Diabetic foot syndrome:Immune-inflammatory features as possible cardiovascular markers in diabetes

Diabetic foot ulcerations have been extensively reported as vascular complications of diabetes mellitus associated with a high degree of morbidity and mortality. Diabetic foot syndrome(DFS), as defined by the World Health organization, is an "ulceration of the foot(distally from the ankle and including the ankle) associated with neuropathy and different grades of ischemia and infection". Pathogenic events able to cause diabetic foot ulcers are *** the commonest causes of this pathogenic pathway it's possible to consider peripheral neuropathy, foot deformity, abnormal foot pressures, abnormal joint mobility, trauma, peripheral artery disease. Several studies reported how diabetic patients show a higher mortality rate compared to patients without diabetes and in particular these studies under filled how cardiovascular mortality and morbidity is 2-4 times higher among patients affected by type 2 diabetes mellitus. This higher degree of cardiovascular morbidity has been explained as due to the observed higher prevalence of major cardiovascular risk factor, of asymptomatic findings of cardiovascular diseases, and of prevalence and incidence of cardiovascular and cerebrovascular events in diabetic patients with foot complications. In diabetes a fundamental pathogenic pathway of most of vascular complications has been reported as linked to a complex interplay of inflammatory, metabolic and procoagulant variables. These pathogenetic aspects have a direct interplay with an insulin resistance, subsequent obesity, diabetes, hypertension, prothrombotic state and blood lipid disorder. Involvement of inflammatory markers such as IL-6 plasma levels and resistin in diabetic subjects as reported by Tuttolomondo et al confirmed the pathogenetic issue of the a "adipo-vascular" axis that may contribute to cardiovascular risk in patients with type 2 diabetes. When sepsis affects the heart: A case report and literature review

A 59-year-old nursing home patient with Down syndrome was brought to the internal medicine department of our hospital due to fever, cough without expectorate, and dyspnea. A thoracic computed tomography revealed the presence of bilateral basal parenchymal opacities. Her condition deteriorated after admission and troponin reached a peak serum concentration of 16.9 ng/m L. The patient was in cardiogenic shock. In addition to fluid resuscitation, vaso-active amine infusion was administered to achieve hemodynamic stabilization. The differential diagnosis investigated possible pulmonary embolism, myocardial infarction, and myocarditis. Furthermore, a second transthoracic echocardiogram suggested Tako-Tsubo syndrome. This is a septic patient. The purpose of this manuscript is to review studies which formerly examined the possible association between high levels of troponin and mortality to see if it can be considered a positive predictive factor of fatal prognosis as the case of thrombocytopenia, already a positive independent predictive factor of multiple organ failure syndrome, and generally to characterize risk profile in a septic patient.

Despite the great successes achieved in the fields of virology and diagnostics,several difficulties affect improvements in hepatitis c virus(HcV)infection control and eradication in the new *** HcV infections still occur,especially in some of the poorest regions of the world,where HcV is endemic and long-term sequelae have a growing economic and health *** HcV vaccine is still no available,despite years of researches and discoveries about the natural history of infection and host-virus interactions:several HcV vaccine candidates have been developed in the last years,targeting different HcV antigens or using alternative delivery systems,but viral variability and adaption ability constitute major challenges for vaccine *** new antiviral drugs for HcV therapy are in preclinical or early clinical development,but different limitations affect treatment *** predictors are important tools,as they provide some guidance for the management of therapy in patients with chronic HcV infection:in particular,the role of host genomics in HcV infection outcomes in the new era of direct-acting antivirals may evolve for new therapeutic targets,representing a chance for modulated and personalized treatment management,when also very potent therapies will be *** the present review we discuss the most recent data about HcV epidemiology,the new perspectives for the prevention of HcV infection and the most recent evidence regarding HcV diagnosis,therapy and predictors of response to it.

Rheological Regime of the upper Mantle beneath the Taiwan Strait and Its Tectonic Significance

This paper deals with deformation textures and fabrics of mantle-derived xenoliths and dislocation microstructures of olivine in the upper mantle in the Penghu Islands, Taiwan. According to the calculation of the chemical composition of xenolith minerals (pyroxene), the equilibrium temperatures and pressures were 986-1116@ and 1.50-2.60 GPa, respectively. Deformation events in the upper mantle may fall into three sequences' (1) uniform steady-state flow deformation with high temperatures and low stresses, (2) shear flow deformation with high temperatures and relatively high stresses on diapiric margins of the upper mantle, and (3) extraction deformation of {110} glide bands with low temperatures and high strain rates. Deformation events and thermal structure of the upper mantle in the study area show that eastern Fujian and the Penghu Islands are characterized by very similar rheological properties of the upper mantle. Volcanism of basalts in the Penghu Islands is related to hot spots of the upper mantle under neath the Taiwan Strait, and diapirism and upwelling of the upper mantle are the dominant factors responsible for rifting of the continental margins in eastern china.

Breast cancer metastasis to the stomach

Aim: This study focuses on the stomach as an unusual but not rare site of metastasis of breast cancer. Methods: We performed a literature search on gastric metastasis from breast cancer searching for reviews from 2000 to 2018 and case reports from 2013 to 2018. We found 11 reviews and 36 case reports and we compared their findings about important aspects of gastric metastasis, such as disease free survival, overall survival, symptoms, endoscopic findings, therapy, histology, and immunohistochemistry. Results: The incidence of stomach as site of metastasis of breast cancer ranges from 5% to 18%. Reviews and case reports reached similar conclusions about several of the aforementioned aspects: invasive lobular breast cancer (ILc) is mainly responsible for gastric metastases;disease free survival can vary greatly ranging from 0.5 months to 30 years;gastric metastases usually present with non-specific symptoms, even though five patients in case reports were asymptomatic;linitis plastica is the most common endoscopic finding;immunohistochemistry is essential for differentiating primary gastric cancer from metastasis;the preferred treatment is systemic therapy, but surgery is still an option in case of emergency;median overall survival of patients with gastric metastasis from breast cancer is 24 months. conclusion: Breast metastasis to the stomach should be considered in any patient suspecting gastric neoplasm previously treated for breast carcinoma, especially if the treated carcinoma was ILc.

Seed Priming with Beta-Amino Butyric Acid Improves Abiotic Stress Tolerance in Rice Seedlings

We studied the influence of seed priming with beta-amino butyric acid（BABA） on the growth, physiological and biochemical parameters of seedlings with varied abiotic stress tolerance, which were raised and grown under unstressed and stressed（Nacl/PEG-6000） conditions. under stressed conditions, the growth of rice seedlings was less when compared to control plants. After BABA priming, the seedling growth increased both under unstressed and stressed conditions as compared to the respective controls. BABA priming of rice seeds caused increase in the photosynthetic pigment content of the leaves, modified the chlorophyll a fluorescence related parameters and also enhanced the photosystem activities of seedlings when compared to their respective non-primed controls. BABA priming also caused increased mitochondrial activities of the rice seedlings. Moreover, BABA priming significantly reduced malondialdehyde content in the seedlings and also resulted in accumulation of proline especially in the Nacl tolerant variety Vyttila 6. BABA seed priming also enhanced the activity of nitrate reductase enzyme and activities of antioxidant enzymes like guaiacol peroxidase and superoxide dismutase. The presence of BABA was detected by high performance thin layer chromatography analysis in the rice seeds whereas in the seedlings it was not detected. Thus, it can be inferred that the seed priming effect of BABA mainly occurred within the seeds, which was further carried to the seedlings. It is concluded that BABA priming of seeds improved the drought and salinity stress tolerance of all the three rice varieties and it was significantly evident in the drought tolerant variety Vaisakh and Nacl tolerant variety Vyttila 6, when compared to the stress sensitive variety Neeraja.

confinement effects in methanol to olefins catalysed by zeolites： A computational review

Small pore zeolites, containing 8-rings as the largest, are widely employed as catalysts in the process of methanol-to-olefins （MTo）. Reactants and products dif- fuse with constraints through 8-rings and this is one of the reaction bottlenecks related to zeolite micropore topology. Small pore zeolites and silicon-aluminophosphates （SAPos） containing cavities, where olefins are mainly formed through the hydrocarbon pool （HP） mechanism, are frequently tested for MTo. Shape selectivity of transition states within the side-chain methylation will be reviewed as this is one of the controlling steps of the MTo process, with particular attention to the role of hexam- ethylbenzene （HMB） and heptamethylbenzenium cation （HeptaMB~）, which are the most tipically detected reaction intermediates, common to the paring and side-chain routes within the HP mechanism. The relative stability of these and other species will be reviewed in terms of confinement effects in different cage-based zeolites. The role of the different alkylating agents, methanol, dimethyl ether （DME）, and surface methoxy species （SMS） will also be reviewed from the computational viewpoint.
