Clinical profiles and outcomes of acute type A aortic dissection and intramural hematoma in the current era: lessons from the first registry of aortic dissection in China

Background::Acute type A aortic dissection(ATAAD)and acute type A intramural hematoma(ATAIMH)are life-threatening diseases with high *** better understand their clinical features in the Chinese population,we analyzed the data from the first Registry of Aortic Dissection in China(sino-RAD)to promote the understanding and management of the ***::All patients with ATAAD and ATAIMH enrolled in sino-RAD from January 1,2012 to December 31,2016 were *** data of patients’selection,history,symptoms,management,outcomes,and postoperation complications were analyzed in the *** continuous variables were compared using the student’s t test for normal distributions and the Mann-Whitney u test for non-normal *** variables were compared using the Chi-square test or Fisher exact ***::A total of 1582 patients with ATAAD and 130 patients with ATAIMH were *** mean age of all patients was 48.4 *** with ATAAD were significantly younger than patients with ATAIMH(48.9 years vs.55.6 years,Ps were dominant,but the male ratio of patients with ATAAD was significantly higher compared to those with ATAIMH(P=0.01).The time range from the onset of symptom to hospitalization was 2.0 *** patients of ATAIMH had hypertension than that of ATAAD(82.3%vs.67.6%,Pst and back pain were the most common clinical *** tomography(CT)was the most common initial diagnostic imaging modality.84.7%received surgical treatment and in-hospital mortality was 5.3%.Patients with ATAAD mainly received surgical treatment(89.6%),while most patients with ATAIMH received medical treatment(39.2%)or endovascular repair(35.4%).Conclusions::Our study suggests that doctors should comprehensively use clinical examination and genetic background screening for patients with ATAAD and ATAIMH and further shorten the time range from symptoms onset to intervention,achieving early diagnosis

Experimental and Molecular simulations for Evaluating the Effect of Lubricity Improvers on the Property of Jet Fuel

since the 1990 s, the kerosene fuel(code: JP-8) had been applied in the ground equipment provided with direct injection compression ignition engines in the u.s. army, resulting in increased occurrence of injection pump failures. Anti-wear additives must be used in the single fuel due to its poor lubricity. In the present work, lubricity improvers were selected on the basis of molecular simulation theoretically and these agents were evaluated to improve the lubricity of jet fuel using the high frequency reciprocating rig(HFRR) apparatus and the ball-on-cylinder lubricity evaluator(BOCLE). It was revealed that dimer acid with higher value of adsorption energy on the Fe(110) plane surface had more efficient lubricity promoting properties than that of naphthenic acid. The experimental results suggested that the dimer acid had a better tribological behavior compared with that of naphthenic acid used as lubricity improver of jet fuel. And addition of anti-wear additives at a dosage of 15 μg/g was able to promote the lubricity of jet fuel to a required level on BOCLE, while a higher concentration over 80 μg/g was needed to improve the lubricity to a demanded value of diesel on HFRR.

The ORF9b protein,derived from the nucleocapsid’s open-reading frame in both sARs-CoV and sARs-CoV-2,serves as an accessory protein crucial for viral immune evasion by inhibiting the innate immune *** its significance,the precise regulatory mechanisms underlying its function remain *** the present study,we unveil that the ORF9b protein of sARs-CoV-2,including emerging mutant strains like Delta and Omicron,can undergo ubiquitination at the K67 site and su.s.quent degradation via the proteasome pathway,despite certain mutations present among these ***,our investigation further uncovers the pivotal role of the translocase of the outer mitochondrial membrane 70(TOM70)as a su.s.rate receptor,bridging ORF9b with heat shock protein 90 alpha(HsP90α)and Cullin 5(CuL5)to form a *** this complex,CuL5 triggers the ubiquitination and degradation of ORF9b,acting as a host antiviral factor,while HsP90αfunctions to stabilize ***,treatment with HsP90 inhibitors such as GA or 17-AAG accelerates the degradation of ORF9b,leading to a pronounced inhibition of sARs-CoV-2 ***-cell sequencing data revealed an up-regulation of HsP90αin lung epithelial cells from COVID-19 patients,suggesting a potential mechanism by which sARs-CoV-2 may exploit HsP90αto evade the host *** study identifies the CuL5-TOM70-HsP90αcomplex as a critical regulator of ORF9b protein stability,shedding light on the intricate host–virus immune response dynamics and offering promising avenu.s.for drug development against sARs-CoV-2 in clinical settings.

Full noncontact laser ultrasound(Lus)imaging has several distinct advantages over current medical ultrasound(us)technologies:elimination of the coupling mediu.s.gel/water),operator-independent image quality,improved repeatability,and volumetric *** light-based ultrasound utilizing tissue-penetrating photoacoustics(PA)generally uses traditional piezoelectric transducers in contact with the imaged tissue or carries an optical fiber detector close to the imaging *** PA,the Lus design presented here minimizes the optical penetration and specifically restricts optical-to-acoustic energy transduction at the tissue surface,maximizing the generated acoustic source *** an appropriate optical design and interferometry,any exposed tissue surfaces can become viable acoustic sources and *** operates analogously to conventional ultrasound but uses light instead of piezoelectric ***,we present full noncontact Lus results,imaging targets at~5 cm depths and at a meterscale standoff from the target *** results demonstrating volumetric imaging and the first Lus images on humans are presented,all at eye-and skin-safe optical exposure *** progression of Lus imaging from tissue-mimicking phantoms,to excised animal tissue,to humans in vivo is shown,with validation from conventional ultrasound *** Lus system design insights and results presented here inspire further Lus development and are a significant step toward the clinical implementation of Lus.

AIM:This paper describes the procedure of detection of Helicobacter pylori(***)in bile specimens in patients suffering from benign diseases of biliary ducts(lithiasis with/without nonspecific cholangitis). METHODs:The group of 72 patients entering the study consisted of 32 male and 40 female(45 % and 55 %, respectively).Bile was obtained during ERCP in 68 patients, and during cholecystectomy in 4 patients.A fast urease test (FuT)to determine the existence of *** in gastric mucosa was carried out for all the patients during the endoscopic *** existence of genetic material of *** was determined by detection of ure A gene by the method of nested *** results of this reaction were shown by electrophoresis on 10 g·L^(-1)agarose gel in a band of 256 bp. REsuLTs:The majority of the patients included in our study had biliary lithiasis without signs of cholangitis(48 patients, 67 %),whereas other patients were complicated by cholangitis(17 patients,24 %).seven patients(9 %)had normal ERCP,forming thus the control *** the group of patients with lithiasis 26 patients(24.2 %)had positive PCR of *** in bile and among the patients with associated cholangitis positive PCR was detected in 9 patients(52.9 %).Among the seven patients with normal ERCP only one(14 %)had positive PCR of ***.A high percentage of *** infection of gastric mucosa was observed(57 patients,79 %).It was also observed that its slightly higher positivity was in the patients with distinct bile pathology:81% FLIT positive patients in the group with choledocholithiasis alone and 76 % in the group with choledocholithiasis associated with ***-one percent of the patients with regular findings had positive ***: The prevalence of H. pylori infection both in bile and in gastric mucosa in patients with benign diseases of biliary ducts does not show a statistically significant difference in relation to the prevalence of the same with the patients
