The prevalence of vision impairment and refractive error in 3654 first year students at Tianjin Medical University

AIM：To determine the prevalence of vision impairment（VI）and refractive error in first year university students at the Tianjin Medical University.METHODS：This is a cross-sectional observational cohort study of VI and refractive error among first year university students at the Tianjin Medical University.The first year university students were involved in this study and were given a detailed questionnaire including age,birth date,and spectacle wearing history.A standardized ophthalmologic examination including visual acuity（VA）,slit-lamp examination,non-cycloplegic auto-refraction,objective refraction,fundus photography,and examination of their spectacles were recorded.RESULTS：A total of 3654 participants were included in this study.Totally 3436（94.03%） individuals had VI in this population.Totally 150（4.10%） individuals had VI due to ocular disease,including amblyopia,congenital cataract,retinal atrophy or degeneration,strabismus,congenital nystagmus,refractive surgery orthokeratology.Totally 3286（89.93%） subjects had VI due to refractive error.Only 218（5.97%） students were emmetropia.Moreover,refractive error was the main cause for the VI（95.63%）.Totally 3242（92.52%） students were myopia and the prevalence of mild,moderate,and high myopia subgroup was 27.05%,44.35%,and 21.26% respectively.Totally 44（1.29%） subjects were hyperopic.The rates of uncorrected visual acuity（UCVA）,presenting visual acuity（PVA）and best corrected visual acuity（BCVA）which better than 20/20 in both eyes were 5.65%,22.32% and 82.13% respectively.The rates of correction,under correction and well correction in myopia subjects were 82.73%,84.39% and 15.61%,respectively. CONCLUSION：We present a high prevalence of refractive errors and high rates of under correction refractive error among first year university students.These results may help to promote vision protection work in young adults.

Neuroinflammation and the NACHT,LRR,and PYD domains-containing protein 3 inflammasome play crucial roles in secondary tissue damage following an initial insult in patients with traumatic brain injury(TBI).Maraviroc,a C-C chemokine receptor type 5 antagonist,has been viewed as a new therapeutic strategy for many neuroinflammatory diseases.We studied the effect of maraviroc on TBI-induced neuroinflammation.A moderate-TBI mouse model was subjected to a controlled cortical impact device.Maraviroc or vehicle was injected intraperitoneally 1 hour after TBI and then once per day for 3 consecutive days.Western blot,immunohistochemistry,and TUNEL(terminal deoxynucleotidyl transferase-mediated dUTP nick-end labeling)analyses were performed to evaluate the molecular mechanisms of maraviroc at 3 days post-TBI.Our results suggest that maraviroc administration reduced NACHT,LRR,and PYD domains-containing protein 3 inflammasome activation,modulated microglial polarization from M1 to M2,decreased neutrophil and macrophage infiltration,and inhibited the release of inflammatory factors after TBI.Moreover,maraviroc treatment decreased the activation of neurotoxic reactive astrocytes,which,in turn,exacerbated neuronal cell death.Additionally,we confirmed the neuroprotective effect of maraviroc using the modified neurological severity score,rotarod test,Morris water maze test,and lesion volume measurements.In summary,our findings indicate that maraviroc might be a desirable pharmacotherapeutic strategy for TBI,and C-C chemokine receptor type 5 might be a promising pharmacotherapeutic target to improve recovery after TBI.

Globular adiponectin ameliorates insulin resistance in skeletal muscle by enhancing the LKB1-mediated AMPK activation via SESN2

Adiponectin has been demonstrated to be a mediator of insulin sensitivity;however,the underlined mechanisms remain unclear.SESN2 is a stress-inducible protein that phosphorylates AMPK in different tissues.In this study,we aimed to validate the amelioration of insulin resistance by globular adiponectin(gAd)and to reveal the role of SESN2 in the improvement of glucose metabolism by gAd.We used a high-fat diet-induced wild-type and SESN2/C57BL/6J insulin resistance mice model to study the effects of six-week aerobic exercise or gAd administration on insulin resistance.In vitro study,C2C12 myotubes were used to determine the potential mechanism by overexpressing or inhibiting SESN2.Similar to exercise,six-week gAd administration decreased fasting glucose,triglyceride and insulin levels,reduced lipid deposition in skeletal muscle and reversed whole-body insulin resistance in mice fed on a high-fat diet.Moreover,gAd enhanced skeletal muscle glucose uptake by activating insulin signaling.However,these effects were diminished in SESN2/mice.We found that gAd administration increased the expression of SESN2 and Liver kinase B1(LKB1)and increased AMPK-T172 phosphorylation in skeletal muscle of wild-type mice,while in SESN2/mice,LKB1 expression was also increased but the pAMPKT172 was unchanged.At the cellular level,gAd increased cellular SESN2 and pAMPK-T172 expression.Immunoprecipitation experiment suggested that SESN2 promoted the formation of complexes of AMPK and LKB1 and hence phosphorylated AMPK.In conclusion,our results revealed that SESN2 played a critical role in gAd-induced AMPK phosphorylation,activation of insulin signaling and skeletal muscle insulin sensitization in mice with insulin resistance.

Signature Based on Six Autophagy-related Genes to Predict Prognosis of Head and Neck Squamous Cell Carcinoma

Objective The present study aimed to develop an autophagy-related gene prognostic prediction model to provide survival risk prediction for head and neck squamous cell carcinoma(HNSCC)patients.Methods The K-mean cluster analysis was performed on HNSCC samples based on the expression values of 210 autophagy-related genes for candidate signature gene selection.LASSO Cox regression analysis was generated using the potential genes and the risk score was calculated from the prognosis model.The risk score was processed as an independent prognostic indicator to construct the nomogram model.The immune status including immune cell infiltration ratio and checkpoints of patients with HNSCC in high-and low-risk groups was also explored.Results LASSO Cox regression analysis was performed on the selected autophagy-related genes.According to the lambda value corresponding to the number of different genes in the LASSO Cox analysis,six genes(GABARAPL2,SAR1A,ST13,GAPDH,FADD and LAMP1)were finally chosen.The risk score based on the genes was generated,which was an independent prognostic marker for HNSCC.The prognostic prediction model(nomogram)was further optimized by the independent prognostic factors(risk score),which can better predict the prognosis and survival of patients.With the risk score and prognosis model,eight types of immune cells and six key immune checkpoints(CTLA4,PD1,IDO1,TDO2,LAG3,TIGIT)displayed expression specificity.Conclusion This study identified several potential prognostic biomarkers and established an autophagy-related prognostic prediction model for HNSCC,which provides a valuable reference for future clinical research.

Particulate pollution is a global risk factor that seriously threatens human health.Fine particles(FPs)and ultrafine particles(UFPs)have small particle diameters and large specific surface areas,which can easily adsorb metals,microorganisms and other pollutants.FPs and UFPs can enter the human body in multiple ways and can be easily and quickly absorbed by the cells,tissues and organs.In the body,the particles can induce oxidative stress,inflammatory response and apoptosis,furthermore causing great adverse effects.Epidemiological studies mainly take the population as the research object to study the distribution of diseases and health conditions in a specific population and to focus on the identification of influencing factors.However,the mechanism by which a substance harms the health of organisms is mainly demonstrated through toxicological studies.Combining epidemiological studies with toxicological studies will provide a more systematic and comprehensive understanding of the impact of PM on the health of organisms.In this review,the sources,compositions,and morphologies of FPs and UFPs are briefly introduced in the first part.The effects and action mechanisms of exposure to FPs and UFPs on the heart,lungs,brain,liver,spleen,kidneys,pancreas,gastrointestinal tract,joints and reproductive system are systematically summarized.In addition,challenges are further pointed out at the end of the paper.This work provides useful theoretical guidance and a strong experimental foundation for investigating and preventing the adverse effects of FPs and UFPs on human health.

Engineered macrophage-biomimetic versatile nanoantidotes for inflammation-targeted therapy against Alzheimer’s disease by neurotoxin neutralization and immune recognition suppression

Immune recognition of excessive neurotoxins by microglia is a trigger for the onset of neuroinflammation in the brain,leading to neurodegeneration in Alzheimer’s disease(AD).Blocking active recognition of microglia while removing neurotoxins holds promise for fundamentally alleviating neurotoxin-induced immune responses,but is very challenging.Herein,an engineered macrophage-biomimetic versatile nanoantidote(OT-Lipo@M)is developed for inflammation-targeted therapy against AD by neurotoxin neutralization and immune recognition suppression.Coating macrophage membranes can not only endow OT-Lipo@M with anti-phagocytic and inflammation-tropism capabilities to target inflammatory lesions in AD brain,but also efficiently reduce neurotoxin levels to prevent them from activating microglia.The loaded oxytocin(OT)can be slowly released to downregulate the expression of immune recognition site Toll-like receptor 4(TLR4)on microglia,inhibiting TLR4-mediated pro-inflammatory signalling cascade.Benefiting from this two-pronged immunosuppressive strategy,OT-Lipo@M exhibits outstanding therapeutic effects on ameliorating cognitive deficits,inhibiting neuronal apoptosis,and enhancing synaptic plasticity in AD mice,accompanied by the delayed hippocampal atrophy and brain microstructural disruption by in vivo 9.4T MR imaging.This work provides new insights into potential AD therapeutics targeting microglia-mediated neuroinflammation at the source.

Impact of Cold Spells on Ischemic Stroke Severity and Clinical Outcomes in tianjin, China

There are several studies on the impact of extreme weather on the incidence and mortality of stroke, but the research on the effect of extreme weather events on severity and outcome of ischemic stroke is scarce. This study aimed to evaluate the impact of cold spells on the severity and clinical outcomes of patients with initial acute ischemic stroke in a hospital-based study. We enrolled 553 patients with initial ischemic stroke during the cold seasons between 2016 and 2019. Patients were separated into the mild stroke group and moderate-to-severe stroke group according to the National Institutes of Health Stroke Scale(NIHSS) of the U.S., and good outcome group and poor outcome group according to modified Rankin Scale(mRS) scores. There are nine different definitions of cold spells according to intensities and duration time. After adjustment for climate factors, air quality index, and common risk factors, it is found that cold spells were associated with moderate-to-severe neurological deficits and clinical outcomes in overall ischemic stroke patients. Furthermore, the delay effect for stroke severity started at the beginning of the cold wave(Lag 0) and lasted up to 14 days(Lags 0–14). In addition, when a cold spell was set as a daily mean temperature < 3rd or 5th percentile and with a duration ≥ 2 or 3 consecutive days, cold spells had a significant impact on clinical outcomes, and there was a definite delay effect of at least 7 days(Lags 0–7) and it lasted up to 14days(Lags 0–14). At last, we concluded that cold spells may be a contributory factor for more severe neurological deficits and worse outcomes in patients with initial ischemic stroke.

Lactobacillus levels and prognosis of patients with acute myocardial infarction

OBJECTIVE To explore the relationship between Lactobacillus and prognosis of acute myocardial infarction(AMI)patients treated by percutaneous coronary intervention(PCI)and its correlation with clinical parameters.METHODS Consecutive patients with AMI in the coronary care unit of tianjin Chest Hospital in China who received emergency PCI between July 2017 and December 2018 were enrolled.Subjects’fecal 16 S r DNA gene sequencing data were analyzed and subjects were categorized into low,medium and high level groups according to stool Lactobacillus measurements.The primary endpoints were major adverse cardiac events.Cox regression analysis was used to analyze the relationship between Lactobacillus and prognosis.Spearman correlation analysis and trend tests were used to assess the relationship between Lactobacillus and the clinical indicators.RESULTS The data of 254 patients were included in the analysis.Mean age was 65.90±11.56 years,and 152 patients(59.84%)were male.Follow-up time was 652(548.25-753.00)days.Multivariate Cox regression analysis showed a significantly lower risk of major adverse cardiac events in patients with Lactobacillus>7.1 copies/g[adjusted hazard ratio(HR)=0.216,95%CI:0.094-0.493,P<0.001]compared to patients with Lactobacillus≤3.6 copies/g.Statistically significant differences were shown in ST-segment elevation myocardial infarction(STEMI)(HR=0.217,95%CI:0.085-0.551,P=0.001).Lactobacillus was a protective factor for male smokers aged over 60 years whose brain natriuretic peptide was over 1,000 pg/m L.Spearman correlation analysis showed that Lactobacillus correlated negatively with white blood cells,neutrophils,high-sensitivity C-reactive protein,Troponin T,creatine kinase,creatine kinase-MB and brain natriuretic peptide(downward trend),and correlated positively with left ventricular ejection fraction(upward trend).CONCLUSIONS This study is the first to reveal the correlation between Lactobacillus and inflammation and myocardial damage after STEMI.STEMI patients,especially male smokers aged over

Dear Editor,Although progressive respiratory failure is the primary cause of death in patients with COVID-19,thromboembolic complications further increased the mortality rates.Endothelial dysfunction,a pro-inflammatory and pro-coagulant state characterized by increased interaction with leukocytes and platelets,is reported to play a key role in COVID-19-associated thrombosis;1 however,its underlying mechanism remains unclear.

Role of platelet-derived extracellular vesicles in traumatic brain injury-induced coagulopathy and inflammation

Extracellular vesicles are composed of fragments of exfoliated plasma membrane,organelles or nuclei and are released after cell activation,apoptosis or destruction.Platelet-derived extracellular vesicles are the most abundant type of extracellular vesicle in the blood of patients with traumatic brain injury.Accumulated laboratory and clinical evidence shows that platelet-derived extracellular vesicles play an important role in coagulopathy and inflammation after traumatic brain injury.This review discusses the recent progress of research on platelet-derived extracellular vesicles in coagulopathy and inflammation and the potential of platelet-derived extracellular vesicles as therapeutic targets for traumatic brain injury.
