Background:Various extracts from Citrullus colocynthis seeds were recently examined for their possible favorable effects on glucose homeostasis,other metabolic variables,and pancreatic islet size in streptozotocin-induced diabetic ***:Considering the effectiveness of insulin upon target cells as another process susceptible to be influenced by plant extracts,the present study is used to compare the effects of either crude or defatted aqueous extracts from *** seeds upon Na+transport in A6 kidney ***:The intensity of electrical currents,taken as the potential difference divided by the epithelial resistance was measured in monolayers of A6 kidney cells exposed at the basolateral side to either crude untreated aqueous(EI)or defatted aqueous(EII)extracts,as well as their fractions EI1 and EII1 containing components with high molecular weight(>5 kDa),and EI2 and EII2 containing components with low molecular weight(<5 kDa).The effect of insulin was to assess the functional integrity of the A6 ***:All extracts caused a rapid and sustained increase in current *** defatted aqueous extract(EII)was more efficient than the non-defatted aqueous extract(EI)in terms of increasing the intensity of current in the A6 *** components of the defatted aqueous extract responsible for such a greater efficiency were located in the fraction containing components with low molecular weight and acted in a time-related progressive fashion upon Na+***:These data suggest that colocynth seed extracts mainly containing components with low molecular weight increased the current intensity in A6 cells,thus suggesting that colocynth may affect Na+transport in kidney cells.

The Asian lotus(Nelumbo nucifera)has a history of cultivation in Asia dating back over 3,000 years where it has been an important food crop producing edible rhizomes and seeds as well as flowers of great aesthetic and cultural ***,we de novo assembled the plastomes of 316 lotus accessions including five North American lotus(***)and 311 Asian lotus(***)to construct a pan-plastome genome map,and investigate the phylogeography and genetic diversity among the only two extant species within this living fossil lineage.A total of 113 unique genes were annotated and plastome sizes varied between 163,457 and 163,672 bp with only minor differences in each of the four major genomic *** most abundant nucleotide differences among plastomes were single nucleotide variants followed by insertions/deletions and block substitutions mainly found in intergenic spacer regions of the large single copy portion of the *** well-supported genetic clusters were resolved using multiple different population structure *** different lotus types(flower,seed,rhizome,or wild)were disproportionally assigned to multiple different genetic *** pattern indicates that the domestication of Asian lotus involved multiple genetic origins and possible matrilineal *** mapping of accessions also revealed that genetic diversity is unevenly distributed with eastern China possessing the highest genetic diversity and regions such as Yunnan,Indonesian,and Thailand possessing unique *** results provide an important maternal history of Nelumbo and necessary groundwork for future studies on intergenomic gene transfer,cytonuclear incompatibility,and conservation genetics.

A Case of Serous Tubal Intraepithelial Carcinoma Diagnosed by Cytology during the Operation

We experienced a case of serous tubal intraepithelial carcinoma (stic), which is the earliest morphologically recognizable precursor of pelvic high grade serous carcinoma, diagnosed by cytology during the operation. A 48-year-old Japanese woman visited to our department because of abnormal cytological result and the left adnexal mass on female cancer screening. Pre-operatively, she was diagnosed as cervical intraepithelial neoplasia (CIN) 3, adenomyosis, and the left adnexal cystic lesion with papillary growth. At laparotomy, the left adnexal tumor turned out to be cystic fallopian tube with small papillary growth on the inner surface, but abnormal findings were not present on bilateral ovaries. The cytology of the imprint smears from the papillary projection and ascitic fluid showed positive, suggesting serous adenocarcinoma. Histopathological examination of the lesion revealed the left fallopian tube lesion was stic with immunohistochemically positive reactivity against p53. No metastases including disseminated lesions were noted. The patient received four courses of systemic chemotherapy, and had no recurrent signs 10 months after the operation.

背景脑外伤后出现的高血糖与预后不良相关，本实验研究了脑外伤儿童围手术期发生高血糖的概率及危险因素。方法以1994年至2004年间，港景医学中心（成人与儿童一级创伤中心）开展脑外伤（TBI）急诊开颅术的13岁以下儿童做回顾性群组研究。分别在手术前（从到达急诊室至开始全身麻醉）、手术中（全身麻醉期间）及手术后早期（手术后首个24小时）测试每例患者的血糖值，计算高血糖（血糖2200mg/dl）及低血糖（血糖〈60mg／d1）的发生率。持续性高血糖定义为在3个实验阶段中（手术前、手术中、手术后早期）有任意两阶段出现血糖升高，而暂时性高血糖是指有任意一阶段出现血糖升高。采用多元10~stic归分析来推测围手术期高血糖的独立预测因子，数据表示为经修正的比值比（AOR）（95％可信区间），P〈0．05认为有统计学意义。结果在每个实验阶段中至少记录1次血糖值：手术前（86[82％]），手术中（94[89％]），和手术后（101[97％]）。在麻醉期间，有64％儿童的血糖测定次数不足每小时1次。47例（45％）儿童在3个实验阶段中至少有1次血糖升高，有29例儿童（28％）发生了暂时性高血糖，18例儿童（17％）发生了持续性高血糖。围手术期高血糖的独立预测因素为年龄〈4岁（AOR[95％CI]；3．5[1．2～10．6]），格拉斯哥昏迷评分≤8（AOR[95％CI]；7．2[2．4～21．5]）以及合并有硬膜下血肿的复合损伤（AOR[95％CI]；34．7[2．3—525．5]）。6例儿童使用胰岛素进行治疗，两例儿童出现了非胰岛素相关性低血糖。结论围手术期高血糖是常见的，而手术中低血糖也并不少见。为了有效监测手术中高血糖及低血糖的发生概率，需要增加手术中血糖测定频率。年龄〈4岁、严重的脑外伤和合并硬膜下血肿的复合损伤是围手术期高血糖发生的危险因素。
