Military nurses are exposed to various occupational *** contributes to adaptation to critical situations and *** care is an essential part of holistic care which,in recent years,has attracted the attention of health-related *** aim of this study was to determine the attitude of military nurses,faculty members,and military nursing students toward spirituality and spiritual *** a cross-sectional descriptive study,214 military nurses,faculty members,and nursing students of Baqiyatallah University of Medical Sciences in Tehran were selected according to inclusion criteria by simple sampling method and completed the spirituality and spiritual care rating *** mean score of attitude toward spirituality and spiritual care in clinical nurses was 69.4(standard deviation[SD]±12.6),in nursing professors 74.6(SD±6.6),and in students 67.5(SD±4/11).The majority of the research community was at a high and desirable level(between 63 and 92,which was the maximum score of the questionnaire).The results of this study showed that military nurses,faculty members,and nursing students of Baqiyatallah University of Medical Sciences in Tehran have a positive attitude toward *** tend to provide spiritual ***,it is necessary to provide a suitable platform for providing spiritual care.

背景缓释剂型能够延长局麻药的镇痛效果并降低其血浆峰浓度，这使我们可以更加安全地使用较大剂量的局麻药，延长镇痛的作用时间。之前我们报道了一种主动载入多囊大囊泡（1argemultivesicularvesicles，LMVVs）的布比卡因（多囊脂质体布比卡因），并证明在动物和人体内可以使镇痛作用时间延长5倍以上。本研究中，我们给出多囊脂质体布比卡因在人体内的药物代谢动力学数据。方法健康志愿者皮下注射0．5％单纯布比卡因20ml，1周后，注射2％的多囊脂质体布比卡因20ml，进行前瞻性非盲法交叉对照研究。结果8例受试者被纳入研究，未观察到局麻药的主观不良反应。通过血浆浓度一时间曲线模型获得血浆峰浓度及达峰时间。两组血浆峰浓度无显著差异（单纯布比卡因与脂质体布比卡因的浓度分别为0．87±0．45μg／ml和0．83±0．34μg／ml，P=0．83，无显著性差异）。这些数值远远低于被公认的中毒血浆浓度，即2-4μg／ml。达峰时间在脂质制剂中延长7倍（262±149分钟与37．5±16分钟，P〈0．01）。结论尽管以新的脂质制剂形式注入的布比卡因总剂量增加至4倍，两组的布比卡因血浆峰浓度并无差异。脂质体布比卡因消除和向血浆再分布的延迟与此前报道的长效制剂缓释效应导致的药代动力学效应的延长是相符的。

The Change in Pulse Rate and Behavioral Score by Using Video Assisted Induction of Pediatric Anesthesia

Purpose: Objective of this study was to determine whether video assisted anesthesia induction reduced pediatric patients’ stress. Methods: With approval from the local ethics committee and parental informed consent, 75 children undergoing minor surgery were investigated in this prospective observational study. Patients were divided into three groups: group 1 was aged two to three years old, group 2 was aged four to six years old and group 3 was aged from seven to ten years old. The following three characteristics were evaluated: 1) the pulse rate at four points (the ward, the entrance at the operating room, mask notification and the mask fit);2) the behavioral score in the operating room;3) the amount of pain killers after the operation. Results: In group 1 (N = 20), there was a significant difference between the control group and the video assisted group regarding the percentage change in pulse rate based on the children’s ward when the patients looked at the mask. In group 2 (N = 26), there was no significant difference regarding any points. In group 3 (N = 29), there was a significant difference between control and video assisted group regarding the percentage change in pulse rate based on the children’s ward for all points. Also, regarding to the behavioral score, there was a significant difference between the control group and the video assisted group of all ages. However, there was no significant difference regarding the use of NSAIDs in the postoperative period between the control and the video assisted group. Conclusion: These results show that the video assisted anesthesia induction is effective for pediatric patients.

Syngnathia, congenital fusion of the mandible and maxilla, is a rare anomaly. We describe fiberoptic intubation of the trachea whilst controlling the airway with a nasal tube. A simple modification of this technique, previously described in the management of the difficult pediatric airway [1], simplifies the procedure with less potential for complications.

Self-Reported Assessment of Health Status and Psychological Condition among Hospital Medical Residents in Iran

Psychological stress and illness among hospital residences have been subject to increasing scientific scrutiny;however very few studies have been published to describe the extent of health status and psychological stresses in residents in different specialties especially in our country that our study was mainly focused on this subject. Methods: The study subjects were medical residents in public hospitals covering 14 different specialties in Tehran in 2007. A self-administered questionnaire elicited information related to socio-demographic profile, specialty, duty hours, sleep quality, physical activity level, number of night shifts per month, mood changes, sense of depression after night shifts, fatigue, use of antidepressant and tranquilizer, and their opinion regarding their health status on a 4-point Likert scale. Results: 66.7% of participants were male with the mean age of 32.9 ± 4.2 years. 62.1% of the residents felt mood changes after nightly shifts that could adversely affect their daily living, quality of work, and social relationships. The overall prevalence of the use of antidepressant drugs was 20.0% which was significantly higher in women than men. Also, 24.4% of them reported consumption of sedative and hypnotic drugs. 15.6% reported complete healthy status while 9.4% reported partial illness. Complete healthy status was more reported among the residents of anesthesiology and pediatrics while illness was more reported by residents in pathology field. Residents’ satisfaction with their status was positively correlated with the year of residency and marriage, while dissatisfaction was more reported in divorced ones as well as in those with higher number of nightly shifts. Conclusion: Notable number of hospital residents in Iran experience significant stressors and emotional and mental health problems. 目的发布欧洲重症医学学会(European Society of Intensive Care Medicine, ESICM)关于神经重症监护患者液体治疗的专家共识和临床实践推荐意见。设计在2016年10月召开的ESICM LIVE 2016会议上召集了一个由22名国际专家组成的共识委员会,随后委员会成员通过电话会议和在线讨论制定本专家共识。方法回顾人群、干预、对比和结局(population, intervention,comparison, and outcomes, PICO)问题,根据需要进行更新并得到证据概要。本共识关注3个主要议题:(1)神经重症监护患者的液体复苏和维持;(2)高渗液体在颅内压控制中的应用;(3)蛛网膜下腔出血后迟发性脑缺血的液体管理。经过广泛的文献检索,应用推荐分级的评估、制订与评价(Gradingof Recommendations Assessment, Development and Evaluation, GRADE)系统原则评估证据质量(从高到极低),制定治疗推荐(强或弱),并在适用时发布最佳实践声明。基于文献和专家意见提供的综合证据,应用改良德尔菲方法(使用序贯方法以避免偏倚和误解)产生最终共识声明。结果最终共识包含总共32条声明,其中13条为强推荐,17条为弱推荐,另外2条声明未提供推荐意见。结论我们发布了一份关于神经重症监护患者液体治疗的共识声明和临床实践推荐。

Anesthetic management of a patient with COVID-19 under incubation period undergoing total?hip arthroplasty

One of the characteristics of COVID-19 epidemiology is that asymptomatic infection is also a source of infection. There was no fever and/or respiratory symptoms in this patient before the operation, and routine blood test showed decreased lymphocyte count and lymphocyte percentage, and novel coronavirus pathogen detection was not carried out in the hospital, and the detection was not performed, chest imaging examination showed no obvious ground glass shadow, so viral pneumonia was not considered. The medical staff involved in the operation all adopted standard secondary medical protection measures. Five days after the operation, the patient tested positive for novel coronavirus nucleic acid, which was a confirmed case of COVID-19. Epidemiological investigation and analysis of the patients with preoperative in COVID-19 incubation period, however, none of the medical staff involved in surgical anesthesia and none of the patients in the next operation were infected, the reason is that the author''s hospital has strict training for all staff in infection control, surgical anesthesiology medical staff have mastered the relevant basic knowledge and operation skills, have a good sense of infection control in the operating room, and can standardize the implementation. This case also suggested that attention should be paid to the patients with significant preoperative simple lymphocyte level decline, and measures should be taken to identify asymptomatic infection early.

Neural Modulation of Hemiparetic Shoulder Pain by Repetitive Ultrasound-Guided Suprascapularis Nerve Block

Background: Neural blockade is widely used in clinical practice to alleviate acute or chronic pain, including pain during rehabilitation. To date there is little controlled evidence to confirm the efficacy of nerve blocks in hemiparetic shoulder pain after stroke. Design: This study is a prospective, open label, cohort trial reporting result from a cohort of stroke patients affected by shoulder pain. Aim: As a cohort study report, in which it is often firstly reported the possibility of an association between an observed effect and a specific environmental based on detailed clinical evaluations and histories, we aim to firstly provide clues in identifying Suprascapularis Nerve blockade as further valuable approach for shoulder pain after stroke. Population: We studied a cohort of patients affected by hemiparetic shoulder pain after Stroke. Methods: Our protocol foresees nerve blocks to be performed each 3 out of 4 days (treatment lasting 30 days) in conjunction with a rehabilitation program with the first aim to provide the window of opportunity to proceed with effective rehabilitation. 47 potential study subjects fulfilled the study criteria and were enrolled. Twenty-four subjects were randomised to the study Group to receive SSNB for the pain of their hemiparetic shoulder while 23 subjects randomized to the control Group whose member did not receive SSNB. They received serial blocks each 3 out of 4 days during ***: Both treatment reported a reduction in the intensity of their shoulder pain, according to data collected from day 1 through day 42 (6 weeks). Study Group patients, receiving SSNBs, reported significant improvement from entry through the whole follow-up period. The efficiency data were higher for SSNB Group after 2 weeks and again for SSNb group at the end of treatment. BACKGROUND Liver resection is the mainstay for a curative treatment for patients with resectable hepatocellular carcinoma(HCC),also in elderly *** this,the evaluation of patient condition,liver function and extent of disease remains a demanding process with the aim to reduce postoperative morbidity and *** To identify new perioperative risk factors that could be associated with higher 90-and 180-d mortality in elderly patients eligible for liver resection for HCC considering traditional perioperative risk scores and to develop a risk *** A multicentric,retrospective study was performed by reviewing the medical records of patients aged 70 years or older who electively underwent liver resection for HCC;several independent variables correlated with death from all causes at 90 and 180 d were *** coefficients of Cox regression proportional-hazards model for sixmonth mortality were rounded to the nearest integer to assign risk factors'weights and derive the scoring *** Multivariate analysis found variables(American Society of anesthesiology score,high rate of comorbidities,Mayo end stage liver disease score and size of biggest lesion)that had independent correlations with increased 90-and 180-d mortality.A clinical risk score was developed with survival *** This score can aid in stratifying this population in order to assess who can benefit from surgical treatment in terms of postoperative mortality.
