Objective:To explore the efficacy and safety of Juan Bi Pill(JBP)in treatment of active rheumatoid arthritis(RA).Methods:From February 2017 to May 2018,115 participants from 4 centers were randomly divided into JBP group(57 cases)and placebo group(58 cases)in a 1:1 ratio using a random number table *** received a dose of JBP(4 g,twice a day,orally)combined with methotrexate(MTX,10 mg per week)or placebo(4 g,twice a day,orally)combined with MTX for 12 *** were required with follow-up visits at 24 and 48 weeks,attending 7 assessment *** were undergo disease activity assessment 7 times(at baseline and 2,4,8,12,24,48 weeks)and safety assessments 6 times(at baseline and 4,8,12,24,48 weeks).The primary endpoint was 28-joint Disease Activity Score(DAS28-ESR and DAS28-CRP).The secondary endpoints included American College of Rheumatology(ACR)criteria for 20%and 50%improvement(ACR20/50),Health Assessment Questionnaire Disability Index(HAQ-DI),clinical disease activity index(CDAI),visual analog scale(VAS),Short Form-36(SF-36)score,Medial Outcomes Study(MOS)sleep scale score,serum erythrocyte sedimentation rate(ESR),C-reactive protein(CRP),tender joint count,swollen joint count,and morning *** adverse reactions were observed during the ***:After 12 weeks of treatment,DAS28-ESR and DAS28-CRP scores in both groups were lower than before treatment(both P<0.01),while the remission rate of DAS28-ESR and DAS28-CRP and low disease activity of JBP group were higher than those in the placebo group(both P<0.01).JBP demonstrated better efficacy on ACR20 and ACR50 compliance rate at 12 and 48 weeks comparing to placebo(all P<0.05).The CDAI and HAQ-DI score,pain VAS and global VAS change of RA patients and physicians,the serum ESR and CRP levels,and the number of tenderness and swelling joints were lower than before treatment at 4,8,12,24,48 weeks in both groups(P<0.05 or P<0.01),while the reduction of above indices in

Dear Editor,Phenotypic drug discovery,which observes compound effects on physiology,has given way to targeting specific molecules,but has revived by using modern methods to find drugs based on therapeutic outcomes in disease models(Vincent et al.,2022).Osteoarthritis(OA)is a prevalent chronic joint disorder,primarily affecting knee joints,with osteoclast activation due to low-grade inflammation resulting in increased bone resorption and structural changes in the joint(Katz et al.,2021).This imbalance and the discharge of inflammatory mediators perpetuate the cycle of inflammation and tissue ***,controlling inflammation and modulating osteoclastogenesis are critical therapeutic targets for managing OA.

Cutaneous nerve regeneration in diabetic wounds remains a challenge for current clinical ***,a natural protein-based functional hydrogel is developed using keratin as the sole matrix,and protocatechuic aldehyde(PA)and zinc ions as the building blocks through dual-dynamic crosslinking reactions of thiol-aldehyde addition and catecholzinc ions *** unique structural design endows the hydrogel with excellent injectability,skin adhesion,self-healing and antibacterial ability,and *** addition,this hydrogel acts as a drug carrier for the loading and sustained release of phellopterin(PP),a natural compound extracted from traditional chinese medicine Angelica *** findings demonstrate that the PP-loaded hydrogel promotes cutaneous nerve regeneration,angiogenesis and tissue remodeling in the full-thickness wounds in both db/db and streptozotocin-induced C57BL/6 diabetic mice,notably by the synergistic effect of zinc ions and PP,showing a great potential in diabetic wound therapy.

Promoting synaptic plasticity and inducing functional reorganization of residual nerve fibers hold clinical significance for restoring motor function following spinal cord *** stimulation targeting the nerve roots has been shown to improve motor function by enhancing nerve conduction in the injured spinal cord and restoring the synaptic ultrastructure of both the sensory and motor ***,our understanding of the neurophysiological mechanisms by which nerve root magnetic stimulation facilitates motor function recovery in the spinal cord is limited,and its role in neuroplasticity remains *** this study,we established a model of spinal cord injury in adult male Sprague–Dawley rats by applying moderate compression at the T10 *** then performed magnetic stimulation on the L5 nerve root for 3 weeks,beginning on day 3 *** day 22 post-injury,we observed that nerve root magnetic stimulation downregulated the level of interleukin-6 in the injured spinal cord tissue of ***,this treatment reduced neuronal damage and glial scar formation,and increased the number of neurons in the injured spinal ***,nerve root magnetic stimulation decreased the levels of acetylcholine,norepinephrine,and dopamine,and increased the expression of synaptic plasticity-related m RNA and proteins PSD95,GAP43,and Synapsin *** together,these results showed that nerve root magnetic stimulation alleviated neuronal damage in the injured spinal cord,regulated synaptic plasticity,and suppressed inflammatory *** findings provide laboratory evidence for the clinical application of nerve root magnetic stimulation in the treatment of spinal cord injury.

Extensive use of avermectin(AVM)in agriculture can seriously contaminate fish in *** acid(FA)is a strong antioxidant sourced from plants and is widely applied in food and medicine,but its protective function on the immunological damage caused by AVM is still *** this study,carp were chronically exposed to(1/1096 hr LC_(50))AVM for 30 day(s),with a dietary supplement of FA(400 mg/kg)to assess its effects on carp *** experimental groups(n=10)included:control,AVM-exposed(2.404μg/L),FA+AVM co-treated,and FA ***-term AVM exposure altered spleen tissue,reducing serum complement C3(p<0.01)and immunoglobulin M levels(p<0.001),and increasing proinflammatory tnf-α(p<0.001),il-6(p<0.001),il-1β(p<0.001),and inos mRNA levels,whilst down-regulating the anti-inflammatory tgf-β(p<0.05).Additionally,it disrupted the balance of oxidative stress indicators such asMDA(p<0.001),T-AOC,GSH,and CAT,leading to spleen tissue apoptosis(42.4%).However,the addition of FA reversed these conditions,elevated the anti-inflammatory factor,and improved spleen immune function following chronic exposure to AVMin ***,the ability to restore oxidative homeostasis in carp by adjusting the Nrf2/NQO-1 axis protected the health of spleen *** discovery also supports the development of fish feed for aquaculture.

Integrating machine learning and human use experience to identify personalized pharmacotherapy in traditional chinese medicine:a case study on resistant hypertension

OBJECTIVE: To enhance the understanding of identifying personalized pharmacotherapy options in traditional chinese medicine(TCM), and further support the registration of new TCM drugs. METHODS: Generalized Boosted Models and XGBoost were employed to construct a classification model to identify the bad prognosis factors in resistant hypertension(RH) patients. Furthermore, we used association analysis to explore the rules of "symptomsyndrome" and "symptom-herb" for the major influencing factors, in order to summarize prescription pattern and applicable patients of TCM. RESULTS: Patients with major adverse cardiac events mostly have complex symptoms of phlegm, stasis, deficiency and fire intermingled with each other, and finally summarized the human experience of using chinese herbal medicine to precisely intervene in some symptoms of RH patients on the basis of conventional Western medical treatment. CONCLUSIONS: Machine learning algorithms can make full use of human use experience and evidence to save clinical trial resources and accelerate the development of TCM varieties.
