Veins are easy to obtain,have low immunogenicity,and induce a relatively weak inflammatory ***,veins have the potential to be used as conduits for nerve ***,because of the presence of venous valves and the great elasticity of the venous wall,the vein is not conducive to nerve *** this study,a novel tissue engineered nerve graft was constructed by combining normal dissected nerve microtissue with an autologous vein graft for repairing 10-mm peripheral nerve defects in *** with rats given the vein graft alone,rats given the tissue engineered nerve graft had an improved sciatic static index,and a higher amplitude and shorter latency of compound muscle action ***,rats implanted with the microtissue graft had a higher density and thickness of myelinated nerve fibers and reduced gastrocnemius muscle atrophy compared with rats implanted with the vein ***,the tissue engineered nerve graft had a lower ability to repair the defect than autogenous nerve *** summary,although the tissue engineered nerve graft constructed with autologous vein and nerve microtissue is not as effective as autologous nerve transplantation for repairing long-segment sciatic nerve defects,it may nonetheless have therapeutic potential for the clinical repair of long sciatic nerve *** study was approved by the Experimental Animal Ethics Committee of Chinese PLA General Hospital(approval No.2016-x9-07)on September 7,2016.

背景对于多数麻醉医师来讲急诊气道管理是必须掌握的一项重要技术，我们总结了一个一级创伤中心10年来的经验，评估创伤患者入院后24小时内气管插管的结果。方法通过该中心1996年7月至2006年6月的创伤登记、质量管理记录、收费系统记录，得出入院后1小时内、24小时内需要插管的患者人数，并且回顾每个气管切开患者的医疗记录，了解其特征和无法进行经口或经鼻插管的原因。结果所有插管都在一名处理创伤经验丰富的麻醉医师的指导下进行。研究期间，标准插管法为直接喉镜下快速诱导插管。入院后1小时内，6088名患者进行插管，其中21名患者（0．3％）最终行气管切开。入院后24小时内，累计32000名患者进行插管，共31名行气管切开。意外的困难气道是气管切开的主要因素。31名患者中有4名死于外伤，无人死于插管失败。结论由经验丰富的麻醉医师指导，直接喉镜下快速诱导气管插管是有效的急诊气道管理方法，以其为核心的急诊气道管理规则使得气道管理成功率很高。

The design of orthopedic biomaterials has gradually shifted from“immune-friendly”to“immunomodulatory,”in which the biomaterials are able to modulate the inflammatory response via macrophage polarization in a local immune microenvironment that favors osteogenesis and implant-to-bone *** the well-known effects of bioactive metallic ions on osteogenesis,how extracellular metallic ions manipulate immune cells in bone tissue microenvironments toward osteogenesis and subsequent bone formation has rarely been ***,we investigate the osteoimmunomodulatory effect of an extracellular bioactive cation(Mg^(2+))in the bone tissue microenvironment using custom-made poly lactic-co-glycolic acid(PLGA)/MgO-alendronate microspheres that endow controllable release of magnesium *** results suggest that the Mg^(2+)-controlled tissue microenvironment can effectively induce macrophage polarization from the M0 to M2 phenotype via the enhancement of anti-inflammatory(IL-10)and pro-osteogenic(BMP-2 and TGF-β1)cytokines *** also generates a favorable osteoimmune microenvironment that facilitates the proliferation and osteogenic differentiation of bone marrow mesenchymal stem *** in vivo results further verify that a large amount of bony tissue,with comparable bone mineral density and mechanical properties,has been generated at an early post-surgical stage in rat intramedullary bone defect *** study demonstrates that the concept of in situ immunomodulated osteogenesis can be realized in a controlled magnesium tissue microenvironment.

The glenohumeral joint is the most commonly dislocated joint of the body and anterior instability is the most common type of shoulder *** resonance (MR) imaging,and more recently,MR arthrography,have become the essential investigation modalities of glenohumeral instability,especially for pre-procedure evaluation before arthroscopic *** associated with glenohumeral instability are variable,and can involve the bones,the labor-ligamentous components,or the rotator *** instability is associated with injuries of the anterior labrum and the anterior band of the inferior glenohumeral ligament,in the form of Bankart lesion and its variants;whereas posterior instability is associated with reverse Bankart and reverse Hill-Sachs *** instability often has no labral pathology on imaging but shows specific osseous changes such as increased chondrolabral *** article reviews the relevant anatomy in brief,the MR imaging technique and the arthrographic technique,and describes the MR findings in each type of instability as well as common imaging pitfalls.

Rapid and effective repair of injured or diseased bone defects remains a major challenge due to shortages of *** hydrogels that respond to internal and external stimuli to achieve therapeutic actions in a spatially and temporally controlled manner have recently attracted much attention for bone therapy and *** hydrogels can be modified by introducing responsive moieties or embedding nanoparticles to increase their capacity for bone *** specific stimuli,smart hydrogels can achieve variable,programmable,and controllable changes on demand to modulate the microenvironment for promoting bone *** this review,we highlight the advantages of smart hydrogels and summarize their materials,gelation methods,and ***,we overview the recent advances in developing hydrogels that respond to biochemical signals,electromagnetic energy,and physical stimuli,including single,dual,and multiple types of stimuli,to enable physiological and pathological bone repair by modulating the ***,we discuss the current challenges and future perspectives regarding the clinical translation of smart hydrogels.

Management of sedation in the critical care unit is an ongoing challenge. Benzodiazepines have been commonly used as sedatives in critically ill patients. The pharmacokinetic and pharmacodynamic properties that make benzodiazepines effective and safe in critical care sedation include rapid onset of action and decreased respiratory depression. Alprazolam is a commonly used benzodiazepine that is prescribed for anxiety and panic disorders. It is frequently prescribed in the outpatient setting. Its use has been reported to result in a relatively high rate of dependence and subsequent withdrawal symptoms. Symptoms of alprazolam withdrawal can be difficult to recognize and treat in the critical care setting. In addition, other benzodiazepines may also be ineffective in treating alprazolam withdrawal. We present a case of alprazolam withdrawal in a critically ill trauma patient who failed treatment with lorazepam and haloperidol. Subsequent replacement with alprazolam resulted in significant improvement in the patient's medication use and clinical status.

With the development of tissue engineering and regenerative medicine,it is much desired to establish bioimaging techniques to monitor the real-time regeneration efficacy in vivo in a non-invasive ***,we tried magnetic resonance imaging(MRI)to evaluate knee cartilage regeneration after implanting a biomaterial scaffold seeded with chondrocytes,namely,matrix-induced autologous chondrocyte implantation(MACI).After summary of the T2 mapping and the T1-related delayed gadolinium-enhanced MRI imaging of cartilage(dGEMRIC)in vitro and in vivo in the literature,these two MRI techniques were tried *** this study,18 patients were followed up for 1 *** was found that there was a significant difference between the regeneration site and the neighboring normal site(control),and the difference gradually diminished with regeneration time up to 1 year according to both the quantitative T1 and T2 MRI *** further established the correlation between the quantitative evaluation of MRI and the clinical Lysholm scores for the first ***,the MRI technique was confirmed to be a feasible semiquantitative yet non-invasive way to evaluate the in vivo regeneration of knee articular cartilage.
