Balance improvement could contribute to ankle stability for the prevention of ankle *** electrical stimulation(FES)is an effective way of augmenting muscle activity and improving *** study investigated the effect of FES of peroneal muscles on single-and double-leg *** healthy females(age=23:1±1:6 years,height=1:63±0:07m,and weight=63:7±9:9 kg)performed single-and double-leg standing balance tests with eyes open and closed before and after 15-minute FES intervention during treadmill running at a comfortable,self-selected *** of peroneal muscles was provided bilaterally,using an Odstock Dropped Foot *** total excursion of the centre of pressure(COP)was calculated to assess the standing balance control *** total excursion of COP in single-and double-leg stance with eyes open reduced significantly after FES intervention by 14.7%(p0:05).Limb preference did not account for balance *** significant difference in total excursion of COP was found between preferred and less preferred limbs with both visual conditions(p>0:05).FES of peroneal muscles improved standing balance control with eyes open in double-leg and single-leg stance and with eyes closed in double-leg *** improvements in balance control with FES treatment did not vary concerning limb preference.

背景与有家的妇女相比,无家可归妇女的妊娠概率要高2倍,但是其接受孕期保健的概率低得多。复杂的生理、心理和社会因素以及合并症(包括滥用药物和精神疾病),使无家可归妇女罹患产科并发症、产后抑郁和孩子享受不到社会福利的风险增加。目的探讨有妊娠史的无家可归妇女对围生期保健的看法,并在此基础上提出改进围生期保健的措施。设计与场所研究设计为质性研究。采用立意抽样法从3个社区抽取有妊娠史的无家可归的妇女作为研究对象。方法对研究对象进行半结构式访谈。在访谈过程中,使研究对象最大限度地表达自我看法。访谈进行至数据饱和为止。对访谈内容进行录音、逐字记录,并进行主题分析。由研究组成员对主题进行独立验证。结果共11例年龄不同(18~40岁)和产次不同(生育1~5个孩子)的妇女纳入了研究。大多数在童年经历过创伤和悲痛事件、患有精神疾病并滥用过药物。影响其与全科医生交流的最重要的主题是"不信任"和"担心孩子享受不到社会福利"。她们经历过卫生和社会保健从业人员的羞辱,并缺乏有效的支持网。虽然不信任卫生和社会保健从业人员,但是她们通常会预约医生,但又不向医生说出自己的需求,这不利于其获得必要的医疗服务。其次重要的主题为"尽最大努力照顾孩子""因为妊娠而享有的整体生物心理社会护理""产后缺乏能使孩子获得社会福利的支持。结论妊娠为无家可归的妇女提供了一个关键机会,使其可以获得能解决其复杂需求的护理,并使其自我照护能力得到提升,尽管其并不信任卫生和社会保健从业人员。缺乏产后支持和担心孩子享受不到社会福利加剧了这些妇女的忧伤、精神健康危机,使其反复滥用药物,从而导致无家可归……并可能导致这一循环持续进行下去。

Background:traumatic brain edema(TBE)is caused by a specific water channel mediated by membrane ***-4(AQP4)plays an especially important role in this process,but the relationship between AQP4 and TBE remains *** purpose of this study was to explore expression of AQP4 in the hippocampus after traumatic brain injury(TBI),as well as the effect of brain edema on skeletal protein and its function in hippocampal ***:The adult male Wistar rats we divided into a sham group and a TBI group,the latter of which was further divided into 1,3,6,12,24 and 72 hours(h)and 15 days(d)post injury subgroups.A proper TBI model was established,and brain edema was assessed in each group by water *** measured the abundance of various proteins,including hypoxia inducible factor-1α(HIF-1α),AQP4,microtubule-associated protein 2(MAP2),tau-5 protein,phosphorylated level of TAU,synaptophysin,cyclic adenosine monophosphate response element binding protein(CREB),phosphorylated CREB and general control nonrepressed 2,in each *** neurons and spatial memory test were analyzed in different time ***:Compared with that in the sham group,the level of AQP4 in hippocampal neurons began to significantly increase at 1 h post TBI and then decreased at 15 d post *** this time frame,AQP4 level peaked at 12 and 72 h,and these peaks were closely correlated with high brain water ***-1αdisplayed a similar ***,levels of MAP2 began to decrease at 1 h post TBI and then increase at 15 d post *** addition,the most severe brain edema in rats was found at 24 h post TBI,with neuronal loss and hippocampal dendritic spine *** to those in the sham group,rats in the TBI groups had significantly prolonged latency and significantly shortened exploration ***:AQP4 level was closely correlated with severity of brain edema,and abnormal levels thereof aggravated such severity after TBI.

In Mali, chest injuries remain a real public health problem and are associated with heavy morbidity and mortality. Faced with a resurgence of urban civil violence and the explosion of road accidents, we decided to conduct this study in order to describe the epidemiological, clinical and therapeutic aspects of thoracic trauma in the emergency department. Method and Material: This is a descriptive prospective study over a period of one year in the emergency department of the CHU Gabriel Touré. Including all patients admitted for thoracic trauma. Analysis and Entry: Data were entered and analyzed on SPSS software version 20.0. The test was significant for a p value Results: We recorded 21,090 appeals in our structure among which 1284 patients were suspected of thoracic trauma. The diagnosis of thoracic trauma was retained in 119 (0.56%) patients. All patients were transported to the emergency room without prehospital medicalization. Clinical presentation was dominated by dyspnea in 54.6% of patients, however pain was the almost constant symptom in conscious victims. Various traumatic mechanisms had caused these lesions of the thorax, of which road traffic accidents represented half of the causes, followed by urban civil violence in 28.6% of patients. Landslides and falls from a great height were responsible for 19.5% of chest injuries. Open chest trauma was the type of lesion found in a third of the cases. This type of injury was exclusively due to blows and injuries during the brawls. Standard chest radiography was performed as the first intention in 60% of patients compared to 10.9% for the pleuropulmonary ultrasound. Thirteen patients required ventilatory assistance after orotracheal intubation. The average length of stay was 65.23 hours. Carotid-cavernous fistula is an abnormal communication between the carotid artery and the cavernous sinus. The fistula is classified based on its etiology, haemodynamic and anatomical configuration. The most common type is the direct high flow fistula resulting from trauma. Indirect fistula which is less common results from underlying conditions such as hypertension, collagen vascular diseases, pregnancy amongst others. It is in regards with this uncommon occurrence of indirect fistula that we report a case of forty years old woman who presented with a left protruding eyeball and headache a month after delivery and was diagnosed with spontaneous type D indirect carotid cavernous fistula with rapidly progressing symptoms. She was referred abroad for interventional radiological services due to unavailability of such services in our facility and country.
