Is new American Thyroid Association risk classification for hereditary medullary thyroid carcinoma applicable to Chinese patients? A single-center study

Objective: The American Thyroid Association (ATA) proposed a new risk classification for hereditary medullary thyroid carcinoma (MTC) in 2015. This study aimed to assess whether the new guidelines are suitable for the Chinese population, and reported our experience on prophylactic thyroidectomy. Methods: A total of 73 patients from 22 families were screened as rearranged during transfection (RET) mutation carriers from 2010 to 2016 in Cancer Hospital, Chinese Academy of Medical science; the medical history for each patient was collected. Based on the initial treatment, we identified the risk factors for poor prognosis by univariate and multivariate logistic regression. Then, 4 RET mutation carriers were enrolled for prophylactic thyroidectomy, and their pathological data and follow-up outcomes were recorded. Results: In univariate and multivariate logistic regression analyses, age at initial surgery and risk classification were significant risk factors for stage III/IV hereditary MTC at initial diagnosis. The likelihood was increased by 11.6% per year of age at initial surgery [95% confidence interval (95% CI), 1.040-1.198; P=0.002). It was 7.888 times more likely to have III/IV stage disease for ATA highest risk patients, compared to ATA moderate risk individuals (95% CI, 1.607-38.717; P=0.003). Postoperative pathological results showed all 4 multiple endocrine neoplasia type 2A (MEN2A) patients had C-cell hyperplasia (CCH); multifocal malignancies were detected in 3 of them. All 4 patients were cured biochemically, and none developed permanent hypoparathyroidism. Conclusions: In Chinese individuals, hereditary MTC aggressiveness is in line with the new ATA risk classification. Germline RET gene mutation carriers should undergo prophylactic thyroidectomy according to basal serum calcitonin levels.

Objective: To investigate the effects of composite sophora colon-soluble Capsule(CsCC) on gut microbiota-mediated short-chain fatty acids(sCFAs) production and downstream group 3 innate lymphoid cells(ILC3s) of dextran sulfate sodium(Dss)-induced ulcerative colitis(uC) mice model. Methods: The main components of CsCC were analyzed by hybrid ultra-high-performance liquid chromatography ion mobility spectromety quadrupole time-of-flight mass spectrometry(uHPLC-IM-QTOF/Ms). Twenty-four male BALB/c mice were randomly divided into 4 grou.s.n=6) by using a computer algorithm-generated random digital, including control, Dss model, mesalazine, and CsCC grou.s. A Dss-induced colitis mice model was established to determine the effects of CsCC by recording colonic weight, colonic length, index of colonic weight, and histological colonic score. The variations in ILC3s were assessed by immunofluorescence and flow cytometry. The results of gut microbiota and sCFAs were acquired by 16s r DNA and gas chromatography-mass spectrometry(GC-Ms) analysis. The expression levels of NCR^(+)ILC3^(-), CCR6^(+)Nkp46-(Lti) ILC3^(-), and ILCreg-specific markers were detected by enzyme-linked immunosorbent assay, and real-time quantitative polymerase chain reaction and Western blot, respectively. Results: The main components of CsCC were matrine, ammothamnine, sophora flavescens neoalcohol J, and sophora oxytol u. After 7 days of treatment, CsCC significantly alleviated colitis by promoting the reproduction of intestinal probiotics manifested as upregulation of the abundance of Bacteroidetes species and specifically the Bacteroidales_s24-7 genus(PsCFAs, the content of butyric acid increased the most after CsCC treatment. Meanwhile, compared with the model group, Lti ILC3s and its biomarkers were significantly downregulated and NCR^(+) ILC3s were significantly elevated in the CsCC group(Purther experiments revealed that ILC3s were differentiated from Lti ILC3s to NCR^(+)

Background: Pediatric casualties account for a notable proportion of encounters in the deployed setting based on the humanitarian medical care mission. Previously published data demonstrates that an age-adjust shock index may be a useful tool in predicting massive transfusion and death in children. We seek to determine if those previous findings are applicable to the deployed, combat trauma ***: We queried the Department of Defense Trauma Registry(DODTR) for all pediatric subjects admitted to us and Coalition fixed-facility hospitals in Iraq and Afghanistan from January 2007 to January 2016. This was a secondary analysis of casualties seeking to validate previously published data using the shock index, pediatric age adjusted. We then used previously published thresholds to determine patients outcome for validation by age grouping, 1–3 years(1.2), 4–6 years(1.2), 7–12 years(1.0), 13–17 years(0.9).Results: From January 2007 through January 2016 there were 3439 pediatric casualties of which 3145 had a documented heart rate and systolic pressure. Of those 502(16.0%) underwent massive transfusion and 226(7.2%) died prior to hospital discharge. Receiver operating characteristic(ROC) thresholds were inconsistent across age grou.s.ranging from 1.0 to 1.9 with generally limited area under the curve(AuC) valu.s.for both massive transfusion and death prediction characteristics. using the previously defined thresholds for validation, we reported sensitivity and specificity for the massive transfusion by age-group: 1–3(0.73, 0.35), 4–6(0.63, 0.60), 7–12(0.80, 0.57), 13–17(0.77, 0.62). For death, 1–3(0.75, 0.34), 4–6(0.66–0.59), 7–12(0.64, 0.52), 13–17(0.70, 0.57). Objective. To investigate the characteristics of urotensin II (u II) receptor in the rat airway smooth muscle and the effect and signal transduction pathway of u II on the proliferation of airway smooth muscle cells. Methods. using 125I uII binding assay to measure the Bmax and Kd of u II receptor. using the 3H TdR incorporation to determine the effect of u II on the proliferation of airway smooth muscle cells and its signal transduction pathway. using Fura 2/AM to measure the effect of u II on the cytosolic free calcium concentration. Results. 1. 125I uII binding increased with the time and reached saturation at 45min. The Bmax was (11.36±0.37)fmol/mg pr and Kd was (4.46±0.61)nmol/L. 2. u II increased 3H TdR incorporation of the airway smooth muscle cells in a dose dependent manner. 3. H7, PD98059 and nicardipine, inhibitors of PKC, MAPK, calcium channel, respectively, significantly inhibited u II stimulated 3H TdR incorporation of airway smooth muscle cells. W7, inhibitor of CaM PK, had no effect. 4. Cyclosporin A, inhibitor of CaN, inhibited 3H TdR incorporation of the airway smooth muscle cells induced by u II in a dose dependent manner. 5. u II promoted cytosolic free calcium concentration increase by 18%. Conclusions. 1. There was u II receptor in the rat airway smooth muscle. 2. The effect of u II stimulated 3H TdR incorporation of airway smooth muscle cells was mediated by such signal transduction pathway as Ca2+, PKC, MAPK and CaN, etc.

The risk of pathogen transmission continu.s.to increase significantly in the presence of tick vectors due to the trade of livestock across *** Ghana,there is a lack of data on the incidence of tick-borne pathogens that are of zoonotic and veterinary *** study,therefore,aimed to determine the prevalence of such pathogens in livestock using molecular approaches.A total of 276 dry blood spots were collected from cattle(100),sheep(95)and goats(81)in the Kassena-Nankana *** samples were analyzed using Polymerase Chain Reaction(qPCR)and conventional assays and sanger sequencing that targeted pathogens including Rickettsia,Coxiella,Babesia,Theileria,Ehrlichia and *** overall prevalence of 36.96%was recorded from the livestock DBs,with mixed infections seen in 7.97%***,the prevalence of infections in livestock was recorded to be 19.21%in sheep,14.13%in cattle,and 3.62%in *** pathogens identified were Rickettsia spp.(3.26%),Babesia ***(8.70%),Theileria orientalis(2.17%),Theileria parva(0.36%),Anaplasma capra(18.48%),Anaplasma phagocytophilum(1.81%),Anaplasma marginale(3.26%)and Anaplasma ovis(7.25%).This study reports the first molecular identification of the above-mentioned pathogens in livestock in Ghana and highlights the use of dry blood spots in resource-limited *** addition,this research provides an update on tick-borne pathogens in Ghana,suggesting risks to livestock production and human *** studies will be essential to establish the distribution and epidemiology of these pathogens in Ghana.

Crimean-Congo hemorrhagic fever(CCHF),caused by Crimean-Congo hemorrhagic fever virus(CCHFV),is endemic in Africa,Asia,and Europe,but CCHF epidemiology and epizootiology is only rudimentarily defined for most *** we summarize what is known about CCHF in Central,Eastern,and south-eastern *** multiple international and country-specific databases using a One Health approach,we defined disease risk and burden through identification of CCHF cases,anti-CCHFV antibody prevalence,and CCHFV isolation from vector *** identified 2313 CCHF cases that occurred in 1944–2021 in the three examined *** Asian countries reported the majority of cases(2,026).In Eastern Asia,China was the only country that reported CCHF cases(287).In south-eastern Asia,no cases were ***,we leveraged our previously established classification scheme to assign countries to five CCHF evidence *** countries(China,Kazakhstan,Kyrgyzstan,Tajikistan,Turkmenistan,and uzbekistan)were assigned to level 1 or level 2 based on CCHF case reports and the maturity of the countries’surveillance *** countries(Mongolia and Myanmar)were assigned to level 3 due to evidence of CCHFV circulation in the absence of reported CCHF *** countries in Eastern and south-eastern Asia were categorized in levels 4 and 5 based on prevalence of CCHFV vector ***,this paper describes the past and present status of CCHF reporting to inform international and local public-health agencies to strengthen or establish CCHFV surveillance systems and address shortcomings.
