With the digestive endoscopic tunnel technique(DETT), many diseases that previously would have been treated by surgery are now endoscopically curable by establishing a submucosal tunnel between the mucosa and muscularis propria(MP). Through the tunnel, endoscopic diagnosis or treatment is performed for lesions in the mucosa, in the MP, and even ou.s.de the gastrointestinal(GI) *** present, the tunnel technique application range covers the following:(1)Treatment of lesions originating from the mucosal layer, e.g., endoscopic submucosal tunnel dissection for oesophageal large or circular early-stage cancer or precancerosis;(2) treatment of lesions from the MP layer, per-oral endoscopic myotomy, submucosal tunnelling endoscopic resection, etc.; and(3) diagnosis and treatment of lesions ou.s.de the GI tract, such as resection of lymph nodes and benign tumour excision in the mediastinum or abdominal cavity. With the increasing number of DETTs performed worldwide, endoscopic tunnel therapeutics, which is based on DETT, has been gradually developed and optimized. However, there is not yet an expert consensus on DETT to regulate its indications, contraindications, surgical procedure, and postoperative *** International DETT Alliance signed up this consensus to standardize the procedures of DETT. In this consensus, we describe the definition, mechanism,and significance of DETT, prevention of infection and concepts of DETTassociated complications, methods to establish a submucosal tunnel, and application of DETT for lesions in the mucosa, in the MP and ou.s.de the GI tract(indications and contraindications, procedures, pre-and postoperative treatments, effectiveness, complications and treatments, and a comparison between DETT and other operations).

stiefel–Whitney classes and topological phases in band theory

We review the recent progress in the study of topological phases in systems with space–time inversion symmetry *** an anti-unitary symmetry which is local in momentum space and satisfies I2 sT= 1 such as PT in two dimensions(2 D)and three dimensions(3 D) without spin–orbit coupling and C2 T in 2 D with or without spin–orbit coupling, where P, T, C2 indicate the inversion, time-reversal, and two-fold rotation symmetries, respectively. under IsT, the Hamiltonian and the periodic part of the Bloch wave function can be constrained to be real-valued, which makes the Berry curvature and the Chern number vanish. In this class of systems, gapped band structures of real wave functions can be topologically distingu.s.ed by the stiefel–Whitney numbers instead. The first and second stiefel–Whitney numbers w1 and w2, respectively,are the corresponding invariants in 1 D and 2 D, which are equivalent to the quantized Berry phase and the Z2 monopole charge, respectively. We first describe the topological phases characterized by the first stiefel–Whitney number, including1 D topological insulators with quantized charge polarization, 2 D Dirac semimetals, and 3 D nodal line semimetals. Next we review how the second stiefel–Whitney class characterizes the 3 D nodal line semimetals carrying a Z2 monopole *** particular, we explain how the second stiefel–Whitney number w2, the Z2 monopole charge, and the linking number between nodal lines are related. Finally, we review the properties of 2 D and 3 D topological insulators characterized by the nontrivial second stiefel Whitney class.

Background:The use of laparoscopic(LLR)and robotic liver resections(RLR)has been safely performed in many institutions for liver tumou.s.A large scale international multicenter study would provide stronger evidence and insight into application of these techniqu.s.for huge liver tumou.s.10 ***:This was a retrospective review of 971 patients who underwent LLR and RLR for huge(≥10 cm)tumors at 42 international centers between ***:One hundred RLR and 699 LLR which met study criteria were *** comparison between the 2 approaches for patients with huge tumors were performed using 1:3 propensity-score matching(PsM)(73 vs.219).Before PsM,LLR was associated with significantly increased frequency of previous abdominal surgery,malignant pathology,liver cirrhosis and increased median *** PsM,RLR and LLR was associated with no significant difference in key perioperative outcomes including media operation time(242 vs.290 min,P=0.286),transfusion rate rate(19.2%vs.16.9%,P=0.652),median blood loss(200 vs.300 mL,P=0.694),open conversion rate(8.2%vs.11.0%,P=0.519),morbidity(28.8%vs.21.9%,P=0.221),major morbidity(4.1%vs.9.6%,P=0.152),mortality and postoperative length of stay(6 vs.6 days,P=0.435).Conclusions:RLR and LLR can be performed safely for selected patients with huge liver tumou.s.with excellent *** was no significant difference in perioperative outcomes after RLR or LLR.

Background:Traumatic pancreatic injuries are rare,and guidelines specifying management are controversial and difficult to apply in the acute clinical *** to sparse data on these injuries,we carried out a retrospective review to determine outcomes following surgical or non-surgical management of traumatic pancreatic *** hypothesize a higher morbidity and mortality rate in patients treated surgically when compared to patients treated ***:We performed a retrospective review of data from four trauma centers in New York from 1990–2014,comparing patients who had blunt traumatic pancreatic injuries who were managed operatively to those managed *** compared continuous variables using the Mann-Whitney u test and categorical variables using the chi-square and Fisher’s exact *** analysis was performed to determine the possible confounding factors associated with mortality in both treatment ***:Twenty nine patients were managed operatively and 32 *** was a significant difference between the operative and non-operative grou.s.in median age(37.0 vs.16.2 years,P=0.016),grade of pancreatic injury(grade I;30.8 vs.85.2%,P value for all comparisonsury severity score(Iss)(16.0 vs.4.0,P=0.002),blood transfusion(55.2 vs.15.6%,P=0.0012),other abdominal injuries(79.3 vs.38.7%,P=0.0014),pelvic fractures(17.2 vs.0.00%,P=0.020),intensive care unit(ICu)admission(86.2 vs.50.0%,P=0.003),median length of stay(LOs)(16.0 vs.4.0 days,Ps.3.1%,P=0.010).Conclusions:Patients with traumatic pancreatic injuries treated operatively were more severely injured and suffered greater complications than those treated *** greater morbidity and mortality associated with these patients warrants further study to determine optimal triage strategies and which su.s.t of patients is likely to benefit from surgery.

studies have found a u.s.aped relationship between sleep duration and chronic kidney disease(CKD)risk,but limited research evaluated the association of reallocating excessive sleep to other behavior with *** included 104538 participants from the nationwide cohort of the Risk Evaluation of Cancers in Chinese Diabetic Individuals:A Longitudinal study,with self-reported time of daily-life *** isotemporal su.s.itution models,we found that su.s.ituting 1 h of sleeping with sitting,walking,or moderate-to-vigorous physical activity was associated with a lower CKD ***-time physical activity displacement was associated with a greater prevalence reduction than occupational physical activity in working *** stratified analysis,a lower CKD prevalence related to su.s.itution toward physical activity was found in long *** pronounced correlations were observed in long sleepers with diabetes than in those with prediabetes,and they benefited from other behavior su.s.itutions toward a more active *** u.s.aped association between sleep duration and CKD prevalence implied the potential effects of insufficient and excessive sleep on the kidneys,in which the pernicious link with oversleep could be reversed by time reallocation to physical *** divergence in the predicted effect on CKD following time reallocation to behavior of different domains and intensities and in subpopulations with diverse metabolic statuses underlined the importance of optimizing sleeping patterns and adjusting integral behavioral composition.

AIM: To construct the natural immune Fab antibody phage display libraries of colorectal cancer and to select antibodies related with colorectal cancer. METHODs: Extract total RNA from tissue of local cancer metastasis lymph nodes of patients with colorectal cancer. RT-PCR was used to amplify the heavy chain Fd and light chain kappa and the amplification products were inserted successively into the vector pComb3 to construct the human libraries of Fab antibodies. They were then panned by phage display technology. By means of Dot immunoblotting and ELIsA, the libraries were identified and the Fab phage antibodies binding with antigens of colorectal cancer were selected. REsuLTs: The amplified fragments of Fd and kappa gained by RT-PCR were about 650 bp. Fd and kappa PCR products were su.s.quently inserted into the vector pComb3, resulting in a recombination rate of 40% and the volume of Fab phage display library reached 1.48 x 10(6).The libraries were enriched about 120-fold by 3 cycles of adsorption-elution-multiplication (panning). Dot immunoblotting showed Fab expressions on the phage libraries and ELIsA showed 5 clones of Fab phage antibodies which had binding activities with antigens of colorectal cancer. CONCLusION: The natural immune Fab antibody phage display libraries of colorectal cancer were constructed. They could be used to select the relative antibodies of colorectal cancer.
