first time that mRNA vaccines have been used on a global *** mRNA vaccines correspond exactly to the definition of gene therapy of the American and European regulatory *** regulations require excretion studies of these drugs and their products(the translated proteins).These studies have not been done for mRNA vaccines(nor for adenovirus vaccines).There are numerous reports of symptoms and pathologies identical to the adverse effects of mRNA vaccines in unvaccinated persons in contact with freshly vaccinated *** is therefore important to review the state of knowledge on the possible excretion of vaccine nanoparticles as well as mRNA and its product,the spike *** mRNA-carrying lipid nanoparticles spread after injection throughout the body according to available animal studies and vaccine mRNA(naked or in nanoparticles or in natural exosomes)is found in the bloodstream as well as vaccine spike in free form or encapsulated in exosomes(shown in human studies).Lipid nanoparticles(or their natural equivalent,exosomes or extracellular vesicles(EVs))have been shown to be able to be excreted through body fluids(sweat,sputum,breast milk)and to pass the transplacental *** EVs are also able to penetrate by inhalation and through the skin(healthy or injured)as well as orally through breast milk(and why not during sexual intercourse through semen,as this has not been studied).It is urgent to enforce the legislation on gene therapy that applies to mRNA vaccines and to carry out studies on this subject while the generalization of mRNA vaccines is being considered.

Why are children and many adults not affected by COVID-19? Role of the host immune response

Investigation of Factors Affecting Preparedness of Reporting Adverse Drug Reactions among Nurses in Public and Private Hospitals in Dar Es Salaam, Tanzania

The current study assessed preparedness among nurses about reporting adverse drug reactions in public and private hospitals. Nurses in hospitals are among health providers very close to patients and are involved in the process of administering medications especially to inpatients. A cross sectional study was conducted among nurses in public and private hospitals in Dar es Salaam city in Tanzania to compare their preparedness on reporting adverse drug reactions (ADRs). A total of 384 nurses were involved in this study, of which 50% were drawn from public hospitals and 50% from private hospitals. Majority of respondents (75.25%) in public and 84.73% in private hospitals said they have not received any training about reporting ADRs. Of the few trained nurses, 85.43% and 96.55%, in public and private hospitals, respectively, said they have been trained in a seminar only once, after they started working as nurses. Respondents in public (19.17%) and private (32.8%) hospitals reported to stock ADRS forms (Yellow cards). Less than 50% of respondents agreed to have access to reference materials such as a text books named “Good Dispensing Practice” and a “List of Registered Medicines”. Further results showed more than two third of all respondents in private (74.25%) and public (73.5%) hospitals reported that lack of training affects the process of reporting ADRs in terms of ability and tendency. In this study we found training, availability of Yellow cards and presence of a focal person are among important contributing factors to preparedness of reporting ADRs among nurses in public and private hospitals.

BACKGROUND Gardnerella vaginalis(***)is a facultative anaerobic bacteria known to cause bloodstream ***,cases are very rare in *** is very limited clinical experience in the treatment of bloodstream infections caused by ***,there is an urgent need for effective antibacterial drugs to treat patients with bloodstream infections caused by *** SUMMARY A woman who underwent a cesarean section presented with a sudden onset of high fever 1-d *** blood cultures suggested an infection due to ***,and treatment with cefoperazone-sulbactam was *** 5 d of treatment,there was a decrease in the hemogram;however,the temperature and C-reactive protein levels remained *** on clinical experience and a review of literature,the treatment was modified to include ornidazole in combination with *** a week of treatment,the temperature,hemogram and C-reactive protein levels returned to normal,and blood cultures turned negative,suggesting a therapeutic effect of the combination *** This case highlighted the effective use of cefoperazone-sulbactam combined with ornidazole for bloodstream infection caused by *** following a cesarean section.

Factors Affecting Medication Non Adherence in Type 2 Sudanese Diabetic Patients

Background: Medications non-adherence is responsible for treatment failure and a reason of higher cost of medical care. It had been proved that, in general, diabetic patients are non-adherent to their treatment and only a small number of them were found to be adhering with all aspects of diabetic care. Objectives: To assess and evaluate the most common factors associated with medications adherence among Type 2 Diabetes Mellitus (DM) patients attending Ribat University Hospital Diabetic Clinic (RUHDC) Khartoum state, Sudan. Methods: Single Centre Descriptive cross sectional hospital based study was carried out among 351 patients using structured questionnaire and patients cards. Results: A total of 351 patients were recruited in the study;65.8% were females. About 64.4% were on oral medications and 35.6% on insulin. Adherence to medications among total Type 2 DM patients was 45%. Main barriers to medication adherence were drugs unavailability (34.3%) and forgetfulness (30.7%). Medications knowledge was 41.2% for patients on oral medications and 38.4% for insulin users. Conclusion: Adherence to anti-diabetic drugs in this study was found to be sub-optimal but considered reasonable in comparison with that reported by many African countries. Poor medications knowledge, drug brand unavailability and forgetfulness were the main reasons for medications non adherence. Family support, improving healthcare system and changing patents’ behavior will be needed to improve medications adherence.
