New simulation model for bone formation markers in osteoporosis patients treated with once-weekly teriparatide

Daily 20-mg and once-weekly 56.5-mg teriparatide（parathyroid hormone 1–34） treatment regimens increase bone mineral density（BMD） and prevent fractures, but changes in bone turnover markers differ between the two regimens. The aim of the present study was to explain changes in bone turnover markers using once-weekly teriparatide with a simulation model. Temporary increases in bone formation markers and subsequent decreases were observed during once-weekly teriparatide treatment for 72 weeks. These observations support the hypothesis that repeated weekly teriparatide administration stimulates bone remodeling, replacing old bone with new bone and leading to a reduction in the active remodeling surface. A simulation model was developed based on the iterative remodeling cycle that occurs on residual old bone. An increase in bone formation and a subsequent decrease were observed in the preliminary simulation. For each fitted time point, the predicted value was compared to the absolute values of the bone formation and resorption markers and lumbar BMD. The simulation model strongly matched actual changes in bone turnover markers and BMD. This simulation model indicates increased bone formation marker levels in the early stage and a subsequent decrease. It is therefore concluded that remodeling-based bone formation persisted during the entire treatment period with once-weekly teriparatide.

Effectiveness of adaptive servo-ventilation

Adaptive servo-ventilation(ASV) has been developed as a specific treatment for sleep-disordered breathing, in particular Cheyne-Stokes respiration with central sleep apnea(CSA). Heart failure patients often have sleep-disordered breathing, which consists of either obstructive sleep apnea(OSA) or CSA. Other medical conditions, such as stroke, acromegaly, renal failure, and opioid use may be associated with CSA. Continuous positive airway pressure(CPAP) therapy is widely used for patients with OSA, but some of these patients develop CSA on CPAP, which is called treatmentemergent CSA. CPAP can be useful as a treatment for these various forms of CSA, but it is insufficient to eliminate respiratory events in approximately half of patients with CSA. As compared to CPAP, ASV may be a better option to treat CSA, with sufficient alleviation of respiratory events as well as improvement of cardiac function in heart failure patients. In patients without heart failure, ASV can also alleviate CSA and relieve their symptom. Recently, ASV has been widely used for patients with various forms of CSA. ASV may be also used in the setting without CSA, but it should be assessed more carefully. Clinicians should have a better understanding of the indications for ASV in each setting.

Background: The nutritional support after hematopoietic stem cell transplantation (HSCT) has not been well established due to the scarcity of clinical trials. To conduct international clinical trials in Asia, we performed the questionnaire survey to investigate the current standard of nutritional support after HSCT. Method: We sent the questionnaire to the physicians nominated by the Asia Pacific Blood and Marrow Transplantation (APBMT) members of each country/ region. Result: We received 15 responses from 7 different countries/regions. The target calorie amount is 1.0 - 1.3 × basal energy expenditure (BEE) in 11 institutes when partial parenteral nutrition is used. When total parenteral nutrition (TPN) is used, the target calorie amount is 1.0 - 1.3 × BEE in 9 institutes and 1.3 - 1.5 × BEE in 4 institutes. Lipid emulsion is routinely used in 12 institutes. Multivitamins and trace elements are routinely added to TPN used in most institutes. It is still uncommon to use the immunonutrition. Blood glucose levels are routinely monitored in all institutes, but the target range varies (<110 in 2 institutes, <150 in 4 institutes, and <200 in 8 institutes). Conclusions: Basic nutritional support is similar in participating institutes. However, the target glucose level varies and the use of immunonutrition is rather rare. These points can be the theme of future clinical trials.

Background: Phlebotomy is performed to reduce excessive iron accumulation in hepatic tissue. We studied serum alanine aminotransferase (ALT) normalization rates and 50%reduction in initial serum ALT (ALT50%reduction rate) in patients with hepatitis C viral (HCV) infection and investigated the factors that influenced the response to phlebotomy therapy. Methods: We evaluated 23 consecutive patients with HCV infection who underwent phlebotomy. Phlebotomy was performed a few times per week, then a few times per month, and 200-400 ml of blood was removed at each session, depending on the clinical response. During the course of therapy, hemoglobin (Hb), serum ALT, and ferritin levels were assessed monthly. Results: In patients with Hb of less than 11 g/dl, the ALT50%reduction rate was 87.5%. In patients with a serum ferritin level of less than 10 g/dl the ALT50%reduction rate was 83.3%. In patients with Hb of less than 11 g/dl, the ALT normalization rate was 50%, and in those with a serum ferritin level of less than 10 g/dl, the ALT normalization rate was 41.7%. Multivariate analysis identified ALT less than 100 IU/l at the start of phlebotomy as an independent factor associated with ALT normalization. Of the 7 patients who showed no response to phlebotomy, 85.7%were obese (body mass index ≥25 kg/m2), and 40%showed more than 30%steatosis on liver histology. The cumulative ALT normalization rate in relation to the total volume of blood loss was 43.9%with a blood less or more than 3l, and thus was optimal above 3l. Conclusions: Although the sample number was relatively small, the results of our study suggest that phlebotomy is effective therapy for HCV patients who are nonobese, show little or no steatosis on liver histology, and have a baseline serum ALT level of less than 100IU/l.

Background: Medical staff provide care to spouses of terminal cancer patients through trial and error by meeting their various support needs and spousal factors regarding their anticipatory grief. Studies on the association between spousal characteristics and anticipatory grief have been inconclusive;additionally, there has been insufficient research on support needs for anticipatory grief of spouses. This study aimed to explore the spousal characteristics and social support needs predicting anticipatory grief in spouses of patients with cancer at the end of life. Methods: This was a cross-sectional study. Eligible spouses (n = 102) completed a self-report questionnaire in two hospitals with palliative care units in Japan. The questionnaire included demographic information, a tool assessing social support needs of spouses, and the Anticipatory Grief Scale for Family Caregivers. Results: Simple regression analyses indicated that patient age, chemotherapy, no treatment, ECOG PS3, children aged under 20 years, total score of “social support needs regarding the disease and treatment of the patients” and subscale scores (“medical condition and cure,” “daily life and social support,” and “intimacy and employment”), and total score of “social support needs of the spouses” and subscale scores (“family psychological issues and social support” and “intimacy, employment, and society”) were significant variables (all p p Conclusions: Patients having no experience of “chemotherapy” and higher “social support needs of the spouses” in Model 1, and greater spousal needs of “family psychological issues and social support” in Model 2 were significant predictors of severe anticipatory grief. Medical staff should pay attention to these risk factors that predict anticipatory grief among spouses.

Background: In Japan, there are few studies of long-term (more than 1 month) interferon (IFN) therapy for chronic hepatitis B (CHB). In this retrospective study, we investigated the efficacy and predictors of response to 6-month IFN therapy. Methods: We analyzed 66 Japanese patie nts with CHB who were treated with IFN for 6 months. They comprised patients who were hepatitis B e antigen (HBeAg)-positive (n = 45) and -negative (n = 21). One (2%), 8 (12%), and 51 (77%) patients were infected with hepatitis B virus (HBV) genotypes A, B, and C, respectively. Responders in patients positive for HBeAg were defined as those who showed normalization of serum alanine aminotransferase (ALT) level, HBeAg loss, and HBV DNA negativity at 6 months after completion of IFNtherapy. In patients negative for HBeAg, responders were defined as those patients who showed normalization of ALT level and HBV DNA negativity at the same 6-month time point. Results: Of the 45 patients with HBeAg at the commencement of IFN therapy,9 (20%) were responders. Young patients, especially those with a high serum ALT level, were significantly more likely to respond to IFN therapy. Of the 21 patients negative for HBeAg, 13 (62%) were responders. There were no significant differences in clinical characteristics between responders and nonresponders among patients negative for HBeAg. Multivariate analyses identified HBeAg negativity and young age as independent factors associated with a positive response to 6-month IFN therapy. However, long-term follow-up of the treated patients showed a fall in the response rate. Conclusions: The response rate to 6-month IFN therapy among HBeAg-positive patients was low. However, young patients may require long-term IFN therapy.

Background. Approximately 30%of patients with chronic hepatitis C have normal serumalanine amino transferase (ALT) levels. While interferon (IFN) mono therapy is approved for patients with chronic hepatitis C infection, the effectiveness of such therapy for chronic hepatitis C patients with normal ALT levels at commencement of treatment remains poorly understood. Methods. Ninety four individuals (M/F, 54∶40; median age, 46 years) with normal ALT levels (< 50 IU/ 1) at the commencement of treatment who were positive for both antihepatitis C virus (HCV) and serum HCV RNA were studied. Among this group, 18 individuals (M/F, 9∶9; median age, 50 years) had had persistently normal ALT levels for at least 3 months prior to treatment. All patients received their first course of IFN therapy in this study. Results. Forty three (45.7 %) of 94 individuals had lost serum HCV RNA at 6 months after cessation of therapy (complete response; CR). The proportion of patients with genotype 2a and HCV RNA level over 1 Meq/ml who showed CR was significantly lower in those with normal ALT levels than in those with elevated ALT levels (23.8%vs 55.6%; P = 0.0189). Two patients who had persistently normal ALT levels and HCV RNA level over 1 Meq/ml were non responders (NR) and had ALT flare ups after IFN therapy. Patients with HCV RNA levels of less than 1 Meq/ml did not show differential responses based on ALT levels. Conclusions. Our data suggest that IFN therapy is effective for patients with normal ALT levels and less than 1 Meq/ml HCV RNA. Thus, such patients should be considered for curative IFN therapy.

AIM: To investigate risk factors for development of hepatocellular carcinoma(HCC) in patients with hepatitis C virus-related liver cirrhosis(LC-C).METHODS: To evaluate the relationship between clinical factors including virological response and the development of HCC in patients with LC-C treated with interferon(IFN) and ribavirin, we conducted a multicenter, retrospective study in 14 hospitals in Japan. All patients had compensated LC-C with clinical or histological data available. HCC was diagnosed by the presence of typical hypervascular characteristics on computed tomography and/or magnetic resonance ***: HCC was diagnosis in 50(21.6%) of 231 LC-C patients during a median observation period of 3.8 years after IFN and ribavirin therapy. Patients who developed HCC were older(P = 0.018) and had higher serum levels of pretreatment alpha-fetoprotein(AFP)(P = 0.038). Multivariate analysis revealed the following independent risk factors for HCC development: history of treatment for HCC [P < 0.001, odds ratio(OR) = 15.27, 95%CI: 4.98-59.51], AFP levels of ≥ 10 ng/m L(P = 0.009, OR = 3.89, 95%CI: 1.38-11.94), and des-γ-carboxy prothrombin(DCP) levels of ≥ 40 m AU/mL at 24 wk after the completion of IFN and ribavirin therapy(P < 0.001, OR = 24.43, 95%CI: 4.11-238.67).CONCLUSION: We suggested that the elevation of AFP and DCP levels at 24 wk after the completion of IFN and ribavirin therapy were strongly associated with the incidence of HCC irrespective of virological response among Japanese LC-C patients.
