Rotablation in the treatment of high-risk patients with heavily calcified left-main coronary lesions

Objective Heavily calcified left-main coronary diseases (LMCA) remain a formidable challenge for percutaneous interventions (PCI). This study was to investigate the safety and efficacy of using rotational atherectomy (RA) in treating such lesions in actual practice. Methods From February 2004 to March 2012, all consecutive patients who received RA for heavily-calcified LMCA lesions in our cath lab were en-rolled. The relevant clinical and angiographic characteristics at the time of index PCI, as well as the clinical follow-up outcomes, were re-trieved and analyzed. Results A total of 34 consecutive patients were recruited with a mean age 77.2 ± 10.2 years. There were 82.4%pre-sented with acute coronary syndrome and 11.8%with cardiogenic shock. Chronic renal disease and diabetes were seen in 64.7%and 52.9%, respectively. Triple-vessel coronary disease was found in 76.5%of them. The mean SYNTAX score was 50 ± 15 and EuroSCORE II scale 5.6 ± 4.8. The angiographic success rate was 100%with a procedural success rate of 91.2%. The mean number of burrs per patient was 1.7 ± 0.5. Crossing-over stenting was used in 64.7%. Most stents were drug-eluting (67.6%). Intra-aortic ballon pump was used in 20.6%of the procedures. Three patients died during hospitalization, all due to presenting cardiogenic shock. No major complication occurred. Among 31 hospital survivors, the major adverse cardiac events (MACE) rate was 16.1%, all due to target lesion revascularization or target vessel revas-cularization. Conclusions In high-surgical-risk elderly patients, plaque modification with RA in PCI of heavily-calcified LMCA could be safely accomplished with a minimal complication rate and low out-of-hospital MACE.

Background and study aims: Endoscopic retrograde chola-ngiopancreatography (ERCP) is the diagnostic procedure of choice in patients with biliary strictures and no culprit mass lesion on abdominal imaging, but it is limited in its diagnostic accuracy. The aim of this prospective study was to determine the value of intraductal ultrasound (IDUS) in distinguishing between benign and malignant biliary strictures in this clinical setting. Patients and methods: Sixty-one patients with painless jaundice and no mass lesion on abdominal computed tomography, who were found to have a biliary stricture at ERCP, underwent IDUS with a high-frequency (20-MHz) wire-guided probe. Histo-patho-logical confirmation or clinical follow-up was used to establish the final diagnosis. The diagnostic performances of IDUS, ERCP, and IDUS plus ERCP in the identification of malignant strictures were evaluated. Results: Forty-three patients had malignant strictures and 18 had benign strictures. ERCP produced 25 false-negative diagnoses, 22 of which were identified as malignant by IDUS. IDUS provided seven false-negative and three false-positive diagnoses. The proportion of patients with malignant strictures who tested positive with IDUS was 2.06 times that of ERCP (95% CI, 1.37- 3.10; 83.3% vs. 40.5% , P=0.0004). When used in conjunction, IDUS increased the accuracy of ERCP from 58% to 90% . Patients with operable lesions on IDUS and no contraindication to surgery underwent resection; most patients with pancreatic parenchymal invasion on IDUS underwent EUS, which identified a pancreatic mass in more than 50% of cases. Patients with negative IDUS and a low clinical suspicion for malignancy were treated endoscopically, while a more aggressive work-up was performed in all patients with high pretest probability, regardless of the IDUS results. BACKGROUND Hepatocellular carcinoma(HCC) is the most common type of primary liver cancer. Current guidelines for HCC management recommend surveillance of high-risk patients every 6 mo using ultrasonography. Serum biomarkers, like alpha-fetoprotein(AFP), protein induced by vitamin K absence/antagonist-Ⅱ(PIVKA-Ⅱ) and lectin-reactive AFP, show suboptimal performance for detection of HCC, which is crucial for successful resection or treatment. Thus, there is a significant need for new biomarkers to aid early diagnosis of HCC. Studies have shown that the expression level of human microRNAs(miRNAs), a small, non-coding RNA species released into the blood, can serve as an early marker for various diseases, including *** To evaluate the diagnostic role of miRNAs in HCC as single markers, signatures or in combination with known protein *** Our prospective, multicenter, case-control study recruited 660 participants(354 controls with chronic liver disease and 306 participants with HCC) and employed a strategy of initial screening by two independent methods, real-time quantitative PCR(n = 60) and next-generation sequencing(n = 100), to assess a large number of miRNAs. The results from the next-generation sequencing and real-time quantitative PCR screening approaches were then combined to select 26 miRNAs(including two putative novel miRNAs). Those miRNAs were analyzed for their diagnostic potential as single markers or in combination with other miRNAs or established protein biomarkers AFP and PIVKA-Ⅱ via real-time quantitative PCR in training(n = 200) and validation cohorts(n = 300).RESULTS We identified 26 miRNAs that differentiated chronic liver disease controls from(early) HCC via two independent discovery approaches. Preliminary results from an investigation of the U-238/Th-234 parent/daughter pair in the nearshore environments of Lake Michigan show that short-lived, naturally-occurring radionuclides are useful in determining the time scales of sediment transport processes on the orders of days to weeks. While the long term, depositional sink for some new particles entering the lake, either from rivers or shoreline erosion, has been well documented, the aggregate time scale of the processes by which these particles move from source to sink is not well quantified. These relatively 搉ew?particles may undergo numerous episodes of resuspension and redeposition before making their way through the coastal margin and entering the long term focusing processes active in the depositional basins of the lake. These episodic events can be large in scale and magnitude, resuspending as much or more material than is permanently deposited in the lake on an annual basis. Th-234, a particle reactive radionuclide with a 24 day half-life, would appear to be well suited as a tracer for following rapid particle movement, particle residence times in the overlying water, and alongshore transport processes. The remotely operated vehicle (ROV) deployed technique for collection of surface sediments residing in non-depositional (hard bottom) lakebeds and some preliminary time series results for transient excess Th-234 inventories in the nearshore of Lake Michigan are described.

Evaluating Training Programs for Electroacupuncture Techniques with Skin Temperature as a New Index

Training in acupuncture techniques has a long history of thousands of years. It has been individually handed down from person (teacher) to person (student). However, techniques and training have not been scientifically evaluated because individual differences may exist among evaluators. In animal studies, some researchers have reported that acupuncture stimulation dilates blood vessels of the skin and skeletal muscles. These studies also reveal an association between skin temperature (ST) and blood circulation volume on the skin. Our previous studies have reported that acupuncture stimulation, especially that of electroacupuncture (EA), can elevate ST. Therefore, we monitored the instructive effects and level of EA techniques with ST and propose that we can bring monitoring ST into training/education of EA as a new index of technical assessment. Moreover, ratio of changes might be used as new criteria for retraining. Healthy students (n = 14) were given with 10 minutes of EA stimulation on the tibialis anterior: Zusanli (ST36) and Tiaokou (ST38). Their ST was monitored before and during stimulation as well as for 30 minutes after stimulation. All subjects showed a nominal increase in ST. At the time, ratios of changes were also calculated. Two subjects did not reach the average of 1.3%. This suggests that the technical level of the therapist was inadequate. Thus, observation of ST elevation and calculation of the average ratio of ST change (elevation) could be applied to a new scientific index of technical assessment in acupuncture treatment training. However, further research (e.g., larger-scale studies, adjustment for gender differences, or other age subjects) is required to support these findings.

AIM: To explore the association of serum insulin, insulin resistance, and β-cell dysfunction with gallstone disease (GSD) in type 2 diabetics. METHODS: We used a community-based study conducted between 1991 and 1993 in Kinmen, Taiwan to identify type 2 diabetics. A screening program for GSD was performed in 2001 by a panel of specialists who employed real-time ultrasound sonography to examine the abdominal region after the patient had fasted for at least 8 h. Screening was conducted in 2001 on 848 patients diagnosed with type 2 diabetes. The HOMA method was used to compare the profile differences for insulin resistance (HOMA IR) and β-cell dysfunction (HOMA β-cell). RESULTS: We studied 440 type 2 diabetics who attended sonography check-ups. After excluding eight insulin-treated diabetics, the prevalence of GSD among the remaining 432 was 13.9% (26/187) among males and 14.7% (36/245) among females. After adjustment for other GSD-associated risk factors in addition to age and obesity, GSD risk increased among females with levels of serum insulin [4th vs 1st quartile odds ratios (OR) = 4.46 (95%CI: 1.71-11.66)] and HOMA IR [4th vs 1st quartile OR = 4.46 (95%CI: 1.71-11.66)]. Better HOMA β-cell function was significantly related to decreased risk of GSD [4th vs 1st quartile OR = 0.16 (95%CI: 0.03-1.70)]. Among males, age and central obesity were the most significant risk factors for GSD. No association of GSD with serum insulin, HOMA IR, and HOMA β-cell was observed among males. CONCLUSION: Serum insulin, insulin resistance, and β-cell dysfunction are risk factors for GSD in females, but not males with type 2 diabetes.

Drought is one of the critical limitations to agricultural soils and crop *** of water is increasing due to climate change that lead to increasing threats to global food ***,ecofriendly and cost effective strategies are highly desirable for mitigating drought stress along with sustainable and smart agricultural *** aim of the study was to mitigate DS using seed priming and exogenous supplementation ofβ-aminobutyric acid(BABA)in linseed(Linum usitatissimum L.).Different doses(0,50,100 and 150μM)of BABA were used for seed priming agent and foliar spraying under three soil moisture levels viz.,25%(SM25),45%(SM45)and 65%(SM65).The response variables of both experiments included different agro-botanical traits and oxidative stress indicators such as melondialdehyde content,free proline accumulation,and antioxidant defense in *** linseed plants showed water stress at SM25 that reduced plant height,number of branches per plant,time taken to flower initiation and heading,and root and shoot dry ***,the number of capsules and seeds per capsule showed a significant decline at SM25,which led to a drastic reduction in 100-seed weight yield in linseed plants in both ***,seed priming and foliar supplementation with of BABA(50–100μM)significantly improved these morpho-agronomical attributes in linseed plants under *** results revealed that the BABA was fully active in linseed plants at ***,the combination of SM25 with BABA significantly improved the antioxidant enzymes superoxide dismutase(SOD),catalase(CAT),ascorbate peroxidase(APX),and peroxidase(POD)activity,which significantly enhanced DS tolerance in linseed *** findings might be useful to oil seed breeders and farmers linseed for breeding program in linseed plants as well as sustainable agricultural production of oil seed crop plants.
