Aim:The aim was to analyze the feasibility of salvage liver transplant after liver resection in hepatocellular carcinoma(HCC)with hepatitis C virus(HCV)***:All the patients diagnosed with HCC with HCV etiology who underwent living donor liver transplant from July 2002 to November 2012 were *** recurrence rate,mortality,and prognostic factors were analyzed and compared between primary transplant and salvage transplant for up to 5 years ***:One hundred and nine patients underwent a liver transplant for HCC associated with HCV etiology within the University of California,San Francisco *** were post-hepatectomy salvage transplants and 91 were primary *** follow-up time was 31 ***,3 and 5 years overall survival rates were 76%,76%and 65%in the salvage group,and 92%,85%and 85%in primary transplant group *** difference in overall survival rates was statistically signifi cant(P=0.031).However,recurrence-free survivals for 1,3 and 5 years were 72%,72%and 46%for salvage group,and 91%,73%and 46%for primary transplant group;which were not statistically signifi cant(P=0.328).Conclusion:Salvage transplantation for post-hepatectomy recurrence for patients with HCC associated with HCV-related chronic liver disease seems to offer inferior overall survival rates than primary transplantation.

The scarcity of donor livers has increased the interest in donation after cardiac death(DCD) as an additional pool to expand the availability of organs. However, the initial results of liver transplantation with DCD grafts have been suboptimal due to an increased rate of complications, as well as decreased graft survival. These challenges have led to many developments in DCD donation outcome, as well as basic and translational research. In this article we review the unique characteristics of DCD donors, nuances of DCD organ procurement, the effect of prolonged warm and cold ischemia times, and discuss major studies that compared DCD to donation after brain death liver transplantation, in terms of outcomes and complications. We also review the different methods of donor treatment that has been applied to ameliorate DCD organ outcome, and we discuss the role of machine perfusion techniques in organ reconditioning. We discuss the two major perfusionmodels, namely, hypothermic machine perfusion and normothermic machine perfusion; we compare both methods, and delineate their major differences.

BACKGROUND: Few studies have been performed to assess health-related quality of life (HRQOL) in liver transplantation (LT) patients in the mainland of China. This study aimed to investigate the HRQOL of post-LT patients in a single center. METHODS: HRQOL was evaluated by the SF-36 (Chinese version) questionnaire in 60 patients (LT group) who had received LT for benign end-stage liver disease (BELD). Fifty-five patients with BELD (BELD group) and 50 healthy volunteers from the general population (GP group) were also evaluated, and the results were compared among the three groups. RESULTS: There was a significant difference among the three groups in terms of the scores of eight domains in the SF-36 (P0.025), but higher scores for the remaining seven domains (P0.025), but had lower scores for the remaining three domains (P<0.025). Lower family income was found to be associated with reduced physical function and mental health scores (P<0.05). Better education was associated with increased mental health scores (P<0.05). CONCLUSIONS: LT patients generally have a good HRQOL although some respects of their HRQOL remains to be improved. Lower family income and poor education are important factors relating to the poor HRQOL of LT patients.

Tissue Response of an Osteoinductive Bioceramic in Bone Defect Rabbit Model

HA/TCP and HA rods （φ5 mm×10 mm） were made for implantation in New Zealand white rabbit with different condition. Sixty three rabbit were divided into three groups： group 1 （n=18）, group 2 （n=27） and group 3 （n=18）. In group 1, 10 mm radius was defected, and one HA/TCP rod was implanted in the muscle a distant away from the bone defect area. In group 2, also, 10 mm radius was defected, one HA rod was implanted in the muscle a distant away from the bone defect area. In group 3, two HA/TCP rods were implanted in the dorsal muscle of the rabbit with bone intact. Histological observation showed that in group 1, some new bone was found only two months after implantation （n=2）, and obvious immature woven bone could be observed in these bioceramics from the 3rd month on. However, in group 3, bone began to be found 6 months after implantation （n=2）. In group 2, we could not find any bone tissue up to 9 month’s observation. These results suggest that, first, the bone defect model could significantly accelerate bone formation at non-osseous sites in rabbits; second,. HA/TCP bioceramics were confirmed with osteoinductive property while HA bioceramics without osteoinductive property nearly. Thus, bone defect might be a good animal model for further researches for osteoinductive bioceramics.

Advances in stem cell science and potential clinical applications have brought clinical medicine closer to the actualization of Regenerative Medicine—an extension of transplantation of organs and cells and implantation of bioprosthetics and biodevices. The goal of such therapeutics will be intervention prior to onset of severe individual disability, enhance organ function and enhance patient performance status without incurring the economic impacts of standard organ transplantation. Regenerative Medicine is already demonstrating proof of principle or efficacy in restora- tion of myocardial contractility, joint mobility and function, immune competence, pulmonary function, immunologic self- tolerance, motor function and normal hemoglobin production with the next targets—diabetes mellitus (type I and type II), neurologic injury, hepatic dysfunction preparing to enter trials. Expenditures on health care needs of an aging U.S. citizenry approximate 20-25% ($3 trillion) of U.S. GDP currently and may to grow to 40% of U.S. GDP by 2025. As the potential of Regenerative Medicine is clinically realized, the societal impact and economic benefits will be disproportionately magnified in the economies of industrialized nations. The experi- ence of the Department of Health and Human Services (HHS), United Network for Organ Sharing (UNOS), the National Bone Marrow Donor Registry (NBMDR), and the National Vaccine Injury Compensation programs (NVICP) can help ensure that as Regenerative Medicine strives to achieve clinical benefits while avoiding decimation of therapeutic options by product liability and medical malpractice concerns—concerns that crippled the U.S. vaccine manufacturing industry until the creation of the NVICP. The first 50 years of organ/cell/tissue transplantation demonstrates that clinical reality of allogeneic and autologous transplantation can antedate complete understanding of the basic science underlying successful transplantation. Liver transplantation(LTx)is an established option for the treatment of end-stage liver disease,acute liver failure,and hepatic malignancies(1-3).Despite significant advances in clinical practice and scientific research on LTx,liver allograft dysfunction remains a significant clinical *** allograft dysfunction(EAD)is a milder form of primary graft dysfunction that correlates with postoperative complications,higher mortality rates,and decreased graft survival(4).The frequency of EAD ranges from 15%to 30%after LTx from donors after brain death(DBD),reaching 68.4%after LTx from donors after cardiac death(DCD)(5).For living donor liver transplantation(LDLT),the prevalence of EAD is comparatively low,accounting for 18.1%of the recipients in our transplant center(4).

Background Genomic selection involves choosing as parents those elite individuals with the higher genomic estimated breeding values(GEBV)to accelerate the speed of genetic improvement in domestic *** after multi-generation selection,the rate of inbreeding and the occurrence of homozygous harmful alleles might increase,which would reduce performance and genetic *** mitigate the above problems,we can utilize genomic mating(GM)based upon optimal mate allocation to construct the best genotypic combinations in the next *** this study,we used stochastic simulation to investigate the impact of various factors on the efficiencies of GM to optimize pairing combinations after genomic selection of candidates in a pig *** factors included:the algorithm used to derive inbreeding coefficients;the trait heritability(0.1,0.3 or 0.5);the kind of GM scheme(focused average GEBV or inbreeding);the approach for computing the genomic relationship matrix(by SNP or runs of homozygosity(ROH)).The outcomes were compared to three traditional mating schemes(random,positive assortative or negative assortative matings).In addition,the performance of the GM approach was tested on real datasets obtained from a Large White pig breeding *** Genomic mating outperforms other approaches in limiting the inbreeding accumulation for the same expected genetic *** use of ROH-based genealogical relatedness in GM achieved faster genetic gains than using relatedness based on individual *** GROH-based GM schemes with the maximum genetic gain resulted in 0.9%-2.6%higher rates of genetic gainΔG,and 13%-83.3%lowerΔF than positive assortative mating regardless of *** rates of inbreeding were always the fastest with positive assortative *** from a purebred Large White pig population,confirmed that GM with ROH-based GRM was more efficient than traditional mating *** Compared with traditional mating schemes,genomic
